1.Preparation of PLGA/PEG micruspheres and its sustained drug release property

Objective To prepare PLGA/PEG microsphcres for sustained release of bovine serum albumin (BSA), and evaluate the effect of the micrnsphere composition, proportion of the components and integration of PEG on the size, BSA encapsulation rate and in vitro BSA release properties of the microspheres. Methods BSA-loaded PLGA/PEG microspheres were prepared by water-in-oil-in-water double emulsions-solvent evaporation method (W/O/W). The morphology of the microspheres was observed with scanning elect'on microscope, the diameter was determined by Mastersizer 2000, and the encapsulation efficiency and in vitro BSA release were evaluated by UV spectrophotometry. Results Increased GA to LA ratio was associated with increased size of the microspheres and accelerated BSA release. The incorporation of PEG obviously increased the BSA encapsulation rate to as much as 88.22% with also increased BSA release rate. The BSA-PLGA/PEG biodegradable microspheres prepared was characterized by high encapsulation rate, low burst release, and slow BSA release for over 3 weeks at high drug concentrations. Conclusion BSA-PLGA/PEG microspheres with relatively high encapsulation efficiency and proper drug loading can be prepared by adjusting the parameters during the preparation process.
2.Nape Needling at Six Points with Electroacupuncture Combined with Huayu J ingbi Decoction in the Treatment of Cervical Spondylotic Vertebralartery:A Retrospective Study

Objective To analyze the clinical efficacy of nape needling at six points with electroacupuncture combined with Huayu Jingbi Decoction in the treatment of cervical spondylotic vertebralartery(CSA).Methods A retrospective study was conducted to collect 126 patients with CSA admitted to Haikou Hospital of Traditional Chinese Medicine from May 2021 to May 2023.According to different treatment regimens,they were divided into observation group(59 cases)and control group(67 cases).The observation group was treated with nape needling at six points with electroacupuncture combined with Huayu Jingbi Decoction,while the control group was treated with conventional acupuncture combined with Huayu Jingbi Decoction.Two weeks was a course of treatment,and both groups were treated for two courses.The changes of neck disability index(NDI)and cervical physiological curvature were observed before and after treatment in the two groups.The changes of hemorheology and hemodynamic indexes were compared before and after treatment between the two groups,and the clinical efficacy of the two groups was evaluated.Results(1)The total effective rate of the observation group was 96.61%(57/59),and the control group was 77.61%(52/67).The efficacy of the observation group was superior to that of the control group,and the difference was statistically significant(P<0.05).(2)After treatment,the NDI scores and cervical physiological curvature of the patients in the two groups were significantly improved(P<0.05),and the observation group was significantly superior to the control group in improving the NDI scores and cervical physiological curvature,with a statistically significant difference(P<0.05).(3)After treatment,the whole blood high cut viscosity,whole blood low cut viscosity,plasma viscosity and fibrinogen of patients in the two groups were significantly improved(P<0.05),and the observation group was significantly superior to the control group in improving whole blood high cut viscosity,whole blood low cut viscosity,plasma viscosity and fibrinogen,and the differences were statistically significant(P<0.05).(4)After treatment,the Vs,Vm,PI and RI of the two groups of patients were significantly improved(P<0.05),and the observation group was significantly superior to the control group in improving Vs,Vm,PI and RI,with statistically significant differences(P<0.05).Conclusion Nape needling at six points with electroacupuncture combined with Huayu Jingbi Decoction has remarkable efficacy in the treatment of cervical spondylotic vertebralartery,which can significantly improve the cervical spine function of the patients,improve the blood rheology and haemodynamic indexes,and promote the recovery of the cervical spine function of the patients.
3.Identification of TGF-beta/Smads pathway in human prostate cancer cell lines and its significance.

