1.Evaluation of the Efficiency and Safety of Redunin Injection in Treating Pharyngoconjunctial Fever in Children

OBJECTIVE:To evaluate the efficacy and safety of Redunin injection in treating pharyngoconjunctial fever in children.METHODS:81 children with pharyngoconjunctial fever were randomly divided into two groups:42 cases in treatment group were treated with Redunin injection(0.6~0.8 mL?kg-1?d-1) that diluted in 5% Glucose injection iv gtt q.d for 3~5 days,and 39 cases in control group with Ribavirin injection(10 mg?kg-1?d-1) that diluted in 10% Glucose iv gtt q.d for 3~5 days.Both group received routine fluid therapy plus symptomatic treatment.The duration of fever,and the time for the extinction of pharyngalgia and conjunctival congestion as well as the side effects in two groups were evaluated.RESULTS:The duration of fever and the time for the extinction of pharyngalgia and conjunctival congestion in the treatment group were significantly shorter than in the control group(P

OBJECTIVE:To evaluate the permeation enhancing effect of Flos Caryophylli volatile oil versus azone on diclofenac sodium.METHODS:Percutaneous absorption experiment was performed on the apparatus of isolated skin,and the penetrated amounts of diclofenac sodium at different time were measured,then the steady-state flux and enhancing rate were calculated,and the deposit effect of diclofenac sodium was investigated.RESULTS:Flos Caryophylli volatile oil and azone all had promotion effect on percutaneous absorption of diclofenac sodium,especially when they used in combination.Used either in combination or alone,they all had remarkable enhancement property on deposit effect of diclofenac sodium(P

Objective To observe clinical effect of Danshentong capsule on invisible blood loss after total hip arthroplasty postoperative.Methods 90 cases were randomly divided into observation group and control group,which were chosen from January 2011 to March 2013, 45 cases in each group. Control group received low molecular weight heparin for prevention invisible blood loss, postoperative.Observation group were given Danshentong capsule for prevention invisible blood loss postoperative.Changes of related indicators were followed up and recorded.Results Hemoglobin were (112.20 ±16.17),(124.79 ±11.53)g/L of observation group 3 and 7 days postoperation, were higher than control group (94.84 ±14.93),(103.52 ±10.62) g/L (P<0.05).Patients in observation group 3 and 7 days platelet values (258.31 ±32.95),(242.16 ±31.06) ×109/L, than control group (225.14 ±30.48,214.95 ±28.68) ×109/L (P<0.05).Patients in observation group hidden hemorrhage (420.59 ±53.96), lower than control group (868.05 ±65.37) U/mL (P<0.05).Postoperative adverse reactions observed ratio of 6.67% in observation group, lower than control group 26.67%(P<0.05).Conclusion Danshentong capsule in hip replacement surgery occult bleeding prevention clinical exact effect, no significant side effects, is worthy of further research and application.
4.Anterior decompression and stabilization with anterior or posterior instrumentation for treatment of the serious thoracolumbar burst fractures

[Objective]To evaluate the clinical effect for the treatment of severe thoracolumbar vertebral burst fracture by the method of anterior vertebral decompression and fusion combining anterior or posterior fixation,and investigate its indications in different conditions.[Method]At the standard of Denis,twenty-eight cases were treated by anterior vertebral decompression and fusion combining anterior or posterior fixation.In a total of 28 cases,25cases were single vertebral fractures,2 cases were two adjacent vertebral fractures and 1 case was two vertebral fractures comparted by one unfractured centrum,the level of vertebral destruction was ruled by LSC scale of McCormack.[Result]26 cases were reviewed,all patients' pain were relieved obviously after the operation,(VAS scale improved),the height and shape of the fractured centrums recovered obviously(Cobb's angle improved 14.5? in average).After an average of 21.5 months follow-up,the pain all disappeared,the intervertebral grafted bone reached bony union,no Cobb's angle increased and no apparent implication occurred in all cases.[Conclusion]Application of anterior vertebral decompression and fusion combining anterior or posterior fixation can increase the stability of the anterior column of the spine,decompress completely,the intervertebral bone graft with kinds of interbody fixation enables the patients to move as early as possible,it is an effective method in treatment of severe thoracolumbar vertebral burst fracture.
5.Pathological features of biopsy prostatic adenocarcinomas (report of 112 cases)

