2. Association between single nucleotide polymorphism of 309 T/G in murine doubleminute 2 gene and the susceptibility of non-small cell lung cancer: A Meta-analysis

Objective: To evaluate the association between a single nucleotide polymorphism (SNP) of 309 T/G in the promoter of murine doubleminute 2 (MDM2) gene and the susceptibility of non-small cell lung cancer (NSCLC). Methods: A computer-based online search was performed by using Cochrane Library, PubMed, EMBase, China National Knowledge Infrasrtucture (CNKI), Wanfang database and VIP database. The case-control studies were selected according to defined inclusion and exclusion criteria. After quality evaluation and data abstraction, a meta-analysis was performed by using STATA 12.0 software. The odds ratio (OR) of the association between MDM2 SNP 309T/G and NSCLC susceptibility was calculated. Then the subgroup analysis, sensitivity analysis and publication bias test were performed. Results: A total of 8 case-control studies were eligible for this analysis, including 5 343 NSCLC patients and 6 652 healthy controls. Meta-analysis showed that MDM2 SNP 309GG could significantly increase the risk of NSCLC [TT vs GG, OR = 0.77, 95% confidence interval (CD = 0.62-0.96; GT vs GG, OR = 0.83, 95% CI = 0.75-0.92; TT+GT vs GG, OR = 0.82, 95% CI = 0.75-0.91]. In the subgroup analysis of ethnicity, MDM2 SNP 309GG could significantly increase the risk of NSCLC for Asian people [TT vs GG, OR = 0.62, 95% CI = 0.52-0.73; GT vs GG, OR = 0.78, 95% CI = 0.67-0.90; TT vs GG+GT, OR = 0.71, 95% CI = 0.59-0.86; TT+GT vs GG, OR = 0.73, 95% CI = 0.63-0.84]. In the subgroup analysis of gender, MDM2 SNP 309GG could significantly increase the risk of NSCLC for female population [TT vs GG, OR = 0.70, 95% CI = 0.54-0.91; GT vs GG, OR = 0.69, 95% CI = 0.54-0.90; TT+GT vs GG, OR = 0.70, 95% CI = 0.55-0.89]. Conclusion: MDM2 SNP 309GG may be a potential biomarker for NSCLC risk, particularly for Asian people and women. Copyright© 2014 by Tumor.

4.Treating maxillary dental arch crowding and protrusion of the patient with mandibular molars missing by implanting miniscrew anchorage.

OBJECTIVETo study the clinical effectiveness of implanting miniscrew serving as anchorage instead of molars in treating maxillary dental arch crowding and protrusive patients mandibular molars complete missing.
METHODSEight adult patients aged from 22 to 38, whose maxillary dental arch were crowding and protrusion, with mandibular molars missing were chosed. At the missing side, a miniscrew was implanted on the buccal surface, 11-13 mm away from the distal end of the second premolar. After implanting, a self-made miniscrew traction cap was attached to its supergingival section by keyway retention. Premolars and anterior teeth were tracted in turn to distal end. Intermaxillary traction II was made necessarily on the traction cap.
RESULTSSuccessful results were acquired after treating together with normal overbite, overjet and right occlusion relation. It took 24 months in the longest course, 15 months in the shortest course and 20.8 months on average.
CONCLUSIONThe implanting miniscrew anchorages could be used in maxillary dental arch crowding and protrusive patients with single molars complete missing.

5.Clinical application of maxillary sinus lift with Summers osteotome.

