1.Improved ability of demonstrating ocular masses on 3.0 T MR scanner combined with an 8-channel eye surface phased array coil: a multi-center study

Objective:To investigate the value of the 8-channel eye surface phased array coil in improving image quality and demonstrating ocular masses on 3.0 T MR scanner.Methods:From July 2018 to January 2020, the data of orbital MRI in 692 patients with ocular masses on 6 medical centers were prospectively collected. The patients were simple randomly assigned into 8-channel eye surface phased array coil group (413 patients) or 8-channel head phased array coil group (279 patients), with the same MRI sequences. The signal to noise ratio (SNR) and contrast to noise ratio (CNR) were calculated in orbital anatomy structures and masses (eyelid mass, intraocular mass, lacrimal mass and orbital mass). The image quality scores including motion artifact, mass margin, the relationship between the mass and adjacent structures, and overall image quality were recorded. The differences of image quality between the two groups were compared by two independent sample t-test or Wilcoxon rank test. Results:The SNR and CNR were higher in eye surface coil group than those in head coil group ( P<0.05). The scores of ocular movement artifacts were higher in head coil group than those in surface coil group ( P<0.05). The scores of intraocular mass margin, the relationship between the mass and adjacent structures, and overall image quality were higher in surface coil group than those in head coil group ( P<0.001). There were no significant differences in mass margin, the relationship between the mass and adjacent structures, and overall image quality scores of eyelid, lacrimal gland, and orbital mass between the two groups ( P>0.05). Conclusion:3.0 T MR scanner combined with the 8-channel eye surface phased array coil can improve the SNR and CNR of orbital MR images, the demonstration of the intraocular mass margin and the relationship between the mass and adjacent structures.
2.CRISPR genotyping of Yersinia pestis in the plague natural foci of Qinghai-Tibet Plateau

Objective:To investigate the genotypes of clustered regularly interspaced short palindromic repeats (CRISPR) of Yersinia pestis ( Y. pestis) and it's regional distribution in the plague natural foci of Qinghai-Tibet Plateau. Methods:In this study, 1 004 strains of Y. pestis isolated from different hosts and vectors in different regions from 1954 to 2011 were selected as experimental objects, which preserved in Qinghai Institute for Endemic Disease Prevention and Control, and DNA of Y. pestis was extracted by traditional phenol-chloroform method. Three CRISPR loci (YPa, YPb and YPc) were amplified by PCR and sequenced, respectively, and the CRISPR sequences were compared with the CRISPRDictionary database recently reported in the literature to identify CRISPR spacer. For the spacer discovered for the first time at each CRISPR locus, Blast sequence alignment was performed in the National Center for Biotechnology Information (NCBI) database to speculate the source of gene sequence. Genotyping of Y. pestis from Qinghai-Tibet Plateau was performed based on the polymorphism of CRISPR spacer array. Results:A total of 53 spacers were found in 1 004 strains of Y. pestis, of which 6 spacers were newly discovered, namely a105, a106, a107, b51, b52 and c14. The 1 004 strains of Y. pestis were divided into 10 groups with 44 different CRISPR genotypes, and 15 new genotypes were found. The dominant genotypes of Y. pestis were G26-a1', G7, G22, G24-a1', G22-a1', G9 and G26-a1'a60 in Himalayan Marmot plague natural foci, and G37-a6' in Qinghai Microtinae plague natural foci. Conclusion:The CRISPR genotypes of Y. pestis in the plague natural foci of Qinghai-Tibet Plateau are high diverse and have significant regional distribution characteristics.
3.Improved ability of demonstrating ocular masses on 3.0 T MR scanner using PROPELLER: a multi-center study

Objective:To investigate the value of periodically rotated overlapping parallel lines with enhanced reconstruction (PROPELLER) in improving ability of demonstrating ocular masses on 3.0 T MR scanner.Methods:This study was a multi-center prospective study involving 6 centers. From July 2018 to January 2020, totally 413 patients with ocular masses from 6 centers were prospectively enrolled, and all of them underwent T 1WI and T 2WI, PROPELLER T 1 FLAIR and T 2WI, and contrast-enhanced scans. The signal intensity of eyelid, vitreous body, lacrimal gland, intraorbital segment of optic nerve, and orbital masses of eyelid, intraocular, lacrimal gland and retrobulbar were measured by two radiologists, and the signal to noise ratio (SNR) and contrast noise ratio (CNR) were calculated. The 5-point scoring method was used to evaluate the motion artefacts, tumor edges and the relationship between the tumor and adjacent structures, and the overall score of image quality was calculated. Paired t-test or Wilcoxon signed rank test was used to compare the image quality between PROPELLER and non-PROPELLER images. Results:The SNR and CNR of PROPELLER T 2WI were higher than those of non-PROPELLER T 2WI (all P<0.001). The SNR and CNR of PROPELLER T 1 FLAIR were lower than those of non-PROPELLER T 1WI (all P<0.05). The scores of artefacts and overall image quality in PROPELLER images were higher than those in non-PROPELLER images (all P<0.001). The tumor edge and the relationship between the tumor and adjacent structures scores of eyelid, intraocular, and lacrimal gland masses in PROPELLER images were higher than those in non-PROPELLER images (all P<0.001),while compared to non-PROPELLER images, retro-global masses in PROPELLER images showed no significant differences (all P>0.05). Conclusion:PROPELLER can reduce ocular motion artefacts, effectively improve image quality and ability of demonstrating anterior (eyelid, intraocular, and lacrimal gland) masses.
4.Potential inappropriate medication and its influencing factors among elderly hypertensive patients in community general practice clinic

