1.Optimal treatment for malignant glaucoma

Abstract?AlM:To investigate the choice of different treatments for malignant glaucoma.? METHODS: ln this retrospective case series, 21 malignant glaucoma patients ( 21 eyes ) admitted in Wuhan General Hospital of Guangzhou Military Command from May 2012 to May 2013 were analyzed. Sixteen eyes ( 76%) developed malignant glaucoma after filtration surgery, 3 eyes ( 14%) after EX - PRESS glaucoma filtration device, 2 eyes ( 10%) after glaucoma filtration Ahmed valve implantation. Main Outcome of corrected visual acuity, intraocular pressure ( lOP ) , anterior chamber depth and complications were detected.?RESULTS: lOP recovered by drug control in 13 eyes, anterior chamber depth. Four eyes were treated by vitreous water- bag aspiration combined with anterior chambers reconstructing. Two eyes were treated by cataract extraction and intraocular lens implantation. Two eyes were treated by posterior capsule excision combined with anterior vitrectomy. lOP before and after treatment was 29. 81±4. 98, 12. 71±3. 77mmHg, respectively (P=0. 00). Anterior chamber depth before and after treatment was 0.41± 0. 34, 2. 13 ± 0. 54mm, respectively (P = 0. 00). Corrected visual acuity before treatment was 0. 19 ± 0. 17, after treatment was 0. 20±0. 16 (P= 0. 36). Except for vitreous hemorrhage in 1 eye, there were no ocular or systemic adverse events observed in all patients.? CONCLUSlON: lt is good to diagnose malignant glaucoma in early period, and treated it step by step. For this can reduce lOP and restore anterior chamber.
2.Long-Term Following Up and Curative Effect of Continuous Stosstherapy with Cyclophosphamide in Treatment of Refractory Nephrotic Syndrome in Children in Zhuang Nationality

0.05).2.Two days after treatment,total protein(TP) and albumin(Alb) of hemotoplasma were significantly higher than before treatment(all P0.05).3.Toxic side effects: gastrointestinal reaction was seen in 20 cases ((52.63%)),reduction of WBC in 2 cases(5.26%),baldness in 38 cases(100%) and heart impairment in 1 case(2.63%).The sperm test and menstrual onset were normal in 18 cases,whose ages ≥ 8 years at beginning of treatment,who were clinically cured and whose ages ≥17 years at the time of last following up,7 of them who had married could have children.Conclusions Combining CTX(conti-)(nuous) stosstherapy with prednisone standard scheme treated the children in Zhuang nationality with NS,We could got a good curative effect.By using this treatment,not only the plasma TP and Alb level could be increased rapidly and the urine protein could be conversed to negative quickly.The accumulated amount of CTX was less than 80 mg/kg,but the treatment was safety in the recent term, and no impairment of sexual gland was seen at long-term following up.

Background Stromal cell-derived factor-1_?(SDF-1_?)has been demonstrated to be essential for stern cell mobilization/homing.Recent evidence indicates that SDF-1_? has been expressed in injured carotid arter- ies.Besides,high SDF-1_? plasma levels are clinically associated with stable coronary artery disease.Objective To investigate whether SDF 1 involves in mobilization of endothelial progenitor cells(EPC)and reendothelialization after vascular injury.Methods SDF-1_? was detected by RT-PCR and Western blot in carotid arteries of mice at different time points after wire-induced injury.SDF-1_? determination in peripheral blood samples and BM was per- formed by SDF-1_? enzyme-linked immunosorbent assay(ELISA)kit.EPC in peripheral blood collected at different time points after vascular injury were quantified by flow cytornetry.In subgroup,blocking SDF-1 rnonoclonal anti- body was injected,peripheral blood EPC were quantified after vascular injury and reendothelialization of injured ar- teries was determined 14 days later.Results Expression of SDF-1_? was evident at day 1,and peaked at day 3 after arterial injury.A rise in plasmatic concentration of SDF-1_? and a significant reduction of SDF-1_? in bone marrow concentration was noticed at all time points following injury.The amount of circulating EPC was increased shortly after induction of vascular injury and persisted up to 7 days(P
4.EMG characteristics of lower extremities during movement of white crane spreads its wings in Tai Chi Quan

