1.The anal verge: localization with multi-slice spiral CT

Objective To determine and evaluate the method of localization of anal verge by mutislice spiral CT. To provide an imaging reference for operative guidance of low-rectal cancer. Methods Forty eight consecutive adult patients suspected of abnormalities other than rectal disease were evaluated with abdominal and pelvic CT scans since August, 2009. They were divided into two groups based on sex and age. There were 23 men and 25 women. The ages of young group were 28 to 50 years and the average age was 41 years. The ages of elderly group were 52 to 81 years and the average age was 64 years. A small cotton ball dipped with contrast media was put at the anal verge as a marker and CT scans were performed with 64-slice spiral CT scanner. The distances between the cotton balls and the lower margin of the pubis combination (La), the lower margin of the 5th sacral vertebra (Lb), the inferior aperture of minor pelvis(Lc) and the lower margin of the basement of external anal sphincter (Ld) were measured on the midsagittal images obtained by MPR. The averages, the standard deviations(s), the 95% and 80% confidence intervals of La, Lb, Lc and Ld were calculated. We took the intervals of ± 1.96 s or ± 1.28 s < 5 mm as the standard of meeting the clinical requirement. The distances meeting the clinical requirement between different sex groups or age groups were compared with t test. Results The Ld was (9.8 ± 1.2) mm, ofwhich intervals of ± 1.96 s and ± 1.28 s were 4. 8 mm and 3.0 mm respectively, meeting the clinical need.This distance was also measured conveniently. There were no significant differences ( P > 0.05 ) between two different sex groups [ male group, ( 10.0 ± 1.2) mm], female group, (9.6 ± 1.2) mm and between two age groups [ young group, (9.6 ± 1.2) mm, elderly group, ( 9.9 ± 1.3 ) mm ]. Conclusions The lower margin of the basement of external anal sphincter was a useful anatomic lanmark for localizing the anal verge,and could be definitely identified on the middle sagittal pelvic CT image. The distance between the structure and anal verge is constant enough and can be used in measuring distance from low rectal lesion to the anal verge.

Objective To study the relationship between polyps of middle meatus and the position of the inferior border of the middle concha related to the semilunar hiatus, and to evaluate the diagnostic value of coronal CT scan of the nasal cavity after ephedrine spray by means of comparison of CT images and clinical findings Methods The coronal CT studys of 102 cases clinically suspected as sinusitis and/or nasal polyps were included in this study 1% ephedrine was sprayed in nasal cavity of the patients before CT study and the patient were asked to take supine position with head over extended over 5 minutes Axial and coronal CT scan was taken with supine position, 2 mm slice thickness and high resolution reconstruction Observation was focused on contraction of the nasal mucosa and soft tissue mass of the middle meatus (polyps) The middle concha was divided into 3 types according the position of its inferior border related to the semilunar hiatus: superior hiatus type (the inferior border of the middle concha superior to the semilunar hiatus), hiatus type (the inferior edge of the middle concha at the level of the semilunar hiatus), inferior hiatus type (the inferior edge of the middle concha inferior to the semilunar hiatus) Statistic comparing the occurrence of the middle meatus polyps with different middle conchas type and measuring the length of the middle conchas of inferior hiatus type from the level of the semilunar hiatus to the inferior border were performad Results Good and better contraction of the nasal mucosa with clear manifestation of the lesion was seen in 98 of 102 cases Nasal polyps of middle meatus were diagnosed in 71 sides of nasal cavities of 47 cases and confirmed surgically and/or clinically 39 superior hiatus and hiatus middle conchas were found ipsilateral to the polyps (54 9%) Of 204 sides of 102 cases, superior hiatus type middle conchas was found in 18 sides of nasal cavity with middle meatus polyps in 11 sides (61 1%); hiatus type in 38 sides with middle meatus polys in 28 sides (73 7%); and inferior hiatus type in 148 sides with middle meatus polys in 32 sides (21 6%) The difference was markedly significant statistically Conclusion Coronal CT scan after ephedrine spray into the nasal cavity can depict the mass in the nasal cavity better, especially nasal polyps with swollen mucosa The method is simple and easy to perform The poorly developed middle concha and the exposed middle meatus may be one of the causes of polyps of middle meatus It clues on that the middle concha should be preserved as much as possible during the endoscopic sinus surgery, opening the ethmoidal infundibulum and bullas, to decrease post operative recurrence of middle meatus polyps
3.A randomized comparison of clinical efficacy of patient controlled intravenous analgesia and routine analgesia for post-prostatectomy pain

