1.Anomalous systemic arterial supply to normal basal segments of the left lower lobe of the lung:characteristic findings on chest radiography and spiral CT

Objective To summarize the characteristic findings on chest radiography and spiral CT of anomalous systemic arterial supply to normal basal segments of the left lower lobe of the lung.Methods Five patients with anomalous systemic arterial supply to normal basal segments of the left lower lobe of the lung were retrospectively reviewed.Chest radiography,and contrast-enhanced spiral CT scan were analyzed. Results On posteroanterior chest radiographs,the findings in 5 cases of this anomaly were all shown as a retrocardiac mass,narrowing lower lobar pulmonary artery shadow,and without normal lower lobar pulmonary artery branches but there were dilated abnormal markings in left lower lung zone.On contrast- enhanced spiral CT scans,the involved left lower lobe of the lung had mild volume loss but normal bronchial trees in all 5 cases and areas of ground-glass opacity in 2 cases.The absence of normal left lower lobar pulmonary artery distal to the origin of superior segmental artery,anomalous systemic artery originating from the descending aorta,diffuse dilatation of the systemic arterial branches distributed in the basal segments of the left lower lobe were also found in all the patients.One patient who underwent angiography had similar vessel features with spiral CT.Conclusion There are some characteristic findings on chest radiography in this anomaly.the contrast-enhanced spiral CT scans are important in diagnosing and avoiding invasive angiography and life-threatening lung biopsy.
2.The synergistic effect of amygdalin and HSYA on the IL-1beta induced endplate chondrocytes of rat intervertebral discs.

The effect of amygdalin joint hydroxysafflor yellow A (HSYA) on the endplate chondrocytes derived from intervertebral discs of rats induced by IL-1beta and the possible mechanism were studied and explored. Chondrocytes were obtained from endplate of one-month SD rat intervertebral discs and cultured primary endplate chondrocytes. After identification, they were divided into normal group, induced group, amygdalin group, HSYA group and combined group. CCK-8 kit was adopted to detect the proliferation of the endplate chondrocytes. FCM was measured to detect the apoptosis. Real-time PCR method was adopted to observe the mRNA expression of Aggrecan, Col 2 alpha1, Col 10 alpha1, MMP-13 and the inflammatory cytokines IL-1beta. The protein expression of Col II, Col X was tested through immunofluorescence. Compared with the normal group, the proliferation of the endplate chondrocytes decreased while the apoptosis increased (P < 0.05). With down regulation of the mRNA expressions of Aggrecan, Col 2 alpha1 and up regulation of the mRNA expressions of Col 10 alpha1, MMP-13, IL-1beta (P < 0.05), the protein expression of Col II decreased while the protein expression of Col X increased. Compared with the induced group, amygdalin group, HSYA group, the combined group could inhibit the apoptosis and promote the proliferation (P < 0.05). They could increase the mRNA expressions of Aggrecan and Col 2 alpha1 while decrease the mRNA expressions of Col 10 alpha1, MMP-13 and IL-1beta (P < 0.05). They could also enhance the protein expression of Col II while reduce the protein expression of Col X. The effect of the combined group was significantly better than that of amygdalin and HSYA. Amygdalin joint HSYA could inhibit the degeneration of the endplate chondrocytes derived from intervertebral discs of rats induced by IL-1beta and better than the single use of amygdalin or HSYA.

3.Comparison of the therapeutic effect on skeletal fluorosis and impact on urine fluoride value among fire needle therapy, electroacupuncture and calcium carbonate D3.

OBJECTIVETo observe the impacts on skeletal fluorosis pain, joint motor dysfunction and urine fluoride excretion in the treatment with fire needle therapy, electroacupuncture and calcium carbonate D3.
METHODSThe randomized controlled trial was adopted. Ninety-five patients were randomized into a fire needle group (31 cases), an electroacupuncture group (33 cases) and a calcium carbonate D3 group (31 cases). In the fire needle group and the electroacupuncture group, Ashi points, Dazhui (GV 14), Geshu (BL 17), Quchi (LI 11), Hegu (LI 4), Xuehai (SP 10) points were selected and stimulated with fire needle and electroacupuncture separately, three times a week. In the calcium carbonate D3 group, calcium carbonate D3 tablets was prescribed for oral administration, 600 mg each time, twice a day. The duration of treatment was 2 months in the electroacupuncture group and calcium carbonate D3 group and 1 month in the fire needle group. VAS score, the range of motion (ROM) and urine fluoride value were compared before and after treatment in the patients of the three groups.
RESULTSAfter treatment, VAS value and ROM were improved significantly in the patients of the three groups (all P < 0.05), the difference was not significant in comparison of the three groups (all P > 0.05). After treatment, the urine fluoride value was increased significantly in the fire needle group [(7.89 +/- 3.61) mg/L vs (9.81 +/- 4.17) mg/L, P < 0.01] and was increased in the electroacupuncture group [(7.53 +/- 3.46) mg/L vs (8.97 +/- 4.21) mg/L, P < 0.05]. The difference was not significant in comparison before and after treatment in the calcium carbonate D3 group (P > 0.05).
CONCLUSIONThe fire needle therapy, electroacupuncture and calcium carbonate D3 all have the clinical value in the prevention and treatment of skeletal fluorosis and the difference in the therapeutic effect has not been discovered among them yet at present. But it has been found that the fire needle therapy and electroacupuncture display the active significance in the promotion of urine fluoride excretion.

