1.Prognosis of subclinical Cushing's syndrome:comparison of surgical ablation of incidentaloma with conservative approach

Forty-eight patients with subclinical Cushing's syndrome(SCS)were evaluated.Eleven of them underwent adrenalectomy(Group 1)and the other 37 cases did not(Group 2).Serum and urine corticosol, plasma ACTH and parameters related to metabolic syndrome(such as waist circumference,blood pressure,blood lipids and fasting plasma glucose)were measured.The data at diagnosis were compared with those during the survey.The results indicated that patients with SCS had a significantly high prevalence of metabolic syndrome.The symptoms and signs of metabolic syndrome could be improved after removing the tumor.Otherwise there is no improvement,some patients will even develop into overt Cushing's syndrome.

One new dicyclopeptide cyclo-(L-N-methyl Glu-L-N-methyl Glu) (1), together with one new natural dicyclopeptide cyclo-(L-methyl Glu ester-L-methyl Glu ester) (2), and two known dicyclopeptides cyclo-(L-methyl Glu ester-L-Glu) (3), and cyclo-(L-Glu-L-Glu) (4), were isolated from the aerial parts of Dianthus chinensis L. Their structures were determined by spectroscopic analyses and chemical methods.

3.Placement of a Long Intestinal Tube in Patients with Early Postoperative Small Bowel Obstruction under Fluoroscopic Guidance.

OBJECTIVETo investigate the placement of a long tube into the small intestine under fluoroscopic guidance and to evaluate its decompression effect on early postoperative small bowel obstruction (EPSBO).
METHODSFifty-four patients with EPSBO requiring decompression between April 2010 and July 2014 were enrolled in the study. Insertion of a long tube was guided by fluoroscopy. We first used the guide wire to pass the pylorus and then used the 10 Fr feeding tube as an exchangeable tube to put the superstiff wire into the duodenum. Finally the long tube could be passed over the guide wire through the pylorus into the intestine. The total procedure time, the radiation exposure time, and the incidence of complications were evaluated.
RESULTSThe long tubes passed into the jejunum on initial insertion for all patients, so the success rate of this technique was 100%. The long tube was inserted into ileum in 18 patients. The mean total procedure time was 34.4 ± 8.6 minutes, and the mean radiation exposure time 18.9 ± 6.8 minutes. A total of 47 patients (87%) experienced full recovery following long-tube decompression and without the need for surgical intervention.
CONCLUSIONSUsing the wire-exchange technique, it is easy to place a long tube into the small bowel under fluoroscopic guidance. This decompression method is safe and effective for management of EPSBO.

5.Isolation and Characterization of Collagenolytic Enzyme-Producing Strain from Rotten Hides and Primary Analysis of the Enzyme Property

Bacteria isolated from rotten hides,one identified as Pseudomonas aeraginosa later, were shown to decompose fresh hides completely in 48 hours at room temperature. Strongly collagenolytic activity was detected when native collagen from calfskin was used as substrate. The optimal reaction pH value and temperature of the collagenace produced by P.aeraginosa are 7.5 and 32℃ respectively. EDTA and EGTA inhabit the collagenolytic activity severely while PMSF has little effect on it. Not only media components but also fermentation conditions have differents effect on the production of this collagenolyic enzyme.
6.Usage status of periapical films undertaking in root canal treatment.

