1.Clinical Study on Needling Peri-ocular Points and Abducens Oculi for Abducens Paralysis

ObjectiveTo compare the efficacy between needling peri-ocular points plus abducens oculi and needling peri-ocular points alone in treating abducens paralysis, for evaluating the effectiveness and advantage of needling peri-ocular points plus abducens oculi.MethodFollowing the random number table, 86 patients with postnatal unilateral abducens oculi were divided into two groups: 43 cases in the control group were intervened by conventional acupuncture at peri-ocular points including bilateral Taiyang (EX-HN5), Cuanzhu (BL2), Fengchi (GB20), Hegu (LI4) and Waiguan (TE5), plus Baihui (GV20), Sizhukong (TE23) and Tongziliao (GB1) on the affected side; 43 cases in the treatment group were intervened by acupuncture at peri-ocular points and abducens oculi, i.e. to puncture the abducens oculi (1~3 mm behind the attached point of the extrarectus to eyeball) in addition to the treatment given to the control group. After 3 treatment courses, the total effective rate, average recovery time and improvement of strabismus angle were observed.ResultThe total effective rate was 93.0% in the treatment group, significantly higher than 81.4%in the control group (P<0.05); of the 28 cured patients in the treatment group, the average recovery time was (34.51±7.91)d, versus (41.88±7.87)d in the 22cured patients in the control group, and the difference was statistically significant (P<0.01). There was a significant difference in comparing the improvement of strabismus angle between the two groups (P<0.01); after treatment, the strabismus angle was (11.23±6.32)° in the treatment group versus (14.14±6.85)° in the control group, and the difference was statistically significant (P<0.05).ConclusionNeedling peri-ocular points and abducens oculi can improve the strabismus angle, shorten treatment duration and reduce patient’s sufferings in treating postnatal abducens paralysis, significantly superior to conventional acupuncture at peri-ocular points.
2.Study on Determination Method of Luteolin-7-O-glucoside in Compound Luobuma Granule

Objective To establish determination method of luteolin-7-O-glucoside in Compound Luobuma Granule. Methods Phenomenex luna C18 column (4.6 mm×150 mm, 5 μm) was used;aceto-0.5% acetic acid (14∶86) was set as the mobile phase at flow rate of 0.8 mL/min;the detection wavelength was 348 nm. Results The linear range of luteolin-7-O-glucoside was in the range of 0.031 9-0.796 3 μg (r=0.999 6), and the average recovery was 100.85% (n=6). Conclusion The method is specific, simple, and can improve quality standard and increase the controllability of Compound Luobuma Granule.
3.Study on Quality Standard for Lianqiaobaidu Pills

Objective To imporve the quality standard of Lianqiaobaidu Pills. Methods The TLC method was established for ephedrae herba, sophorae flavescentis radix, paeoniae radix rubra, and RP-HPLC methed was established for the determination of chlorogenic acid in Lianqiaobaidu Pills. Zorbax SB-C18 (4.6 mm×150 mm, 5 μm) column was used with methanol-acetonitrile (2∶1) - 0.04% phosphoric acid (10∶90) as the mobile phase at flow rate of 0.8 mL/min. The detection wavelength was 326 nm. Results TLC chromatographic spots were clear, and negative control had no interference. The linear range of chlorogenic acid was 0.042 2-0.464 2 μg (r=1), the average recovery (n=6) was 98.96%. Conclusion The method is simple, reliable, with strong exclusive feature, and suitable for quality control of Lianqiaobaidu Pills.
5.Expression of Toll like receptor 4 in injured brain tissue after traumatic brain injury

