1.Analysis of the Factors of Influence on the Therapeutic Effect of Primary Hepatic Carcinoma after Interventional Therapy

Objective To evaluate the factors of influence on the prognosis and survival rate in patients of primary hepatic cellular carcinoma(HCC) after interventional therapy.Methods 258 cases of HCC were treated with interventional therapy,the data of these cases and the survival rate were analysed.Results The prognosis in patients with HCC was relation to the hepatic function,therapeutic methods and accumulation of iodized oil by analysis of Cox Regression.The cumulative survival rate was different in HCC with different pathological type,different grade of hepatic function,with or without tumor embolus in portal vein,different cumulation of iodized oil and different therapetic methods,there was significance in statistics between most of them.Conclusion Cox Regression model can be used to analyse synthetically the factors that significantly affected the survival period in the patients with HCC.
2.Inducing Mesenchymal Stem Cells from Adult Human Adipose Tissue to Differentiate into Cardiomyocytes In Vitro

ObjectiveTo study the differentiation of adipose tissue-derived mesenchymal stem cells (ADMSCs) into cardiomyocytes in vitro. MethodsADMSCs were isolated and purified by the method of digesting and adhering to the culture plastis. The third generation of cells were determined with immunocytochemistry method and induced by 5-aza. On the 7th, 14th, 21st, 28th day after being induced, the surface antigen of myoblast cell and cardiac myocyte were determined, the expression of gene of GATA4 and Nkx2.5 were tested, and the content of atrial natriuretic polypeptide (ANP) assayed. ResultsImmunocytochemical staining showed CD44, CD13, CD105 positive, CD45, CD34, HLA-DR, factor Ⅷ negative. After being induced by 5-aza, the direction of cell arraying was gradually similar. The cells were stained positively for Desmin, α-Sarcomeric Actin, myosin heavy chain and Troponin T. The cells express GATA4 and Nkx2.5 and secrete ANP. ConclusionThere was mesenchymal stem cells in human adipose tissue. 5-aza may induce ADMSCs to differentiate into cardiomyocytes in vitro.
3.Functional magnetic resonance imaging on acupuncturing Yuan-Source and He-Sea acupoints of stomach Meridian of Foot-Yangming.

OBJECTIVETo explore the functional brain localization with magnetic resonance imaging (MRI) after acupuncturing the Yuan-Source and He-Sea acupoints of Stomach Meridian of Foot-Yangming (ST).
METHODSThe study was performed in 30 healthy volunteers who underwent acupuncture at Yuan-Source acupoint (Chongyang, ST42) and He-Sea acupoint (Zusanli, ST36) (ST group). Ten of these were also underwent acupuncture at the non-acupoints as the control group. Blood oxygenation level dependent functional MRI was performed.
RESULTSIn the ST group, signal increasing areas were demonstrated in bilateral superior temporal gyri (Broadmann 22), bilateral supramarginal gyri (Broadmann 40), bilateral cerebellar hemispheres, bilateral cingulate gyri and isthmus of cingulate gyri (Broadmann 32, 30), bilateral superior parietal lobules (Broadmann 7); signal decreasing areas were shown in bilateral orbital gyri (Broadmann 11), bilateral temporal pole (Broadmann 38), right inferior frontal gyrus (Broadmann 47) and right medial occipitotemporal gyrus (Broadmann 36). In the control group, signal increases areas were demonstrated in superior temporal gyri, precentral gyri, cingulate gyri, thalamus, insula and cerebellum. The size, signal intensity and number of increasing areas in control group are less than in ST group.
CONCLUSIONCombined acupuncture of Yuan-Source and He-Sea acupoints of ST can activate and decrease the multiple brain regions of "splanchnic brain" and thus reach a new functional balance to relieve pain.

4.Value of the hypointensive internal septation on MR for the differentiation between benign and malignant breast lesions

Objective To investigate the diagnostic value of internal septation for differentiating benign from malignant breast lesions.Methods A total of 26 patients were included in the study,in which 12 patients had 20 lesions of breast carcinoma and 14 patients had 25 lesions of fibroadenoma diagnosed either pathologically or clinically.The differential diagnoistic value of the hypointensive internal septation was analyzed.Results The signal intensity of fibroadenomas and malignant lesions on T_2-weighted fat- suppressed images could be classified as iso- to hyper- intensity,hypointensity and mixed intensity. According to the signal intensity classification,there were 5,11 and 4 cases in patients with breast carcinoma respectively,while 11,10,4 cases in patients with fibroadenoma respectively.There was no statistical difference in the distribution between the two patient groups(?~2=1.764,P=0.414).The shape of fibroadenomas and malignant lesions could be classified as irregular、roundish or lobulated.According to the morphological classification,there were 12,7 and 1 case in patients with breast carcinoma respectively, while 1,7,17 cases in patients with fibroadenoma respectively.There was statistical difference in the distribution between the two patient groups(?~2=23.262,P=0.000).The typical features of fibroadenomas were as follows:lobulated shape,hypointensive internal septations on T_2-weighted or postcontrast images. The diagnostic sensitivity of the three imaging features for fibroadenoma was 68%(17/25),52%(13/25), and 72%(18/25)respectively;and the diagnostic specificity was 95%(19/20),90%(18/20),95% (19/20)respectively.Conclusion The internal septation is a rather specific sign for diagnosis of fibroadenomas.

