Objectives To inspect the levels of peripheral blood leukocyte and lymphocyte functionassociated antigen-1(LFA-1)expression in renaI allograft recipients before and after renal transplantation.Methods Using monoclonal antibody-flow cytometer fluorescence technique,the expression of LFA-1 in lymphocytes of 33 patients with renal transplantation was detected at 1 day before operation and 3,6,9,15 days after operation by flow cytometric immunological technique,Results The level of LFA-1 expression after transplantation was significantly decreased and maintained at a low level.and ascended suddenly during the course of graft rejection.The level of LFA-1 expression in rejection was obviously higher than in non-rejection and after the eontrollring of rejection.Conclusions LFA-1 might Dlay an important role in graft rejection.Monitoring of the expression of LFA-1 is helpful for the diagnosis of the graft rejection.

To investigate the correlation between activation of B lymphocytes and CD11a, CD54 expression in renal allograft recipients, the dynamic changes in CD19, CD11a and CD54 levels in lymphocytes at different time points were examined by using mAb-FACS-immunofluores-cence technique in 10 renal allograft recipients in stable course(stable group) and 10 with acute rejection(rejection group) after operation. The expression levels of CD19 and CD54 in lymphocytes were significant higher than those of the pre-operation levels in both stable group and rejection group on the 3rd day after kidney transplantation, and the CD19 expression level was significant higher before rejection in the rejection group than that in stable group at the same time points; The expression of CDlla increased from (31. 9 ?12. 4)% to (49. 5 ? 20. 2)% during rejection and decreased significantly after anti-rejection treatment. Objective To investigate the association of age,precipitating factors and seasons of symptom onset of urinary stones with patients who have or have no family history. Methods The questionaires were used to survey 360 urinary stone cases with family history,1200 cases without family history and 1000 normal controls in Chongqing Area. Results In the 360 patients with family history,the morbidity rates of the first degree relatives and second degree relatives were 10.71%(147/1372) and 1.68%(54/3214), respectively.But in those without family history,the morbidity rates of the first degree relatives and second degree relatives was 0.66%(23/3480) and 0.12%(9/7345),respectively.Among the patients with family history,the mean age of morbidity for the males (26.35 years) was younger than that of the females (36.4 years, t=1.75,P 0.05).There was no significant difference of the precipitating factors and symptom onset seasons between patients with or without family history,while the enviropmental factors had significant effects on the morbidity of urinary stone patients,without regard to family history(? 2=0.23, P
5.The value of soluble triggering receptor expressed on myeloid cells-1 in the diagnosis of bacterial pleural effusion

Objective To investigate the value of soluble triggering receptor expressed on myeloid cells-1(sTREM-1) in the diagnosis of bacterial pleural effusion. Methods The levels of sTREM-1 in pleural effusion were determined in 30 patients with bacterial pleural effusion, 33 patients with malignant pleural effusion,31 patients with tuberculous pleural effusion and 28 patients with transudate by quantitative ELISA assay. The levels of CRP in pleural effusion were assayed using immunonephelometry method. The diagnostic value was assessed by receiver operating characteristic ROC curve analysis. Results The levels of sTREM-1[(1255.2±248.6)ng/L] in bacterial pleural effusion were significantly higher than those in malignant [(125.6±22.4)ng/L], tuberculous[(184.5±36.5)ng/L] and transudate groups[(92.5±20.8)ng/L] (P<0.05). For ROC curve, when the cut off value of sTREM-1 was set at 425 ng/L, the sensitivity , specificity ,accuracy, positive predictive value and negative value was 93.3%,90.3%,91.9%,93.2% and 93.3%, respectively. Conclusions Detection of sTREM-1 in pleural effusion is helpful to differentiate pleural effusion of bacterial origin from those with other etiologies.
6.Preventing inflammatory reaction with estrogen after cerebral ischemia/reperfusion injury

