1.Clinical significance of peripheral blood leukocyte and lymphocyte function-associated antigen-1 expression in renal allograft recipients

Objectives To inspect the levels of peripheral blood leukocyte and lymphocyte functionassociated antigen-1(LFA-1)expression in renaI allograft recipients before and after renal transplantation.Methods Using monoclonal antibody-flow cytometer fluorescence technique,the expression of LFA-1 in lymphocytes of 33 patients with renal transplantation was detected at 1 day before operation and 3,6,9,15 days after operation by flow cytometric immunological technique,Results The level of LFA-1 expression after transplantation was significantly decreased and maintained at a low level.and ascended suddenly during the course of graft rejection.The level of LFA-1 expression in rejection was obviously higher than in non-rejection and after the eontrollring of rejection.Conclusions LFA-1 might Dlay an important role in graft rejection.Monitoring of the expression of LFA-1 is helpful for the diagnosis of the graft rejection.
2.Effect of facilitation techniques combined with acupuncture in patients with hemiplegia after stroke

ObjectiveTo study the effect of facilitation techniques combined with acupuncture on movement function and activity of daily living(ADL) of the stroke patients.Methods200 patients with stroke were divided randomly into 4 groups,group A for facilitation techniques with acupuncture,group B for facilitation techniques only,group C for acupuncture therapy only,and group D for control.The recovery grades of the Brunnstrom and Functional Independent Measure(FIM) were applied to assess movement function and ADL before and after treatment.ResultsThe number of patients whose limbs recoveried up to Brunnstrom 4 in group A、B、C after treatment was higher than that in group D as while as the scores of FIM(P<0.05).The FIM scores and the number of patients whose whose limbs recoveried grades(>4 grades) of the Brunnstrom in group B and C were no difference(P>0.05),The FIM scores and the number of patients whose recovery grades(>4 grades) of the Brunnstrom in group A were significant higher than that in group B and C (P<0. 05).ConclusionsFacilitation techniques combined with acupuncture can improve the movement function and ADL of the stroke patients.
3.Effects ofβ-elemene on radiosensitivity of Colo320 cells transplanted tumor nude mice model in vivo

Objective To explore the radiosensitivity ofβ-elemene on Colo320 cells transplanted tumor in nude mice and its molecular mechanisms.Methods Colo320 cells transplanted tumor model was established by cell suspension inoculation in nude mice.Then the transplanted mice with 0.8-1.0 cm3 tumor were randomly divided into 4 groups:control,β-elemene (40 mg/kg),radiation (4 Gy),andβ-elemene (40 mg/kg)+ radiation (4 Gy) groups,with four to five mice in each.Tumor weight and morphology were observed in each group.In addition,the apoptosis of tumor cells was measured by TUNEL and the expression of Fas gene was detected by immunohistochemistry.Results The nude mice transplanted tumor model was successfully established.Compared with that in control and simple radiation groups,tumor weight was significantly decreased inβ-elemene combined with irradiation group (P <0.05).At the same time,the apoptosis rate and the expression of Fas gene in tumor cells were significantly increased (P <0 .0 5 )inβ-elemene combined with irradiation group compared with control and simple radiation groups. Conclusion β-elemene could enhance the radiosensitivity of Colo320 cells transplanted tumor in nude mice probably by inducing Fas-mediated apoptosis of tumor cells.
4.Reconstruction of the hip joint stability with the dual-mobility acetabular cup

BACKGROUND: Patients with hip instability due to cerebral palsy, hemiplegia, infantile paralysis and extensive damage in gluteus medius, appear with high dislocation rate after arthroplasty, which is a great challenge for clinicians.OBJECTIVE: To investigate the reconstruction of the hip joint stability with the dual-mobility acetabular cup, and to prevent the dislocation after replacement in patients with hip neuromuscular lesions.METHODS: Twelve cases of hemiplegia, infantile paralysis, developmental dysplasia of the hip and recurrent dislocation after hip arthroplasty admitted in the Orthopedic Treatment Center, the Second Affiliated Hospital of Henan University of Chinese Medicine from January 2010 to July 2014 were enrolled, then underwent joint replacement or revision with dual-mobility cup, and the dynamic stability of the hip was achieved by adjusting the abductor lever arm.RESULTS AND CONCLUSION: (1) The followed-up time was from 20 to 60 months. (2) One year later, one case suffered Vancouver A right femoral fracture and received conservative treatment at 1 year postoperatively; one case of dislocation at postoperative 1 week, and dislocation, infection and loosening occurred in none cases. (3) These results manifest that those patients with neuromuscular disease and hip instability treated with hip joint arthroplasty using dual-mobility acetabular cup can reconstruct the stability of the hip joint and prevent the occurrence of postoperative dislocation.
5.Electrophysiological characteristics and catheter ablation left-sided accessory pathways of Mahaim fiber