4.Prevention and treatment of anastomotic leakage following anterior resection for low rectal cancer

Objective To analyze retrospectively the clinico-pathological features that influencing the occurrence of anastomotie leakage after low anterior resection of rectal cancer, as well as its management and outcome of patients. Methods The data of 513 patients underwent anterior resection for low rectal cancer from june 1999 to June 2007 were reviewed. Results The incidence of anastomotic leakage was 4.5 %(23/ 513). 20 patients underwent conservative therapy,while 3 patients underwent reoperation, all patients were cured without ileostomy or colostomy.Conclusion The occurrence rate of anastomotie leakage is closely re- lated to the type of operation,the Dukes staging,Diabetes Mellitus and Diarrhea. Local irrigation via the drainage tube is the main strategic point to manage the leakage.
5.TURP plus endocrine therapy (ET) versus α1A-blockers plus ET for bladder outlet obstruction in advanced prostate cancer.

OBJECTIVETo compare the effect of transurethral resection of the prostate combined with endocrine therapy (TURP + ET) with that of αlA-blockers combined with ET ((αlA-b + ET) in the treatment of bladder outlet obstruction (BOO) in patients with advanced prostate cancer (PCa), and to investigate the safety of the TURP + ET for the treatment of PCa with BOO.
METHODSWe retrospectively analyzed 63 cases of PCa with BOO, 28 treated by αlA-b + ET and the other 35 by TURP + ET. We obtained the residual urine volume (RV), maximum urinary flow rate (Qmax), International Prostate Symptom Score (IPSS), and quality of life score (QoL) before and after treatment along with the overall survival rate of the patients, followed by comparison of the parameters between the two methods.
RESULTSAt 3 months after treatment, RV, IPSS, and QoL in the TURP + ET group were significantly decreased from (137.8 ± 27.6) ml, (22.3 ± 3.6), and (4.2 ± 0.8) to (29 ± 13.6) ml, (7.8 ± 2.1), and (1.6 ± 0.5) respectively (P < 0.05), while Qmax increased from (5.6 ± 2.1) ml/s to (17.6 ± 2.7) ml/s (P < 0.05); the former three parameters in the αlA-b + ET group decreased from (133.6 ± 24.9) ml, (21.5 ± 3.2), and (4.7 ± 1.1) to (42 ± 18.3) ml, (12.8 ± 2.6), and (2.5 ± 0.7) respectively (P < 0.05), while the latter one increased from (6.3 ± 2.4) ml/s to (11.7 ± 2.3) ml/s (P < 0.05), all with statistically significant differences between the two groups (P < 0.05). The overall survival rate of the TURP + ET group was not significantly different from that of the αlA-b + ET group (51.4% vs 46.4% , P > 0.05).
CONCLUSIONTURP + ET is preferable to αlA-b + ET for its advantage of relieving BOO symptoms in advanced PCa without affecting the overall survival rate of the patients.

6.Studies on the chemical constituents of Lycianthes biflora.

AIMTo study the chemical constituents of Lycianthes biflora.
METHODSColumn chromatography was used to separate the chemical constituents. IR, MS, 1HNMR, 13CNMR and 2D-NMR technique were used to determine the structures of the isolated constituents.
RESULTSFive compounds were isolated from this plant. Their structures were identified to be bifloride A (1), N-trans-cinnamoyltyramine (2), liquiritigenin (3), N-trans-p-coumaroyloctopamine (4), 1-O-beta-D-glucopyranosyl-2-N-2'-hydroxypalmitoyl-sphinga-4- trans-8-trans-dienine (5).
CONCLUSIONCompounds 1 and 2 are new compounds, the others were isolated from this plant for the first time. Compound 2 showed inhibitory effects on P-388.

7.Treatment outcomes of multivisceral resection for locally advanced right colon cancer.

OBJECTIVETo investigate long-term survival after multivisceral resection in patients with locally advanced right colon cancer.
METHODSThe clinical data and survival of 13 patients with locally advanced right colon cancer were retrospectively analyzed.
RESULTSThere were 8 males and 5 females with a mean age of 58.6 years. Location of the primary tumor included hepatic flexure(n=6), transverse colon(n=2), and ascending colon(n=5). Three patients had duodenal invasion alone, 9 had involvement of duodenum and other organs, and 1 had pancreas and stomach involvement. Right colectomy and pancreaticoduodenectomy and(or) resection of other organs were performed. The 1-, 3-, and 5-year survival rates were 69%, 54%, and 30%, respectively.
CONCLUSIONRight colectomy combined with multivisceral resection is a promising procedure for selected patients with locally advanced colon cancer.

