2.The experimental research of three-dimensional ultrasound in real-time intellectual positioning navigation

Objective To explore the accuracy and practicability of the three-dimensional ultrasound in real-time intellectual positioning navigation.Methods The phantom was applied in the real-time intellectual positioning guided biopsy of three dimensional ultrasound,free-hand ultrasound-guided biopsy and bracket-guided biopsy with different depths(5 cm,10 cm) and different sizes of the targets (6 mm and 15 mm).The time spent,the length of the sample and the success rate of biopsy were compared among three groups,and statistical analysis was done.Results ①The different sizes of targets with different depths were shown in three kinds of biopsy that the real-time intellectual positioning guided biopsy of three-dimensional ultrasound spent more time than bracket-guided biopsy,less than free hand ultrasound-guided biopsy.The difference was significant (P <0.05).The aquired red-dyed sample was longer in the real-time intellectual positioning guided biopsy of three-dimensional ultrasound compared to free-hand ultrasound-guide biopsy and bracket-guided biopsy,the difference was significant (P <0.001).②For real-time IP-guided biopsy of three-dimensional ultrasound with same depth,larger size of targets(15 mm) spent less time than smaller saize of targets (6 mm),the difference was significant (P < 0.05).However,compared to smaller saize of targets(6 mm),success rate of biopsy in larger size of targets(15 mm) had no significant difference (P =0.238 at depth of 5 cm,P =0.479 at depth of 10 cm).③For real-time intellectual positioning guided biopsy of three-dimensional ultrasound with same size,time spent was less in the targets at the depth of 5 cm than in the targets at the depth of 10 cm,the difference was significant (P<0.05).The aquired red-dyed sample was longer in the targets at the depth of 5 cm than in the targets at the depth of 10 cm,and success rate of biopsy in the targets at the depth of 5 cm was higher too (P <0.05).Conclusions The real-time intellectual positioning guided biopsy of three dimensional ultrasound spend less time with high accuracy and success rate of biopsy,especially for smaller and deeper targets,can be clinically applied with accuracy and practicability.

5.Study on knowledge discovery in traditional Chinese medical case records

Traditional Chinese medical case records in the previous dynasties are vital to the development of traditional Chinese medical theory, but the tremendous amount of data are far beyond a person's ability for comprehension. According to information science, traditional Chinese medical case record data are complicated and intricate experiential data. New technology and methods are needed to solve this difficulty. Knowledge discovery technology plays an important role in analyzing data and uncovering important data patterns, and it will be a useful method in processing such data. This paper briefly presents the methods of knowledge discovery in traditional Chinese medical case record study, and puts forward some necessary academic methods.

Fibrolase is a non-hemorrhagic zinc metalloproteinase isolated from southern copperhead snake venom (Agkistrodon contortrix contortrix) and is capable of degrading fibrin clots resulting from purified fibrinogen or from blood plasma. Alfimeprase, a truncated form of fibrolase, as a clinical agent was successfully completed PhaseII clinical trials.The cDNA of alfimeprase was amplified by recursive PCR, digested with BamHI and HindII, and cloned into pET43.1a, pMALp2X and pMALc2X vectors to generate fusions with NusA, MBP and sMBP(with signal peptide), respectively. Nus/alfimeprase was expressed in soluble form by co-expressing with chaperone FkpA and inducing with1mmol/L IPTG. The fusion protein accounted for about 25 % of total protein following cell lysis. Alfimeprase was successfully purifiesd by Ni-NTA affinity chromatography and cleaved by enterokinase. The results demonstrate the fibrinolytic activity of recombinant alfimeprase using fibrin plate assays and fibrinogen hydrolysis.
7.Prenatal diagnosis of congenital fetal heart abnormalities in twins by Yagel's heart examination method

Objective To investigate the value and clinical significance of congenital heart diseases (CHD)detection in twins.Methods A total of 1103 twins were included in this study(127 twins were at high risk for CHD).The fetal hearts were scanned by ultrasound using Yagel's heart examination method. Autopsies were done when the pregnancy was terminated.And blood samples from fetal hearts or umbilical veins were used to evaluate fetal chromosomes.A close follow-up was conducted for normal heart cases and another heart examination was done within three months after birth.Results(1)12 twins(1.09%,12/ 1103)had CHD.Among them,4 cases were from the high risk for CHD group(33.3%,4/12),and 8 cases(66.7%,8/12)were from the low risk pregnancy group.(2)Two twins suffered from the same CHD (one pair were both TOF,and the other pair were both rhabdomyoma).One pair of twins had different abnormalities(one baby was TOF,and the other was duodenal atresia with a normal heart).All three pairs of twins chose termination and autopsies were conducted.Unanimous conclusions between prenatal ultrasound and autopsy were obtained.Nine twins were CHD in one baby and a normal heart in the other baby.Seven of them had the same conclusion after delivery.(3)Two twins with CHD were found with fetal abnormal chromosome.(4)1091 cases were not found having any abnormality,however,one fetus from one twin pair was diagnosed with ventricular septal defect(VSD)with abnormal chromosome after birth,and one fetus from another twin pair had patency of ductus arteriosus after birth.(5)The sensitivity of Yagel's heart examination was 82.4% and specificity was 100% in twins.Conclusion Yagel's heart examination is an effective and time-saving method to scan fetal hearts in twins.
8.Investigation of non-ionizing radiation hazards from physiotherapy equipment in 16 medical institutions.

