1.The primary study of postmortem interval estimation by the lipid in puparium cases of Diptera:Calliphoridae

Objective To explore the value of lipidic degradation in puparium cases of C.megacephala during weathering in postmortem interval estimation. Methods The puparium cases of reared C.megacephala were collected, and laid outdoor. They were taken back 5 days and 10 days later, and then be stored at -20℃. The samples of control group were cleaned and cryopreserved immediately. The spectra were collected and preprocessed. The peak position and peak height were read and performed statistical analysis by SPSS 19.0 software. Results Compared to control group, the positions of Vas CH3 band showed no shift, and the Vas CH2 band showed blue shift, meanwhile, the Vs CH2 band disappeared in both experimental groups, moreover, the Vs CH3 band showed red shift only in 10d group;except of the Vs CH3 band in 5d group, the intensities of both the other two lipidic bands reduced in both experimental groups. Compared with 5day group, the Vs CH3 band showed red shift in 10d group, meanwhile, the differences of all the lipidic bands had statistical signiifcance. Conclusion Detection of lipidic degradation in puparium cases of C. megacephala during weathering by micro-FTIR provides a novel way to estimate postmortem interval and performs its potential for forensic applications.
2.The forensic significance of characteristics of the protein degradation in puparium cases ofChrysomyia megacephala

Objective To observe the protein degredation of empty puparium cases ofC. megacephala after weathering and to explore its practical significance on postmortem interval estimation in forensic science. MethodsThe standardized feeding was used inC. megacephala, and the empty puparium cases were collected and put into the forest. They were taken back 5 days, and 10 days later, respectively. The spectra were collected and preprocessed, and then the absorption peaks of amides were read, and curve fitting was performed in the average spectra of each group. The statistical analysis was performed by SPSS 19.0.Results Compared to control group, ifve days post weathering, the position of amideⅠabsorption peak showed blue shift, but the amideⅡabsorption peak showed no shift, moreover, none of the amide absorption peaks showed changes inpeak intensity. For the secondary structures, α-helix decreased and β-sheet increased slightly, however, β-turn did not change; Ten days post weathering, the positions of both amideⅠandⅡabsorption peaks showed blue shift, and an obviousshoulder peak appeared in amide I absorption peak. Meanwhile, both of amideⅠandⅡabsorption peaks showed decreased in peak intensity. Moreover, for the secondary structures, α-helix and β-sheet showed the same tendency as in the 5th day group, except for the β-turn increased dramaticlly.Conclusion The spectra of the empty puparium cases of C. megacephala showed that the absorption peaks of amideⅠandⅡpresented certain characteristic features within ten days post weathering, and estimate relative long-time postmortem interval.
3. Relationship between glycosylated hemoglobin and hemorrhagic transformation after cerebral infarction

Objective:
To assess the relationship between hemoglobin A1c (HbA1c) and hemorrhagic cerebral infarction (HI) in patients with acute cerebral infarction.
Methods:
From January 2014 to June 2016, in the Lianyungang Hospital Affiliated to Xuzhou Medical University, 426 patients with acute anterior circulation infarction were included. The blood sugar status before stroke was expressed by HbA1c. HbA1c and fasting blood glucose (FBG) were measured on the second day after admission. The National Institutes of Health Stroke Scale (NIHSS) was used to assess the severity of neurological function at admission. The modified Rankin scale (mRS) was used to evaluate the prognosis at discharge. CT or MRI/SWI examination was performed to determine whether there was HT. Logistic regression was used to evaluate the risk factors for HT and short-term prognosis after cerebral infarction.
Results:
Of the 426 patients enrolled, 93 (21.8%) appeared HT, 60 (14.1%) had hemorrhagic cerebral infarction (HI) and 33 (7.7%) had parenchymal hemorrhage (PH). Multivariate analysis showed that HbA1c and infarct volume were independent predictor of HT. When patients were grouped according to fasting blood glucose (FBG<7.8 mmol/L or ≥ 7.8 mmol/L), the predictive effect of HbA1c on HT was found in both groups. 4.Correlation between blood pressure variability and cognitive impairment in patients with acute ischemic stroke

ObjectiveToinvestigatethecorrelationbetweenbloodpressurevariabilityandcognitive impairment in ischemic stroke. Methods The inpatients with acute ischemic stroke were enroled. The demographic and clinical data were colected. The coefficient of variation of blood pressure within 7 days after onset w as calculated. Montreal Cognitive Assessment w as used to evaluate the cognitive function at three month after onset. Multivariate logistic regression analysis w as used to identify the relationship betw een the coefficient of variation of blood pressure w ithin 7 days and the cognitive impairment at 3 months after onset. Results A total of 708 patients w ith acute ischemic stroke w ere enrol ed in the study. At 3-month folow-up, 510 patients (72.0%) had cognitive impairment and 198 (28.0%) had normal cognitive function. The coefficient of variation for systolic blood pressure ( 8.3 ±1.2 vs.8.7 ±1.4; t= -3.299, P=0.001) and coefficient of variation for diastolic blood pressure ( 7.8 ±1.3 vs.8.0 ±1.5; t= -2.529, P=0.012) in the cognitive impairment group w ere significantly higher than those in the normal cognitive function group. With the first quintile as a reference, after adjusting other confounding factors, multivariate logistic regression analysis show ed that cognitive impairment at 3 months after onset w as significantly associated w ith coefficient of variation for systolic blood pressure. The odds ratios and 95 % confidence intervals for the 2-5 quantile groups w ere 2.33 (1.18-4.6), 2.31 (1.15-4.66), 2.70 (1.29-5.65), and 4.82 (1.92-12.1), respectively ( al P<0.05 ). Conclusion Systolic blood pressure variability in the acute phase of ischemic stroke is associated w ith cognitive impairment.
5.Dual metabolic platform to analyze differential metabolites in hepatitis B virus-related liver cirrhosis patients with pre-sarcopenia

