3.Bilateral facetectomy combined with pedicle screw fixation and interbody fusion in the treatment of bilateral lumbar foraminal stenosis in 41 cases

Objective To explore the clinical efficacy in bilateral lumbar foraminal stenosis (LFS) after treatment with bilateral facetectomy combined with pedicle screw fixation and interbody fusion.Methods A total of 41 cases of patients with bilateral LFS underwent bilateral facetectomy combined with pedicle screw fixation and interbody fusion from February 2010 to August 2013 in Department of Spine Surgery,the People's Hospital of Tianjin City,were retrospectively analysed.The clinical efficacy was assessed by Oswestry disability index (ODD questionnaire and visual analogue scale (VAS) before and after operation,anterior and posterior disc height and L1-S1 angle were measured as well.Then the ODI and VAS scores,and changes in anterior and posterior disc height and L1-S1 angle were calculated at the time of the last patient follow-up visit.Results All 41 patients were followed up for 12 to 36 months,with an average of (26.2±2.4) months.Compared with preoperation,at the time of the last follow-up visit the back pain VAS score,leg pain VAS score and ODI were decreased,while the anterior and posterior disc height were increased,there were statistically significant differences (P<0.05).No statistically significant difference was found in L1-S1 angle between preoperation and postoperation (P>0.05).Conclusion The short-term clinical curative effect of posterior bilateral facetectomy combined with pedicle screw fixation and interbody fusion in the treatment of bilateral LFS is satisfactory.
4.Primary exploring of "dynamic character" of acupoint.

The dynamic character of acupoint has been observed extensively, but it is rarely analyzed systematically. The aim of this paper is to analyze the essence of this unique dynamic character of acupoint based on old and modern research literatures. Modern studies are summarized on qi and blood changes on the same acupoint at different times, in different physiological and pathological conditions and different syndrome types. On the basis of the literatures, the scientific base of dynamic character of acupoint is explored preliminarily. The authors propose that the dynamic character of acupoint should be focus on the modern theory of acupuncture and clinical treatment and modern preventive medicine, and the authors also put forward ideas and prospect on the essence, basis and application direction of dynamic character of acupoint.

5.Clinical observation on meridian detection and diagnosis in 66 patients with hypertension.

OBJECTIVETo observe the meridian reaction in patients with hypertension, to explore the significance of meridian diagnosis.
METHODSSixty-six patients with hypertension received two different methods of meridian detection. Kang Wei Human Body Meridian Analysis and Diagnosis System was applied to detect the volt-ampere characteristic curve on the Yuan (source) point of the 12 meridians. Five minutes after the first examination, the traditional method of meridian detection was used to examine the meridian reactions, such as pressing pain, tuber and cord-like reactants along the 12 meridians at forearms, lower limb, head and back.
RESULTSThere were sixty-five cases of abnormal volt-ampere characteristic curve among sixty-six patients with hypertension. The number of cases about Kidney Meridian of Foot-Shaoyin and Bladder Meridian of Foot-Taiyang were both 28 with the highest abnormal rate being 43.1%; Using traditional method of meridian detection, there were 41 cases of abnormal meridian reactions, about 28 cases of Governor Vessel, with the highest abnormal rate being 68.3%.
CONCLUSIONThe abnormal meridian reactions exist in Governor Vessel, Kidney Meridian and Bladder Meridian in patients with hypertension. Therefore, this finding provides clinical evidence in meridian diagnosis.

6.Current situation and prospects of scale application in acupuncture research in China.

In recent years, the standardized scales have been increasingly adopted for acupuncture researches, making leaps in both quality and quantity. By analyzing the current situation of scale application in acupuncture researches in the past five years, the present paper points out the major problems at present, including inadequate use of the scale, insufficiency of the scale in reflecting the advantages and characteristics of acupuncture and moxibustion as well as lack of confirmatory test of reliability and validity of the scales, and probes into the future study thinking.

7.Analysis on Hantaan virus in hemorrhagic fever patients with renal syndrome in Heilongjiang

Objective To separate and amplify Hantaan virus(HV)in serum of hemorrhagic fever patients with renal syndrome(HFRS)in Heilongjiang,and look for its difference from intemational standard type strain(76-118strains).Methods HVs of different phase in the 8erum of 50 HFRS patients were separated and amplified by RTnested-PCR,its products were analyzed the amplified by sequencing.Results Detectable rate of HV in the patients serum was 36.36%(8/22)in 7 days after onset,it Was 13.04%(3/23)in patients having an onset 8 days to 14 days earlier,5 cases were not detectable 15 days after onset.Comparing the sequence of HV S gene fragment,sample 1,9,18,31,37,38,44 strain had a homology of 90.24%,86.72%,89.97%,89.16%,86.45%,87.26%and 89.43%to 76-118 strains,respectively.Conclusions The positive rate is the highset in 7 days after onset.Nucleotide sequence difference exists between pathogenic strain of Heilongjiang's HV and international standard strain,indicating that not only hosts but also locations can affect HV.
8.Emergency repair of severe complex defect in forearm by transplantation of free flap and functional reconstruction