Objective:To investigate the status quo of potentially inappropriate medication (PIM) among elderly hypertensive patients in community general practice clinic and related influencing factors.Methods:A total of 767 prescriptions for elderly hypertensive patients in a community health service center in Beijing from October 2020 to August 2021 were reviewed. The prevalence of PIM was assessed based on the criteria of potentially inappropriate medications for older adults in China(2017). Logistic regression analysis was employed to analyze the influencing factors for PIM. Results:The survey showed that 198 elderly patients had PIM with 244 person-doses. The top three drugs with high rate of PIM were benzodiazepine-estazolam (64 person-doses), clopidogrel (53 person-doses) and insulin (35 person-doses). Univariate analysis showed that PIM was significantly associated with types of medication;and underlying diseases hypertension with hyperlipidemia, coronary heart disease, type 2 diabetes, osteoarthritis, upper respiratory tract infection and insomnia (χ 2=82.58, 13.65, 17.74, 7.52, 10.34, 68.19,respectively,all P<0.05). Multivariate logistic regression analysis showed that the types of medication, hypertension complicated with upper respiratory tract infection, and insomnia were independent influencing factors for PIM ( OR=1.55, 2.47, 9.05; P<0.05). Conclusion:The study shows that PIM is more common in elderly hypertensive patients in community general practice clinics,which is associated with types of medication, hypertension complicated with upper respiratory tract infection and insomnia. It is suggested that general practitioner working in community clinics should be aware of PIM, minimize the number of prescription drugs, and choose new drugs or non-drug treatments to reduce the occurrence of PIM.
5.Effectiveness of TBL+ CBD method applied in the course teaching of Introduction to General Practice

Objective:To verify the application value of team-based learning (TBL) joint case-based discussion (CBD) method applied in the course teaching of Introduction to General Practice.Methods:Students from two clinical oriented classes in the same grade were divided into the experimental group ( n=35) and the control group ( n=35). The control group implemented the traditional teaching method, and the experimental group implemented the TBL joint CBD method. The teaching effect were compared between the two groups. SPSS 20.0 was used for frequency description and statistical test analysis. Non-normal distribution data were tested with Mann-Whitney U test. Results:After the teaching, the average rank of multiple-choice questions (35.28) and analytic questions (42.24) in the experimental group were significantly higher than those of the control group (32.68 and 25.52, respectively, P<0.05). Conclusion:The TBL joint CBD method applied in the course of general practice helps to improve students' test achievement, enhance their overall quality, and lay a foundation for training comprehensive competencies of general practitioners.
6. CT and MRI features of adult rhabdomyosarcoma of the extremities

Objective:
To investigate the CT and MRI features of adult rhabdomyosarcoma (RMS) of extremities.
Methods:
A retrospective analysis was carried out on CT and MRI findings of 10 patients with rhabdomyosarcoma of extremities confirmed by surgery and pathology in Wenzhou People′s Hospital and the Second Affiliated Hospital of Wenzhou Medical University. The site, size, shape, margin, density or signal, adjacent tissue changes and enhanced degree of the mass were observed.
Results:
In the 10 cases, the mass was located in the thigh in 6 cases, in the calf in 3 cases, and in the upper arm in 1 case. The maximum diameter of the tumor was 2.9-22.5 (12.0 ± 7.2) cm. The masses were oval in 2 cases and irregular in 8 cases. The margins of the masses were clear in 2 cases and unclear in 8 cases. There was invasive destruction of adjacent bone in 3 cases, intratumoral necrotic cystic changes in 8 cases, intratumoral hemorrhage in 2 case, and lymph node metastasis in 3 cases. Four patients underwent plain CT scan: CT presented isodensity in 1 case and slightly lower density in 3 cases. The mass density was uniform in 1 case and uneven in 3 cases. CT value of the solid part of the mass was 27-47 (42 ± 6) HU in average. Three patients underwent enhanced CT scan: all the enhanced masses showed uneven enhancement, including 1 case of mild enhancement and 2 cases of moderate enhancement, with no enhancement of necrotic cystic lesion area and clear in lesion boundary. Nine patients underwent plain MRI scan: mass of 9 cases presented iso-signal or slightly lower signal on T1WI, including 4 cases with iso-signal and 5 cases with slightly lower signal. There were 7 cases of uneven signals and 2 cases of uniform signals. The mass presented high signal on T2WI, including 8 cases with uneven signals and 1 case with uniform signals, and the necrotic cystic lesion showed higher signal on T2WI. Seven patients underwent enhanced MRI scan: all the enhanced masses showed uneven enhancement, including significant enhancement in 5 cases, moderate enhancement in 2 cases, no enhancement in necrotic cystic lesion area, and "perivascular" phenomenon in 4 cases.
Conclusions
CT or MRI examination shows that the soft tissue tumor of the extremities presents low density on CT, equal or slightly low T1WI signal on MRI, and high signal on T2WI, with necrotic cystic lesion inside peripheral blood vessel density in the tumor, without calcification and significantly or moderately uneven enhancement should consider the possibility of RMS should be considered.
7.Research progress on lncRNA related to osteosarcoma