Objective To assess the clinical efficacy of patient controlled intravenous analgesia (PCIA) with mixture of fentanyl and droperidol for post-prostatectomy pain. Methods 104 patients undergone prostatectomy were randomly divided into two groups. In group A, the patients received PCIA.In group B, they received routine analgesia.The pain scores of visual analogue pain scale (VAS) and control of bladder spasm were recorded at various time postoperatively, and the time of continuous infusion of bladder, the time of indwelling catheter and the mean hospitalization time were also recorded. 4.Effects of sevoflurane preconditioning on lung injury induced by renal ischemia-reperfusion in rats

Objective To evaluate the effects of sevoflurane preconditioning on lung injury induced by renal ischemia-reperfusion (I/R) in rats.Methods Forty adult male Wistar rats,aged 3 months,weighing 200-250 g,were randomly divided into 5 groups (n =8 each):sham operation group (group S); renal I/R group (group I/R); 1.1% sevoflurane preconditioning group (group SP1); 2.2% sevoflurane preconditioning group (group SP2); and 3.3% sevoflurane preconditioning group (group SP3).Renal I/R was induced by clamping bilateral renal pedicles for 45 min followed by 6 h reperfusion.In SP1-SP3 groups,1.1%,2.2% and 3.3% sevoflurane were inhaled for 1 h,respectively,before ischemia,followed by 10 min washout.The rats were sacrificed by exsanguination at 6 h of reperfusion,and lungs were removed for determination of wet/dry lung weight (W/D) ratio,myeloperoxidase (MPO) activity and expression of angiotensin converting enzyme (ACE) mRNA,and for microscope examination of pathologic changes which were scored.Results Compared with group S,the pathological score,W/D ratio,MPO activity and expression of ACE mRNA were significantly increased in I/R and SP1-SP3 groups (P < 0.05).Compared with group I/R,the pathological score,W/D ratio,MPO activity and expression of ACE mRNA were significantly decreased in SP1-SP3 groups (P < 0.05).Compared with SP1 and SP3 groups,the pathological score,W/D ratio,MPO activity and expression of ACE mRNA were significantly decreased in group SP2 (P < 0.05).Compared with group SP1,the W/D ratio was significantly decreased (P <0.05),and no significant changes were found in the other parameters in group SP3 (P > 0.05).Conclusion Sevoflurane preconditioning can attenuate lung injury induced by renal I/R and inhibition of ACE synthesis is involved in the mechanism.
5.THE STUDY OF NON-CYTOTOXIC CHEMOTHERAPY IN GASTRIC CANCER

objective To study the inhibition effect and its mechanism of a non - cytotoxic chemotherapy drug in the growth of gastric cancer. Methods The inhibition effect was observed in the growth of xenotransplanted gastric cancer cells MKN45 with a gastrin receptor(GR) antagonist. On the basis of the determination of expressed GR in 34 patients with gastric cancer, we evaluated the preliminary clinical effect of PGL to gastric cancer with the positive GR. Results It is less tumor area, weight and gastric cancer cells DNA Index, average DNA contents and S phase fraction in PGL group than in control group. It is the least in PGL + SST group. GR is easy to be expressed in advanced gastric cancer of the body, cardia and fundus. With the 16-21 months follow- up of 14 patients with positive GR gastric cancer, we find that 2 patients died in control group of 8 patients, and all patients are alive in PGL group. Conclusion PGL can regulate the growth of gastric cancer cells MKN45. It would be able to improve the prognosis of gastric cancer.
6.A clinical analysis of 69 cases of aldosterone-producing adenoma of the adrenal gland

Objective To study the diagnosis and treatment of aldosterone producing adenoma of the adrenal gland. Methods From January 1988 to January 2000,69 cases of aldosterone producing adenoma of the adrenal gland were diagnosed and treated.The diagnosis was all confirmed on open surgery and pathological studies. Results In all the patients,serum potassium level resumed to normal range in a week postoperatively.48 patients have been followed up for 3 months to 10 years,in 45 of them the blood pressure dropped to normal.In the other 3, drugs are still needed to control blood pressure after operation. Conclusions Spironolactonum test and serum aldosterone assay were the main methods of qualitative diagnosis of aldosterone producing adenoma.The location of the growth was mainly assessed on B ultrasono graphy and CT, the diagnostic accuracy being 81.2 percent and 100 percent respectively.Operation was the effective treatment method for aldosterone producing adenoma.
7.Efficiency evaluation of county-level public hospital based on data envelopment analysis

