1.Pregnane X Receptor(PXR) and Constitutive Androstane Receptor(CAR) in the Multidrug Resistance

Chemotherapy is one of the three most common treatment modalities for cancer .However , chemotherapy as current firstline therapy induces significant side effects and limited efficacy ,leading to multidrug resistance and fast recurrence challenging the patient survival rate .Drug metabolizing enzymes ( DMEs) and efflux transporters promote the metabolism,clearance,and detoxification of chemotherapeutic agents .Nuclear receptors, especially pregnane X receptor (PXR,NR112)and constitutive androstane activated receptor (CAR,NR113),reg-ulate the expressions of target genes that could encode phase I DMEs ,phase II DMEs,and efflux transporters in the development of multidrug resistance ( MDR) during chemotherapy .Recent studies have revealed that PXR and CAR play pivotal roles in MDR of various human carcinomas .And their expressve levels or activation statuses could predict the risk of drug resistance in the patients subjected to chemotherapy .Accordingly ,PXR/CAR antag-onists,combining with existing chemotherapeutics that activate PXR /CAR,are promising options that could over-come MDR in cancer.
2.The research progress of amniotic fluid-derived mesenchymal stem cells in cancer therapy

Amniotic fluid-derived mesenchymal stem cells(AF-MSCs)are multipotential,nonhemato-poietic progenitor cells that have an inherent ability to self -renew and to differentiate into many cell types ,inclu-ding osteoblasts,chondrocytes and adipocytes.Despite their high proliferation rate,no in vivo tumorigenicity has been reported and their karyotype was normal even after extensive expansion in culture .Several studies have dem-onstrated the inherent tumortropic properties of MSCs and their successful use as cellular vehicles of therapeutic agents for the treatment of several types of cancer .This article reviews recent articles on studying the role of amni-otic fluid-derived mesenchymal stem cells in cancer therapy .
3.Study on the effects of psychological and behavior intervention for convalescent cancer patients

Objective To explore the mode of psychological and behavior intervention which suitable for convalescent cancer patients,and evaluate its clinical effects in order to provide empirical basis for nursing care of cancer patients. Methods havior intervention condition. All the patients were evaluated by General Self-Efficacy Scale (GSES),ProfiIe of Mood States-Short Form(POMS-SF) and Quality of Life Questionnaire-Core 30(QLQ-C30)before and after the intervention. Result Compared with the control group,the scores of self-efficacy and quality of life were significantly improved in the experimental group (P< 0.01),while the total score and factor scores of POMS-SF such as tension-anxiety (TA),anxiety-hostility(AH),fatigne-inertia (FI),and confusion-babelism(CB)were significantly decreased(P<0.05). Conclusion Psychological and behavior intervention is proved to be feasible and effective to improve the mental health status,self-efficacy and quality of life in convalescent cancer patients.
4.Postoperative radiotherapy for completely resected stage Ⅲ A-N2 non-small cell lung cancer

Current studies show that postoperative adjuvant radiotherapy (PORT)may improve local control and overall survival for Ⅲ A-N2 NSCLC.With modern radiotherapy technology and techniques,the adverse effect of PORT is moderate and tolerated.Besides,modern PORT doesn′t decrease the survival.Inves-tigation concerning postoperative target volume for PORT is rare,so that definite agreement has not been reached.Available informations suggest that clinical target volume should include bronchial stump,involved lymph nodes,ipsilateral hilar nodes,subcarinal nodes and ipsilateral lower paratracheal nodes.
5.Observation of short-term curative effect of Suxiao Jiuxin Pill on coronary heart disease and angina pectoris

AIM: To observe the short-term curative effect of Suxiao Jiuxin Pill on coronary heart disease (CHD) and angina pectoris. METHODS: 187 patients were divided into two groups at random:the treatment group with 105 cases and the control group with 82 cases. The treatment group was given Suxiao Jiuxin Pill 6 pills tid and the control group was given Compound Danshen Dripping Pill 6 pills tid orally. The treating period was 2 weeks. Before and after treatment angina pectoris was observed and electrocardiogram was determined. RESULTS: Suxiao Jiuxin Pill could significantly alleviate syndrome of CHD in patients, lessen electrocardiogram and decrease dosage of nitroglycerin. 6.Construction of a cDNA library from Agkistrodon acutus venom gland and identification of Agkihagin, a novel transcript for metalloproteinase

AIM To construct a non-normalized cDNA library from Agkistrodon acutus venom gland as an imtial step to develop new and more effective venom by genetic engineering technique for screening and expressing target genes. METHODS The total RNA was extracted from fresh venom gland using Trizol. mRNA was reversely transcripted to cDNA using superscriptⅡ reverse transcriptase. Second-strand synthesis was performed using DNA polymeraseⅠ. After adding EcoRⅠ adaptor, phosphorylating the end and digesting with XhoⅠ, the cDNA was collected in five fractions (<0.25 kb, 0.25-0.5 kb, 0.5-1 kb, 1-2 kb and >2 kb) using the QIAquick Gel Extraction kit and ligated to pBluescriptⅡ vectors. The five libraries obtained were plated by infecting E.coli DH10B, constructing a cDNA library of Agkistrodon acutus venom gland. Sequencing clones at random, 8696 high quality 5′ end expressed sequenced tags (ESTs) were obtained and analyzed. The initial sequences were assembled into 2855 clusters. Among which, one of the clusters (Agkihagin) consisting of 74 ESTs was identified as a novel metalloprtoteinase based on RT-PCR and sequence analysis. RESULTSThe titers of library were 2.048×106. The novel metalloproteinase belonged to PⅢ type metalloproteinase. Its open reading frame was composed of 1827 nucleotides and coded a pre-zymogen of 608 amino acid with zinc-binding domain for metalloproteinase and Asp-Glu-Cys-Asp(DECD) domain for disintegrin. CONCLUSION The capacity of cDNA library of venom gland is above the general level of cDNA library. It would be a helpful platform to construct a catalog for transcripts in the venom gland of the Agkistrodon acutus. The sequence analysis indicates that the deduced amino acid sequence of the identified gene for metalloproteinase share the highest 87% identity with the metalloproteinase genes of other snakes in the GenBank. It lays a good foundation for the study of structure-function relationships of snake venom metalloproteinases.