Objective Discuss the different between minimally invasive and conventional thoracotomy mitral valve replacement surgery.Methods Select 141 cases from February 2009 to December 2012 in our hospital suffer mitral valve replacement surgery.69 cases minimally invasive surgery and the 72 cases conventional thoracotomy mitral valve replacement surgery,mechanical valve 90 cases,the bioprosthesis 51 cases.The establishment of cardiopulmonary bypass is through the femoral artery and vein and the right jugular vein with cannulation.Under the guide of transesophageal echocardiography (TEE) and adjust the the intubation position to the inferior vena cava and superior vena cava junction.Double-lumen endotracheal intubation in trachea.Transthoracic approach through the right side of the stemum 4 intercostal,the left lung unilateral breathing and fight lung collapse.Open the pericardium with minimally invasive surgical instruments away 2 cm from the phrenic nerve.Transthoracic chitwood clamp blocking the ascending aorta,HTK or crystalloid cardioplegia aortic root perfusion.Arrest heart minimally invasive mitral valve replacement surgery.After CPB,unplug the femoral artery and vein catheter,6-0 prolene suture femoral artery reconstruction pathway.Results Minimally invasive compared to the conventional median thoracotomy mitral valve replacement surgery have no significant difference in operative time,cardiopulmonary bypass time,aortic clamping timeand the intensive care unit (ICU) time.Conclusion Overcome the learning curve,minimally invasive mitral valve surgery have many advantages than the conventional median thoracotomy surgery is a safe,effective,and easy to spread surgery.
6.Impacts of the Decoction of Fortifying the Spleen and Supplementing Qi on the cellular immunity of chronic Hepatitis B virus carriers

Objective To observe the impacts of the method of fortifying the spleen and supplementing Qi on the cellular immunity of chronic hepatitis B virus(HBV)carriers.Methods Asymptomatic HBV carriers group(ASC group,n=60)with HBV DNA positive were randomly divided into Fortifying the Spleen and Supplementing Qi decoction group(A group,n=30)and conventional treatment group(B group,n=30),the peripheral blood T lymphocyte subsets were detected with flow cytometey and compared with those of the healthy people(The normal control group,n=18).All the patients were treated,and 24 weeks after treatment the indexes were measured again.Results As compared with those of normal controls,the numbers of CD4+ T cells and CD8+ T cells were decreased significantly in HBV carriers(P<0.01);There was a statistical significance between the values before and complete the treatment with the decoction of fortifying the spleen and supplementing Qi(P<0.05 or P<0.01),and compared with conventional treatment group,there Was a statistical significance(P<0.05).Conclusion T-cell subset function was disturbed in the HBV carriers.The method of fortifying the spleen and supplementing Qi could improve the cellular immunity of carriers with chronic hepatitis B.
7.Clinical effect of reconstructive new tricuspid valvar annulus technique and prosthetic ring in the anatomic correction of Ebstein's anomaly: 60 cases report