ObjectiveTo study electrophysiological characteristics and catheter ablation left-sided accessory pathways of Mahaim fiber.MethodsTwenty-eight patients underwent catheter ablation for Mabaim fiber and 3 patients was left-sided accessory pathways.All of them were male with 34,20,38 years old.The tachycardia history was1-5years. Electrophysiological characteristics,target sitesand electrocardiogram(ECG,12 leads) were recorded through catheter from coronary sinus(CS),high right atrium(HRA),His bundle and ablation.ResultsOne patient's ECG showed little preexcitation,the other two were normal. Atrial stimulation could be easily induced tachycardia.Their ECG showed wide QRS waves (right bundle branch block or similar right bundle branch block).The characteristics of accessory pathways were slow and anterograde decremental conduction as well as blocked by adenosine triphosphate injection.Ventricular-atrial conduction was via atrioventricular node with centripetal decreasing transmission during ventricular pacing.During tachycardia,the His bundle electrogram resulted in a V-H-A pattern,the retrograde A in His bundle was pioneer.The atriofascicular pathways of 2 patients were completely ablated respectively in left posterior lateral and before the free wall of mitral valve ring.The nodoventricular pathway of 1 patient was successfully ablated in left middle posterior lateral of mitral valve ring.ConclusionsThe left-sided accessory pathways of Mahaim fiber is an uncommon accessory pathway,which has specific electrophysiological characteristics.It is effective and safe to ablate accessory pathway with radiofrequency.
6.EFFECTS OF HCY ON THE EXPRESSION OF HCY-2 AND THE PROLIFERATION IN HUMAN FETUS ARTERIAL SMOOTH MUSCLE CELLS

Objective To study the homocysteine induced gene(HCY-2) expression in human fetus arterial smooth muscle cells(hASMCs) in culture and explore the effects of homocysteine(HCY) and folic acid on HCY-2 expression and cell proliferation of hASMCs. Methods Immunohistochemistry ABC staining method was used to observe and analyze HCY-2 expression in hASMCs in culture.The image analysis system was used to research of hASMCs quantificationally.The effect of different HCY concentration on the proliferation of hASMCs was investigated by the cell counting. Results Immunoreactive substance of HCY-2 was chiefly found in cytoplasm of hASMCs.The expression of HCY-2 could be affected by HCY concentrations.There was a positive dose-dependent correlation with HCY concentrations in the culture medium.Folic acid increased the expression of HCY-2.The different concentration of HCY enhanced the proliferation of hASMCs,and this enhancement was maximal at the concentration of 1.25 mmol/L of HCY,while the proliferation was decreased when the concentration of HCY was over 1.25 mmol/L.Conclusion HCY increases the expression of HCY-2,and affects the proliferation of hASMCs.HCY is inhibited by folic acid.
7.Observation of macular microvascular changes in eyes of patients of type 2 diabetes without clinical ;features of diabetic retinopathy by optical coherence tomography angiography

Objective To observe the morphological changes of macular capillary in type 2 diabetic mellitus (DM) patients without clinical features of diabetic retinopathy (DR) by optical coherence tomography angiography (OCTA). Methods This is a prospective clinical case-control study. Forty-three eyes of 22 patients with DM without clinical features of DR (case group) and 40 control eyes of 20 age-and sex-matched healthy physical examination subjects (control group) were enrolled in this study. All subjects underwent OCTA examination with mode of retinal blood flow imaging, macular 3 mm×3 mm and 6 mm×6 mm area, signal strength>45. Foveal avascular zone (FAZ) area, foveal capillary density, parafovea capillary non-perfusion, and micro-aneurysm in shallow capillary vessel layer were evaluated. Results In case group, the mean FAZ area was (0.397±0.141) mm2 and the mean foveal capillary density was (44.6±0.62)%. In control group, the mean FAZ area was (0.253±0.112) mm2 and the mean foveal capillary density was (48.6±0.58)%. FAZ area of eyes in case group was larger than that in control group (t=1.017, P<0.05). There was no difference of foveal capillary density between two groups (t=1.499, P>0.05). The spider web-like FAZ and normal foveolar avascular zone were observed in eyes of control group. The parafovea capillary non-perfusion, abnormal foveolar avascular zone, micro-aneurysm and tortuosity of vessels were observed in eyes of case group. Parafovea capillary non-perfusion (χ2=4.542), micro-aneurysms (χ2=5.183) were seen more often in case group than control group (P<0.05). Conclusion Type 2 DM patients have abnormal retinal vascular microcirculation before DR using OCTA, including larger FAZ area, parafovea capillary non-perfusion, abnormal foveolar avascular zone, micro-aneurysm and tortuosity of vessels.
8.A comparative study of Utrecht interstitial applicator and ring interstitial applicator in three-dimensional conformal brachytherapy for cervical cancer