Objective To investigate the blood fatty levels in different genotypes of matrix metalloproteinases (MMP) and the carotid atherosclerosis (CAS) in patients with essential hypertension (EH).Methods Four hundred and eighty-eight cases of Han and Uygur in patients with EH were selected,and MMP-2-735 C/T and MMP-3-1171 5A/6A polymorphism was measured by polymerase chain reactionrestriction fragment length polymorphism.They were divided into two groups:EH with CAS group (CAS group,293 cases,Han 171 cases,Uygur 122 cases) and EH without CAS group (NS group,195 cases,Han 105 cases,Uygur 90 cases).Blood fatty factors were analyzed according to the different MMP genotypes.Results According to MMP-2 genotypes,in Hart CAS group,the low density lipoprotein cholesterol (LDL-C) levels in MMP-2 CT+TT genotypes were significantly higher than those in CC genotype(P < 0.05),and high density lipoprotein cholesterol levels were significantly lower than those in CC genotype(P < 0.05); In Uygur CAS group,the triglyceride levels in MMP-2 CT+TT genotypes were significantly higher than those in CC genotype(P < 0.05).According to MMP-3 genotypes in Han NS group,the total cholesterol and LDL-C levels in 6A/6A genotype were significantly higher than those in 5A/5A+5A/6A genotypes (P < 0.05).Conclusion The blood fatty levels are different in patients with different MMP genotypes,which may probably influence the CAS.
8.Curative Effect of Adhesive Intestinal Obstruction with Integrated Traditional Chinese and Western Medi-cine

Objective: To study the curative effect of adhesive intestinal obstruction with integrated traditional Chinese and western medicine. Methods: A total of 128 patients with adhesive intestinal obstruction were randomly divided into 2 groups ( 64 patients in each) . The western medicine group was treated with conventional therapies. Traditional Chinese and western medicine group received acupuncture and moxibustion on the basis of western medicine treatment. After 3 days, abdominal X?ray plain film examination was done. The recovery of gastrointestinal function, cure rate, hospitalization days and cost was compared. Results: The symptom alle?viation rate of abdominal pain and distension, nausea and vomitting, and exhaust and defecation in Chinese and Western medicine group were higher than those in western medicine group ( P<0?05) . The cure rate was higher than that in western medicine group ( P<0?05). Hospitalization days and cost were lower than those in western medicine group (P<0?01). Conclusion: Compared with treatment with western medicine, it has quick effect, high cure rate, short treatment course, low cost in the treatment of adhesive intestinal obstruction with integrated traditional Chinese and western medicine and acupuncture.
9.Effects of long term use of beclomethasone dipropionate nasal spray on bone density with perennial allergic rhinitis.

OBJECTIVE:
To study the effects of long term use of beclomethasone dipropionate (BDP) nasal spray on bone density with perennial allergic rhinitis (AR) in adults.
METHOD:
A 5-year randomized study was conducted on the effects of BDP nasal spray on serum calcium, phosphorus, alkaline phosphatase, and bone density determined before and after the treatment in 36 adult patients with perennial AR. 20-45 years of age, were randomly divided into 3 groups. That is group A (nasal spray 1 - <3 year), group B (nasal spray BDP 3 - <5 year) and group C (nasal spray BDP > or =5 year). The data were analyzed by paired t test.
RESULT:
The perennial AR were followed up for more than > or =1 year, > or =3 year and > or =5 year to observe the influences of nasal spray BDP. There were no significant difference between the data examined before and after the treatment (P > 0.05). Bone development is not influenced by nasal spray BDP < or =400 microg/d within 5 years.
CONCLUSION
Long term use of BDP nasal spray in adult patients does not lead to osteoporosis if the lowest effective steroid dose is given.

10.Obstertric nurse's ability to treat maternal pain in cesarean section and its influencing factors

Objective To investigate the pain management ability of obstetric nurses in puerperants undergoing cesarean section, and to provide some reference for improving the professional knowledge and service ability of obstetric nursing staff. Methods From November 2017 to November 2018, 162 staff in charge of nursing puerperants undergoing cesarean section in Obstetrics Department of 3 ClassⅢ Grade A hospitals were selected by convenience sampling method. General Information Questionnaire, Nurses' Knowledge and Attitudes Survey Regarding Pain(NKAS) were used to investigate and study and compare the pain management ability of nurses with different data. A total of 162 questionnaires were sent out and 156 valid questionnaires were recovered with a recovery rate of 96.30%. Results The scores of NKAS scale of 156 nurses were (15.16±5.42). The scores of "general knowledge of pain", "knowledge of analgesics","knowledge of pain assessment" and "knowledge of pain intervention" were (3.47±0.95), (6.89±1.24), (3.07±0.73) and (1.65±0.98), respectively. There was significant difference in NKAS scores between nurses with different ages, nurses' professional level, education and working years, and the difference was statistically significant (P<0.05). Multiple linear regression analysis showed that nurses' education and working years were the influencing factors of their pain management ability (P<0.05). Conclusions The pain management ability of obstetric caesarean section nurses in Nanjing Class Ⅲ Grade A Hospital is low. It is suggested to strengthen the training of pain management ability of obstetric caesarean section nurses with short working length and low professional levels in order to provide better nursing service for puerperants.