OBJECTIVETo introduce and evaluate the procedure and the effect of maxillary sinus lift with closed technique by Summers osteotome, bone grafting and simultaneous implant placement.
METHODS66 cases with severely resorbed alveolar bone in maxillary posterior region received sinus lift with Summers osteotome, simultaneously bone grafting and implants placement. The final restoration was finished at 6 months postoperatively.
RESULTSThe operation procedure were eventless in the 66 cases, the sinus floor were elevated by 2-5 mm, three-dimensional reconstruction of the CT scan pictures showed the smooth dome profile of the lifting sites and no signs of laceration on the membrane, and there were no maxillary antritis after operation. After 6 months, no significantly bone graft resorption and good osseointegration were noticed in X-ray imaging. The final restoration was finished at this time. 12-24 months after the restoration, all implants inserted were remain, the hard and soft tissue were healthy, prosthesis were stable and functioned. X-ray showed good osseointegration in the lifting sites, the vertical resorption around the implants were less than 1 mm.
CONCLUSIONWith properly use of Summers osteotome, scraps of the bone in the implant sockets can be pushed into the sinus, these autogenous bone scraps were in favor of the osseogenesis and the sinus floor can be easily elevated by the method with very infrequent complications. It enlarged indication of dental implants and avoided operation of harvesting autogenous bone in other site. The method is simple and valuable to clinical application.

6.Effect of siRNA targeting c-Myc and VEGF on human colorectal cancer cells.

OBJECTIVETo investigate the biological behavioral effects of specific siRNA expression plasmids targeted against c-Myc and vascular endothelial growth factor (VEGF) on human colorectal cancer cell line Volo.
METHODSThe expression plasmids with small interfering RNA (siRNA) aiming at c-Myc and VEGF were designed and constructed respectively, then transfected into Volo cells by eukaryocyte transfection technique. The protein expressions of c-Myc and VEGF were detected by Western blotting. Cellular proliferation, apoptosis, cycle distribution and invasion character were analyzed by tetrazolium bromide colorimetry (MTT), flow cytometry (FCM), TUNEL assay and matrigel invasion assay respectively.
RESULTSEnzymatic digestion and DNA sequencing confirmed that the c-Myc and VEGF specific siRNA expression plasmids were constructed successfully. After plasmids were transfected into cells, the protein expressions of c-Myc and VEGF were significantly down-regulated respectively as compared with control group (P<0.01). The cellular proliferation inhibitory rates in c-Myc siRNA group, VEGF siRNA group and c-Myc+VEGF group were (59.20+/-5.05)%, (32.31+/-3.48)% and (75.81+/-7.89)% respectively, which were higher than that in control group [(6.80+/-1.45)%] (all P<0.05). The cell apoptosis rate in above 3 groups were (40.50+/-4.37)%, (21.30+/-2.98)% and (62.59+/-9.66)% respectively, which were higher than that in control group [(2.90+/-0.36)%] (all P<0.05). The cell invasion rates in VEGF siRNA group and c-Myc+VEGF siRNA group were (7.34+/-3.65)% and (2.80+/-1.02)%, which were lower than that in control group [(18.57+/-7.46)%] (P<0.05). The effect of c-Myc+VEGF siRNA group was greater.
CONCLUSIONSThe specific siRNA efficiently silences the expression of c-Myc and VEGF, subsequently, suppresses the cell proliferation, triggers the cell apoptosis and inhibits the cell invasiveness in these transfected colorectal cancer Volo cells. In addition, the synergism of siRNA-c-Myc and siRNA-VEGF in transfected cells can be found.

7.Quantification of left and right ventricular systolic function in patients with dilated cardiomyopathy using real-time three-dimensional echocardiography.

OBJECTIVE:
To assess left and right ventricular systolic function in patients with dilated cardiomyopathy (DCM) using real-time three-dimensional echocardiography (RT-3DE).
METHODS:
Fifty DCM patients and 50 normal subjects were enrolled. Left and right ventricular systolic function parameters including end-systolic volume (ESV) and end-diastolic volume (EDV), stroke volume (SV) and ejection fraction (EF) were measured with RT-3DE. The systolicdyssynchrony index (SDI) for left ventricular systolic function was also measured in the same time. The study compared the data of the left and right ventricular systolic function parameters between the DCM group and the control group. Cardiac magnetic resonance (CMRI) was performed in a subgroup of the 30 DCM patients to confirm RT-3DE measurements.
RESULTS:
The results of EDV, ESV and SDI measured by RT-3DE were significantly higher in patient group with DCM than those in the control group (P<0.001). The result of EF was significantly lower in patients with DCM than in normal subjects (P<0.001), but SV showed no significant difference between the two groups (P>0.05). In the DCM group, the results showed a significantly negative correlation between left ventricular ejection fraction (LVEF) and SDI (r=-0.697, P<0.001), and there was also a moderate correlation between LVEF and right ventricular ejection fraction (RVEF) (r=0.496, P<0.01). The results of ESV, EDV and EF showed no significant differences as measured by RT-3DE or CMRI in the patient group (P>0.05), and there was also good correlation between the two measurements (LVEF: r=0.89, P<0.01; RVEF: r=0.85, P<0.01).
CONCLUSION
Left and right ventricular systolic function in DCM could be evaluated by RT-3DE with left and right ventricular systolic function parameters.

