Objective To study the expression of ILD patients and health men during exercise,furthermore provide the reference of application of exercise test in ILD.Methods Cycle exercise test was used in 36 patients with ILD and 20 normal subjects,The oxygen uptake per minute(VO2),Borg Scale(BS),SpO2,and other index of exercise test were detected.Results Compared with normal subjects,The VO2max in ILD patients was decrease markedly;The BS was increased,SpO2 decreased markedly during exercise.Conclusions The results indicate that the hypoxemia and dyspnea are the limited factors of exercise in the patients with ILD;The capacity of exercise in ILD decrease compare with normal subjects.
2.Study on the Expression of Dyspnea and SpO_2 in COPD and Silicosis Patients During Exercise

Objective To study the expression of dyspnea and SpO2 with the patients with COPD and silicosis during exercise, further more to provide the reference in clinic treatment.Methods Cycle exercise test was used in 20 patients with COPD and 20 silicosis patients , The oxygen uptake per minute (VO2) ,Borg Scale (BS), SpO2, and other index of exercise test were detected. Results Dyspnea index as Borg scale was closely correlated with oxygen uptake (VO2) during exercise in all the patients. 3.Analysis of the application value of arthroscopic assisted percutaneous internal fixation and open reduction and internal fixation in treatment of tibial plateau fracture

Objective To compare the value of arthroscopic assisted percutaneous internal fixation and open reduction and internal fixation in treatment of tibial plateau fracture.Methods The clinical data of 180 patients of tibial plateau fracture were analyzed retrospectively,including 112 patients of type Ⅱ fracture (group A) and 68 patients of type Ⅲ fracture (group B).The patients in group A and group B were divided into group A1 (56 patients),A2 (56 patients),B 1 (34 patients),B2 (34 patients) respectively according to the odd number and even number of admission date.The patients in group A1 and B1 received open reduction and internal fixation and the patients in group A2 and B2 received arthroscopic assisted percutaneous internal fixation.The postoperative functional recovery was compared and evaluated between the same type of tibial plateau fractures under different treatments.Results In group A1:the postoperative functional recovery,the excellent was 12 patients,the good was 26 patients,the middle good was 12 patients,the bad was 6 patients.The rate of the excellent and the good was 67.9% (38/56).In group A2:the postoperative functional recovery,the excellent was 19 patients,the good was 34 patients,the middle good was 3 patients.The rate of the excellent and the good was 94.6%(53/56).The rate of the excellent and the good between group A1 and group A2 had significant difference (P < 0.01).After operation for 6 months,the HSS scores of knee joint and degree of flex and extend in group A1 and A2 had significant difference [(73.21 ± 8.32) scores vs.(85.58 ±9.89) scores,(35.6 ±6.6)° vs.(55.1 ±8.8)°] (P <0.01).In group B1:the postoperative functional recovery,the excellent was 8 patients,the good was 16 patients,the middle good was 7 patients,the bad was 3 patiens.The rate of the excellent and the good was 70.6%(24/34).In group B2:the postoperative functional recovery,the excellent was 12 patients,the good was 21 patients,the middle good was 1 patient.The rate of the excellent and the good was 97.1%(33/34).The rate of the excellent and the good between group B1 and group B2 had significant difference(P < 0.01).After operation for 6 months,the HSS scores of knee joint and degree of flex and extend in group B1 and B2 had significant difference [(75.32 ± 7.99) scores vs.(86.41 ±10.21) scores,(33.9 ±7.2)° vs.(56.6 ± 10.5)°](P<0.01).Conclusions The efficacy of arthroscopic assisted percutaneous internal fixation is better than open reduction and internal fixation either in type Ⅱ fracture or in type Ⅲ fracture.This method is mini-invasive and is worthy to spread.
4.Study of the expression of endocrine gland-derived vascular endothelial growth factor in the spinal cord tumors