Objective To analyze how the athletes control postural balance through observing their joint angular motion change,center of pressure (COP) change and EMG activities of lower extremities for both athletes and beginners when performing the movement of white crane spreads its wings in Tai Chi Quan. Methods Twenty subjects were divided into 2 groups as 10 athletes and 10 beginners of Tai Chi Quan in each. When performing the movement of white crane spreads its wings, their EMG activities of 10 skeletal muscles, two-dimensional angular motion of 3 large joints in bilateral lower extremities, and COP displacements in lateral and anterior-posterior directions were recorded. Each performing trial took 8 seconds and repeated 5 times. All data were statistically processed for EMG, motion and balance analysis, and the EMG data were normalized to compare the differences between the athletes and beginners of Tai Chi Quan. Results The EMG activities of tibialis anterior(Ta), biceps femur(bF), gluteus medium(Gm) in support leg of athletes were significantly greater than those of the ipsilateral muscles in support leg of beginners, and the same was the EMG activities of gastrocnemius (G) and rectus femur(rF) in virtual leg of athletes compared with the beginners. The maximal average angular motion of 3 large joints and the coxa -joint motion of support leg in athletes were both significantly greater than that in beginners, but the anterior-posterior COP displacement in athletes was significantly smaller than that in beginners. Conclusions When performing the movement of white crane spreads its wings in Tai Chi Quan, athletes could oppose the COP change by enhancing muscle activities to maintain the postural balance, while beginners couldn’t make such active muscle activities against the uncontrolled anterior-posterior COP displacement. This may preliminarily explain the reason why Tai Chi Quan exercise can improve the muscle strength in lower extremities and enhance the ability of postural control.
5.Hepatic VX2 tumor after portal vein occlusion in rabbits:evaluation with DSA

Objective To study the value of DSA for hepatic vascular anatomy,and to evaluate the efficacy of portal vein occlusion in rabbits with hepatic VX2 tumor.Methods Twenty New Zealand white rabbits were randomly divided into two groups with 10 in each group,including test group A and positive control group B of ham operation.For the test group A,portal branch ligation(PBL)was performed for the left external branch after 3 weeks of the tumor implantation to the left external lobe.Two weeks later,the DSA of hepatic artery and portal vein were performed in all of the rabbits.Results The total displaying effectiveness of the branches of hepatic artery by DSA was better than that by vascular perfusion.There was hypovascular blood supply to hepatic artery implantation of the tumor in the test group A,comparing with that of the group B.Conclusion DSA can clearly display spacial details of the hepatic vascular anatomy in rabbits,and play an important role in post-procedual evaluation of the portal vein occlusion in rabbits.
6.Observation on therapeutic effect of cervical spondylosis of vertebral artery type treated with both acupuncture and mild moxibustion.

OBJECTIVETo observe the clinical therapeutic effects on cervical spondylosis of vertebral artery type treated with both acupuncture and mild moxibustion and investigate the mechanism of action.
METHODSSixty cases were randomly divided into the combined therapy group of acupuncture and mild moxibustion (combined therapy group) and the acupuncture group, 30 cases for each group. In combined therapy group, Baihui (GV 20), Fengchi (GB 20) and cervical Jiaji (EX-B 2) were punctured, and Baihui(GV 20), Dazhui (GV 14), Mingmen (GV 4), Shenque (CV 8) and Guanyuan (CV 4) were treated with mild moxibustion; in acupuncture group, acupuncture was simply applied, the acupoints selection and manipulation were the same as those in the combined therapy group. The average velocity meter and the resistive index of vertebral and basal arteries were observed before and after treatment by TCD examination, the therapeutic effects and clinical symptom-sign scores were observed too.
RESULTSThe total effective rate was 93.3% (28/30) in combined therapy group, and 73.3% (22/30) in acupuncture group. The therapeutic effect in combined therapy group was superior to that in acupuncture group (P < 0.05). The improvements of symptom-sign scores in two groups after treatment were more significant than that before treatment, and the score in the combined therapy group was superior to that in acupuncture group (P < 0.05). The average velocity meter and the resistive index of vertebral and basal arteries after treatment were improved in both groups, in which, the improvement in combined therapy group was more apparent (all P < 0.05).
CONCLUSIONThe therapeutic effects on cervical spondylosis of vertebral artery type treated with both acupuncture and mild moxibustion are preferable; by this therapy, the average velocity meter of vertebral and basal arteries are accelerated and the resistive index is decreased.