Objective To report the outcome of emergency repair of severe complex defect in forearm by transplantation of free flap and simultaneous functional reconstruction.Methods From Mar.1994 to Aug.2003,4 cases with severe complex defect in forearm was repaired by transplantation of free skin flap, free skin flap combined with fibula flap,or fibula osteocutaneous flap in emergency.Simultaneously the flexion and extension function were repaired by muscle transfer and/or tendon grafting,tenonectomy.Results All the cases were successful.Follow-up period ranged from 1 to 3 years postoperatively.The blood-supply,tex- ture and elasticity of transferred flaps were excellent with good bone healing.Opposition of thumb with four fin- gers was good.Sensory recovery of the hand was satisfactory.Conclusion Transplantation of free flap com- bined with simultaneous functional reconstruction is an ideal method in emergency repair of severe complex de- fect in forearm.
9.Imaging features of active Crohn′s disease on conventional ultrasound and contrast-enhanced ultrasound

Objective To investigate the imaging features of active Crohn′s disease on conventional ultrasound and contrast-enhanced ultrasound (CEUS). Methods The imaging features of 20 patients with an established diagnosis of Crohn′s disease on transabdominal high-frequency ultrasound and contrast-enhanced ultrasound in Shanghai Tenth People′s Hospital from August 2011 to December 2012 were studied retrospectively. Contrast-specific imaging modes were performed and the ultrasound contrast agent was SonoVue. The thickness of inner, outer and all layers of intestinal walls in the lesion area were observed;the ratio between inner and outer bowel wall thickness was calculated;Limberg classiifcation was determined by Power-Doppler results. Likewise, contrast-enhanced ultrasound was used to evaluate the degree and area of bowel wall enhancement, as well as the changes over time. Variance analysis was applied to compare intestinal wall thickness, arrive time of contrast agent, time to peak and washing time of patients with Crohn′s disease from different Limberg groups, and further comparison between groups were anlysed with LSD-t test. Results The intestinal wall thickness of all 20 patients was larger than 4 mm, while the mean thickness of intestinal walls was (8.8±0.4) mm (range 5.5-12.0 mm);the ratio between inner and outer wall thickness was greater than 1.0. Limberg classiifcation wasⅡin 2 patients,Ⅲin 8 patients andⅣin 10 patients. There were two enhancement patterns shown on contrast-enhanced ultrasound:Pattern 1 in 13 (13/20, 65.0%) patients showing simultaneous enhancement in both inner and outer intestinal walls at the same time. Pattern 2 in 7 (7/20, 35.0%) patients showing outward enhancement from inner to outer wall with a predominance of inner wall. The wall thicknesses of patients with Crohn′s disease from Limberg Ⅱgroup, Limberg Ⅲgroup and Limberg Ⅳgroup were (6.6±0.1), (7.5±0.4) and (10.2±0.4) mm respectively. The thicknesses of inner bowel walls were (3.6±0.6), (5.0±0.2) and (7.3±0.3) mm respectively. CEUS time to peak was (30.5±2.1), (26.9±2.4) and (21.0±1.6) s respectively. The wash-in time of the contrast agent was (18.0±5.7), (10.6±1.0) and (8.7±1.2) s respectively. As the Limberg level increased, the thickness of the entire and inner bowel wall both increased, while CEUS time to peak and wash-in time of the contrast agent became longer. These difference was statistically significant. In addition, the ratio between inner and outter wall thickness also increased as the Limberg level increased, however, the difference was statistically insigniifcant. Likewise, the outer bowel wall thickness and the arrival time of the contrast agent in patients with Crohn′s diseases from different Limberg level groups showed no statistical significance. Conclusions The patients with active Crohn′s disease always showed thickened bowel walls, higher Limberg level and complete or partial enhancement of bowel wall on CEUS. There were some correlations between the above-mentioned ifndings.
10.Treatment of superficial infantile hemangiomas with topical propranolol.

OBJECTIVETo evaluate the efficacy and safety of 1% propranolol ointment in the treatment of superficial infantile hemangiomas (IHs).
METHODSA retrospective chart review was performed on 49 children (34 female and 15 male) with a median age of 4.1 months (range, 1-10 months). A total of 58 superficial IHs were treated with 1% propranolol ointment. Topical propranolol was applied three times daily for a mean duration of 21.1 weeks (range, 5-59 weeks). Changes in the size, texture, and color of the tumor were monitored and recorded at regular intervals. The treatment response was evaluated using a 3-point scale system: good, partial, and no response. Adverse effects after medication were evaluated and managed accordingly.
RESULTSOf the 49 cases, 26 (53.1%) demonstrated good response, 17 (34.7%) showed a partial response, and 6 (12.2%) had no response. The total effective rate was 87.8% . No systemic complication was observed in any of the patients.
CONCLUSIONSTopical therapy with 1% propranolol ointment may be a safe and effective method for the treatment of superficial IHs and can be used as an adjuvant treatment measure during the wait-and-see period.
