2.A Retrospective Study of Chinese Herbal Medicine Combined with Systemic Chemotherapy and/or Regional Arterial Perfusion for Pancreatic Cancer with Liver Metastases.

OBJECTIVETo evaluate the efficacy and safety of Chinese herbal medicine combined with systemic chemotherapy and/or regional arterial perfusion for pancreatic cancer with liver metastases (PCLM).
METHODSWe retrospectively selected 292 patients with PCLM who were treated by Chinese herbal medicine combined with systemic chemotherapy and/or regional arterial perfusion at Tianjin Medical University Cancer Hospital from January 2001 to December 2010. All patients were assigned to the Western medicine treatment group (157 cases) and the integrative medicine treatment group (135 cases). Patients in the Western medicine treatment group were treated with gemcitabine (GEM)-based chemotherapy, and partial of them received regional arterial perfusion. Those in the integrative medicine treatment group additionally took Chinese herbs of clearing heat and eliminating mass for at least 4 weeks. The median survival time (MST) , adverse reactions and the incidence of complications were observed.
RESULTSThere was no statistical significance in general data between the two groups (P > 0.05). There was statistical difference in MST between the two groups (4.8 months vs 5.5 months, P < 0.05). No death occurred during chemotherapy or regional arterial perfusion. All toxic or adverse reactions were tolerable.
CONCLUSIONChinese herbal medicine combined with systemic chemotherapy and/or regional arterial perfusion was effective and safe, and it could be optimally selected as palliative therapy for PCLM.

3.Investigation on therapeutic effect and mechanism of acupuncture at Fengchi (GB 20) and "Gongxue" for treatment of patients with vertebral-basilar insufficiency.

OBJECTIVETo compare the difference of the therapeutic effect of acupuncture and western medicine on the patients with vertebral-basilar insufficiency (VBI), and investigate its mechanism.
METHODSAccording to the different velocity of blood flow recorded by Transcranial Doppler (TCD), sixty patients with VBI were divided into an acupunture group (31 cases) and a western medicine group (29 cases). In the acupuncture group, the patients were punctured at Fengchi (GB 20) and "Gongxue" (Extra). In the western medicine group, the patients were orally given the Flunarizine hydrochlorid capsules. Before and after two-week treatment, the clinical symptoms and related data of TCD were compared, analyzed and evaluated.
RESULTSThe clinical symptoms were obviously improved in the acupuncture group, which was better than the western medicine group (P < 0.05). Acupuncture not only could up-regulate the velocity of vertebral-basilar blood flow (VBF) on the patients with slower VBF of peak-systolic phase (Vs), end-diastolic phase (Vd) and mean value (Vm), but also reduced the VBF on the patients with faster Vs, Vd and Vm. In comparison of the data before and after treatment, there were significantly differences in two groups (P < 0.05), however, there were no difference between the two groups (all P > 0.05). On the index of vascular pulsation (PI), resistance index (RI) and Vs/Vd, there were no difference in both groups before and after treatment (all P > 0.05).
CONCLUSIONAcupuncturing at Fengchi (GB 20) and "Gongxue" has bidirectional and beneficial function of regulation on the VBF, but no active role on the compatibility of vertebral-basilar blood vessel.

4.Studies on drug release from aminophylline konjac glucomannan matrix tablet.

OBJECTIVEIn vitro aminophylline release from matrix tablets with konjac glucomannan (KGM) were studied to elevate the feasibility of KGM used as carrier materials to prepare matrix tablets.
METHODKGM hydrophilic matrix tablets were prepared by direct compression method with aminophylline as the model drug. The effects of test methods, pH values, ionic strength of dissolution media and rotation speeds on drug release were studied by in vitro dissolution experiment.
RESULTThe MDT value tested by Paddle method was less than that tested by Basket method (P < 0.05). Among the rate of drug release in different dissolution media, distillded water is the fastest, pH 6. 8 PBS is the second, 0.1 mol x L(-1) HCL is the slowest. MDT increased with increasing the ionic strength of dissolution media (P < 0.05). MDT decreased with increasing the rotation speed, but the rate of drug release did not increase when the rotation speed was more than 100 r x min(-1) (P > 0.1). The mechanism of drug release were diffusion and erosion.
CONCLUSIONKGM can be used in sustained delivery systems as a good candidate of hydrophilic polymer.