OBJECTIVETo investigate the non-ionizing radiation hazards from physiotherapy equipment in medical institutions and to explore feasible control measures for occupational diseases.
METHODSOn-site measurement and assessment of ultra-high-frequency radiation, high-frequency electromagnetic field, microwave radiation, and laser radiation were carried out in 16 medical institutions using the methods in the Measurement of Physical Agents in Workplace (GBZ/T189-2007).
RESULTSAll the investigated medical institutions failed to take effective protective measures against non-ionizing radiation. Of the 17 ultra-short wave therapy apparatus, 70.6%, 47.1%, and 17.64% had a safe intensity of ultra-high-frequency radiation on the head, chest, and abdomen, respectively. Of the 4 external high-frequency thermotherapy apparatus, 100%, 75%, and 75%had a safe intensity of high-frequency electromagnetic field on the head, chest, and abdomen, respectively. In addition, the intensities of microwave radiation and laser radiation produced by the 18 microwave therapy apparatus and 12 laser therapeutic apparatus met national health standards.
CONCLUSIONThere are non-ionizing radiation hazards from physiotherapy equipment in medical institutions, and effective prevention and control measures are necessary.

9.Comparative observation on curative effects of stenosing tenovaginitis of flexor digitorum treated with acupuncture knife therapy and blocking therapy.

OBJECTIVETo compare the curative effects on stenosing tenovaginitis of flexor digitorum treated with acupuncture knife therapy and blocking therapy so as to explore the better method.
METHODSThree hundred and four cases were randomly divided into acupuncture knife therapy group (164 cases) and blocking therapy group (140 cases). In acupuncture knife therapy group, hooking and cutting were applied on the pressure points and scleromas with Ren's hooked acupuncture knife; routine blocking therapy was applied in blocking therapy group. The curative effects of both groups after 1-2 treatments and the recurrence after treatment in following 6 months were observed.
RESULTSThe curative rate was 98.2% (161/164)in acupuncture knife therapy group, superior to that of 65.7% (92/140) in blocking therapy group; the recurrence rate was 1.9% (3/161) in acupuncture knife therapy group, inferior to that of 37.0% (34/92) in blocking therapy group. The differences are significant between groups (both P < 0.01).
CONCLUSIONThe curative effect of stenosing tenovaginitis of flexor digitorum treated with acupuncture knife therapy is superior to that of blocking therapy, with low recurrence rate.

10.The value of virtual touch tissues quantification in evaluation of diabetic nephropathy

Objective To investigate the clinical value of virtual touch tissues quantification in the evaluation of kidney stiffness in patients of diabetic nephropathy .Methods A total of 90 cases of diabetic nephropathy were divided into 3 groups:infinitesimal albuminuria ,microalbuminuria and massive proteinuria groups.And other 30 health subjects were taken as control group.The shears wave velocity ( Vs) which reflected the tissue elasticity was measured.The Vs values were compared among different groups.Results In all groups,the highest Vs was present in renal cortex .And compared with the renal cortex ,the Vs of the renal medulla and renal sinus have statistically significant differences [ the normal control group:( 3.65 ± 0.26)m/s,(2.72 ±0.35) m/s,(1.83 ±0.54) m/s,t =9.30,18.20,both P <0.05;Infinitesimal albuminuria group:(2.98 ±0.28)m/s,(2.47 ±0.33)m/s,(1.65 ±0.31)m/s,t=5.97,15.57,both P<0.05;microalbuminuria group:(2.55 ±0.22) m/s,(2.22 ±0.28) m/s,(1.54 ±0.21) m/s,t =3.86, 11.83,both P<0.05;massive proteinuria group:(1.99 ±0.28)m/s,(1.49 ±0.30)m/s,(1.01 ±0.39)m/s, t=5.85,11.48,both P<0.05].The renal cortex Vs of Infinitesimal albumonuria group ,microalbuminuria group and massive proteinuria group show a gradually decreasing trend .And the renal cortex Vs of microalbuminuria group and massive proteinuria group have statistically significant differences compared with the normal control group(t=11.79,17.79,both P<0.05).Conclusions Virtual touch tissues quantification technique can reflect the renal tissue elasticity .It will contribute to the assessment of renal function in patients with early diabetic nephropathy .