OBJECTIVETo determine the TGF-beta/Smads signal pathway in different human prostate cancer cell lines, and to explore the role of the TGF-beta/Smads pathway in the progression and metastasis of prostate cancer and its possible mechanisms.
METHODSWe detected the expressions of the key proteins involved in the TGF-beta/Smads pathway, TbetaR II, Smad2/3, p-Smad2 and Smad4, in prostate cancer cell lines LNCaP, PC-3, DU145, IF11 and IA8 with different metastatic potentials by Western blotting.
RESULTSTbetaR II was expressed highly in PC-3, DU145, IF11 and IA8, but extremely lowly in LNCaP. Smad2/3 was highly expressed in all the cell lines with different intensity, while p-Smad2 only in PC-3 and DU145. The expression of Smad4 was detected in LNCaP, PC-3 and DU145, but not in IF11 and IA8.
CONCLUSIONThe status of the TGF-beta/Smads pathway differs in the cell lines with different metastatic potentials, only active in PC-3 and DU145. The TGF-beta/Smads pathway may be involved in the invasion and metastasis of prostate cancer through altering the expressions of the key proteins in it.

4.Relationship between lower urinary tract symptoms and objective measures of benign prostatic hyperplasia: a Chinese survey.

BACKGROUNDThe enlarged prostate leads to obstruction and lower urinary tract symptoms (LUTS), which comprise frequency, urgency, weak stream, straining and nocturia. This study was conducted in a large series of patients to evaluate the relationship between LUTS as stipulated in the International Prostate Symptom Score (IPSS) and the objective parameters related to benign prostatic hyperplasia (BPH).
METHODSWe enrolled 1295 BPH patients from seven centers. The patients were either at first diagnosis of BPH or had discontinued medical treatment for at least 3 months. Those with several other diseases that may be potential risk factors affecting urinary symptoms were excluded from the study. Age, IPSS, prostate volume, peak flow rate, urine volume and post-voiding residual urine volume were measured. The relationship between IPSS and objective parameters were quantified by means of Spearman correlation coefficients. The differences in these parameters between the groups with mild, moderate or severe symptoms were also evaluated.
RESULTSStatistically significant correlations were found between IPSS and objective parameters by means of Spearman correlation coefficients. When the patients were divided into three groups with different severities of symptoms, there were significant differences in peak flow rate, urine volume, prostate volume, residue urine volume and quality of life, whereas average age and prostate-specific antigen levels were similar. However, there was evident overlap of these parameters between the groups. The same results were found when the irritative or obstructive subscore of IPSS was considered.
CONCLUSIONSThe correlation between objective parameters of BPH and LUTS is significant. However, it is hard to predict the severity of symptoms by these parameters.

5.Gene mapping of autosomal dominant retinitis pigmentosa in a Chinese family.

BACKGROUNDThe autosomal dominant form of retinitis pigmentosa (ADRP) can be caused by mutations in 14 genes and further loci remains to be identified. This study was intended to identify mutations in a Chinese pedigree with ADRP.
METHODSA large Chinese family with retinitis pigmentosa was collected. The genetic analysis of the family suggested an autosomal dominant pattern. Microsatellite (STR) markers tightly linked to genes known to be responsible for ADRP were selected for linkage analysis. Exons along with adjacent splice junctions of PRPF31 were amplified by polymerase chain reaction (PCR) and screened by direct sequencing.
RESULTSThe caused gene of ADRP was mapped to 19q13.4 between markers D19S572 and D19S877, with a maximum LOD score of 3.01 at marker D19S418 (recombination fraction = 0).
CONCLUSIONThe affected gene linked to the 19q13.4 in a Chinese family with ADRP, which is different from other mutations at the same loci in other Chinese families.

6.Expression of Smad4 in prostate cancer LNCaP and ARCaP cell lines with different metastatic potentials and its significance.

OBJECTIVETo observe the expression of Smad4, the core of TGF-beta/Smads signal transduction pathway in different prostate cancer cell lines, and explore their molecular mechanism of bone metastatic potential.
METHODSThe Millicell polycarbonate filter coated with matrigel was used to confirm the invasive potency of LNCaP and ARCaP cell lines (IF11 and IA8). The expressions of the Smad4 protein and mRNA in these prostate cancer cells with different metastatic potentials were detected by Western blotting and RT-PCR, respectively.
RESULTSARCaP cell lines (IF11 and IA8) exhibited a stronger potency of invasion than LNCaP (P < 0.01). The Smad4 protein and mRNA highly expressed in the LNCaP cell line that was well-known with a low metastatic potential, but not in the ARCaP (IF11 or IA8) cells with high metastatic potentials (P < 0.01).
CONCLUSIONSmad4 expresses differently in LNCaP and ARCaP cell lines with different metastatic potentials and, as a tumor suppressive gene, its deficient expression may play an important role in the invasion and metastasis of advanced prostate cancer.

