1.Therapeutic effect of DC-CIK cells combined with S-1 in the treatment of non-small-cell lung cancer

Objective To observe the clinical effect of DC-CIK cells combined with S-1 in the treatment of non-small-cell lung cancer.Methods 76 patients with non-small-cell lung cancer were randomly divided into the observation group and the control group on average.The control group was treated with S-1,and the observation group was treated with DC-CIK cells combined with S-1.For the observation group,the peripheral venous blood was collected before the treatment for DC cells and CIK cells cultivation.The expression of CD4+/CD8+,CD4+ and NK cells in the peripheral blood of the two groups was detected by flow cytometry before the treatment and after one week of the treatments.Besides,the number of white blood cells and platelet count were also measured and the symptoms of non-small-cell lung cancer were observed.Results The percentage of CD4+/CD8+,CD4+ and NK cells in the peripheral blood of the observation group after the treatment was (1.65±1.03),(34.56±8.90) and (18.68±7.98),respectively,which was significantly higher than (1.32±0.70),(29.07±7.15) and (15.28±8.23) in the control group (all P<0.05).There was no significant difference in the percentage of CD8+ cells between the observation group (25.56± 8.90) and the control group (26.64±6.77) (P>0.05).The vomiting,leukopenia and thrombocytopenia in the observation group were less than those in the control group (all P<0.05).The one-year survival rate was 52.63% in the observation group which was significant higher than 42.11% in the control group (P<0.05).Conclusions DC-CIK cells combined with S-1 is a safe and effective treatment for non-small-cell lung cancer,which can effectively improve the clinical efficacy and prolong the survival time of patients.
2.Laparoscopic Splenectomy Combined with Lower Oesophagus Transection in the Treatment of Hepatic Cirrhosis Complicated with Portal Hypertension

Objective To study the short-term prognosis of laparoscopic splenectomy combined with lower esophagus transection in the treatment for hepatic cirrhosis induced portal hypertension.Methods The clinical data of 7 cases of portal hypertension treated by laparoscopic eplenectomy combined with lower esophagus transaction were retrospectively analysisd.Results Among of the patients,six cases were undertaken with total laparoscopic technique and only one case done with hand-assisted laparoscopic operation and one case turned to open surgery.Average operation time was 330 min and operation hemorrhage was 780 ml.None of cases died,bled or occurred pancreatical fistula.Mean time of bowel function recovery(70 h) and postoperative hospitalization(12.7 d).1 case complicated with hydrothorax recovered after conservative therapy.Conclusion Cincial short-term effects of patients treated with laparoscopic splenectomy combined with lower esophagus transection for hepatic cirrhosis induced portal hypertension is satisfied,so it is a safe and feasible way for future clinical practice if conducted by the skilled and competent surgeon.
3.Experience of Transabdominal Gastroesophageal Stapling in Management of Variceal Bleeding in Portal Hypertension(Report of 48 cases)

Objective To investigate the effect of transabdominal gastroesophageal stapling in management of variceal bleeding in portal hypertension with Hassabi operation and sum up our clinical experience.Methods Retrospective analysis of 48 cases with portal hypertension operated by transabdominal gastroesophageal stapling between May,1998 and Oct.2005.The elimination of oesophagus venous、the rebleeding rate、the operation mortality、the stricture and the fistula rate and the index that influences the liver function were maily observed.Results No case suffered rebleeding and hepatal encephalopathy during the hospitaligation of the patients.Esophageal varices disappeared or reduced in all cases.One case resulted from stricture and one case died of multiple organ failure.Conclusions Transabdominal extensive oesophagogastric devascularization combined with gastrooesophageal stapling is an effective and safe procedure for control of acute variceal haemorrhage with satisfactory short-term control,but the long-term effect needs to be long follow-up for the patients.
4.The Expression of Secondary Lymphoid Tissue Chemokine(SLC)in Pancreatic Carcinoma and its Clinical Significance

Objective To Investigate the expression of secondary lymphoid tissue chemokine (SLC) and its relationship with angiogenisis,pathological feature and prognosis of pancreatic cancer (PC).Methods SLC mRNA expression and microvessel density(MVD) were detected in 22 cases with PC with immunohistochemistry and reverse transcription-polymerase chain reaction(RT-PCR) respectively and compared with its clinical features.Results Mean MVD of PC was 57?12 and enhanced expression of SLC mRNA was detected in 17 cases(77%),The positive rate of SLC mRNA was significantly lower in cases of without metastasis and at early clinical stage(stage Ⅰ+Ⅱ)than that with metastasis and at advanced stage(stage Ⅲ+Ⅳ).MVD was significant higher in SLC mRNA-enhanced PC than in SLC mRNA reduced PC(P

