In recent years, there has been significant progress in the research of oncolytic viruses for the therapy of gliomas. The latest clinical trial results related to the modification, effectiveness, and safety of oncolytic viruses have brought hope for the development of glioblastoma treatments. Modified oncolytic viruses, particularly those based on the herpes simplex virus, have gained approval in Japan. Clinical trials involving recombinant poliovirus have shown better-than-expected survival outcomes with a strong safety profile. Notably, the first-time report of adenovirus in combination with immune checkpoint inhibitors for glioblastoma has demonstrated promising survival benefits and safety. However, challenges remain, including the selection of administration routes and the sustainability of treatment effects during oncolytic virus therapy. Therefore, further preclinical and clinical studies are required to improve the effectiveness and optimize treatment strategy for glioblastoma using oncolytic viruses.

3.Application value of biological muscle flap in laparoscopic radical proximal gastrectomy with esophagogastric anastomosis

Objective:To investigate the application value of biological muscle flap in laparo-scopic radical proximal gastrectomy with esophagogastric anastomosis.Methods:The retrospec-tive and descriptive study was conducted. The clinicopathological data of 10 patients with adeno-carcinoma of esophagogastric junction who were admitted to The First Affiliated Hospital of Xi′an Jiaotong University from May 2023 to August 2023 were collected. All patients were males, aged (65±5)years. All patients underwent laparoscopic radical proximal gastrectomy and esophagogastric anastomosis with digestive tract reconstruction using the esophagogastric biological muscle flap. Observation indicators: (1) surgical situations and early complications; (2) follow-up and late com-plications. Measurement data with normal distribution were represented as Mean± SD, and measure-ment data with skewed distribution were represented as M(range). Count data were described as absolute numbers. Results:(1) Surgical situations and early complications. All 10 patients success-fully completed the surgery without conversion to open surgery, and the operation time was (166±18)minutes. Cases with digestive tract reconstruction as end-to-side anastomosis and Overlap anas-tomosis were 1 and 9, respectively. The time of digestive tract reconstruction, the number of lymph node dissected, volume of intraoperative blood loss, time to postoperative first anal exhaust, time to postoperative first intake of liquid food, duration of postoperative hospital stay were (40±12)minutes, 24±6, (41±9)mL, (3.4±0.5)days, (4.1±1.0)days, (8.3±0.7)days in the 10 patients. Of 4 cases with postoperative early complications, 1 case developed pulmonary infection (Clavien-Dindo grade Ⅱ) on the second day after surgery, with pulmonary infection absorbed after 5 days of antibiotic treat-ment. Two cases experienced chest distress and shortness of breath on the third day after surgery, with the diagnosis of a small to moderate amount of pleural effusion after chest B-ultrasound examination. After pleural puncture and active treatment, the symptoms of them were improved and the pleural effusion disappeared. There was 1 case with choking sensation when eating solid food, which was started from the third week after surgery. Upper gastrointestinal imaging revealed mild anastomotic stenosis of Clavien-Dindo grade Ⅰ in the patient, who was improved after conservative treatment. On the 7th day after surgery, all 10 patients underwent upper gastrointestinal angiography, and no anastomotic leakage or stenosis occurred. There was no sign of contrast agent reflux in the supine position and 30° head down position. (2) Follow-up and late complications. All 10 patients were followed up for 59.5(range, 31.0-127.0)days. The esophageal reflux scale score of 10 patients was 1.4±0.3. During the follow-up, 1 case underwent gastroscopy on 40 days after surgery, which showed reflux esophagitis with Los Angeles grade as B and the Clavien-Dindo grade as Ⅰ. There was no clinical symptom such as heartburn or acid reflux. Results of 24-hour pH monitoring showed that the patient experienced 24 instances of reflux in an upright position and 15 instances of reflux in a supine position, with no prolonged reflux. The total reflux time within 24 hours was 75 minutes. The DeMeester score was 38.3. Results of esophageal pressure measurement showed that the esophageal contraction morphology was normal, but the anastomotic opening was not well relaxed. The rest of 9 cases had no complication such as reflux esophagitis.Conclusion:Biological muscle flap applied in the laparoscopic radical proximal gastrectomy with esophagogastric anastomosis is safe and feasible, with satisfied short-term efficacy.