Objective To explore the relationship of spinal tumors and endocrine gland-derived vascular endothelial growth factor( EG-VEGF) .Methods 77 cases of spinal tumors and 17 cases of vertebral compression fractures were collected.The testing group were grouped according to the pathological type of spinal tumor,the observ-ing group was consisted of vertebral compression fracture cases.The samples of various types of tumors in patients were collected during operation and the positive rate and expression of EG-VEGF mRNA were detected by RT-PCR technique,and the results of each group were statistical analysed.Results The results of relative expression of EG-VEGF mRNA in the unknown primary spinal metastases group,osteosarcoma group and chordoma group were higher, there were no statistically significant difference compared between three groups(all P>0.05).Relative expressions of that in osteoid osteoma group and giant cell tumor group were lower,so was in the observation group,there were no sig-nificant differences in three groups ( all P>0.05);The relative expressions of EG-VEGF mRNA in CUP group, osteosarcoma group and chordoma group were significantly higher than the osteoid osteoma group,with statistical sig-nificance[(0.81 ±0.21),(0.84 ±0.22),(0.79 ±0.41)vs(0.53 ±0.19),t=0.86,0.82,0.81,all P<0.05]. Similar results were also found in EG -VEGF mRNA positive expression rate in three groups.Conclusion EG-VEGF in normal spine spinal vertebrae and primary benign tumor ( osteoid osteoma,giant cell tumor) showed lower expression,which consistent with its tissue specificity,but in primary malignant spinal cancer ( osteosarcoma, chordoma) and unknown primary tumor spinal metastases showed the higher expression, indicating that it may be a measure of an important indicator of spinal cancer, and may play a key role in early diagnosis and treatment of cancer of the spine in the future.
5.Detection of Carcinoma of Urinary Bladder by Fluorescence in situ Hybridization

OBJECTIVE To study the molecular cytogenetic alterations of transitional cell carcinoma of the urinary bladder with exfoliated cells by fluorescence in situ hybridization(FISH) analysis of chromosome-specific probes.METHODS FISH was performed using 3,7,9 and 17 of chromosome-specific probes to examine chromosome aberration of exfoliated cells in 50 urine samples from patients with transitional cell urinary bladder carcinoma.RESULTS The frequency of numerical aberration of chromosomes 3,7,9 and 17 was 28%,32%,56% and 38% in urinary exfoliated cells,respectively.Loss of chromosome 9 was the most common finding,but it was not correlated with pathological grade of cancer and stage of the disease.Abnormality of chromosomes 3,7 and 17 was associated with the clinical stage.CONCLUSIONS A number of chromosome aberrations are detected in transitional cell carcinoma of the urinary bladder by FISH technique which provides a basis for further understanding of its molecular pathogenesis.It is a rapid,accurate and very sensitive method and can be used in clinical diagnosis.
6.Nosocomial Infection Analysis and Resistance Induced by Pseudomonas aeruginosa During 2005-2006

OBJECTIVE To investigate the change in antimicrobial resistance to Pseudomonas aeruginosa in our hospital from Jan 2005 to Dec 2006 and direct the using of drugs reasonably. METHODS P. aeruginosa was identified by API and VITEK2 system, and its antimicrobial resistance was determined by Kirby-Bauer method and VITEK2 system. The antimicrobial resistance rates were analyzed by WHONET5.4 software. RESULTS The resistant rates of P. aeruginosa to SAM, CTX, and CRO were higher than 70%. The resistant rate to CIP was the lowest (about 30% or so). CONCLUSIONS P. aeruginosa is still a major pathogenic bacterium in our hospital. It is very important to select antibiotics correctly according to the results of susceptibility tests.
7.Prediction model for probability of malignancy in solitary pulmonary nodules on 18F-FDG PET/CT of smokers with pulmonary fibrosis