Objective To investigate the expression of Toll like receptor 4(TLR4)in the injured brain tissue atter traumatic brain injury(TBI) and explore the potential role of TLR4/NF-κB in the secondary brain injury.Methods Thirty-six SD rats were randomly divided into control group(n=1 2),TBI inducement for 1 d group(n=6),TBI inducement for 3 d group(n=12)and TBI inducement for 7 d group(n=6).TBI models of the later 3 groups were induced by Feendy's free-falling,and rats of the control group are only performed exposure ofdura of the right parietal lobe.TLR4 mRNA expression in the injured brain tissue was studied by RT-PCR,NF-κB binding activity was detected by electrophoretic mobility shift assay (EMSA),and the TNF-α and IL-6 levels were detected by enzyme linked immunosorbent assay(ELISA).Immunohistochemistry was employed to determine the protein expression of TLR4 in the brain tissues of control group and TBI inducement for 3 d group.Results The TLR4 mRNA expression,NF-κB binding activity,and the levels of TNF-t and IL-6 in rats of the TBI inducement for 1,3 and 7 d groups were significantly increased as compared with those in the control group(P<0.05).The protein expression level of TLR4 in the brain tissue of control group was rare,and a large number of TLR4-positive immunostained cells,including cortical glial cells and neurons,were noted in the brain tissue of TBI inducement for 3 d group.TLR4 mRNA level was positively correlated to the NF-κB activity(r=0.786,P=0.000),and positive relations were also noted between TLR4 mRNAlevel and both TNF-α and IL-6 levels(r=0.517,P=0.010; r=0.503,P-0.012).Conclusion TBI could induce concomitant and persistent up-regulation of TLR4 expression and NF-κB binding activity in the injured brain tissue.TLR4/NF-κB might play a central role in the secondary injury after TBl.
6.Association of peripheral white blood cell count with carotid intimal-medial thickness in middle-aged and elderly Chinese of suburban area of Shanghai

Objective To investigate the association between peripheral white blood cell (WBC) count and the carotid intimal-medial thickness (cIMT) in middle-aged and elderly Chinese of suburban area of Shanghai.Methods A total of 2 519 subjects aged 40 years or older from Jiading District,Shanghai were recruited to undergo questionnaire interview,anthropometric measurements,fasting blood sampling,and carotid B-mode ultrasonography.After excluding subjects with biochemical index missing,WBC count > 10× 109/L as well as those with known myocardial infarction,cerebral infarction,coronary heart disease,and those taking lipid-lowing agents,2 239subjects were included in this analysis.Results Increased WBC count was associated with more unfavorable metabolic risk profiles.With the increasing quartiles of WBC count,cIMT were 0.558,0.570,0.573,and 0.587mm respectively(P for trend < 0.01).As compared with participants in the first WBC quartile,the risks of cIMT thickening were increased by 79%,109% in males and 29% in females (P> 0.05 in females).Multiple linear regression model showed that peripheral WBC count was an independent risk marker for cIMT thickening,especially for neutrophil and monocyte count.Conclusion A high-normal WBC count was significantly associated with cIMT thickening in middle-aged and elderly Chinese of suburban area of Shanghai,especially in males.
7.Development of a novel screening assay for inhibitors targeting HIF-1alpha and P300 interaction.

Hypoxia is a general characteristic of most solid malignancies and intimately related to cancer progression. Homeostatic response to hypoxia is primarily mediated by hypoxia inducible factor-1alpha (HIF-1alpha) that elicits transcriptional activity through recruitment P300 coactivator. Targeting the interaction of HIF- alpha and P300 would thus constitute a novel approach for cancer treatment by suppressing tumor angiogenesis and metastasis. Here, a screening assay was developed for inhibitors targeting the interaction between HIF-1alpha and P300. The nucleotide sequence of human HIF-1alpha and P300 were cloned into pBIND and pACT vectors, named pBIND-HIF1alpha and pACT-P300. The interaction of HIF-1alpha and P300 was identified in HEK293 cell using mammalian two-hybrid system. And compound chetomin decreased their interaction in this mammalian two-hybrid system. We further verified HIF-1 inhibition effect of chetomin in U251-HRE cells. Therefore, we established a screening assay combined HIF-1alpha and P300 mammalian two-hybrid system and U251-HRE reporter assay for HIF-1 selective inhibitors.

8.Leihong granule intervened in-stent restenosis after endovascular therapy for lower extremity arterial occlusive diseases: a clinical observation.