Infectious bone defect is bone defect with infection or as a result of treatment of bone infection. It requires surgical intervention, and the treatment processes are complex and long, which include bone infection control,bone defect repair and even complex soft tissue reconstructions in some cases. Failure to achieve the goals in any step may lead to the failure of the overall treatment. Therefore, infectious bone defect has been a worldwide challenge in the field of orthopedics. Conventionally, sequestrectomy, bone grafting, bone transport, and systemic/local antibiotic treatment are standard therapies. Radical debridement remains one of the cornerstones for the management of bone infection. However, the scale of debridement and the timing and method of bone defect reconstruction remain controversial. With the clinical application of induced membrane technique, effective infection control and rapid bone reconstruction have been achieved in the management of infectious bone defect. The induced membrane technique has attracted more interests and attention, but the lack of understanding the basic principles of infection control and technical details may hamper the clinical outcomes of induced membrane technique and complications can possibly occur. Therefore, the Chinese Orthopedic Association organized domestic orthopedic experts to formulate An evidence-based clinical guideline for the treatment of infectious bone defect with induced membrane technique ( version 2023) according to the evidence-based method and put forward recommendations on infectious bone defect from the aspects of precise diagnosis, preoperative evaluation, operation procedure, postoperative management and rehabilitation, so as to provide useful references for the treatment of infectious bone defect with induced membrane technique.
5.Study on the toxicity of the extract from Miao medicine Wikstroemia indica to zebrafish

OBJECTIVE To study the toxicity of the extract from Mi ao medicine Wikstroemia indica to zebrafish . METHODS Zebrafish embryo model was used as the object ,after exposure to W. indica extract (10,20,40 μg/mL),the number of spontaneous twitching within 1 min,heart rate within 10 s,the occurrence of malformation and death were detected and recorded . Zebrafish was used as the object ,after exposure to W. indica extract(10-100 μg/mL)for 24,48 and 72 h,and the median lethal concentration(LC50)of W. indica extract to zebrafish at different time points were calculated . After exposure to low ,medium and high concentration (27,37,51 μg/mL)of W. indica extract,the liver phenotype ,hepatocyte apoptosis and lipid deposition of zebrafish were observed ,and the activities of alanine aminotransferase (ALT),aspartate aminotransferase (AST)and lactate dehydrogenase(LDH)in liver tissue were detected . RESULTS Compared with blank group ,the number of spontaneous twitching , malformation rate (except for 10 μg/mL group )and mortality of embryos increased significantly in 10,20,40 μg/mL groups of W. indica extract,and the heart rate (except for 10,20 μg/mL group )of embryos decreased significantly (P<0.05). LC50 of W. indica extract to zebrafish at 24,48 and 72 h were 39.850,28.300 and 21.490 μg/mL,respectively. After drug treatment ,the transparency of liver area of zebrafish in low ,medium and high concentration groups of W. indica extract reduced and their shape were enlarged;apoptosis and lipid deposition increased ;the activities of ALT ,AST and LDH in liver tissue were significantly increased (P<0.05). CONCLUSIONS Miao medicine W. indica extract had develop mental toxicity to zebrafish embryos and he patotoxicity to zebrafish .
6.Study on nephrotoxicity of the extracts from different parts of Miao medicine Wikstroemia indica

