1.Clinical Significance of Serum Kynurenine and Uric Acid Levels Detection in Chronic Myeloid Leukemia Patients

Objective To investigate the clinical significance of kynurenine (KYN) and uric acid (UA) levels in chronic myeloid leukemia patients (CML).Methods 30 CML patients (CML group) and 25 healthy controls (Control group) were enrolled in the hospital from Mar 2014 to Dec 2015.Serum KYN and tryptophan (TRP) of all subjects were determined by high performance liquid chromatography,and KYN/TRP ratio (KTI) were calculated.Leukocytes were counted by automat ic blood cell analyzer.Serum UA were detected by enzyme method.Correlation between UA and KTI,leukocytes were analyzed by Pearson correlation.Results The serum KTI of the CML group (49.76±19.06) before treatment was significantly higher than that of the control group (27.87-±-5.06,t=4.470,P=0.001).KTI cutoff value was 40 by ROC analysis.There had correlation between KTI and leukocytes (r=0.743,P=0.001) and no correlation between KTI and ages (r=0.205,P =0.276).KTI (64.7±17.8) and the leukocytes [(96.7±64.1) × 109/L] of 10 cases of KTI≥40 of CML patients before treatment was significantly higher than that of KTI (27.0± 10.9,t=7.102,P =0.001) and the leukocytes[(10.1 ± 5.2) × 109/L,t=4.285,P=0.002] after treatment.The serum UA in KTI<40 CML patients (n=15,354.98±103.9 μmol/L) was significantly lower than that of KTI≥40 CML group CML (n=15,484.98±132.1 μmol/L,t=1.432,P=0.045).The UA in CML group was associated with KTI (r=0.573,P=0.001) and leukocytes (r=0.537,P=0.002).Conclusion The increase of KTI in CML patients suggested that an increased activity of indoleamine-2,3-dioxygenase (IDO).IDO activity may associated with CML morbidity and therapy effect.The detection of KTI may contribute to the diagnosis of CML and assessing therapy effect of CML.
2.Influence of lung IL-10 on dendritic cells in multiple organs dysfunction syndrome

Objective To explore the influence of lung IL-10 on interstitial dendritic cells in multiple organ dysfunction syndrome(MODS),and its role in immunological dysfunction.Methods The model of MODS was replicated by injecting zymosan into the peritoneal cavity of C57BL/6 mice.The mice were then randomly divided into normal groups as well as the groups of 3-6 hours,12-48 hours,5-7 days and 10-12 days post trauma.The level of CCR7 mRNA in lung was detected by RT-PCR and the proteins of CCR7 and IL-10 were immunohistochemically stained.The number of mononuclear cells in MHC-II positive peripheral blood was determined by flow cytometry.Results At the early stage of injury(3-6 hours),the number of IL-10 positive cells and the expression of CCR7 increased,but the number of mononuclear cells in MHC-II positive peripheral blood decreased.At the stage of 12-48 hours,the number of IL-10 positive cells and the expression of CCR7 continued to increase obviously,and that of mononuclear cells in MHC-II positive peripheral blood went on decreasing.At the stage of 10-12 days,the number of IL-10 positive cells increased dramatically compared with that in normal group,but the expression of CCR7 declined obviously in contrast to that in 12-48 hours,and the number of mononuclear cells in MHC-II positive peripheral blood decreased to the lowest level.Conclusions The expression of IL-10 in lung could inhibit the excessive immunological damage induced by dendritic cells on the one hand,and it probably leads to immunosuppression by reducing the CCR7 expression of dendritic cells and the number of mononuclear cells in MHC-II positive peripheral blood on the other hand.
3.Effect of phacoemulsification versus extracapsular cataract extraction in patients with intumescent senile cataract