Objective To study the pathomorphological f eatures of prostatic adenocarcinoma from biopsy. Methods Of 1147 cases of prostatic biopsy specimens,112 cases were adenocarcinoma,1030 cases benign prostatic hyperplasia and 5 cases nonspecific granulomatous prosta titis.Histological grades and the differential diagnosis between well-different iated adenocarcinoma and hyperplasia were conducted.Immunohistochemical analysis (34?E12) were done in some cases. Results 112 cases of adenocarcinoma were diagnosed according to the architectural changes, cytologic al atypia and invasion. 23 cases(20.5%)were of well-differentiated carcinoma ,38 cases(33.9%)moderately-differentiated carcinoma and 51 cases(45.5%) po or-differentiated carcinoma. 21 cases(18.8%) were of low grade prostatic intrae pithelial neoplasia(PIN Ⅰ)and 75 cases(67.0%)of high grade PIN(PIN Ⅲ) were found with in tumor tissue.With immunohistochemical stain,98 in 102 cases of BP H were positive reactive for 34?E12 in the basal layer,while 36 of 40 cases of prostatic adenocarcinoma were negative. Conclusions Ro utine pathological morphological features,combined with immunohistochemical test s is valuable for the pathological diagnosis of prostatic adenocarcinoma.
6.Correlativity Analysis Between Antimicrobial Consumption and Antibiotic Resistance of Pseudomonas Aeruginosa

OBJECTIVE: To discuss the correlation between the consumption of antibacterials and the antibiotic resistance of Pseudomonas aeruginosa(PA) in our hospital.METHODS: The total antibacterial consumption expressed as defined daily doses(DDDs/day/100 persons) as well as the synchronous antibiotic resistance rate of PA were computed and the correlation beween the them was analyzed.RESULTS: The resistance rates of PA to cefepime,gentamycin and ciprofloxacin were positively correlated with the consumption of ciprofloxacin.A significant positive correlation was noted between the resistance rates of PA to gentamycin and imipenem cilastatin and the consumption of imipenem.The resistance rates of PA to ceftazidime and ceftriaxone were positively correlated with the consumption of piperacillin/tazobactam.The resistance rates of PA to amikacin and gentamycin were positively correlated with the consumption of amikacin and gentamycin respectively.CONCLUSION: A positive correlation was noted between antimicrobial consumption and antibiotic resistance of PA.
7.Relationship between Obstructive Sleep Apnea Hypopnea Syndrome and Cardiac Arrhythmia

Objective To investigate the relationship between obstructive sleep apnea hypopnea syndrome(OSAHS) and cardiac arrhythmia. Methods 160 snorers were recruited to undergo more than 7 hours nocturnal polysomnography(PSG) and 24 hours ECG monitoring, and were divided into OSAHS group (n=98) and control group (n=62) based on the results of PSG monitoring. Results There were significant differences in sleep apnea hypopnea index(AHI), SpO_2 and cardiac arrhythmia frequency between the OSAHS and control groups. Accompanying with the breath pause, SpO_2 decreased, and heart rate was periodic changes. The frequency of cardiac arrhythmia obviously increased when SpO_2 was lower than 80%. Conclusion During sleep the low oxygen saturations and fluctuation of autonomic nervous system play important roles in the cardiac arrhythmia in OSAHS patients.
8.Improved Z-open door expensive laminoplasty for cervical ossification of the posterior longitudinal ligament

[Objective]To investigate the effects of the expansive Z-open-door laminoplasty for the ossification of posterior longitudinal ligaments(OPLL).[Methods]Thirty-six cases of OPLL were operated by the expansive Z-open-door laminoplasty.According to JOA score(17 Points) the proximate effects of all patients were analyzed.[Results]All cases were followed-up for average 18.6 months.JOA improved from 8.3 preoperatively to 13.8 postoperatively.Improved rate averaged 58.6%.Image show:3 of 31cases of anterior protruding type were changed to straight type,5 cases of straight type still kept initial type.None of all cases changed into posterior protruding type after operation.The flexion and extention range of the neck were less 1/3 and the axis-ward pain happened commonly.[Conclusion]Improved expensive Z-open door laminoplasty for DPLL can decompression clearly.It has advantages of adeguate canal expansion,preventing re-dosing the door,and being safe and reliable.
9.Comparative study of complications after D1 and D2 gastrectomy for gastric cancer

Objective To analyze the early mortality and morbidity of early postoperative complications of gastric cancer patients submitted to D1 or D2 gastrectomy for improving the surgical effect.Methods All consecutive patients who underwent D1 or D2 radical gastric resection between January 2006 and December 2007 were evaluated.Clinicopathologic features of the tumor,the extent of lymphadenectomy,the postoperative mortality and the early morbidity of complication were analysed.Results There were 130 patients admitted for the treatment of gastric cancer.34 underwent D1 dissection and 96 underwent D2 dissection.The early morbidity of D2 dissection was higher than that of D1 dissection (20.6% vs.39.6%,P<0.05).When complications were analyzed individually,there was no significant difference.Although the postoperative mortality was higher in the D2 group,no significant difference was observed(4.2% vs.0,P>0.05).4 patients underwent D2 dissection died.Considering the causes of the 4 died patients,respiratory complication related to anaatomotic and pancreatic leakage was the most important.Conclusion This study indicates that D2 dissection is a safe and effective procedure and the extent of lymphadenectomy may be related with high postoperative complication morbidity and mortality.Improving the surgical dissection skill and standardizing the gastric lymphadenectomy are the key ways to decrease the morbidity and mortality.
10.Microwave radiation affects the phosphorylation of nuclear factor erythroid-2-related factor-2 and activity of protein kinase C in vascular endothelial cells

