Objective To observe the effect of treating non-alcoholic fatty liver with Baogan Kangxiao decoction.Methods 120 cases of non-alcoholic fatty liver were randomly recruited into a control group and a treatment group. The control group was treated with Zhibituo tablet and ursodeoxycholie acid, while the treatment group was treated with Baogan Kangxian decoction on the basis of the control group. Both groups were treated for four weeks. Results the total effective rate was 90.0% and 70.0% in the treatment group and the control group respectively, showing significant difference (χ2=6.266, P＜0.05). Conclusion Baogan Kangxian decotion has obvious effects on nonalcoholic fatty liver in reducing fat and enzyme.

Objective To investigate the optimal therapeutic strategy of severe acute pancreatitis(SAP). Methods The effect of treatment of non operation, endoscopy and operation on cure rate, complication rate and course of the disease in SAP were retrospectively analysed. Results Non operative, endoscopic and operative treatment included 33, 8 and 19 cases respectively.Cure rate, complication rate and course of the disease in non operation group were 85.7%(24/28), 16.7%(4/24), (25?8)days respectively;in endoscope group were 100%(8/8), 12.5%(1/8), (19?4)day respectively;in operation group were 68.4%(13/19), 38.5%(5/13), (57?14)day respectively. Conclusions According to the difference of pathogeny and severity,the treatment should be taken individually.In acute reaction phase of SAP should be treated with non operation,if there isn't existence of promoting facters.For gallstone SAP, endoscopy or operation is the first choice of treatment.If patients can not be cured by non operation or endoscopy,operaton should be performed.

Objective To explore the improvement of minocycline on negative symptoms in patients with schizophrenia and its influence on tumor necrosis factor (TNF)-α.Methods Patients with schizophrenia were randomly assigned into two groups.One was olanzapine combined with placebo group (control group, 37 cases).The other was olanzapine combined with minocycline group (treatment group, 200 mg ·d-1,po,bid, 38 cases).Both groups were assessed by the positive and negative syndrome scale (PANSS), the scale for the assessment of negative symptoms (SANS), and also tested the level of TNF-α before and after 12 weeks treatments.Results After 12 weeks treatments, the scores of SANS and PANSS in treatment group were significantly lower than those of control group (P<0.05).The level of TNF-α in treatment group[(6.5±1.3) pg ·mL-1]was significantly lower than that of control group[(7.6±1.8) pg ·mL-1] (P<0.01).Conclusion Minocycline can improve negative symptoms in patients with schizophrenia, and the mechanism may be related to lower the level of TNF-α.

Objective To investigate clinical efficacy of epirubicin joint docetaxel for triple -negative and non -triple -negative breast cancer,to provide reference for clinical treatment.Methods A retrospective analysis of 80 cases with breast cancer was conducted,26 patients with triple -negative breast cancer were set as study group, and 54 patients with non -triple negative breast cancer were set as control group.All patients used epirubicin docetax-el chemotherapy,the curative effect,survival rate,toxicity were compared between the two groups.Results The chemotherapy response rate of triple -negative breast cancer group was 84.62%,which was significantly higher than 53.70% in non -triple -negative breast cancer group (χ2 =6.45,P <0.01).The 5 -year disease -free survival rate,overall survival rate of triple -negative breast cancer group were 50.00%,73.08%,which were significantly lower than those of non -triple -negative breast cancer group (χ2 =5.89,P <0.01;χ2 =4.19,P <0.01).The inci-dence rates of constipation,nausea,vomiting,leukopenia,neutropenia Ⅲ -Ⅳ degrees reaction were 13.75%, 10.00%,15.00%,13.75%,and the overall incidence rates were 41.25%,42.50%,41.25%,43.75% respective-ly.Conclusion Epirubicin joint docetaxel adjuvant chemotherapy has good short term efficacy for triple negative breast cancer,but its long -term efficacy is poor to non -triple -negative breast cancer.

Objective To explore whether portal vein injection of small interfering RNA(siRNA) targeting connective tissue growth factor(CTGF) could inhibit CTGF expression in rats liver in vivo and prevent rats hepatic fibrosis. Methods Twenty four male rats were equaly divided into four groups. Rats received subcutaneous injection of 40% CCl4 (3 ml/kg) together with portal vein injection of saline every three days for 6 consecutive weeks were served as model group; CCl4 together with portal vein delivery of siRNA (0.1 mg/kg) as preventive group; CCl4 for 2 weeks followed by CCl4 and CTGF siRNA for more than 4 weeks as curative group, and only intraportal injection of saline as control group. Portal vein pressure in all rats at 3 days after the last CCl4 injection were measured, and blood and hepatic tissue from rats were harvested. Serum concentration of transaminases, hyaluronic acid(HA) and procollagen type Ⅲ (PⅢNP) were measured. Expression of CTGF mRNA and protein in rats liver was evaluated by RT-PCR and Western blot, respectively. Inflammation and fibrosis in rats liver was analyzed by H-E and Sirius red collagen staining. The fibrotic area was measured quantitatively by a computer-aided manipulator. Results Compared with model group, the expression of CTGF mRNA and protein in liver in both preventive and curative groups were markedly down-regulated. Inflammation, necrosis and fibrosis in hepatic tissue was significantly attenuated. In addition, the serum concentration of transaminases, HA and PⅢNP were greatly reduced. Objective To study the causes and management of biliary complications after orthotopic liver transplantation.MethodsClinical data of 38 patients who have underwent orthotopic liver transplantation between May 2000 and January 2002 were studied retrospectively. ResultsPostoperative bililary complications occurred in 9 patients(24%), including simple biliary fistula in 4, stricture of hepatoenterostomy, intrahepatic cholelith, biliary stricture with sludge, biliary stricture with cholelith, and intrahepatic biloma and cholelith in one each. Two patients died of the complication, and seven were cured successfully. ConclusionsThe causes of biliary complications are complicated. Early diagnosis and timely management helps to decrease the mortality. Endoscopic retrograde cholangiopancreatography and percutaneous transhepatic cholangiography is a valuable, less traumatic procedure.