Objective:This study aimed to analyze differential metabolites in patients using a dual metabolic platform and to orientate early nutritional intervention in patients with cirrhosis.Methods:The skeletal muscle index (SMI) was calculated based on computed tomography (CT) measurements of skeletal muscle cross-sectional area at the third lumbar vertebra level. Pre-sarcopenia was diagnosed for males with SMI < 46.96 and for females with SMI < 32.46. Fifteen HBV-related liver cirrhosis patients with pre-sarcopenia were included as Group S while fourteen liver cirrhosis without pre-sarcopenia were Group NS. Liquid chromatography-mass spectrometry (LC-MS) and gas chromatography-mass spectrometry (GC-MS) analyses were used to detect differential metabolites and disturbed pathways in the two groups.Results:Five pathways and twenty-eight pathways were defined as disturbed pathways in the plasma of liver cirrhosis patients with pre-sarcopenia by LC-MS and GC-MS, respectively. Most of these pathways are related to amino acid metabolism. Forty-two differential metabolites were imported into the disturbed pathways. Moreover, 3-hydroxypropanal, hydrocinnamic acid, betaine aldehyde, phosphohydroxypyruvic acid, (r)-3-hydroxybutyric acid, and creatinine were identified as potential biomarkers for pre-sarcopenia in HBV-related liver cirrhosis.Conclusions:The study identified a total of 33 pathways and related differential metabolites that were disturbed in HBV-related liver cirrhosis with pre-sarcopenia. The amino acid metabolism, urea cycle, and glyoxylate and dicarboxylate metabolism pathways may be associated with pre-sarcopenia in patients with HBV-related liver cirrhosis. These results provide a direction for nutritional supplementation in liver cirrhosis.
6.The expression and significance of Glypican-3 in Budd-Chiari syndrome complicated with hepatocellular carcinoma

Objective To study the expression and significance of Glypican-3 in Budd-Chiari syndrome (BCS) complicated with hepatocellular carcinoma (HCC).Methods The data of 46 patients with BCS complicated with HCC (the BCS + HCC group) treated in The First Affiliated Hospital of Zhengzhou University from January 2007 to December 2016 were analyzed retrospectively.Another 48 patients with HBV-related HCC (the HBV + HCC group) and 43 patients with hepatic cyst (the hepatic cyst group) were randomly selected as the control groups during the same time period.The differencesin positive rates of Glypican-3 in the liver tissues among the three groups were compared.The BCS + HCC group was further divided into the Glypican-3 positive and Glypican-3 negative subgroups according to the expression of Glypican-3.The differences in gender,age,AFP,HbsAg,Child-Pugh classification,tumor number,extrahepatic metastasis,vascular invasion,Edmondson-Steiner grading and BCLC staging between the two subgroups were compared.The survival time of the two subgroups was compared using the Kaplan-Meier method.Results The expression rates of Glypican-3 in the BCS + HCC group,HBV + HCC group and Hepatic Cyst group were 76.1%,70.8% and 0%,respectively.The levels of Glypican-3 in the BCS + HCC group and the HCC group were significantly higher than that in the hepatic cyst group.The differences were statistically significant (P < 0.05).No statistically significant difference was detected between the BCS + HCC group and the HBV + HCC group (P > 0.05).In the group of patients with BCS + HCC,there was no significant difference in gender,age,AFP,HbsAg,Child-Pugh classification,tumor number and extrahepatic metastasis between the Glypican-3 positive and negative subgroups (P >0.05).However,vascular invasion,Edmondson-Steiner grading and BCLC staging in the Glypican-3 positive subgroup were significantly higher than those in the Glypican-3 negative group,(P < 0.05).The 1-year,3-year and 5-year survival rates were 77.1%,51.0% and 22.8% in the Glypican-3 positive subgroup,compared with 90.9%,63.6% and 45.5% in the Glypican-3 negative subgroup,respectively.There were statistically significant differences between the two groups (P < 0.05).Conclusion Glypican-3 has a stable expression in patients with BCS complicated with HCC,and it is closely related to malignancy of the tumor and prognosis of the patients.

"The Expert Group on Tumor Ablation Therapy of Chinese Medical Doctor Association, The Tumor Ablation Committee of Chinese College of Interventionalists, The Society of Tumor Ablation Therapy of Chinese Anti-Cancer Association and The Ablation Expert Committee of the Chinese Society of Clinical Oncology" have organized multidisciplinary experts to formulate the consensus for thermal ablation of pulmonary subsolid nodules or ground-glass nodule (GGN). The expert consensus reviews current literatures and provides clinical practices for thermal ablation of GGN. The main contents include: (1) clinical evaluation of GGN, (2) procedures, indications, contraindications, outcomes evaluation and related complications of thermal ablation for GGN and (3) future development directions.