BACKGROUND: Nuclear factor erythroid-2-related factor-2 (NF-E2-related factor-2) is an important transcription factor to regulate anti-oxidative stress reaction. Some researches indicate that NF-E2-related factor-2 can be phosphorylated by numerous members of protein kinase C family. In order to investigate generant mechanism of microwave radiation on oxidative stress injury, whether microwave radiation can influence on anti-oxidative regulating system through NF-E2-related factor-2 or not should be further studied.OBJ ECTIVE: To analyze the effect of microwave radiation on phosphorylation of NF-E2-related factor-2 and activity of protein kinase C in vascular endothelial cells.DESIGN: Observational-contrast study.SETTING : Department of Labor Hygiene, the Third Military Medical University of Chinese PLA.MATERIALS: Vascular endothelial cell strain; H332PO4; Protein-A Sepharose (Sigma Company); mono-antibody of NF-E2-related factor-2 (H-300, Santa Cruz); α-mono-antibody of protein kinase C (Santa Cruz); glass microfiber filters(Whatman Company); gel scanning system (Gel Doc 2000, Bio-Rad); liquid scintillation spectrometer (LKB-117, Sweden).METHODS: The experiment was carried out in Laboratory of Electromagnetic radiation and Biological Effect, Department of Labor Hygiene, the Third Military Medical University of Chinese PLA from March to July 2003. ① Analysis of phosphorylation of NF-E2-related factor-2: Vascular endothelial cells were cultured with DMEM medium till the period of productive growth and incubated with 32Pi for 2 hours. And then, cultured bottle was maintained in water bath at 37℃ and performed with microwave radiation in dark chamber, whose reflectivity was about zero. It was regarded as radiation group, and the average power density of radiation was 30 mW/cm2; in addition, the duration of radiation was 30 minutes.Cells did not deal with microwave radiation were regarded as control group. Phosphorylation level of NF-E2-related factor-2 was measured at 2, 4, 8 and 24 hours after radiation with immune coprecipitation-autoradiography technique and dealt with semi-quantitative analysis with gel scanning system. Cells in the control group were analyzed directly. ②Active analysis and expressional measurement of protein kinase C: Cells in the radiation group and the control group were dealt with the same cultured method, condition, radiation styles, dosage and environment as mentioned above. At 2, 4, 8 and 24 hours after radiation, cells were split to extract plasma and membrane protein. Furthermore, activity of protein kinase C was measured with r-32P-ATP labeled liquid scintillation spectrometer; gray value of protein strap was dealt with semi-quantitative analysis with gel scanning system; staining degree of plasma was observed after immunocytochemical staining of protein kinase C. In addition, cells in the control group were measured and observed directly.MAIN OUTCOME MEASURES: ① Phosphorylation level of NF-E2-related factor-2 in radiation group and control group; ②Results of active analysis and expressional measurement of protein kinase C in radiation group and control group.RESULTS: ① Phosphorylation level of NF-E2-related factor-2 in radiation group and control group: Gray value of NF-E2-related factor-2 was higher in radiation group than that in control group at 2, 4 and 8 hours after radiation.Phosphorylation level of NF-E2-related factor-2 reached the peak at four hours after radiation. In addition, results of semi-quantitative scanning analysis showed that, at 2, 4 and 8 hours after radiation, phosphorylation level of NF-E2-related factor-2 was increased 33%, 261% and 141% in radiation group as compared with that in control group,respectively (t = 2.974, 4.209, 4.047, P < 0.05), and then, fallen down to normal value 24 hours later. ② Results of active analysis and expressional measurement of protein kinase C in radiation group and control group: At 2, 4 and 8 hours after microwave radiation, expression of protein kinase C in radiation group was higher than that in control group,especially at the 4 hour. In addition, at 24 hours after radiation, expression of protein kinase C recovered the normal value. Results of immunocytochemical staining showed that staining of plasma was deeper in radiation group than that in control group at 4 hours after radiation. Moreover, results of r-32P-ATP labeled liquid scintillation spectrometer also suggested that, at 2, 4 and 8 hours after radiation, activity of protein kinase C was increased 36%, 93% and 47% in radiation group as compared with that in control group, respectively (t =2.801, 3.654, 3.035, P < 0.05). And then, activity of protein kinase C was decreased after 24 hours, otherwise, activity of protein kinase C reached the peak at 4 hours after radiation.CONCLUSION: Microwave radiation can strengthen the phosphorylation level of NF-E2-related factor-2 in vascular endothelial cells during a special period; meanwhile, it can also cause the increase of expression of protein kinase C.Time effect of activity of protein kinase C is coincidence with phosphorylation level of NF-E2-related factor-2.