7.Analysis of High-risk Factors of Iatrogenic Duodenal Injuries and Surgical Tactics of Treatnent According to Injuries Scaling

Objective To sum up and discern the high risk factors of iatrogenic duodenl injuries and to evaluate the value of the treatment according to the new trauma scaling,so as to to facilitate and guide clinical research.Methods 21 patients with iatrogenic duodenal trauma hospitalized and From Jan.1985 to Oct.2005,in our hospital were collected and retrospectively reviewed,the duodenum organ iatrogenic injury scale was classified and developed depending on the clinical finding and referring to the injury scale by the American Association for the Surgery of Trauma(A.A.S.T.)in 1990.Results The iatrogenic duodenal injuries result from various surgery and endoscopic procedures,most from a sequelae to pertinent biliary operation and(or)iatrogenic technical errors of instrumention.High risk factors responsible for the iatrogenic injuries include inflammatory conglutination,biliary systems anomalies or anatomic variations,duodenal Vater's papilla with neoplasma or diverticulum,surgeohs with less experience or over self-confidence or some procedures with violence and specific correctable errors,and so on.According to the new grading scheme,the duodenal injuries of the 21 cases were classified as follows:grade I-5 cases,grade II-6 cases,grade III-5 cases,grade IV-3 cases,and grade V-2 cases.Among the patients,3 cases were treated by drainage.2 cases underwent simple repair.8 cases were treated with repair and drainage.8 cases underwent enterorrhaphy and drainage.Conclusions Knowledge about causal factors of such iatrogenic processes can play a crucial role in their prevention,correct diagnosis,and management.Having the iatrogenic duodenal scaling done well,appears to be an accurate and practical way to select an optimum and flexible treatment and improve the patients's prognosis.
8.Reduced expression of the CC chemokine SLC mRNA in gastric cancer and metastatic lymph nodes

Objective To investigate the mRNA expression of secondary lymphoid tissue chemokine (SLC) in human gastric carcinoma. Method The study was designed to investigate semi-quantitatively the expression of SLC mRNA in gastric carcinoma and metastatic lymph nodes by reverse transcription-polymerase chain reaction.Result The expression of SLC mRNA in the gastric cancer tissues was markedly reduced compared with that in adjacent noncancer tissues. The average tumor/normal (T/N) ratio determined by RT-PCR was 0.43?0.08 in 29 patients.Metastatic lymph node/normal ratio was 0.63?0.07.Negative lymph node/normal ratio was 1.19?0.11. As a control,the mean G3PDH T/N ratio was 1.16?0.06,there was no correlation between SLC mRNA expression and clinicopathological characteristics in gastric cancer.ConclusionSLC mRNA expression is suppressed in the gastric tumours. SLC may play an important role in the early stage of carcinogenesis.

Objective To evaluate the enhancement effect of SLC(secondary lymphoid tissue chemokine)gene mediated by a repli- cation deficient recombinant adenovirus(Ad)on human pancreatic cancer in vitro and vivo and to investigate the mechanism of action of SLC.Methods Human pancreatic carcinoma cell line ASPC-1 was infected with Ad-SLC and Ad-CFG,and compared with phosphate buffered saline(PBS).MTT(3,-[4,5-dimethyhhiazol-2-yl]-2,5-diphenyhetrazolium bromide)assay was used to estimate the proliferation of ASPC-1 cells;tube formation assay and choriallantoic membrane assay were used to evaluate angiogenesis in vivo and in vitro.Xenografted nude mice with pancreatic cancer were established to observe in vivo tumour growth suppression.Microvessel density re- vealed by CD34 immunohistochemical staining was measured and TIL in tumor region was also evaluated.Results Growth and tube forma- tion of ASPC-1 cells infected with Ad-SLC were suppressed significantly compared with cells infected with Ad-CFG or ceils treated with PBS.Neovascularisation in the Ad-SLC group was less than that in the PBS and Ad-CFG groups,based on chorioallantoic mem- brane results.Volumes of pancreatic tumours in the Ad-SLC group were significantly smaller than those in the PBS and Ad-CFG groups at the end of the treatment period.Microvessel density in the Ad-SLC group was significantly lower than that in the Ad-CFG and PBS groups and TILs in tumor region in the Ad-SLC group was significantly risen much more than that in the Ad-CFG and PBS groups when the mice transferd the same number of TILs from spleen of health mice.Conclusions The SLC gene mediated by adenovirus is efficient for gene therapy for pancreatic carcinoma.Suppression of SLC on proliferation of vascular endothelium cells,attracting TIL to tumor region and angiogenesis may account for its effect.