8.Construction of nano-silver coating on pure titanium surface and its antibacterial effect on Staphylococcus aureus

BACKGROUND:Nano-silver is a new antibacterial material developed based on nano-technology. It is characterized as stable physical and chemical properties, and good electrical, optical, and catalytic performance, but its antibacterial response to Staphylococcus aureus is controversial. OBJECTIVE:To explore the preparation methods of nano-silver coating on pure titanium surface and to explore its antibacterial effect on Staphylococcus aureus. METHODS:(1) Preparation of nano-silver coating on the surface of pure titanium: Hydroxyapatite and silver powder as research objects were fully mixed at a ratio of 20:1. The mixture was ball-milled to ensure the formation of nanosized particles. The mixture of hydroxyapatite and silver powder was put into a stirrer for continuous stirring to ensure the uniform distribution. The mixture was then placed in an ethanol solution followed by insertion of a titanium plate (anode) and a stainless steel plate (cathode). The precipitated powder was obtained on the titanium surface at a voltage of 20 V, and the pure titanium surface with nano-silver coating was prepared after heat treatment in a tube resistance furnace. (2) Observation of antibacterial properties: A minimal inhibitory concentration test was used to determine the antibacterial concentration of the nano-silver coating on the pure titanium surface when shaken with Staphylococcus aureus. A scanning electron microscope was used to observe the structure of Staphylococcus aureus on the pure titanium surface with nano-silver coatings of different mass concentrations. RESULTS AND CONCLUSION:Nano-silver/hydroxyapatite composite coating and hydroxyapatite coating were relatively uniform. The nano-silver/hydroxyapatite composite coating was white in color but slightly yellowish. The hydroxyapatite coating was white in color. The coating surface was rough and bonded firmly. No peeling of the coating was observed under gross observation. The antibacterial ability of the nano-silver group at 37℃ static culture and at 37℃ shaking culture was significantly higher than that of the hydroxy-apatite group (P<0.05). The absorbance value of Staphylococcus aureus at 600 nm in the nano-silver group was lower than that of hydroxyapatite group at 7, 10, 30 hours after intervention (P<0.05). The layer cells on the nano-silver coating become lighter in color compared with those on the hydroxyapatite coating and there were cells that ruptured and died. The number of Staphylococcus aureus on the nano-silver coating was reduced, and a large number of vacuoles were found. These findings indicate that the 20:1 mixture of hydroxyapatite and silver at micron level can be used to prepare nano-silver coating through ball milling, water bath, ultrasound and heat treatment. The prepared coating can exert excellent antibacterial effects on Staphylococcus aureus.
9.Development of disposable anticoagulant negative pressure syringe

Objective To develop a disposable negative pressure syringe to solve the problems in bone marrow aspiration. Methods The syringe was composed of an injector and a self-contained puncture needle.The injector consisted of a barrel,a plunger and a tip.The plunger had its front end placed in the barrel and connected with the tip.The tip clung to the inner wall and could slide along it,and the upper end of the cylinder was connected closely with a negative-pressure sealing cap.A negative-pressure cavity was formed between the sealing cap and the tip,and an anticoagulation layer existed at the cylinder inner wall in the cavity.The blood taking needle was made up of a bone puncture needle and a hub.Results The developed syringe gained stable negative pressure,low time and workload consumption during bone marrow aspiration when compared with the conventional tools. Conclusion The syringe increases the quality of bone marrow aspiration while decreases the workload of medical staffs,and thus is worthy promoting practically.
10.Suicide rate trends in China from 2002 to 2015

Objective:This study aimed to present the time trend of suicide rate in people aged 5 years and over in China from 2002 to 2015,and to describe the rural and urban distribution,and population characteristics of suicide rate in recent 13 years.Methods:The data of suicide rate were based on the China Health Statistics Yearbook published by the National Health and Family Planning Commission (formerly as the Ministry of Health) of the Peoples Republic of China.Poisson regression models were used to test the time trend of the general suicide rate,and region-,gender-and age-specific suicide rates.Results:Since 2002,the suicide rates in urban population,rural population,male and female have decreased significantly.Urban population had higher decreasing trend than rural population,and female had higher decreasing trend than male.Suicide rates in women of reproductive age (15 -49 years) both in rural and urban areas had significantly decreasing trends,and were lower than male with same ages in recent years.There was no statistical difference of the decreasing trend in rural population aged 85 years and over,or young male population in rural area.The annual average suicide rate from 2012 to 2015 was 6.75/100 000.Rural population,male and the older persons had higher suicide rate.Conclusion:The overall suicide rate decreased over the period of 2001-2015.However,it is still important to carry out suicide prevention program,especially in rural area,and in older persons and young male.