Objective To compare the safety and effect of the phacoemulsification (PHACO) versus extracapsular cataract extraction (ECCE) in patients with intumescent senile cataract.Methods 200 eyes from patients with intumescent cataract were included and randomly divided into 2 groups:PE group (108 eyes,received PHACO) and ECCE group (92 eyes,received small incision ECCE) respectively.Superior quadrant sclera tunnel incisions were made with stabs of 2.8 mm diameters.Trypan-blue was used to show the anterior lens capsular membrane.In the PHACO group,a 4 mm diameters continuous curvilinear capsulorhexis (CCC) was made,which was enlarged to 6-7 mm after the intraocular lenses (IOL) implantation.For the ECCE group,a 8 mm-diameter CCC was made.Then the lens nucleus was either phacoemulsificated or delivered and IOL was implanted.The complications during and after surgery and the visual outcomes were recorded and statistically analyzed by SPSS 13.0 software.Results Age,sex and the hardness of the nucleus were comparable between the two groups.The best corrected vision acuity (BCVA) was 0.05 and worse in all patients before surgery.While 3 days after operation,in PE group,105 eyes (97.2%,105/108) gained postoperative vision 0.05 and better,and 82 of them were better than 0.3; in group ECCE,97.2% (89/92) of the operated eyes gained vision 0.05 and better,72 (78.3%,72/92) eyes were better than 0.3.No statistical differences were found between the two groups in postoperative vision recovery.While,there were more failure rates of the CCC,tear of the anterior and posterior capsular,loss of the vitreous and iris injury rate in the ECCE group than in the PE group (20.7% vs.8.3%,17.4% vs.12.0%,7.6% vs.0.9%,15.2% vs.0%,P=0.01,P=0.00,P=0.02,P< 0.001).Prolapse of iris and discoria were found in ECCE group.Conclusions With small CCC,phacoemulsification can lead to better surgical outcomes than small incision ECCE procedures,and the operative and post-operative complications are less in PE group than in ECCE group.

Appropriate bacteria can improve blood sugar disorder. Treating diabetes from the theory of Pi-Wei is effective. Regulating intestinal flora has become a new pathway for treating diabetes from the theory of Pi-Wei. On the basis of intestinal flora, authors discussed the treatment of diabetes from Pi and Wei.

Traditional Chinese medicine market is a typical market with information asymmetry, which may lead to "lemons" problem. Third-Party Certification is efficient path to solve the problem of information asymmetry. For this reason geo-authentic and commodity classes certification seems to be very important. But as we know, information asymmetry happen in the stages of traditional Chinese patent medicines market. So a signaling transfer mechanism, which can deliver the quality signal, is badly needed in TCM market. This paper analysis and design of signaling transfer mechanism based on Third-Party Certification, which called quality prestige index of TCM enterprises (QPITCM). QPITCM can display quality information as a signal run through the TCM market.

Objective To analyse the characteristics of fundus fluorescein angiography(FFA)of iatrogenic retinal vascular occlu- sion.Design Retrospective case series.Participants 9 eyes of 9 patients with iatrogenic retinal vascular occlusion and 16 eyes of 16 patients with non-iatrogenic retinal vascular occlusion in Beijing Tongren Hospital in 2002-2005.Methods All patients were examined with FFA.The difference of circulation time of retinal vessels both in iatrogenic and non-iatrogenic retinal vascular occlusion patients was compared.Main Outcome Measures The starting perfusion time and the finishing time of retinal artery or vein.Results In pa- tients with iatrogenic(4 cases)and non-iatrogenic(12 cases)central retinal artery occlusion,the finishing perfusion time was separately 79.33?87.04s and 19.20?4.61s; the finishing time of retinal vein was separately 128.07?149.11s and 33.16?15.34s.In iatrogenic(4 cas- es)and non-iatrogenic(4 cases)central retinal artery together with central retinal vein occlusion patients,the finishing perfusion time of retinal artery was separately 211.67?371.26s and 30.07?17.26s;the finishing perfusion time of retinal vein was 232.43?358.52s and 48. 81?11.64s.One patient was ocular artery occlusion.FFA showed that choroidal background fluorescence and central artery were perfused slowly,the vascular fluorescence perfusion was interrupted before it came out of optic disk and the perfusion interruption continued until late stage with extensive peripheral non-perfusion areas.Conclusion The perfusion time of the retinal artery and vein in iatrogenic reti- nal vascular occlusion may be much longer than that in non-iatrogenic retinal vascular occlusion.