OBJECTIVE To study the nephrotoxicity of the extracts from different parts o f Miao medicine Wikstroemia indica in healthy rats ,and to provide reference for the study of its toxicity mechanism and clinical drug use. METHODS Using 70% ethanol as solvent ,total ethanol extract of W. indica was extracted with diacolation method. After dispersing the above extract with water,the fractions of corresponding fractions were obtained with petroleum ether ,ethyl acetate and n-butanol,and the rest was the extract of water fraction. SD rats were randomly divided into total ethanol extract group ,petroleum ether fraction group ,ethyl acetate fraction group ,n-butanol fraction group ,water fraction group and blank group ,with 12 rats in each group (half male and half female ). The rats in the administration groups were given the corresponding dose of drug solution intragastrically (total ethanol extract 317.520 mg/kg,petroleum ether fraction 7.875 mg/kg,ethyl acetate fraction 78.435 mg/kg,n-butanol fraction 53.865 mg/kg and water fraction 76.545 mg/kg),once a day ,for conse- cutive 2 weeks,and then stopped taking drug for 2 weeks; rats in the blank group were given equal volume of 1.0% . sodium carboxymethyl cellulose solution intragastrically. Duringthe experiment ,the general conditions of rats were observed. The samples of urine (on the 14th and 28th day ),serum and bilateral renal tissues (on the 15th and 29th day )were taken respectively,the renal index was calculated ,the levels of @qq.com renal function indexes in serum and urine were detected ,and the pathomorphological changes of renal tissues were observed. RESULTS During administration ,compared with blank group ,the rats in the total ethanol extract group and ethyl acetate fraction group showed poisoning behavior and activity characteristics such as mental depression ,decreased activity and diet ,thin stool and decreased body mass. The mental state of the rats in the petroleum ether fraction group ,n-butanol fraction group and water fraction group were slightly worse than that in blank group,and slightly decreased activity and diet as well as thin stool ,and slowly increased body mass were found ;however,there was no significant difference in anal temperature in each group. After 2 weeks of administration ,the renal index in total ethanol extract group ,the serum levels of N-acetylglucosaminidase(NAG),urea nitrogen (BUN)and creatinine (Cr)in total ethanol extract group and ethyl acetate fraction group ,serum level of NAG in n-butanol fraction group and serum level of Cr in water fraction group ,as while as NAG levels in urine of rats in total ethanol extract group and petroleum ether fraction group ,NAG and urinary protein levels in urine of rats in ethyl acetate fraction group were increased significantly (P<0.05 or P<0.01). In the pathomorphological observation ,renal tubules showed different degrees of unclear structure ,cell swelling and a few cell necrosis in the total ethanol extract group ,petroleum ether fraction group and ethyl acetate fraction group ,accompanying by glomerular pyknosis,renal tubular sclerosis and inflammatory cell infiltration ,compared with blank group. After drug withdrawal ,the mental state of rats in the administration groups were significantly improved ,the amount of activity and diet increased ,and the stool tended to be normal. Two weeks after drug withdrawal and recovery ,the levels of above indexes in serum and urine of rats in administration groups returned to be close to that in blank group (P>0.05);the glomerular structure of rats in each administration group gradually recovered clearly ,and cell swelling and inflammatory cell infiltration were rare in total ethanol extract group , petroleum ether fraction group and ethyl acetate fraction group. CONCLUSIONS The total ethanol extract ,petroleum ether fraction and ethyl acetate fraction of Miao medicine W. indica have certain nephrotoxicity and reversibility. The toxic component may
7.Digestive tract reconstruction after laparoscopic proximal gastrectomy and the application prospect of esophagogastric anastomosis with double flap technique

Laparoscopic proximal gastrectomy (LPG) can be selected for the treatment of early upper gastric carcinoma, but gastroesophageal reflux after operation would seriously affect the quality of life of patients. Esophagogastric anastomosis with double flap technique is a digestive tract reconstruction method using the anastomosis between the esophagus and the anterior wall of the stomach. Compared with other digestive tract reconstruction methods, esophagogastric anastomosis with double flap technique can maintain the postoperative body mass of patients in good condition, improve the nutritional status and the long-term quality of life of patients. Esophagogastric anasto-mosis with double flap technique has good anti reflux effects and retain the possibility of endoscopic examination and treatment. By reviewing literatures at home and abroad, and combined with clinical experiences, the authors discuss current status and digestive tract reconstruction methods of LPG, and deeply investigate the application prospect of esophagogastric anastomosis with double flap technique.
8.Effect of visceral fat area on the prognosis of patients with radical gastrectomy

【Objective】 To investigate the predictive value of visceral fat area (VFA) in patients with gastric cancer after radical gastrectomy. 【Methods】 A retrospective analysis was performed on 195 patients who underwent radical gastrectomy in the Department of General Surgery, The First Affiliated Hospital of Xi’an Jiaotong University from January 2014 to December 2017. CT image data and clinicopathological data within 1 week before surgery were collected. VFA was calculated by software, and the patients were divided into VFA-H group (n=96) and VFA-L group (n=99). The relationship between VFA in different groups and long-term prognosis was compared. 【Results】 CT examination results showed that VFA value was (111±62) cm2, and BMI was positively correlated with VFA value (r=0.640, P<0.001). ROC curve showed that VFA was more valuable in predicting the prognosis of gastric cancer (AUC=0.703, P<0.001) and better than BMI. Cox regression analysis of prognostic factors in gastric cancer patients: Univariate analysis showed that age, tumor length, TNM stage and VFA were the influencing factors for prognosis, while multivariate analysis showed that TNM stage III and VFA-L were independent risk factors for prognosis of gastric cancer patients. 【Conclusion】 VFA has a good predictive ability and can be used to evaluate the prognosis of gastric cancer patients after operation.
9.The value of definitive perineal urethrostomy for the treatment of complex adult anterior urethral strictures

