1.Percutaneous screw fixation for the treatment of pelvic fractures under C-arm fluoroscopy.

OBJECTIVETo investigate the applications of percutaneous screw fixation for the treatment of pelvic fractures and its related surgical considerations.
METHODSFrom June 2010 to June 2012,19 patients with pelvic fractures were treated with percutaneous hollow screws. There were 13 males and 6 females, with an average age of 41 years (ranged from 22 to 58 years). Fractures were caused by traffic accidents in 11 cases, by falling down from high place in 8 cases. Based on the Tile classification, there were 15 cases of Tile C type and 4 case of Tile B type. The indexes such as screw inserting time, intraoperative blood loss, complications, functional recovery and reduction conditions were observed. Fixation methods included sacroiliac screws, cannulated screw fixation of the pubic ramus and cannulated screw fixation of the pubic symphysis separation.
RESULTSAnatomical reduction achieved in 7 cases, satisfactory reduction 11 cases, and unsatisfactory reduction 1 case. Union time of fracture union ranged from 8 to 12 weeks (mean 10 weeks). Wound infection,ununion of fracture and nerve injuries were not found. According to the Majeed standards, 12 patients obtained an excellent results, 6 good and 1 fair.
CONCLUSIONPercutaneous screw fixation for the treatment of pelvic fractures under fluoroscopy has several advantages such as less trauma, less blood loss, fewer rates of complications, reliable fixation and no blood transfusion, which can reconstruct the stability of the pelvic ring, but it needs adequate preoperative preparation and high requirements for the surgeon.

2.Comparison of early clinical outcomes between mobile-bearing and fixed-bearing total knee arthroplasty for the valgus knee.

OBJECTIVETo compare the early clinical outcomes of primary total knee arthroplasty in the valgus knee between mobile-bearing prosthesis and fixed-bearing prosthesis.
METHODSFrom January 2011 to December 2013, 17 patients (23 knees) treated by the same surgeon were selected for a retrospective study in the First Hospital of Jilin University, including 2 males and 15 females with a mean age of 61.5 years old (48 to 75 years). The pre-operative diagnosis included osteoarthritis (14 patients, 19 knees) and rheumatoid arthritis (3 patients, 4 knees). The patients with valgus deformity were divided into group A and group B. The patients in group A were treated with the fixed-bearing prosthesis (9 patients, 12 knees), and the patients in group B were treated with the mobile-bearing prosthesis (8 patients, 11 knees). The Knee Society Score (KSS), Hosptial for Special Surgery (HSS), Western Ontario MacMaster (WOMAC), the range of motion (ROM) and femorotibial angle were collected at pre-operation and post-operation follow-up for statistical analysis.
RESULTSAll the patients were followed up, and the duration ranged from 6 to 36 months (mean 25 months). The valgus deformity was corrected in all patients, and there were significant differences for all patients between pre-operation and post-operation at the latest follow-up with regard to the KSS knee score, function score, HSS score, WOMAC score, ROM and femorotibial angle (P<0.01). There were no significant differences in KSS knee score, function score, HSS score, WOMAC score, improvement in ROM and femorotibial angle between two groups at the latest follow-up. However there were significant differences in ROM (P<0.05) between fixed-bearing group (101.8±8.8)° and mobile-bearing group (108.4±7.2)° at the latest follow-up. No case with spin-out of mobile bearing was observed. There were no complications in any patient, such as infection, common peroneal nerve injury, dislocation and instability. The X-ary imaging showed no osteolysis or implant loosening.
CONCLUSIONThe early clinical outcomes of primary total knee arthroplasty by using mobile-bearing prosthesis and fixed-bearing prosthesis are satisfactory for the treatment of the valgus knee, and the short term clinical outcomes of mobile-bearing TKA and fixed-bearing TKA are similar.

3.Temporary vena cava filter for trapping thrombus in prevention of pulmonary embolism

Objective To assess temporary filter in the inferior vena cava (IVC) for trapping thrombus and preventing pulmonary embolism in patients with deep vein thrombosis. Methods In 58 cases of deep vein thrombus in single lower extremity, Antheor temporary filter (ATF) was introduced into IVC prior to anticoagulation and/or vascular intraluminal procedures. Results All IVC filters were successfully introduced. No symptomatic pulmonary embolism was documented. ATF was removed after a mean of 12.0?2.0 days. Thrombus was trapped in 46 cases (79.3%). Thrombus more than 1 cm in size was trapped in 2 cases. Conclusion Temporary filter was safe and effective in prevention of pulmonary embolism.
4.Design and development of the DSA digital subtraction workstation.

According to the patient examination criterion and the demands of all related departments, the DSA digital subtraction workstation has been successfully designed and is introduced in this paper by analyzing the characteristic of video source of DSA which was manufactured by GE Company and has no DICOM standard interface. The workstation includes images-capturing gateway and post-processing software. With the developed workstation, all images from this early DSA equipment are transformed into DICOM format and then are shared in different machines.