OBJECTIVETo determine the current radiographic practices of root canal treatment (RCT) undertaken by endodontists, general dentist practitioners (GDPs) and interns, and to analyze the relationship between the usage of periapical films and the quality of RCT.
METHODSThe patients who had received RCT treatment in West China College of Stomatology were investigated at random and were divided into three groups: Endodontists treating group, GDPs treating group and interns treating group according treating physician. The usage status of periapical film were investigated by the well designed questionnaires. The obturation quality of postoperative films were evaluated. Chi-square analysis was used to compare usage rates of periapical films of three groups. Mann-Whitney test was used to determine the relationship between the usage of periapical films and the quality of RCT.
RESULTS571 teeth of 412 patients were investigated. In endodontists treating group, GDPs treating group and interns treating group, the usage rates of preoperative films were 95.3%, 89.5% and 92.1% respectively. The usage rates of working length films were 5.2%, 1.1% and 5.8% respectively. The usage of master point films was 94.8%, 72.1% and 97.4% respectively. The usage of postoperative films was 97.9%, 76.3% and 95.3% respectively. There were statistically significant differences among the working length films, master point films and postoperative films of three groups (P<0.05). The periapical films of adequate quality RCT and inadequate quality RCT were 3.14+/-0.639 and 2.84+/-0.736 respectively. There was statistically significant difference between periapical films of adequate quality RCT and inadequate quality RCT (P<0.05).
CONCLUSIONRadiographic practices of endodontists and interns are more standardized than GDPs. Quality RCT is related to radiographic practices. Standard radiographic practices would be benefit to RCT.

Primary pigmented nodular adrenal disease (PPNAD) is a kind of autosomal dominant inherited disease. Patient in the study presented with Cushing's syndrome, and clinical and pathological diagnosis of PPNAD was confirmed. It is now confirmed that there are two relevant genes and their mutations may lead to PPNAD. This study showed no mutations in the patient, surpecting if there would be an alternative mechanism or a new gene in playing the role.

9.Comparative research of craniofacial structural characteristics of syndromic and non-syndromic patients with cleft palate.

OBJECTIVETo compare craniofacial structural characteristic of syndromic and non-syndromic patients with cleft palate and to probe into the reason of their maxillary hypoplasia and to lay a foundation for better treatment protocol for syndromic patients with cleft palate.
METHODSThe sample consisted of 8 individuals with Treacher Collin syndrome, 9 patients with Pierre Robin syndrome, and 40 patients with non-syndromic cleft palate which met certain criterions. Lateral cephalometric radiographs were obtained from each study subject. A total of 18 variables, comprising 9 angular, 7 linear, and 2 ratio measurements, were studied. SPSS 12.0 software was used to describe the statistics data, consisting of means, SD, and t test.
RESULTSThe tendency for smaller value in SNB and larger value in ANB indicate a severer anteroposterior mandibular hypoplasia in the syndromic type cleft. There was no difference between syndromic and non-syndromic cleft palate patients with regards to maxillary anteroposterior development. No significant difference on the development in cranial base area was found either.
CONCLUSIONThis interplay of cranio-facial bones is very important in the development of this complex, which will not only affect the growth but also the defect development. In this study, the patients with the two syndromes showed the similar growth potential of maxilla with the non-syndromic cleft patients.

10.Polymyxin B antagonizing biological activity of lipopolysaccharide.

OBJECTIVETo investigate the mechanism of polymyxin B (PMB) antagonizing the biological activity of lipopolysaccharide (LPS).
METHODSThe affinity of PMB for LPS and lipid A was assayed by biosensor, and the neutralization of PMB for LPS (2 ng/ml) was detected by kinetic turbidimetric limulus test. The releases of TNF-alpha and IL-6 in murine peritoneal macrophages a (PMphi) after exposure to LPS (100 ng/ml) were detected, and the expression levels of TLR4, TNF-alpha and IL-6 mRNA in PMphi induced by LPS (100 ng/ml) were measured by RT-PCR.
RESULTSPMB had high-affinity to LPS and lipid A with dissociation equilibrium constants of 18.9 nmol/L and 11.1 nmol/L, respectively, and neutralized LPS in a dose-dependent manner. Furthermore, PMB could markedly inhibit the expressions of TLR4, TNF-alpha and IL-6 mRNA and the release of cycokines in LPS-stimulated murine peritoneal macrophages.
CONCLUSIONSPMB neutralizes LPS and inhibites the expression and release of cycokines in macrophages, in which the affinity of PMB for lipid A plays an important role.