Osteosarcoma is a clinically common primary malignant bone tumor.Its treatment is still not ideal because of the high de-gree of malignancy and early metastasis.Understanding the molecular mechanism and the biological characteristics of malignant os-teosarcoma,and exploring the effective treatment have become the focus of osteosarcoma research at home and abroad.A long non-coding RNA,lncRNA,is involved in the regulation of a variety of cellular functions and plays an important role in the development of osteosarcoma.Recently,lncRNA has been increasingly reported in osteosarcoma research.It has been found to affect the proliferation, invasion,and migration of osteosarcoma cells by interacting with proteins,mRNAs,and miRNAs.This article reviews the research on ln-cRNA based on recent reports regarding its role in the molecular mechanism of osteosarcoma.

The medical assistance to advanced schistosomiasis patients established by the Chinese government is a major public facility for patients with advanced schistosomiasis. Since the medical assistance to advance schistosomiasis patients in Hu?nan Province started ten years ago,a set of mature and operable programs with whole program management and related technolo?gies has been developed. The author investigated the data on medical assistance to advanced schistosomiasis patients in Hunan Province during the last 10 years(from 2006 to 2015)retrospectively,and found that the program had high therapeutic effect and high satisfaction degree of both patients and the society. In order to improve the management of the medical assistance to ad?vanced schistosomiasis patients and share our experiences of the whole program management and related technologies with the colleagues of other provinces,this paper mainly illustrates the experiences of the program,as well as the existing problems and related strategies.

This paper summarizes the clinical diagnosis and treatment of imported African schistosomiasis,in order to make the therapeutic standards. Imported African schistosomiasis includes mainly schistosomiasis haematobia and schistosomiasis mansoni in China. In order to set up the operational standards,enhance diagnostic and cure rates,and reduce the complica?tions,we review the related literature combined with our experience over years,and summarize,in this paper,the pathogenic mechanism,and key points of clinical diagnosis and treatment of schistosomiasis haematobia and schistosomiasis mansoni,so as to provide the reference for clinical doctors.
10.Accuracy and reliability of three-dimensional craniofacial cone-beam CT superimposition method based on voxel registration

Objective To set up a three-dimensional(3D) craniofacial cone-beam CT(CBCT)superimposition method based on voxel registration,and evaluate the accuracy and reliability of different reference superimposition areas.Methods CBCT scans were taken on a dry skull for three times with a time-interval of two weeks,and CBCT scans were taken on ten adult volunteers twice with a time-interval of one month.The 3D superimposition based on voxel registration was set up by means of Analyze 12.0.The bilateral-zygomatic bone and anterior cranial base were selected as different reference areas for voxel overlapping.The 3D overlapping images were output into Geomagic Qualify 2013,and the surface distances on different craniofacial regions were measured.Three operators repeated three times of these superimposition and measurement.SPSS 19.0 were used to analyze the accuracy and reliability.Results When bilateral-zygomatic bone used as superimposition reference,the mean surface distance range of dry skull were from 0.035 to 0.064 mm,and volunteers from 0.099 to 0.182 mm.When the anterior cranial base used as superimposition reference,the mean surface distance range of dry skull were from 0.038 to0.071 mm,and volunteers from 0.127 to 0.218 mm.All these mean values were less than the CBCT voxel size0.25 mm(P<0.05),and got sub-pixel precision.No significant difference was found between the different operators at different times(P>0.05).Conclusions This study showed the high accuracy and reliability of 3D CBCT superimposition based on voxel registration,either the anterior cranial base or the bilateral-zygomatic bone as reference.This method could be used for evaluating 3D craniofacial treatment effects.