BACKGROUND:Leukocytic infiltration induced by release of inflammatory cytotkines and up-regulation of adhesion molecules is closely associated with the formation of cerebral infarcted focus. The related factors have been widely studied.OBJECTIVE: To explore influence of estrogen on inflammatory reaction in rats after focal ischemia/reperfusion injury.DESIGN: Randomized controlled experiment.SETTING: Nanjing General Hospital, Nanjing Military Area Command of Chinese PLA.MATERIALS:The experiment was performed at the Department of Neurology, Department of Pathology, Nanjing General Hospital, Nanjing Military Area Command of Chinese PLA. Adult male SD rats were selected to establish cerebral ischemic models, with the body mass of 280-350 g.METHODS:The rats were assigned into 3 groups: control group,ovariectomized group and estrogen treatment group (estradiol, 200 μg/kg,subcutaneous injection, once a week for 4 weeks). Four weeks later, models with right middle cerebral artery occlusion (MCAO) for 1 hour and 2 hours as well as reperfusion for 0, 1, 3, 6, 22 and 70 hours were established with thread embolism method. Mean number of infiltrative neutrophils in brain tissue was calculated under microscope with 10 high power fields in ischemic hemisphere with hematoxylin and eosin staining. Expression of nuclear factor-κB was determined with immunohistochemical method.MAIN OUTCOME MEASURES:Infiltration of neutrophils and expression of nuclear factor-κB in brain parenchyma.RESULTS: ①Expression of nuclear factor-κB: There was expression of nuclear factor-κB in the ovariectomized group at hour 1 after ischemia.Positive cells appeared at hour 2 after ischemia in the control group and estrogen treatment group. The expression was in the peak at ischemia for 2hours and reperfusion for 3 hours in the three groups, and decreased gradually. There was slight expression at reperfusion for 70 hours in the ovariectomized group, while there was no clear factor-κB positive cell at reperfusion for 22 hours in the control group and estrogen treatment group.②Infiltrative neutrophils in cerebral ischemic region in the ovariectomized group significantly increased at ischemia for 2 hours and reperfusion for 22 hours. Compared with the estrogen treatment group, there was significant difference (P=0.045). At ischemia for 2 hours and reperfusion for 70 hours,infiltrative neutrophils in the ovariectomized group were more than those in the control group and estrogen treatment group, but there were significant differences only between ovariectomized group and control group.CONCLUSION: Estrogen can inhibit inflammatory reaction after cerebral ischemia/reperfusion injury.
7.Clinical Study of Six-hole Moxibustion Box Therapy for Stable Chronic Obstructive Pulmonary Diseases

Objective To investigate the clinical efficacy and safety of six-hole moxibustion box therapy for stable chronic obstructive pulmonary diseases(COPD).Methods Sixty patients with thoracic facet joint disorder were randomly allocated to treatment and control groups, 30 cases each. The control group inhaled tiotropium bromide inhalation powder spray and the treatment group received six-hole moxibustion box therapy in addition. The COPD Assessment Test (CAT) score and the dyspnea score were recorded and peripheral blood oxygen saturation was measured in the two groups before and after treatment. The adverse reactions were monitored in the two groups. Results There were statistically significant pre-/post-treatment differences in the CAT score and the dyspnea score in the two groups (P<0.05). There were statistically significant post-treatment differences in the CAT score and the dyspnea score between the treatment and control groups (P<0.05). There were statistically significant differences in pre-/post- treatment CAT score difference value and dyspnea score difference value between the two groups (P<0.05). There was a statistically significant pre-/post-treatment difference in peripheral blood oxygen saturation in the treatment group (P<0.05). There was a statistically significant post-treatment difference in peripheral blood oxygen saturation between the two groups (P<0.05).Conclusion Six-hole moxibustion box therapy plus tiotropium bromide inhalation powder spray is safe and effective in treating stable COPD.
8.The toxic effect observation of the retina after vitreous chamber injected with PDGFR-α ASODN

Objective To explore the toxic effect of platelet-derived growth factor receptor (PDGFR)-αantisense oligonucleotide (αASODN) on the retina. Methods Twenty-four healthy adult colored rabbits were selected and randomly divided into four groups in six for each group. Intravitreous injections of 0.1ml different density diluents containing PDGFR-αASODN and liposome were performed in the right eyes in 3 groups. The other group was injected with 0.1 mL balanced salt solution (BSS) as the control group. The left eyes of all animals were not rejected. Slit lamp examination, indirect ophthalmoscopy and electroretinogram (ERG) examination were performed at 1, 7, 14 and 28 days after the injection. On the day 28, the right eyes were harvested, and HE、immunohistochemistry and transmission electron microscopy of retinal tissue were performed . Results The slit lamp, indirect ophthalmoscope examination of all groups were normal in each time. ERG examination yielded no difference in the amplitude of b wave between the treated groups and the normal control group. Pathological changes of the retinal tissue were not observed in the examinations of HE and immunohistochemical at the day 28 after injection. Electron microscope observation of retinal photoreceptor cells in the group D showed the parts of gaps between membranous discs were expanding, parts of were fusing, a few of clearances around cell nucleus were slightly enlarged,and the shapes of the cell nucleus were slightly irregular. Conclusions To inject 0.1 mL PDGFR-αASODN/lip2000 into the vitreous chamber, PDGFR-αASODN can be relatively safe when its concentration is less than 1.5μmol/L.
9.Experimental study of osteogenic differentiation of precartilaginous stem cells induced by calcitonin gene-re-lated peptide