Objective:To establish and validate a malignant risk prediction model of solitary pulmonary nodules (SPNs) with pulmonary fibrosis in long-term smokers based on 18F-flurodeoxyglucose (FDG) PET/CT. Methods:PET/CT images of 222 SPNs combined with pulmonary fibrosis which were shown in integrated CT scan in 169 patients (all males; age 68(63, 75) years) were analyzed retrospectively. All patients were examined in PET/CT Center of the Affiliated Hospital of Qingdao University from January 2011 to December 2019 and all had definite smoking history. The benign and malignant nodules were judged according to the pathological diagnosis or follow-up imaging data of lung lesions (follow-up≥2 years). The clinical characteristics (age, smoking index), morphological characteristics (longest diameter of lesion, density, location, distribution, relative position of fibrosis, spiculation, lobulation, calcification, vacuole, vascular convergence, pleural indentation, emphysema and severity of bilateral pulmonary fibrosis) and metabolic characteristics (maximum standardized uptake value (SUV max)) of the benign and malignant lesions were analyzed by χ2 test and Mann-Whitney U test. Then multivariate logistic regression analysis was applied to select independent risk factors of malignant nodules, and a risk prediction model was established and verified by the area under the receiver operating characteristic (ROC) curve and k-fold cross validation ( k=10) respectively. Results:Among 169 patients, 222 SPNs were detected (157 malignant nodules, 65 benign nodules). Univariate analysis showed that smoking index, speculation, lobulation, vascular convergence sign, calcification, emphysema, nodule size, relative position of nodule and fibrosis, SUV max and severity of bilateral pulmonary fibrosis were significantly different between the benign and malignant nodules ( z values: 2.514-9.858, χ2 values: 4.353-18.442, all P<0.05). Result of multivariate logistic regression analysis showed that calcification, vascular convergence and SUV max were the independent risk factors of malignant nodules combined with pulmonary fibrosis (odds ratio ( OR): 0.048-2.534, all P<0.05). The risk prediction model was as follow: P=1/(1+ e - x), x=-1.839-3.033×calcification+ 0.930×vascular convergence+ 0.754×SUV max(with calcification/vascular convergence=1, without calcification/vascular convergence=0). The area under ROC curve was 0.932(95% CI: 0.895-0.969), and the sensitivity and specificity of the model were 87.9% and 86.2%, respectively. Results of k-fold cross validation showed that the prediction accuracy of 10 test sets was 0.847±0.075, and was 0.862±0.010 in training sets. Conclusions:Calcification, vascular convergence and SUV max are independent risk factors of malignant SPNs combined with pulmonary fibrosis in long-term asymptomatic smokers. The model based on the above variables presents high diagnostic efficiency in diagnosing malignant SPNs.
8.A decrease in proliferative ability and directional differentiation ability of autologous bone marrow mesenchymal stem cells in a rat model of steroid-induced femoral head necrosis

BACKGROUND:In recent years, stem cel therapy for early osteonecrosis of the femoral head has become an alternative method, but the quality of stem cels is a key to the therapeutic outcomes. OBJECTIVE:To evaluate the proliferative ability and directional differentiation ability of autologous bone marrow mesenchymal stem cels in a rat model of steroid-induced femoral head necrosis. METHODS:Twenty Sprague-Dawley rats were randomly divided into control and observation groups with ten in each group. An animal model of steroid-induced femoral head necrosis was built in the observation group, and then bone marrow mesenchymal stem cels from rats in both two groups were isolated and cultured. Cel counting kit-8 was used to detect proliferation of passage 3 cels. Bone marrow mesenchymal stem cels at passage 3 were selected in the two groups for osteogenic and adipogenic induction. Alkaline phosphatase staining and alizarin red staining were adopted at 7 and 14 days of osteogenic induction, and oil red O staining as performed at 21 days of adipogenic induction. RESULTS AND CONCLUSION:The absorbance values of bone marrow mesenchymal stem cels were lower in the observation group than the control group at 1, 3, 5 days of culture, but there was no significant difference between two groups (P > 0.05). Until the 7th day of culture, the absorbance value and alkaline phosphatase activity in the observation group were significantly lower than that in the control group (P < 0.05). Additionaly, there were fewer calcium nodules and lipid droplets in the observation group compared with the control group. These findings suggest that the proliferative ability and directional differentiation ability of autologous bone marrow mesenchymal stem cels from a rat model of steroid-induced femoral head necrosis are both decreased.
9.53 cases with treatment of postoperative digestive tract fistula and associate complications of esophageal or cardiac cancer