Objective To investigate the dosimetric differences between Utrecht applicator and ring applicator in three-dimensional (3D) conformal brachytherapy for locally advanced cervical cancer.Methods Twenty-five patients with locally advanced cervical cancer were treated with magnetic resonance imaging-guided 3D conformal brachytherapy.Utrecht applicator and ring applicator were used interchangeably for 96 cycles.Patients were divided into two groups according to the type of applicator.Each group received 48 cycles of treatment, in which ring applicator was first applied for 26 cycles and Utrecht applicator was first applied for 22 cycles.High-risk clinical target volume ( HR-CTV) , width, thickness, and D90 at the point A level, D2 cm3 of organs at risk (OARs), V7 Gy , W7 Gy,A, V7 Gy ,A, and W/T7 Gy were evaluated and analyzed using paired t-test.Results There were no significant differences in HR-CTV and the width, thickness, and D90 at the point A level between the Utrecht group and the ring group ( P=0.487;P=0.340;P=0.857;P=0.921);there were no significant differences in D2 cm3 values of bladder, rectum, sigmoid, and bowel between the two groups ( P=0.136;P=0.802;P=0.985;P=0.458);there were no significant differences in V7 Gy and T7 Gy,A between the two groups ( P=0.076;P=0.435) .The Utrecht group had a significantly larger W/T7 Gy,A than the ring group ( P=0.002 ) .Conclusions Utrecht applicator is appropriate for patients with relatively large width and width/thickness ratio of HR-CTV at the point A level.
9.DTI and DTT in evaluation of effect of childbearing history on female pelvic floor muscles

Objective To investigates the feasibility of diffusion tensor imaging (DTI) and diffusion tensor tracking (DTT) for evaluation of the effect of childbearing history on female pelvic floor muscles. Methods Forty-six healthy females were divided into two groups: nulliparous and primiparous. MR conventional sequences and DTI were acquired. The optimized FA threshold value was obtained by regulating the FA to fiber tracking. The two groups were compared in terms of ADC, FA, VRA and T2-WT. Results (1)The DTT of FA 0.18 got the highest score in fiber tracking . ( 2 ) The ADC of nulliparous subjects and the subjects who had given birth were (1.24 ± 0.11) ×10-3 mm2/s, (1.33 ± 0.11) ×10-3 mm2/s (P = 0.017). There were no statistical differences in FA, VRA and T2-WT between the two groups (P > 0.05). Conclusions The optimized FA threshold of fiber tracking in pelvic floor muscles is 0.18. DTI and DTT may be used to evaluate the effect of childbearing history on female pelvic floor muscles.
10.Correlation between ICRU reference point dose and dose-volume parameters of organs at risk in three-dimensional conformal brachytherapy for locally advanced cervical cancer

Objective To investigate the correlation between ICRU reference point dose and dosevolume parameters of organs at risk (OARs) under different bladder and rectal filling status in threedimensional conformal brachytherapy for locally advanced cervical cancer.Methods A total of 31 patients who received magnetic resonance imaging-guided three-dimensional conformal brachytherapy for cervical cancer in 96 fractions were enrolled.The ICRU rectal and bladder reference points were determined in the treatment planning system,and the doses at these points were recorded and compared with the dose-volume parameters of the rectum and bladder.The paired t-test was used to analyze the differences between them.Results Bladder DICRU was lower than bladder D0.1cm3 and D1 cm3 (P=0.000 and 0.000),higher than bladder D5 cm3 and D10cm3 (P=0.000 and 0.000),and similar to bladder D2 cm3 (P=0.345).Under the bladder filling status,bladder DICRU was lower than D2cm3.Rectal DICRU was lower than rectal D0.1 cm3 and D1cm3 (P =0.000 and 0.002),higher than rectal D5 cm3 and D 10 cm3 (P =0.000 and 0.000),and similar to rectal D2cm3 (P=0.058).The ICRU bladder and rectal reference point doses were positively correlated with corresponding D2 cm3.In the case of bladder volume ≥ 200 cm3,the ICRU bladder reference point dose underestimated bladder D2 cm3.In the case of rectal volume ≥ 37 cm3,the ICRU rectal reference point dose overestimated rectal D2 cm3.Conclusions In three-dimensional conformal brachytherapy,it is generally safe to use D2 cm3 as an index to evaluate OARs,but when the bladder or rectum is in an empty status,the ICRU bladder or rectal reference point doses should be considered.