5.Effect of five fungicides on growth of Glycyrrhiza uralensis and efficiency of mycorrhizal symbiosis.

In order to obtain the fungicides with minimal impact on efficiency of mycorrhizal symbiosis, the effect of five fungicides including polyoxins, jinggangmycins, thiophanate methylate, chlorothalonil and carbendazim on the growth of medicinal plant and efficiency of mycorrhizal symbiosis were studied. Pot cultured Glycyrrhiza uralensis was treated with different fungicides with the concentration that commonly used in the field. 60 d after treated with fungicides, infection rate, infection density, biomass indexes, photosyn- thetic index and the content of active component were measured. Experimental results showed that carbendazim had the strongest inhibition on mycorrhizal symbiosis effect. Carbendazim significantly inhibited the mycorrhizal infection rate, significantly suppressed the actual photosynthetic efficiency of G. uralensis and the most indicators of biomass. Polyoxins showed the lowest inhibiting affection. Polyoxins had no significant effect on mycorrhizal infection rate, the actual photosynthetic efficiency of G. uralensis and the most indicators of biomass. The other three fungicides also had an inhibitory effect on efficiency of mycorrhizal symbiosis, and the inhibition degrees were all between polyoxins's and carbendazim's. The author considered that fungicide's inhibition degree on mycorrhizal effect might be related with the species of fungicides, so the author suggested that the farmer should try to choose bio-fungicides like polyoxins.

6.Clinical features of sudden hearing loss in children.

OBJECTIVE:
To investigate the clinical features and prognosis of sudden hearing loss in children, so that to guide clinical diagnosis and treatment.
METHOD:
A retrospective review of medical records of 12 hospitalized children (21 ears) suffering from sudden hearing loss was conducted.
RESULT:
The study comprised 8 males and 4 females with a mean age of 6.28 years old, among which 25% had a unilateral hearing loss and 75% exhibited bilateral hearing loss. The mean onset time was 5.42 days. Four children had tinnitus and 3 patients showed dizziness. For the possible etiology. 2 cases had mumps. 6 cases had definite upper respiratory infections history, one experienced trauma, and another one took ototoxic drugs. Five patients were proved to come down with the large vestibular aqua duct syndrome by the CT scan. Among the 21 ears. 7 ears were diagnosed severe hearing loss and 14 ears were diagnosed profound hearing loss. After the treatment of 7 days, 6 ears were cured, one car showed effective change. and 14 cars came out to be ineffective, with the total effective rate of 33.3%. After the 14 days' treatment, there was no improvement for 5 patients.
CONCLUSION
CT scan should he performed on all of the children with hearing loss to exclude the inner ear malformation. Most of the children with sudden hearing loss underwent some clear etiology, showing more severe hearing loss and had a poorer prognosis.

7.Changes in canine position during maxillary molar distalization with mini-implant and CD appliance: an in vitro study

Objective:
To investigate changes in the three⁃dimensional position of the maxillary canine during the
distal movement of the maxillary first molar by a mini⁃implant combined with a CD appliance.
Methods:
Ten typodont
models of class II malocclusion were selected, and one side was randomly chosen as the experimental group. The CD ap⁃
pliance was bonded to the maxillary canine and first molar of the experimental group, and 1.2 mm stainless steel wire
was bent as the anchorage, which was fixed on the model to simulate mini⁃implants implanted in the zygomatic alveolar
ridge. Then, 180 g orthodontic force was applied to the canine of the experimental group; the other side was recognized
as the control group and was not used for strengthening. Six mini⁃implants were implanted in different parts of the mod⁃
el and used as a reference before and after the experiment. The models were placed in an incubator and heated at 56 ℃
in a water bath for 2 min. The models were scanned before and after thermostatic water bath treatment with a 3⁃shape
scanner. Then, the digital models were overlapped through the reference points, and the positions of the canines and
first molars were measured before and after the experiment.
Results:
The sagittal movement distance of the first molar in the experimental group was ( 0.25 ± 0.33) mm, and the vertical movement distance was (0.25 ± 0.28 )mm, which was
significantly different from the control group (P < 0.05), while the transverse change was not significantly different (P >
0.05). There was no significant difference in the root position of the first molar in the experimental group (P > 0.05).
The lateral and vertical displacement distances of the maxillary canine crown in the experimental group were (4.03 ±
2.11) mm and (1.86 ± 1.01) mm, respectively, which were significantly different from those in the control group (P <
0.001), while the sagittal changes showed no significant differences (P > 0.05). In the experimental group, there was no
significant difference in the position of the apex of the maxillary cusp (P > 0.05).
Conclusion
Our in vitro study
showed that the maxillary canines inclined buccally accompanied by a small amount of intrusion during molar distaliza⁃
tion by a mini⁃implant combined with a CD appliance.
8.Comparison of efficacy and safety of pegylated interferon alfa-2a or adefovir dipivoxil monotherapy with combination therapy in HBeAg positive chronic hepatitis B patients