The equilibrium configuration of fluid-phase phospholipid vesicles in aqueous solutions are controlled by bending elasticity. We exploited a method for calculating explicitly the stability of arbitrary symmetric shapes. The morphology of lipid vesicles with topological genus g=2, i.e. with two holes or two handles, is studied. We provided some stable shapes with results explicitly.
6.Surveys on iodine nutritional status of pregnant and lactating women in coastal areas of Zhejiang province

Objective To evaluate the nutritional status of iodine of pregnant and lactating women in coastal and inland areas of Zhejiang province,and to provide scientific basis for iodine supplementation.Methods In 2009,five coastal cities,five coastal counties and three inland areas from Zhejiang province were selected by the cluster sampling method,and one sub-district(township) was randomly selected in each city (county),then one community (village) was randomly selected in each sub-district (township).Thirty pregnant women,thirty lactating women and fifty children aged 8 - 10,were randomly selected in each community(village) as the investigation objects to collect instant urine samples to detect urine iodine,and to collect thirty resident edible salt and some water samples to detect salt iodine and water iodine,if the numbers of water source were more than 5 then 1 water sample was collected by their location of east,south,west,north and center in each community(village),otherwise,the numbers of water samples collected were equal to the number of community (village).Results The medians of salt iodine of coastal city,coastal county and inland area were 28.42,25.29,and 33.24 mg/kg,respectively,and the medians of water iodine correspondingly were 5.41,11.52,and 2.80 μg/L,respectively.The medians of urinary iodine of pregnant and lactating women in Zhejiang province were 148.72 and 161.34 μg/L,respectively,which were less than that of children aged 8 - 10(231.02 μg/L,Z =8.57,8.24,all P < 0.017).The medians of urinary iodine in pregnant women from different areas were ranked in order as inland areas(211.42 μg/L),coastal counties (150.51 μg/L) and coastal cities (123.33 μg/L,Z =2.80,5.31,2.42,all P < 0.017),and in lactating women,the medians of urinary iodine in inland areas( 197.43 μg/L) was higher than that in coastal cities( 139.64 μg/L,Z =4.03,P < 0.017).Conclusions The overall levels of iodine nutrition in lactating and pregnant women in Zhejiang province are in the appropriate level,but that of the pregnant women in the coastal cities is inadequate.
7.Effect of thrombotic burden on the clinical outcome of endovascular recanalization in large vessel occlusion stroke

Objective:To investigate the effect of thrombus burden on the clinical outcome of endovascular recanalization in large vessel occlusive stroke.Methods:Patients with acute anterior circulation occlusion who underwent endovascular treatment within 24 hours after onset in Zhengzhou University People′s Hospital from January 2018 to December 2019 were retrospectively collected. According to the clot burden score (CBS) of DSA, total objectives were divided into CBS≥6 group (24 cases) and CBS<6 group (38 cases). Clinical data of the two groups were collected and the modified Rankin scale (mRS) was used to evaluate the clinical outcome at 90 days after surgery. Independent sample t-test, Wilcoxon rank sum test and χ 2 test were used to compare the clinical data between the two groups. Independent risk factors affecting the clinical outcome were analyzed by binary logistic regression. Results:There were no statistically significant differences in basic demographic data, stroke risk factors and other factors between the CBS≥6 group and CBS<6 group ( P>0.05).The proportion of using tirofiban after surgery in the CBS≥6 group (63.2%, 24/38) was lower than that in the CBS<6 group (87.5%, 21/24) (χ2=4.380, P=0.044). The discharge NIHSS score of the CBS≥6 group was [5.0 (3.3, 7.8) points] lower than CBS<6 group [8.5 (1.8, 14.5) points] ( Z=5.221, P=0.022). The proportion of postoperative mRS 0-2 was (91.7%, 22/24) in the CBS≥6 group higher than CBS<6 group(39.5%, 15/38) (χ2=20.486, P=0.001), there were no statistically significant differences between the two groups ( P<0.05). The results of binary logistics regression analysis showed the CBS groups (OR=0.042, 95%CI 0.007-0.244 , P=0.001) was an independent risk factor affecting good outcome. Subgroup analysis of whether tirofiban was used or not showed there was no statistically significant difference in clinical prognosis between the two groups ( P>0.05). Conclusions:The clinical outcome of CBS≥6 group is significantly better than that of CBS<6 group, and patients with small thrombus burden are more likely to get a good clinical outcome of 90 days.
8.Ultrasonic bone mineral density of calcaneus in 1 816 healthy subjects in Guangxi autonomous region.