Objective Discuss clinical type and treatment of postoperative digestive tract fistula and associate complications of esophageal or cardiac cancer.Methods Analized clinical manifestations and treatment retrospectively of 53 cases of digestive tract fistula after operation of esophageal or cardiac cancer from January,2010 to December,2012.Results All the 10 undergoing surgery are cured by reoperation.2 died of mediastinal infection and sepsis.4 died of respiratory failier,malnutrition,left and right bronchal fistula,respectively.Conclusion There are 4 types of fistula:Ⅰ type of sepsis,Ⅱ type of respiratory failure,Ⅲ type of thoracic infection,Ⅳ type of neck inection.Reoperation within 24 hours is the key of suscessful repairment of intrathoracic anastmotic leakage.Bronchal fistula or respiratory failure caused by multiple thoracic encapsulated effusion is also the indication of surgery.Complete lung reexpansion and complete drainage is the key.Mini-invasive VATS is the method of exact drainage.
10.Study on the nutrition status and clinical evaluation in patients with GOLD Ⅱ and GOLD Ⅲ or chronic obstructive pulmonary disease

Objective To investigate nutrition status and dyspnea in the patients with chronic obstructive pulmonary disease (COPD) between GOLD Ⅱ and GoLD Ⅲ, and test the evaluative validity of disease status by GOLD classification of COPD. Methods Thirty patients with clinically stable COPD were recruited, including 15 patients of GOLD Ⅱ of COPD and 15 patients of GOLD Ⅲ of COPD. Body mass index (BMI), triceps skin-fold thickness (TSF), serum albumin (Alb), and partial pressure of oxygen in arterial blood (PaO2) were measured in each patient. Dyspnea was assessed by the Borg Scale (BS). Exercise stress test was taken by incremental exercise test. Results BMI was significantly lower in the patients of GOLD Ⅲ than that in the patients of GOLD Ⅱ[(19±5 ) kg/m2 vs (23±3) kg/m2,p < 0.05]. TSF was significantly reduced in the patients of GOLD Ⅲ than that in the patients of GOLD Ⅱ[ (8±3) mm vs(13±5) mm, P < 0.01]. Alia in the patients of GOLD Ⅲ was significantly decreased than that in the patients of GOLD Ⅱ [(32±7) g/L vs (36±6) g/L, P <0.05]. The difference of PaO2 between the patients of GOLD Ⅲ and the patients of GOLD Ⅱ was significant [(72±9) nun Hg (1 mm Hg = 0.133 kPa ) vs (78±8) nun Hg, P < 0.01], and the significant difference of BS was found between the patients of GOLD Ⅲ and the patients of GOLD Ⅱ( 5.0±2.0) grades vs (3.0±1.0) grades, P <0.05 ]. In addition, 12 patients in the patients of GOLD Ⅱ took the exercise stress test and 8 patients were found anaerobic threshold (AT), 5 patients in the patients of GOLD Ⅲ took the exercise stress teat and no AT was found. Conclusions The parameters of BMI, TSF, Alb and PaO2 are significantly reduced in the patients of GOLD Ⅲ than those in the patients of GOLD Ⅱ. In the patients of GOLD Ⅲ, BS is higher than that in the patients of GOLD Ⅱ and AT is difficult to obtain, suggesting more severe in degree of impairment. GOLD classification of COPD reflects the disease stares and prognosis in the patients with COPD, as a valuable parameter in clinical practice.