Objective To compare the clinical efficacy and safety of pegylated interferon α-2a (Peg IFN α-2a) or adefovir dipivoxil(ADV) monotherapy and their combination therapy in HBeAg positive chronic hepatitis B (CHB) patients. Methods An open randomized controlled multicenter clinical trial was performed. One hundred and twenty cases with CHB were divided into 3 groups: Peg IFN α-2a monotherapy (group A), ADV monotherapy (group B) and Peg IFN α-2a plus ADV combination therapy (group C). The virological response (VR), serological response (HBeAg, HBsAg clearance and seroconversion), biochemical response (BR) and sustained response (SR) were tested at week 24 and 48 of therapy and week 48 of follow-up after end of treatment (EOT) for'evaluation of therapeutic effects, safety and drug resistance. The efficacy was compared using X2 test. Results At week 48 of treatment, the VR (HBV DNA ≤500 copy/mL) rates were 36. 8%(14/38), 37. 5%(15/40) and 62. 9% (22/35), respectively in groups A, B and C; that in group C was higher than those in groups A and B (X2 = 4. 933, 4. 801, respectively; both P < 0. 05); HBeAg seroconversion rates in three groups were 44. 7% (17/38), 17. 5% (7/40) and 51. 4% (18/35), respectively. At week 48 of follow-up,SR rates in three groups were 34. 2%(13/38), 15. 0%(6/40) and 48. 6% (17/35), respectively; those in groups C and A were higher than that in group B (X2 = 9. 894,P<0. 01;X2 =3. 903, P<0. 05, respectively). Conclusions VRs at week 24 and 48 of Peg IFN α-2a plus ADV combination therapy are better than Peg IFN α-2a or ADV monotherapy. SRs at week 48 of follow-up after Peg IFN α-2a monotherapy and combination therapy are both better than ADV monotherapy.
9.Construction and identification of 2 secreted human GAD65 fragment DNA vaccines.

OBJECTIVE:
To construct and identify 2 secreted human GAD65 fragment DNA vaccines.
METHODS:
The GAD(190-315), GAD(490-570) cDNA and hIL-2 signal peptide cDNA were linked through overlapping PCR, respectively. The fusion gene was cloned into eukaryotic expression vector pBudCE4.1. After the DNA vaccine being determined to contain the correct target nucleotide sequence, the expression of fusion proteins was detected by Western blot.
RESULTS:
The nucleotide sequence of the cloned gene was the same as the reported sequence, and their open reading fragment was correct. The products of these DNA vaccines were expressed and secreted in eukaryotic cell using Western blot.
CONCLUSION
The pBudCE4.1/SGAD(190-315) and pBudCE4.1/SGAD(490-570) secreted human GAD65 fragment DNA vaccines were successfully constructed, which is a foundation for immune prevention of type 1 diabetes.

10.Carbon monoxide inhalation ameliorates conditions of lung grafts from rat brain death donors.

BACKGROUNDSuccessful lung transplantation has been limited by the scarcity of donors. Brain death (BD) donors are major source of lung transplantation. Whereas BD process induces acute lung injury and aggravates lung ischemia reperfusion injury. Carbon monoxide (CO) inhalation at 50-500 parts per million (ppm) can ameliorate lung injury in several models. We examined in rats whether CO inhalation in BD donor would show favorable effects on lung grafts.
METHODSRats were randomly divided into 4 groups. In sham group, donor rats received insertion of a balloon catheter into the cranial cavity, but the balloon was not inflated. In BD-only group, donor rats were ventilated with 40% oxygen after BD confirmation. In BD+CO250 and BD+CO500 groups, donor rats inhaled, after BD confirmation, 250 ppm or 500 ppm CO for 120 minutes prior to lung procurement, and orthotopic lung transplantation was performed. The rats were sacrificed 120 minutes after the lung transplantation by exsanguination, and their blood and lung graft samples were obtained. A total of 8 rats fulfilling the criteria were included in each group.
RESULTSThe inhalation decreased the severity of lung injury in grafts from BD donors checked by histological examination. CO pretreatment reversed the aggravation of PaO2/FiO2 in recipients from BD donors. The CO inhalation down-regulated pro-inflammatory cytokines (TNF-alpha, IL-6) along with the increase of anti-inflammatory cytokine (IL-10) in recipient serum, and inhibited the activity of myeloperoxidase in grafts tissue. The inhalation significantly decreased cell apoptosis in lung grafts, inhibiting mRNA and protein expression of intercellular adhesion molecule-1 (ICAM-1) and caspase-3 in lung grafts. Further, the inhalation activated phosphorylation of p38 expression and inhibited phosphorylation of anti-extracellular signal-regulated kinase (ERK) expression in lung grafts. The effects of CO at 500 ppm were greater than those at 250 ppm.
CONCLUSIONSCO exerts potent protective effects on lung grafts from BD donor, exhibiting anti-inflammatory and anti-apoptosis functions by modulating the mitogen-activated protein kinase (MAPK) signal transduction.
