1.Effects of Zhuangyao Shuanglu Tongnao Formula on neuronal apoptosis of rats with ischemia-reperfusion induced injury

AIM To investigate the effects of Zhuangyao Shuanglu Tongnao Formula on neuronal apoptosis in rats with cerebral ischemia-reperfusion injury based on the study of oxidative stress and inflammatory response.METHODS The rats were randomly divided into the sham operation group,the model group,the edaravone group(3.0 mg/kg),the low,medium and high dose groups(9.0,18.0,36.0 g/kg)of Zhuangyao Shuanglu Tongnao Formula,with 18 rats in each group.The middle cerebral artery occlusion/reperfusion was conducted by thread embolism method to simulate cerebral ischemia reperfusion injury in rats followed by 6 days corresponding drugs administration.Subsequently,the rats had their neurological function deficit scored by Zeal Longa scoring method;their sizes of cerebral infarction areas measured by TTC staining;their pathological damage and apoptosis of neurons in hippocampal CA1 area of ischemic penumbra of the brain tissue detected by HE staining and TUNEL staining;their SOD activity and levels of GSH,MDA,IL-6,IL-1β,TNF-α in brain tissue detected by kits;and their protein expressions of Bax,Bcl-2,caspase-3,cleaved-capase-3,TLR4,NF-κB p65,Nrf2,HO-1 in rat brain tissue determined by Western blot.RESULTS Compared with the model group,the groups intervened with edaravone,medium and high dose of Zhuangyao Shuanglu Tongnao Formula displayed improvements in the scores of nerve function defects,the rate of cerebral infarction,the rate of neuronal apoptosis,the levels of IL-6,IL-1β,TNF-α and MDA in the ischemic penumbra of brain tissues,the protein expressions of Bax and TLR4,the ratio of cleaved-capase-3/caspase-3 and p-NF-κB p65/NF-κB p65(P<0.05),the levels of GSH,the activity of SOD and the protein expressions of Bcl-2,Nrf2 and HO-1(P<0.05).CONCLUSION Being an inhibitor of oxidative stress and inflammatory response,Zhuangyao Shuanglu Tongnao Formula can alleviate brain injury in rats with cerebral ischemia reperfusion injury through the inhibition of neuronal apoptosis and improvement of neural function mediated by the inhibition of TLR4/NF-κB signal pathway and activation of Nrf2/HO-1 signal pathway.

3.Survey on the Construction Status of Forensic Virtual Autopsy Laboratory and the Applicability of Laboratory Accreditation.

OBJECTIVES:
To survey the development status and actual needs of virtual autopsy technology in China and to clarify the applicability of forensic virtual autopsy laboratory accreditation.
METHODS:
The questionnaire was set up included three aspects:(1) the current status of virtual autopsy technology development; (2) the accreditation elements such as personnel, equipment, entrustment and acceptance, methods, environmental facilities; (3) the needs and suggestions of practicing institutions. A total of 130 forensic pathology institutions were surveyed by online participation through the Questionnaire Star platform.
RESULTS:
Among the 130 institutions, 43.08% were familiar with the characteristics of virtual autopsy technology, 35.38% conducted or received training in virtual autopsy, and 70.77% have establishment needs (including maintenance). Relevant elements were suitable for laboratory accreditation.
CONCLUSIONS
Virtual autopsy identification has gained social recognition. There is a demand for accreditation of forensic virtual autopsy laboratory. After the preliminary assessment, considering the characteristics and current situation of this technology, China National Accreditation Service for Conformity Assessment (CNAS) can first carry out the accreditation pilot of virtual autopsy project at large comprehensive forensic institutions with higher identification capability, and then CNAS can popularize the accreditation in a wide range when the conditions are suitable.

4.Automatic Identification of Brain Injury Mechanism Based on Deep Learning.

OBJECTIVES:
To apply the convolutional neural network (CNN) Inception_v3 model in automatic identification of acceleration and deceleration injury based on CT images of brain, and to explore the application prospect of deep learning technology in forensic brain injury mechanism inference.
METHODS:
CT images from 190 cases with acceleration and deceleration brain injury were selected as the experimental group, and CT images from 130 normal brain cases were used as the control group. The above-mentioned 320 imaging data were divided into training validation dataset and testing dataset according to random sampling method. The model classification performance was evaluated by the accuracy rate, precision rate, recall rate, F1-value and AUC value.
RESULTS:
In the training process and validation process, the accuracy rate of the model to classify acceleration injury, deceleration injury and normal brain was 99.00% and 87.21%, which met the requirements. The optimized model was used to test the data of the testing dataset, the result showed that the accuracy rate of the model in the test set was 87.18%, and the precision rate, recall rate, F1-score and AUC of the model to recognize acceleration injury were 84.38%, 90.00%, 87.10% and 0.98, respectively, to recognize deceleration injury were 86.67%, 72.22%, 78.79% and 0.92, respectively, to recognize normal brain were 88.57%, 89.86%, 89.21% and 0.93, respectively.
CONCLUSIONS
Inception_v3 model has potential application value in distinguishing acceleration and deceleration injury based on brain CT images, and is expected to become an auxiliary tool to infer the mechanism of head injury.