7.Effect of hypoxia on proliferation and invasion of non-small-cell lung cancer cell line A549

Objective To study the effect of hypoxia on proliferation and invasion in non-small-cell lung cancer cell line A549 and its molecule mechanism.Methods The non-small-cell lung cancer cell line A549 were stimulated under hypoxia and normoxia for 24 hours in vitro.The proliferation ability was detected by CCK8.The invasion ability was detected by Transwell assay.The mRNA and protein expression levels of HIF-1 oα and β-catenin were detected by RT-PCR and Western blot.Furthermore,β-catenin siRNA was used to investigate the role of β-catenin on hypoxia induced proliferation and invasion.Results The results demonstrated that hypoxia could promote A549 cells proliferation and invasion.The mRNA and protein level of HIF-1oα and β-catenin was upregulated by hypoxia treatment,whereas β-catenin siRNA could block these processes.Conclusion Hypoxia can promote the proliferation and invasion of non-small-cell lung cancer cell line A549 by activating Wnt/β-catenin signaling pathway.
8.Thirty-six critical cases of emergency helicopter transferring between hospitals

Objective:To summarize critical cases of emergency helicopter transferring between hospitals and improve the quality and safety of critical care.Methods:The task records of Guangxi Aviation Medical Rescue Training Base from September 2017 to September 2020 were retrieved. The mission acceptance, implementation results, disease spectrum composition, pre-transfer preparation and medical intervention on board were summarized.Results:① General information: a total of 168 patients of helicopter transfer requests were registered, of which 36 patients were transferred, 35 patients were successful, 1 patient had cardiac arrest during the landing phase, and died several hours after continuous resuscitation. Of the 36 patients 30 were males and 6 were females, with median age of 50.5 (29.8, 66.0) years old, the average transfer time was (54.95±17.89) minutes, and the average transfer distance was (205.74±74.68) km. ② Disease spectrum included 11 cases of stroke (30.55%), 7 cases of trauma (19.45%), 5 cases of severe pneumonia (13.89%), 5 cases of heart and macro-vascular diseases (13.89%), 5 cases of abdominal emergency (13.89%), and 3 other conditions (8.33%).③ Severity: 31 patients (86.11%) were severe (≥15) according to acute physiology and chronic health evaluation Ⅱ (APACHEⅡ) score; 19 patients (52.78%) were high-risk emergency transport (≥6) according to Hamilton early warning score (HEWS); 6 patients (85.71% of trauma patients) were severe trauma (≥16) according to injury severity score (ISS). ④ Preparation before transfer: remote consultation was carried out to evaluate the latest state of the patient's condition, especially the respiratory and circulatory conditions. Relevant items were reviewed and emergency treatments were implemented when necessary. Targeted preparation was made for accidents that might occur during transfer, such as electrocardiogram (94.44%), blood gas analysis (94.44%), brain CT (36.11%) and other auxiliary examinations, endotracheal intubation or tracheotomy (72.22%), deep vein catheterization (91.67%), placement of gastric tube (86.11%) and urinary tube (88.89%), adjustment of sedative (38.89%), vasoactive drugs (58.33%) and drugs for dehydration and lowering intracranial pressure (33.33%), and fixation of fracture (11.11%), etc. ⑤ On-board medical intervention: cardiac monitoring, blood pressure, respiration and blood oxygen monitoring were carried out in all patients. The parameters of patients using ventilator were adjusted in time (66.67%). The dosage of patients using micropump was adjusted in time (91.67%). Other aspects included the use of sedative and analgesics (38.89%), sputum suction nursing (75.00%), all kinds of catheter nursing (endotracheal intubation/incision nursing of 72.22%, indwelling catheter nursing of 88.89%), and cardiopulmonary resuscitation for patient with cardiac arrest (2.78%).Conclusion:As the patients transferred by helicopter are mainly those of critically ill at this stage, the requirements for airborne medical equipment and rescue technology are high, and there is an urgent need to establish technical specifications and personnel training standards.
9.Changes of cognitive function and its correlated factors in patients with first acute cerebral ischemic stroke