OBJECTIVETo study the relationship between ultrasonic bone mineral density (BMD) of calcaneus and age, height, and body weight, and to establish the normal reference value for stiffness index (SI) of healthy subjects in Guangxi autonomous region and provide scientific foundation for the diagnosis and prevention of osteoporosis.
METHODSSI of calcaneus in 1 816 healthy adults aged 11-90 years (775 men, 1 041 women) were measured with quantitative ultrasound. According to their ages, all the men and women were divided into 13 groups by 5-year age group.
RESULTSSI reached peak values in 21-25 year group in men (range: 111.45 +/- 21.19) and in 16-20 year group in women (range: 101.26 +/- 17.51). Then the SI value began to decline with aging. The SI featured by a typical dual-peak curve in women and the decrease rate of SI was faster in women than in men over 61 years. Multivariate linear regression analysis showed significantly negative correlation between SI and age (P < 0.001), positive correlation between SI and body weight (P < 0.05 for men; P < 0.01 for women), and positive correlation between SI and height in women (P < 0.01).
CONCLUSIONSI correlates with age, height, and weight. The acquired SI value may provide a reference for the diagnosis of osteoporosis.

9.Mechanisms of morphine-evoked changes of intracellular calcium in primarily cultured hippocampal neurons.

AIMIn order to explore the neurobiological mechanism of morphine addiction and treatment methods, the acute and chronic effects of morphine on the intracellular free calcium concentration ([Ca2+]i) in cultured hippocampal neurons were investigated.
METHODSChanges of [Ca2+]i induced by morphine in primarily cultured hippocampal neurons were measured by confocal laser scanning microscopy using Ca(2+) -sensitive dye fluo-4 as the calcium fluorescent probe.
RESULTSMorphine actually induced the increase in [Ca2+]i of hippocampal neurons. This process could be blocked by naltrindole (delta opioid receptor antagonist) pretreatment, but not by CTOP (micro opioid receptor antagonist) pretreatment. Pretreatment of the cells with thapsigargin almost completely blocked morphine-evoked response; while pretreatment of the cells with verapamil partially inhibited this response. After exposure to 100 micromol/L morphine for 24 h, intracellular [Ca2+]i increased and the increase could be intensified after adding 10 micromol/L naloxone to the medium.
CONCLUSIONMorphine induced the release of Ca2+ is mainly from inositol 1, 4, 5-trisphosphate (IP3) sensitive stores in hippocampal neuron of rats through activation of delta2 subtype opioid receptor.

10.Surveillance on hemorrhagic fever with renal syndrome in Jiangsu province from 1996 to 2000.

OBJECTIVETo analyze the incidence rate, animal hosts, and human inapparent infection rate from surveillance data collected in Jiangsu province from 1996 to 2000 and the risk factors of hemorrhagic fever with renal syndrome (HFRS).
METHODSData on the incidence rate was collected from the whole province to describe the epidemiologic characteristics of HFRS. Data on the density of rats, proportion of rats with virus, index of rats with virus and the human inapparent infection rate were collected in special areas according to the standardised protocol in the project.
RESULTSThe incidence rate of HFRS reduced continuously in Jiangsu province, with a 42.17% reduction from 1996 to 2000. The ratio between the incidence rate of autumn-winter peak and spring peak had also reduced. The main host in spring was Rattus norvegicus (with a density of 3.07%), while that in autumn was Apodemus agrarius (with a density of 4.64%). The density of main hosts and mixed species of rats had all reduced during the five years of observation. The proportions of Rattus norvegicus, Apodemus agrarius, Mus musculus with virus were relatively high in spring and autumn. The index of Rattus norvegicus and Mus musculus with virus in spring were 0.025 5 and 0.028 4 respectively, while that of Apodemus agrarius with virus in autumn was 0.030 2. The average human inapparent infection rate was 4.73% in the five years.
CONCLUSIONThe incidence rate of HFRS in Jiangsu reduced significantly from 1996 to 2000, but the risk factors still widely existed. Suggesting that the surveillance program needs to be carried out steadily in Jiangsu.