Objective To explore operating efficiency of county-level public hospital by using data envelopment analysis ,provide reference ,and basis for health decision-making .Methods The health resources data of 16 county-level public hospitals were ran-domly selected from 2008 to 2011 ,its relative efficiency were studied by data envelopment analysis method of CCR-CRS and BCC-VRS model ,the change trend from 2008 to 2011 were analyzed through the Malmquist productivity index .Results The average ef-ficiency value was 0 .938 in 16 county-level hospitals in 2011 ,7 hospitals was DEA efficient and 9 hospitals was non-DEA efficient (8 hospitals with Scale efficiency decline gradually ,another one with increasing gradually ) .The total factor productivity increased 2 .5% from 2008 to 2011 average annual ,the productivity first decreased from 2008 to 2009 ,and then increased from 2009 to 2011 . Conclusion The overall efficiency 16 county-level hospitals is not high ,it is necessary to develope hospital scale reasonable , strength hospitals′management level and improve technical ability of doctors .
8.Frequencies distribution of 5-HT2A T102C genotypes in the schizophrenia patients

Objective To investigate the geographic distribution of 5-HT2A T102C genotypes frequency in the PLA schizophrenia patients. Methods The 5-HT2A receptor gene T102C polymorphism was analyzed for genotyping by the method of polymerase chain reaction(PCR)in 177 male PLA schizophrenia patients. Results The distribution of 5-HT2A genotypic frequencies in 177 male PLA schizophrenia patients were 0.07~0.33(? 2=4.44,P=0.617 1) for A1A1, 0.50~0.72 for A1A2 (? 2=1.14, P=0.942 2), and 0.17~0.29 for A2A2 (? 2=0.93, P=0.985 7), respectively. There was no significant correlation between 5-HT2A frequencies and different populations. Conclusion The frequencies of 5-HT2A T102C genotypes in the PLA schizophrenia patients were uniform in different populations.
9.Analysis for risk factors of type 2 diabetes mellitus based on FP-growth algorithm

Objective:We do it to solve the problem of low efficiency in analyzing risk factors of type 2 diabetes mellitus by Apriori Algorithm.Methods: We used the patients’ data from the information department of one tertiary referral hospital in Lanzhou which include course note of disease and their health record form January 2009 to March 2014.We found out that the FP-growth algorithm analyzes risk factors of type 2 diabetes better. And we analyzed the efficiency by programming FP-growth and Apriori algorithm with C#.Results: We can analyze the run time and recorded data, time and support degree.Conclusion:The FP-growth algorithm has a higher efficiency in analyzing risk factors of type 2 diabetes mellitus.
10.The effects of group sandplay therapy on emotional status and coping style in patients with gastric cancer undergoing chemotherapy

Objective:To explore the application of group sandbox game intervention on emotional state and coping style in patients with gastric cancer.Methods:A total of 92 patients with gastric cancer treated with chemotherapy in the First People′s Hospital of Lianyungang from March 2018 to March 2019 were selected by convenient sampling method, they were divided into two groups according to admission time, 46 patients in the control group received conventional care after chemotherapy, and 46 patients in the observation group received group sandplay therapy for three months which was performed after chemotherapy. The Distress Thermometer(DT), Hamilton Depression Scale (HAMD), Hamilton Anxiety Scale (HAMA), Medical Copingmodes Questionnaire (MCMQ) were used to assess the emotional status and coping style of the patients before and after intervention.Results:After intervention, the DT, HAMA and HAMD scores of the observation group were 2.26±2.09, 13.52±2.50 and 16.47±3.61 respectively, and the DT, HAMA and HAMD scores of the control group were 3.36±2.23 and 20.85±4.53, 23.63±4.82, respectively. The DT, HAMA, HAMD scores in observation group were significantly lower than those in the control group, the difference between the two groups was statistically significant ( t value was 2.441, 9.608, 8.064, P<0.05). The dimensional score of the MCMQ measurement surface of the observation group after intervention was 18.35±3.19, and 14.25±3.04 in the control group. MCMQ scale escape and yield dimension scores in the observation group were 15.26±3.14, 10.24±2.56, and 17.81±2.59 and 12.84±2.64 in the control group. After the intervention, the observation group′s MCMQ surface score was higher than the control group, and the escape and yield dimension score was lower than that of the control group ( t value was 6.311, 4.249, 4.795, P<0.05). Conclusions:Group sandplay therapy can relieve negative emotion in patients with gastric cancer undergoing chemotherapy, and improve their coping style.