8.Curative effect of ultrasonic location botulinum toxin A injection for lower limbs spasticity in children with cerebral palsy

Objective To evaluate the clinical effect of ultrasonic location botulinum toxin A (BTX-A)injection for lower limbs spasticity in children with cerebral palsy.Methods Seventy-five children with lower limbs spasticity resulting from cerebral palsy were divided into injection group(n =37) and control group(n =38).Ultrasonography was used to located the injection of BTX-A into the spastic muscles of the legs in injection group.Then rehabilitation training was received the day after the injection.For all patients,muscle spasticity and gross motor function measure were evaluated before treatment and in the second week,the first,third and sixth month treatment using the Modified Ashworth Scale(MAS) and the Gross Motor Function Measure(GMFM) assessment.Results In the second week,the first,third and the sixth month of treatment,the improvement of muscle spasticity and GMFM in the injection group were significantly better than before(all P < 0.01).And the improvement of muscle spasticity were the greatest in the second week and the first month of the injection.The average therapeutic effect in the injection group was significantly better than that in the control group(P <0.05).There was no statistical difference between the evaluation score of the third month and that of the sixth month for injection group.The BTX-A injection had time limit and could be reinject after 6 months.Conclusions The ultrasound located BTX-A injection to treat lower limbs spasticity in cerebral palsy has the advantages of accurate localization,safety,good child patient compliance and more effective than only rehabilitation treatment.
9.Efficacy and mechanism of scalp acupuncture for spastic cerebral palsy.

OBJECTIVE:
To observe the clinical efficacy of scalp acupuncture for spastic cerebral palsy (CP), and to explore its possible mechanism based on brain white matter fiber bundles, nerve growth related proteins and inflammatory cytokines.
METHODS:
A total of 90 children with spastic CP were randomly divided into a scalp acupuncture group and a sham scalp acupuncture group, 45 cases in each group. The children in the two groups were treated with conventional comprehensive rehabilitation treatment. The children in the scalp acupuncture group were treated with scalp acupuncture at the parietal temporal anterior oblique line, parietal temporal posterior oblique line on the affected side, and parietal midline. The children in the sham scalp acupuncture group were treated with scalp acupuncture at 1 cun next to the above point lines. The needles were kept for 30 min, once a day, 5 days a week, for 12 weeks. Before and after treatment, the diffusion tensor imaging (DTI) indexes of magnetic resonance (FA values of corticospinal tract [CST], anterior limb of internal capsule [ICAL], posterior limb of internal capsule [ICPL], genu of internal capsule [ICGL], genu of corpus callosum [GCC], body of corpus callosum [BCC] and splenium of corpus callosum [SCC]), serum levels of nerve growth related proteins (neuron-specific enolase [NSE], glial fibrillary acidic protein [GFAP], myelin basic protein [MBP], ubiquitin carboxy terminal hydrolase-L1 [UCH-L1]) and inflammatory cytokines (interleukin 33 [IL-33], tumor necrosis factor α [TNF-α]), cerebral hemodynamic indexes (mean blood flow velocity [Vm], systolic peak flow velocity [Vs] and resistance index [RI], pulsatility index [PI] of cerebral artery), surface electromyography (SEMG) signal indexes (root mean square [RMS] values of rectus femoris, hamstring muscles, gastrocnemius muscles, tibialis anterior muscles), gross motor function measure-88 (GMFM-88) score, modified Ashworth scale (MAS) score, ability of daily living (ADL) score were observed in the two groups. The clinical effect of the two groups was compared.
RESULTS:
After treatment, the FA value of each fiber bundle, Vm, Vs, GMFM-88 scores and ADL scores in the two groups were higher than those before treatment (P<0.05), and the above indexes in the scalp acupuncture group were higher than those in the sham scalp acupuncture group (P<0.05). After treatment, the serum levels of NSE, GFAP, MBP, UCH-L1, IL-33, TNF-α as well as RI, PI, MAS scores and RMS values of each muscle were lower than those before treatment (P<0.05), and the above indexes in the scalp acupuncture group were lower than those in the sham scalp acupuncture group (P<0.05). The total effective rate was 95.6% (43/45) in the scalp acupuncture group, which was higher than 82.2% (37/45) in the sham scalp acupuncture group (P<0.05).
CONCLUSION
Scalp acupuncture could effectively treat spastic CP, improve the cerebral hemodynamics and gross motor function, reduce muscle tension and spasticity, and improve the ability of daily life. The mechanism may be related to repairing the white matter fiber bundles and regulating the levels of nerve growth related proteins and inflammatory cytokines.