OBJECTIVETo observe the intervention effect of Leihong Granule (LG) in in-stent restenosis (ISR) after endovascular therapy for lower extremity arterial occlusive diseases (LEAOD).
METHODSRecruited 80 LEAOD patients who successfully underwent endovascular therapy (balloon dilation and stent implantation) were randomly assigned to two groups, the control group and the LG group, 40 in each group. Patients in the control group received basic treatment, while those in the LG group additionally took LG for 3 months. Plasma levels of IL-10, IL-18, CRP, and the intima-media thickness (IMT) of lower extremity artery were observed in the two groups between and after treatment. The rate of stent patency, ABI, intermittent claudication, rest pain, and the incidence of amputation the two groups were recorded and observed in the two groups.
RESULTSIn the control group, serum levels of IL-10, IL-18, CRP, and IMT were significantly higher one month after surgery than before surgery (P < 0.05). There was no significant difference in serum levels of IL-10, IL-18, CRP, or IMT between the two groups before surgery (P > 0.05). These indices were obviously lower in the LG group than in the control group after surgery (P < 0.05). Compared with the control group, the incidence rates of intermittent claudication and the rest pain at 6 months and 12 months after surgery significantly decreased (P < 0.05). The stent patency rate at 6 months and 12 months after surgery, and ABI were significantly higher than those of the control group (P < 0.05). There was no statistical difference in the amputation rate between the two groups (P > 0.05).
CONCLUSIONLG might effectively improve ischemic symptoms of affected limbs possibly through lowering the ISR rate after endovascular therapy for LEAOD through preventing immunosuppressive actions.

9.CT angiography-based simulation of the surgical approach in early operation for ruptured aneurysm.

OBJECTIVETo simulate the surgical approaches for intracranial aneurysms using three-dimensional CT angiography (3D-CTA) and assess the value of 3D-CTA in early microneurosurgery for ruptured intracranial aneurysms.
METHODSForty-eight patients with spontaneous subarachnoid hemorrhage due to ruptured intracranial aneurysm were confirmed by early operation. All the patients were classified according to Hunt-Hess, including 11 of grade I, 29 of grade II, and 8 of grade III. CTA was performed before the operation and surgical simulation was conducted. The preoperative findings on CTA and the intraoperative findings were compared and the clinical value of cerebral 3D-CTA was analyzed.
RESULTSPre-operative 3D-CTA clearly displayed the location, size and shape of the aneurysms, the axis direction of the aneurysm apex and the width of aneurysm neck. The spatial relation between the parent aneutysm artery, the aneurysm, the peripheral vessels and the bony structures were also demonstrated. These findings were basically consistent with the intraoperative findings. The Glasgow outcome score was 5 in 41 patients, 4 in 4 patients, 3 in 2 patients, and 2 in 1 patient upon discharge from the hospital.
CONCLUSIONSPreoperative 3D-CTA examination can simulate the surgery for ruptured aneurysms to help improve the surgical success rate.

10.Expression of aldosterone synthesis related enzyme and associated regulatory factor genes in aldosterone-producing adenoma

Objective To investigate the discrepancy of aldosterone synthesis process and potential regulation abnormality between aldosterone-producing adenoma(APA) and normal adrenal(NA) with microarray. Methods cRNA probes labelled with biotin were prepared from mRNA of APAs(APA group,n=10) or NAs(control group,n=7).The probes were hybridized with oligonucleotide microarray of target gene expression profile.Expression levels were read from the fluorescent intensity scanned.The difference of gene expression profile was analyzed by computer software.Differentially expressed genes were verified by real-time RT-PCR. Results Compared with control group,97 genes were up-regulated and 168 genes were down-regulated in APA group.In the genes related to steroid hormone synthesis,only CYP11B2 was significantly up-regulated.In the physiologic regulators of aldosterone synthesis,CYB5A,CYP17A1,DUSP1 and HMGCR were down-regulated,while RENBP and NR1H2 were up-regulated.As a key enzyme in the biosynthesis of cortisol,the expression of CYP17A1 gene was inhibited. Conclusion Among the aldosterone synthesis related enzyme and corresponding regulatory genes in APA,CYP11B2 may be a key synthetase,and the suppressed physiologic regulators of aldosterone synthesis may indicate the existence of neoplastic modulation.
