1.Bioactivity of sol-gel derived apatite/wollastonite porous bioactive glass-ceramic

BACKGROUND: Apatite-wollastonite containing glass-ceramic (AWGC) is a kind of good bone repairing materials with excellent bioactivity, which is prepared by traditional melting process.OBJECTIVE: To observe AWGC prepared with sol-gel method and its bioactivity.DESIGN: Design experiment of materials process and in vitro bioactivity experiment.SETTING: College of materials science and Engineering of Sichuan University.MATERIALS: AWGC.METHODS: This experiment was conducted at the laboratory of College of Materials Science and Engineering of Sichuan University between August 2002 and May 2003. AWGC was prepared from sol-gel and followed by heattreating process. Bioactivity was investigated in vitro by immersing in the simulate body fluid (SBF) at 37 ℃ for 7 days . JL-1155 laser particle analyzer, X-ray diffraction, Fourier transform infrared spectroscopy and scanning electron microscope were used for micro-morphological structure analysis.MAIN OUTCOME MEASURES: ①The crystalline structure and microstructure of sol-gel derived glass-ceramic② The apatite forming process in simulate body fluid③ The diameter of the pore of the sol-gel derived apatite/wollastonite glass-ceramicRESULTS: ①Main crystalline phases of the sol-gel derived glass-ceramic materials were hydroxyapatite/fluoroapatite [Ca10(PO4)6(OH, F)] and β-wollastonite[β-CaSiO3]; Microstructure contained many micro-pores of 2-3μ m;② Sol-gel derived AW glass ceramic had excellent bioactivity: plenty of apatite granules were generated on the surface of the material after soaking for 7 days. ③Porous scaffolds possessed good macro-porous structure with the interconnected macro pores of 300-400 μm in diameter;CONCLUSION: Apatite-wollastonite containing glass-ceramic (AWGC)with excellent bioactivity was developed by sol-gel process. The material is expected to be a good candidate for bone-repairing and bone tissue engineering scaffold materials.
2.The clinical effect of Wiltse approach transforaminai lumbar interbody fusion in treatment of lumbar spinal canal stenosis

Objective To compare the curative effect of transforaminal lumbar interbody fusion by Wihse approach and posterior median approach in treatment of lumbar spinal canal stenosis.Methods Ninety-six patients with lumbar spinal canal stenosis were treated with transforaminal lumbar interbody fusion by Wihse approach (42 cases,Wihse group) and posterior median approach (54 cases,control group).The operation time,exposure time,exposure bleeding volume,length of incision,intraoperative bleeding volume,visual analog score (VAS) of nick at the third day after surgery,and VAS of lumbar and leg,Japanese orthopedic association (JOA) score and Roland-Morris disability questionnaire (RDQ) score at 1 year after surgery were compared between the 2 groups.Results The length of incision,exposure time,operation time,exposure bleeding volume,intraoperative bleeding volume,VAS of nick at the third day after surgery and VAS of lumbar at 1 year after surgery in Wiltse group were (5.0 ± 1.2) cm,(20.0 ± 7.6) min,(150.0 ± 8.4) min,(10.6 ±5.4) ml,(125.5 ± 10.6) ml,(4.5 ±0.6) scores and (1.0 ±0.5) scores,and in control group were (8.0 ± 1.6) cm,(35.2 ±6.8) min,(162.0 ±5.6) min,(84.5 ±7.8) ml,(186.4 ± 15.4) ml,(6.8 ± 0.5) scores and (2.5 ± 0.8) scores],there were statistical differences between the 2 groups (P < 0.01).But there were no statistical differences in the VAS of leg,JOA score and RDQ score at 1 year after surgery between the 2 groups (P > 0.05).Conclusion Two kinds of approach transforaminal lumbar interbody fusions all have good therapeutic effect on lumbar spinal canal stenosis,but the Wiltse approach has advantages of small incision,less intraoperative dissection,less bleeding and low incidence of postoperative lumbodynia.
3.The effect analysis of transforamen lumbar interbody fusion in treatment of lumbar intervertebral disk herniation with unilateral pedicle screw fixation

Objective To explore the effect of transforamen lumbar interbody fusion in treatment of lumbar intervertebral disk herniation with unilateral pedicle screw fixation. Methods Retrospectivly studied the clinical data of 36 patients with lumbar intervertebral disk herniation who were treated through transforamen lumbar interbody fusion with unilateral pedicle screw from January 2011 to October 2012. There were 22 male and 14 female patients, aged form 44 to 68 years old with mean age of 56. The lesion positions included 8 patients in L3/4, 17 patients in L4/5, and 11 patients in L5/S1. The data of operation time, intraoperative bleeding volume, and postoperative drainage volume were collected. Therapeutic effect were evaluated by visual analogue score (VAS), lumbar vertebral score of Japanese Orthopedic Association (JOA) score, and Oswestry dysfunction index (ODI). Results Operation time was 65-100 min with mean time of 85 min. Intraoperative bleeding volume was 100-250 ml with mean volume of 135 ml. Postoperative drainage volume was 100-200 ml with mean volume of 150 ml. Followed up for 12-36 months with mean time of 20 months. No loosening, breakage of screws or displacement of interbody fusion cages were found in the follow up period. The rate of anastomosis was 100%(36/36). The VAS was (7.9 ± 1.2) scores before operation and (1.1±0.3) scores at the end of follow up period, there was significant difference( P<0.05). The JOA score was (10.2±2.6) scores before operation and (23.2±4.3) scores at the end of follow up period, there was significant difference (P<0.05). The ODI was (43.3±6.2) scores before operation and (15.2±4.3) scores at the end of follow up period, there was significant difference ( P<0.05). Conclusion The effect of transforamen lumbar interbody fusion with unilateral pedicle screw fixation is proper for the patients who are suffering from lumbar intervertebral disk herniation, which has advantages of less invasion, less bleeding, better stability, better effect, quicker recovery and less complication.
4.Imaging manifestations of chordoma in the cervical vertebrae