Objective To investigate the effect of calcitonin gene-related peptide ( CGRP ) in inducing os-teogenic differentiation of rat precartilaginous stem cells in vitro and the underlying mechanisms. Methods Rat pre-cartilaginous stem cells ( PSCs) were cultured in complete osteogenesis medium containing DMEM/F-12 medium and different concentrations (0, 10-8,10-9,10-10mol/L) of CGRP, the morphology changes of PSCs were observed. The proliferation of PSCs was examined at different time points by CCK-8. All the PSCs were then randomly assigned to an experimental group and a control group. The PSCs in the experimental group were cultured in complete osteogenesis medium with 10-10 mol/L CGRP , while the control group cultured merely in complete osteogenesis medium was re-ceived no special intervention. Both groups were stained by Alizarin Red and the expression of alkaline phosphatase (ALP) was detected. The osteogenic genes (RUNX2,OPN and BGP) were measured by use of RT-PCR. The activa-tion of Wnt/β-catenin signaling pathway was tested by using Western blotting to evaluate the effect of CGRP . Results Compared to the control group ( the concentration of CGRP was 0 mol/L) , the concentration of ALP was significantly higher in the experimental group, calcium deposition was significantly more obvious, and the expression of the osteogenic genes such as RUNX2,OPN and BGP as well as theβ-catenin protein expression were up-regulated significantly. However, CGRP had no effect on cell proliferation. Conclusion CGRP activated Wnt/β-catenin sig-nal pathway and induced osteogenic differentiation of precartilaginous stem cells.
10.Analysis on spouse donor renal transplantation

Objective To analyze clinical outcomes of the spouse donor kidney transplantation for evaluating the security.Methods Sixty-three cases of living-renal transplantation were divided into two groups, spouse donor group (12 cases) and the other related donor group (53 cases). Twelve cases of spouse-donor(SD)renal transplantation were summarized, that were compared with the nospouse transplantation cases of the other living-related renal transplantation in the same period, which were similar in basic conditions and in immunosuppressant scheme. The observational parameters included average hospitalization time, rate of acute renal necrosis, acute rejection incidence within 1 year, serum creatinine at 7 d, 30 d and 1 year after transplantation.Results The recipient age of the 2 groups was (39±3)years and (37±3)years(P=0.05), dialysis time was (4.7±3.2)months and (4.4±2.9)months(P=0.78), the average hospitalization time was (20.9±8.3)d and (23.0±7.8)d(P=0.41). There was no significant difference between the spouse donor group and the no spouse related donor group. The acute rejection incidence within 1 year was 33.3%(4/12) in spouse group and 3. 9%(2/51) in the other related group, there was significant difference between the 2 groups(P<0.05). The rate of acute renal necrosis was 16.7%(2/12) in spouse group and 3.9%(2/15)in no spouse related group, there was no significant different between the 2 groups(P>0.05).SCr was (206.47±47.22)μmol/L and (163. 75±25.91)μmol/L in spouse group at post-operation 7 d and 30 d, and was (142.79±89.42)μmol/L and (119. 99±15.03)μmol/L in no spouse group. There was significant difference between the groups(P=0.02, P=0.00). One year after operation, SCr was (133. 40±6. 11)μmol/L in spouse group and (121. 00±34.12)μmol/L in no spouse group,there was no significant difference between the 2 groups (P=0.25).Conclusions Preoperative comprehensive assessment of the spouse donors and recipients renal transplantation is the guarantee for the success.Short-term outcomes of spouse donor renal transplantation is ideal, but rate of acute rejection within 1 year is higher than that of the other living-relative donor kidney transplantation, which dose not influence the long-time survival of spouse recipients.