7.Establishment of UPLC Fingerprint and Quantitative Analysis of Multi-components of Shengyutang

Objective:To establish ultra performance liquid chromatography (UPLC) fingerprint of Shengyutang and quantitative analysis method of 11 index components in this famous classical formula. Method:UPLC-diode array detector/evaporative light scattering detector (UPLC-PDA/ELSD) was used, two chromatographic conditions were established by different detectors according to the polarity of chemical components. Conditions of fingerprint 1 were as follows:ACQUITY UPLC HSS T3 column (2.1 mm×100 mm, 1.8 µm) with the mobile phase of acetonitrile (A)-0.6% formic acid solution (C) for gradient elution (0-4 min, 0-4%A; 4-8 min, 4%A; 8-9 min, 4%-8%A; 9-14 min, 8%-9%A; 14-21 min, 9%-15%A; 21-26 min, 15%-17%A; 26-30 min, 17%-20%A; 30-35 min, 20%-32%A; 35-40 min, 32%-40%A; 40-50 min, 40%-80%A; 50-55 min, 80%A), the flow rate of 0.3 mL·min-1, PDA with detection wavelengths of 280 nm and 321 nm, the column temperature at 30 ℃. Conditions of fingerprint 2 were as follows:the CORTECS C18 column (3.0 mm×100 mm, 2.7 µm) with the mobile phase of acetonitrile (A)-water (D) for gradient elution (0-11 min, 19%A; 11-16 min, 19%-25%A; 16-34 min, 25%-28%A; 34-47 min, 28%-47%A; 47-60 min, 47%-80%A), the flow rate of 0.4 mL·min-1, ELSD with drift tube temperature of 95 ℃, the carrier gas (air) flow rate of 2.0 L·min-1, and the column temperature at 30 ℃. UPLC-PDA/ELSD fingerprints of 15 batches of Shengyutang were established, and the similarity was evaluated by similarity evaluation system of chromatographic fingerprint of traditional Chinese medicine (2012 edition) issued by the Chinese Pharmacopoeia Commission, and the contents of eleven index components in this famous classical formula were determined. Result:The similarities of UPLC-PDA/ELSD fingerprints of 15 batches of Shengyutang were >0.98 by comparing with the control fingerprint, 27 and 16 common peaks were identified in fingerprint 1, 2, respectively. Objective:To establish the ultraperformance liquid chromatography (UPLC) fingerprint of Pipa Qingfeiyin substance benchmark, and to establish a quantitative analysis method for simultaneous determination of the contents of five index components, so as to provide reference for the quality control and evaluation of this famous classical formula. Method:ACQUITY UPLC® CSHTM C18 column (2.1 mm×100 mm, 1.7 μm) was used with mobile phase of acetonitrile (A)-0.1% formic acid aqueous solution (B) for gradient elution (0-7 min, 5%-7%A; 7-11 min, 7%-8%A; 11-22 min, 8%-14%A; 22-30 min, 14%-15%A; 30-35 min, 15%-25%A; 35-42 min, 25%-40%A; 42-45 min, 40%-50%A; 45-50 min, 50%-60%A), the flow rate was 0.35 mL·min-1, the column temperature was 25 ℃, the detection wavelengths were 278 nm and 248 nm. UPLC fingerprints of 15 batches of Pipa Qingfeiyin substance benchmark were established, and the "Similarity Evaluation System of Chromatographic Fingerprint of Traditional Chinese Medicine" software (2012 edition) was used for similarity analysis, and the common peaks were assigned. Cluster analysis (CA), principal component analysis (PCA) and orthogonal partial least squares discriminant analysis (OPLS-DA) were used to evaluate the fingerprint data. UPLC fingerprint method was used to simultaneously determine the contents of five components in the substance benchmark. Result:The method validation of fingerprint and determination method was good, the similarities between 15 batches of Pipa Qingfeiyin substance benchmark and their control fingerprint were ≥0.997, 23 common peaks were identified and 11 chromatographic peaks were identified. 9.Application of Virtual Anatomy Technology in Postmortem Examination of Medical Dispute Cases.

Objective To discuss the application value of CT scanning technology in cause of death determination of medical dispute cases. Methods From July 2017 to December 2018, postmortem CT imaging data of 12 medical dispute cases were collected. CT imaging diagnosis results and anatomy findings as well as differences between antemortem and postmortem CT diagnosis were compared. The advantages and disadvantages of CT routine tests of the cadavers in terms of the diagnosis of disease and damage were analyzed. Results The comparison between CT imaging diagnosis and anatomical findings showed that CT scans had advantages in the diagnosis of disease and damage with large differences in density changes, such as atelectasis, pneumonia, calcification, fracture and hemorrhage, etc. The comparison of CT diagnosis in antemortem and postmortem examination showed that the cadavers of medical dispute cases were well preserved and that postmortem CT scan was meaningful for the diagnosis of antemortem diseases. Conclusion Virtual anatomy technology has a relatively high application value in postmortem examination of medical dispute cases. It can provide effective information for the appraisers before the autopsy and can also provide a reference for cause of death analysis when the anatomy cannot be performed.