Objective To analyze the X-ray,CT and MRI findings of chordoma in the cervical vertebrae,so as to improve its diagnostic accu-racy.Methods X-ray,CT and MRI findings of 7 cases of chordoma proved pathologically were analyzed retrospectively.X-ray and CT were undergone in all 7 cases,MRI in 3 cases.Results X-ray demonstrated bony destruction in 7 cases,bony collapse in 1 case. Narrowing of inter-vertebral space was in 6 cases and soft tissue mass was in 3 cases.CT demonstrated bony destruction in 7 cases, calcification within the tumors in 2 cases,involvement of the accessory of the cervical vertebra and transverse foramen in 5 cases. Soft tissue mass was around the cervical vertebra in 4 cases.MRI was undergone in 3 cases.On T1 WI middle signal in 3 cases were showed,on T2 WI low and high mixed signal in 3 cases,and soft tissue masses around the lesion in 3 cases.Conclusion Chordoma of the cervical vertebrae is uncommon.Full understanding X-ray,CT,MRI characteristics is useful to improve the diagnosis,guide clinical surgery treatment and forecast prognosis.
5.Component distribution in gradient biomaterial prepared with multi grades energy PIII.

During the preparation of functional gradient materials (FGM) with plasma immersion ion implantation-ion beam enhanced deposition (PIII-IBED), the combination strength between the coating and the substrate would be greatly affected by the implantation dose and the distribution of implanted ions in substrate. According to the requirements of FGM, an idea of multi grades energy implantation had been suggested, with which the Gauss peak could be moved toward the surface, and the concentration of implanted ions could be maximized at the surface. In this study, the distribution of carbon ions implanted into titanic alloy substrate have been simulated theoretically, and the tentative idea have been confirmed experimentally.

6.Research and development of A-W bioactive glass ceramic.

Studies on bioactive glass and glass-ceramic are important research high-lights in the field of biomedical materials. Due to their bioactivity, these materials can form a tight chemical bond with the living bone, when implanted. As a preeminent kind of these materials, A-W(Apatite/Wollastonite) bioactive glass ceramic has not only the excellent bioactivity and biocompatibility, but also the eminent mechanical properties, so it has been largely applied and developed in clinical practice. The development, preparation, properties, applications and the mechanism of its bond with bone are introduced in this paper. We will also put forward the prospect of the research and development of A-W bioactive glass ceramic.

7.Application of 3D printing technology in the second staged revision of infection after hip replacement

Objective To explore the application of 3D printing technology in the second staged revision of infection after hip replacement.Methods From July 2014 to July 2016,21 patients with postoperative infection after hip replacement needed for phase Ⅱ revision surgery were selected as the 3D printing group,while 21 patients who underwent hip replacement without 3D printing technique were selected as the control group.The acetabulum,femoral and other related data were obtained by CT scanning,using computer simulation and modeling in vitro,3D printing technology was used to print the model,showed the hip joint model and done the operation,after 2 to 3-year-follow-up,compared the effect and the Harris hip joint function score.Results Compared the operation time of the two groups,3D printing group average operation time was (86.4±31.5) min,while the control group was (131.7±29.6) min,the difference was statistically significant (P<0.05);the Harris hip joint function score of 3D printing group was (41.4±7.8) point before operation,and after three years follow-up the score was (91.2 ± 6.9) point,the difference was statistically significant (P<0.05).Conclusion 3D printing technology can be applied in the second stage revision surgery of infection after hip replacement,which has effective result.
8.A novel europium doped apatite/wollastonite porous magnetic bioactive glass ceramic.

A new biocompatible apatite-wollastonite magnetic glass ceramic has been synthesized via sol-gel process. Characteristics of the materials were determined with differential thermal analysis (DTA), X-ray diffraction (XRD), scan electron microscopy (SEM), energy dispersive spectrum (EDS), inductively couple plasma atomic emission spectroscopy (ICP-AES), vibrating sample magnetometer (VSM) and so on. Results showed that the main crystalline phases of the material were hydroxyapatite/fluoroapatite [Ca10(PO4)6(OH, F)), beta-wollastonite[beta-CaSiO3] and calcium europium oxide silicate Ca2Eu8[(SiO4)6O2]. The magnetization of the sample contanining 2% Eu2O3 in weight reached 2.18 emu/g for an applied field of 10 000Oe. Hydroxyapatite layer could form on the surface of the sample while soaking for 14 days in simulated body fluid. Good bioactivity was demonstrated. So it is a potential bone repairing material as well as a hyperthemia treatment material for pateints with cancer.

9.In vitro release of tetracycline hydrochloride from alpha-TCP cement.

Drug release from alpha-TCP cement containing tetracycline hydrochloride (TTCH) was studied in vitro. Results from X-ray diffraction study indicated that TTCH did not prevent the hydration of alpha-TCP. In vitro drug release study showed that TTCH release could sustain over 1200 h, and the release was controlled by two mechanisms: (1) diffusion of free TTCH molecules through the porous cement (square-root-of-time kinetics); (2) dissociation of TTCH from the apatite-TTCH complex (zero-order kinetics). The mechanism controlling release would changed with the variety of the antibiotic content of cement pellets, as a result of TTCH adsorption and bonding on calcium phosphates. The first mechanism was predominantly for low concentration system TTCH-loaded apatite cement systems at the initial release period, and for high concentration TTCH-loaded apatite cement systems. As for low concentration TTCH-loaded apatite cement systems at later release stage, drug release was controlled by the coupling of the two mechanisms.