10.Comprehensive treatment of 352 cases with hypopharyngeal cancer.

OBJECTIVETo review the experience of different surgical construction methods and comprehensive treatments for hypopharyngeal cancer.
METHODSThree hundred and fifty-two cases (According to UICC 2002 criteria, stage I, 3; II, 31; III, 134; IV, 184) with hypopharyngeal cancer were retrospectively reviewed from 1999 to 2005 in the Department of Otorhinolaryngology of Qilu Hospital of Shandong University, including 321 males and 31 females. The median age was 58 years old, ranged from 26 to 82 years old. All the tumors originated from the pyriform sinus (272), posterior pharyngeal wall (61), postcricoid area (19). There were no distant metastasis. Two hundred and fourty-one cases were surgically treated with laryngeal functions preserved and 111 cases without laryngeal functions preserved. All the patients received modified neck dissection, including both unilateral (247 patients) and bilateral (105 patients). Pharyngoesophageal defect reconstruction methods in cases with laryngeal functions preserved were: direct suture in 137, pectoralis major musculocutaneous flap in 62, split graft in 2, pectoralis major musculocutaneous flap combined with the split graft in 5, stomach pulling-up in 12, colon interposition in 23 patients. While in cases without laryngeal functions preserved the methods includes: direct suture in 54, laryngotracheal flap in 54 patients, pectoralis major musculocutaneous flap in 7, laryngotracheal flap combined with pectoralis major musculocutaneous flap in 8, stomach pulling-up in 22, colon interposition in 3 patients. All patients received radiotherapy postoperatively (dose 55 - 75 Gy).
RESULTSThe overall 3 and 5 year survival rates were 65.1% (229/352) and 53.6% (142/265), respectively. For stage I, the 5 year survival rate was 3/3, stage II, 80.6%(25/31), stage III, 65.0% (67/103), stage IV, 36.7% (47/128). The 3 and 5 year survival rates in functionally preserved group were 68.0% (164/241) and 59.7% (114/191), respectively, while in non-functionally preserved group were 58.6% (65/111) and 37.8% (28/74), respectively. The cervical lymph node metastasis was found in 239 sides. Pathologic findings showed that well, moderately and lower differentiated squamous cell carcinomas were 84, 163, 105 cases, respectively. Laryngeal functions (voice, respiration and deglutition) were completely restored in 169 patients and partially restored (voice and deglutition) in 72 patients.
CONCLUSIONSCombined surgery and radiotherapy are the best choice for hypopharyngeal cancer. The continuity of the pharyngoesophagus is restored and the laryngeal function is preserved as far as possible. The preservation of laryngeal function and the laryngeal and pharyngeal reconstruction are based on the premise that the tumor was excised completely.