8.A survey on adolescent scoliosis in Guangzhou

Objective To investigate the prevalence of adolescent scoliosis in Guangzhou for development of effective prevention and treatment program to the disease. Methods From November2007 to July 2009, 30 142 students between 7 to 20 years old in primary and junior middle schools received physical check-up for detection of scoliosis through physical and radiographic examination.Results 211 cases were diagnosed as scoliosis (Cobb angle≥10°) , with the prevalence rate as 0.70%. 192 patients with idiopathic scoliosis (IS) were detected, accounting for 91.00%. There were 19 cases of congenital scoliosis, accounting for 9.00%. Sex ratio of scoliosis was 180/31. The prevalence of scoliosis was significantly lower in boys than that in girls (x2= 112.332, P<0.001 ).Conclusion The crude prevalence of adolescent scoliosis was 0.70% in Guangzhou with majority of idiopathic scoliosis. Investigation on scoliosis among school-age population seemed to be important for the purposes of early diagnosis, selection of effective prevention and treatment.
9.A medium-term analysis on of therapeutic effects of locking proximal humerus plate for the treatment of comminuted fractures of proximal humerus.

OBJECTIVETo investigate the medium-term curative effects of locking proximal humerus plate for the treatment of comminuted fractures of proximal humerus, and provide evidences for the clinical practice.
METHODSFrom August 2005 and April 2008, 23 patients with comminuted fractures of proximal humerus were treated with locking plates, including 12 males and 11 females, aged 27 to 76 years old (averaged 51.5 years old). There were 18 cases of traffic accident injuries, 4 cases of falls injuries, and 1 case injured after heavy pressure. According to Neer classification, 11 cases were three-part fractures, and 12 cases were four-part fractures. Outcomes were assessed with radiography and the Constant-Murley (C-M) shoulder evaluation.
RESULTSAll the patients got primary healing of incisions. Twenty-three patients were followed up, and the duration ranged from 17 to 49 months, with an average of 35.25 months. Twenty patients had fracture healing during 4 to 7 months after operation. There was no significant differences among 3, 6 and 12 months after operation in C-M scoring. The average C-M score was (79.85 +/- 17.23) points (38 to 100 points) at the 12th month after operation, 8 cases got an excellent result, 8 good, 5 fair, and 2 poor. In the LPHP plus bone graft group 6 cases got an excellent result, 4 good, 3 fair, and 1 poor; in LPHP fixation group 2 excellent, 4 good, 2 fair,and 1 poor.
CONCLUSIONThe medium-term curative effect of the locking proximal humerus plate in the treatment of proximal humeral fractures is significant. For the comminuted fractures of proximal humerus combined with osteoporosis and bone defects, bone graft should be performed routinely.

10.New method of left ventricular intubation in anaesthetised rats.

AIMTo search for a new method of left ventricular intubation in rat.
METHODSThe wire of percutaneous transluminal coronary angioplasty (PTCA) was put into carotid artery through external carotid artery. Then supported by guide wire, the PE50 tube was advanced into left ventricle.
RESULTSLeft ventricular intubation with PTCA guide wire could be performed in 30 rats and successfully repeated in 27 animals.
CONCLUSIONThe new left ventricular intubation technology in rats is simple and provides a reproducible method.