Objective To investigate the changes of cognitive impairment and its correlated factors in patients with first acute cerebral ischemic stroke.Methods Five hundred and sixty-eighty patients,admitted to our hospital from July 2010 to June 2011 and met the diagnostic criteria of first acute cerebral ischemic stroke,were chosen in our study; neuropsychological evaluation was conducted within 1 week of acute cerebral ischemic stroke.The comprehensive neuropsychological evaluation battery included mini-mental state examination (MMSE),Digit span,Mattis dementia scale,Rey complex figure test,stroop test,World Health Organization and University of California-Los Angeles auditory verbal learning test,semantic category verbal fluency test and clock drawing test,which contained tests for memory,attention,executive function,information processing capability,visuospatial and visuo constructive functions.Gender,age,histories of hypertension and smoking,and levels of blood pressure,random blood glucose,fasting blood glucose and 2-hour postprandial blood glucose,cholesterin,triglyceride,low density lipoprotein-C (LDL-C) and high density lipoprotein-C (HDL-C) were noted and the relations between cognitive function and both blood sugar and blood lipid levels were analyzed.Results (1) The abnormal rates of these patients in MMSE,Mattis dementia scale,attention deficit disorder,immediate logical memory disorder,delayed logical memory disorder,instant auditory memory disorder,delayed auditory memory disorder,visual memory disorder,executive function disorder,information processing disorder,and visuospatial and visuo constructive disorder were 78.2%,72.5%,26.5%,56.3%,60.9%,74.6%,98.2%,83.8%,62.7%,4.9% and 77.7%,respectively.In patients with different blood glucose levels,significant differences were found on scores of MMSE,Mattis dementia scale,attention scale (P< 0.05); furthermore,the scores ofMMSE,Mattis dementia scale,attention scale in patients with impaired fasting glucose were obviously lower as compared with those in other groups (P<0.05).The scale of above cognitive domains showed no significant difference in patients with different types of hyperlipidemias (P>0.05); the scores of execution ability and visual space structure capability in patients with HDL-C were higher than those in normal density lipoprotein-C and LDL-C (P<0.05),and the higher level of density lipoprotein-C,the higher scores of visual space structure capability (P<0.05).Logistic regression analysis found that the total MMSE scores were associated with gender,age,and levels of fasting blood glucose and 2-hour postprandial blood glucose; the independent risk factors of executive function were gender,age and HDL-C level; the ability of memory was correlated with age and HDL-C level.Visuospatial and visuo constructive function was associated with gender,age and mRS scores; the information processing ability was only correlated with HDL-C level.Conclusion In patients with acute cerebral ischemic stroke,the cognitive impairment is extensive and severe,with a prevalence of about 75%,mainly having disorders in memory,executive ability,spatial structure; glucose levels,especially impaired fasting glucose level show most obvious influence in cognitive impairment;HDL-C level helps keep normal cognitive function; age,gender,HDL-C level are the most important factors of cognitive function.
10.The study of multiple RT-PCR-based reverse dot blot hybridization technique for detecting influenza viruses

Objective To establish a multiplex RT-PCR-based reverse dot blot hybridization technique to detect influenza viruses. Methods Obtain the HA nucleotide sequences of seasonal influenza H1 N1, seasonal influenza H3N2, influenza H1 N1 and human avian influenza H5 N1 from GenBank. Design primers in conservative district and probes t in high variable region respectively, after analyzing the HA nucleotide sequences of influenza virus through the Vector NTI 9.0. Establish and optimize multiple RT-PCR system by comparing amplification efficiency and specificity at different primer concentrations. Establish the reverse dot hybridization system after optimizing the concentration of probes. To compare the sensitivity and specificity of this technique and the general RT-PCR Method through extracting the viral RNA of the mentioned influenza virus which are to be the reference substance. Results Successfully establish a multiplex RT-PCR-based reverse dot blot hybridization technique for detecting influenza viruses. This technique is 100-1000 times more sensitive than gel electrophoresis method, and it has a good specificity.Conclusion Successfully established multiplex RT-PCR-based reverse dot blot hybridization technique for detecting influenza viruses.