Objective To analyze the clinical outcome of definitive perineal urethrostomy for the treatment of complex adult anterior urethral strictures.Methods From June 2012 to February 2018,28 patients who underwent the definitive perineal urethrostomy for a complex anterior urethral stricture were retrospectively reviewed.The mean age of the patients was 62.3 (range 34 to 77) years and stricture etiology was lichen sclerosus in 46.4% (13/28) of the cases,iatrogenic stricture after transurethral resection of the prostate in 35.7% (10/28),failed hypospadias repair in 10.7% (3/28),infection 3.6% (1/28)and idiopathic etiology 3.6% (1/28).The mean stricture length was 9.6 (range 4.5 to 16.0) cm and the stricture length <5 cm in 7.1% (2/28) of the cases,5-10 cm in 35.7% (10/28) and > 10 cm in 57.1% (16/28).0f28 patients 21 (75%) underwent prior urethral dilation,4 (14.3%) underwent direct vision internal urethrotomy (DVIU) and 17 (60.7%) underwent urethroplasty.Among the medical history of interest,hypertension was observed in 17 patients (60.7%),chronic ischemic heart disease in 11 patients (39.3%),diabetes mellitus in 9 patients (32.1%) and chronic obstructive pulmonary disease (COPD) in 9 patients (32.1%).All patients completed the perineal urethrostomy with inverted U-shaped perineal skin flap.The patient was placed in a normal lithotomy position and an inverted U-shaped perineal incision was made.We can get an inverted U-shaped perineal skin flap after dissecting along the bulbospongiosus muscle.The bulbar urethra was opened along the ventral surface and the margins of the spongiosum tissue were closed with perineal skin flap.We analyzed the clinical characteristics of the patients,the outcome of the technique and investigated the satisfaction rates of the patients by outpatient follow-up or telephone interview.Surgical success was defined as functional voiding without the need for further procedures.Results The perioperative complications included 3 cases of urinary tract infection,1 case of bleeding and 1 case of wound dehiscence.Mean follow-up length was 27.2 (range 6.0 to 64.0)months.Of 28 cases 26 (92.9%) were successful and the mean maximum urinary flow rate was 26.5 (range 17.0 to 40.0) ml/s.Postoperatively,2 cases of urethrostomy strictures were found in 2 months and 4 months,respectively.They required periodic outpatient dilation and the younger one was in the waiting list for revision of the perineostomy and the older one refused the revision.Of 28 patients 24 (85.7%) were satisfied with the results obtained with surgery,15 were very satisfied,9 cases satisfied and 4 cases dissatisfied.Conclusions The successful rate of the definitive perineal urethrostomy for the complex adult anterior urethral strictures was high and patients were satisfied with the outcome of the surgery.The definitive perineal urthrostomy is a well-tolerated treatment option for patients with complex anterior urethral strictures,especially for those aged and with extensive comorbidities.
10.Efficacy of treating multilevel cervical spondylotic myelopathy with single-door laminoplasty via Y type nano-bone plate

Objective To explore the application value of single-door laminoplasty via Y type nano-bone plate in treating multilevel cervical spondylotic myelopathy (MCSM) and analyze the short-term efficacy.Methods From January 2013 to December 2016,79 cases of MCSM were treated with single-door laminoplasty via Y type nano-bone plate to evaluate the improvement of post-operative neurological function by the Japanese Orthopaedic Association (JOA) evaluation system.We also measured cervical curvature of cervical X-ray and C5 sagittal diameter of the spinal canal before operation and 6 months after operation to understand the maintenance and enlargement of the spinal canal.The improvement degree of spinal cord compression was evaluated by preoperative and postoperative cervical MRI.Osseous healing on the open door side and the door shaft side was observed with the aid of three-dimensional spiral CT.Results Follow-up ranged from 6 to 36 months,with an average of (20.4±7.9)months.Preoperative JOA score was (8.6 ± 1.3) points and JOA score 6 months after operation was (14.3 ± 1.5)points (P < 0.05).JOA improvement rate was (68.6 ± 15.8)%;postoperative follow-up X-ray and threedimensional spiral CT showed that the spinal canal had satisfying enlargement,the door shaft side all had osseous healing,the open door side osseous healing was not obvious,and there was no lamina collapse or reclosing.Sagittal diameter of theC spinal canal was (8.9±l.1)mm before operation and (15.1±l.1)mm 6 months after operation (P<0.05).The spinal canal enlargement rate was (70.8±22.3)%,cervical curvature was (14.8± 7.0)°preoperatively and (15.1±6.7)°postoperatively with no significant difference (P>0.05).Conclusion EOLP via Y type nano-bone plate is safe and efficacious in treating MCSM.It not only provides a good immediate fixation,but also provides the possibility for the open door side lateral osseous fusion.