Objective The aim of this study is to evaluate the early and midterm results of reconstructive new tricuspid valvar annulus technique and prosthetic ring in the anatomic correction of Ebstein's anomaly.Methods 60 operations for Ebstein's anomaly were performed by the reconstructive new tricuspid valvar annulus technique,prosthetic ring was used to reinforce the new tricuspid valvar annulus,the plastic effect of tricuspid valve was observed real-time by trans-esophagus echocardiography during operation.The follow-up results of the plastic effect of tricuspid valve were studied after operation.Results Mean operation time was (3.1 ± 2.3) h,mean cardiopulmonary bypass time was (148.8 ± 44.6) min,mean aortic cross-clamp time was(112.6 ± 34.1) min.3 patients had tricuspid valve replacement because of bad plastic effect;1 patient had bidirectional shunt Glenn procedure because of right sided heart failure.Hospital mortality was 1.When the patients were discharged,tricuspid regurgitation severe 1,medium 5,mild or no 50;53 being of heart functional class Ⅰ and class Ⅱ,3 of classⅢ.45 patients (80.3 %) were followed-up out-hospital from 6 months to 7 years,mean (24.4 ± 21.4) months.Tricuspid regurgitation severe 1,medium 6,mild or no 38.And the nice stability of tricuspid valve was found during the follow-up period,41 patients being of heart functional class Ⅰ or Ⅱ (NYHA),4 of class Ⅲ.1 need reoperation.Conclusion The reconstructive new tricuspid valvar annulus technique can maintain farthest the dimension and geometric configuration of functional right ventricle,longitudinal plication of the atrialized right ventricle can restore a large ventricular cavity,improve it's pump function,and make valvuloplasty perfect,which widen obviously the indication of tricuspid valvuloplasty for Ebstein's anomaly.The research concluded that this technique can correct anatomically Ebstein's anomaly effectively,and it can get the stable effect of tricuspid valvuloplasty obviously if use prosthetic ring to reinforce the new tricuspid valvar annulus.
8.Clinical application of minimally invasive cardiac surgery in secondary or multipal heart surgery

Objective To review and analyze minimally invasive right thoracotomy and peripheral cannulation as an alternative strategy in redo heart surgery.Methods From June 2012 to December 2016, 23 reoperative heart procedures were performed through a right lateral thoracotomy with about 5 cm in the third or fourth intercostal space,according to the different diagnosis with the corresponding operations.Results We performed 23 redo heart surgeries,there were 10 female and 13 male patients with a mean age of 41.43 years(range,20 to 69 years).4 mitral perivalvular leakage, 1 prosthetic valve dysfunction, 3 mitral insufficiency after mitral valvuloplasty, 1 aortic insufficiency after ventricular septal defect and aortic repair, 1 mitral insufficiency after ventricular and atrial septal defect repair, 3 residual fistula after surgery repair of ventricular septal defect, 1 tricuspid insufficiency after mitral and aortic valve replacement, 1 tricuspid insufficiency after left atrial myxoma removal,1 tricuspid insufficiency after Bentall surgery, 2 tricuspid insufficiency after mitral valve replacement, 1 mitral insufficiency after ventricular septal defect and mitral valvuloplasty, 2 tricuspid insufficiency after the repair of Ebstein abnormality.We respectively carried out repair of valve leakage, mitral valve replacement, aortic valve replacement, mitral valve repair, tricuspid valvuloplasty, postoperative repair of residual shunt of ventricular septal defect, redo repair of Ebstein abnormality.The effect of the whole group operation of operation was satisfactory, no death during operation and severe complications.All patients were under cardiopulmonary bypass,three patients' ascending aortas were bloked, and nine patients were with cardiopulmonary bypass.It indicated tha the operation was possible at operation time,total blood loss and the frequency of blood transfusion.Conclusion The application of closed thoracic cardiopulmonary bypass and minimally invasive right thoracotomy in redo or multiple cardiac surgery,can avoid reoperative sternotomy risk such as hemorrhage, tissue damage and so on.It shortened the operation time, reduced the risk of surgery and blood transfusion rate, can be used in some of the heart surgery patients again and repeatedly.
9.Effects on activation of protein kinase C and phosphorylation of glutamate receptor 2 by electromagnetic irradiation in rat cerebellum

Objective To investigate the effects of electromagnetic irradiation on activation of protein kinase C(PKC)and phosphorylation of glutamate receptor 2(GluR2)in rat cerebellum.Methods Sixty male Wistar rats were divided randomly into a control group and an electromagnetic exposure group(including 5 subgroups ob- served at different time points after the irradiation,eg.0 hour,3 hours,12 hours,24 hours and 72 hours after irradi- ation),with 10 rats in each group.All the rats in the exposure group were exposed to 90 mW/cm2 electromagnetic ir- radiation for 20 minutes,their rectal temperature was detected immediately after irradiation and the specific absorption rate(SAR)value was calculated,activation of PKC was detected with improved TaKai method,the level of cerebellar GluR2 expression and phosphorylation(ser880)was detected by using Western blot.Results Immediately(0 hour)after exposure,the rectal temperature of rats increased 2.99℃,SAR value was 8.66 W/kg.When compared to the control group,it was found that there was no significant difference between the exposure group and the control group with regard to all the parameters at 3,12,24 and 72 hours after exposure,except that the cerebellar PKC acti- vation and GluR2(ser 880)phosphorylation decreased significantly immediately after irradiation.Conclusion The electromagnetic irradiation has injurious effects on cerebellar signal pathway of for motor learning.
