2.Effect of photodynamic treatment on the secretion of tumor necrosis factor α by human vascular endothelial cells

Objective To measure the changes in levels of tumor necrosis factor α(TNF-α) secreted by human endothelial cells EC-304 after hematoporphyrin monomethyl ether-photodynamic treatment (HMME-PDT),and to explore the relationship between cytokines and inflammation initiation after management of nevus flammeus with photodynamic therapy.Methods EC-304 cells were cultured in 6-well plates,and classified into 4 groups:HMME-PDT group pretreated with HMME followed by irradiation with laser,HMME control group treated with HMME only,laser control group irradiated with laser only,and blank control group without any treatment.Culture supematants of EC-304 cells were collected from HMME-PDT group at 12,24 and 48 hours after the irradiation,and from the other three groups at the same time points.The supernatant TNF-α level was measured with enzyme linked immunosorbent assay (ELISA).Results The difference was statistically significant in the supernatant TNF-α level between different time points in each group (F=62.276.P<0.01) and between the 4 groups at each time point (F=11.538,P<0.01).Multiple comparison analysis showed that HMME-PDT group differed significantly from the other 3 control groups in the supernatant TNF-αlevel at each time point (all P<0.01),while no significant difierence was observed among the other three control groups at any time point (all P>0.05).Conclusion HMME-PDT promotes the secretion of TNF-α by EC-304 cells.

4.ECG signals processing and transmission system based on virtual instrument and remote network

Objective To develope a realtime system for ECG signals acquisition,amplification and network transmission.Methods The raw noisy ECG signals underwent gain amplification,denoising and filtering by a system developed by virtual instrument and LabVIEW,and network component programming was carried out based on TCP/IP.Then the processed ECG signals were transmitted to remote terminals with dual communication model.Results The system behaved well in easy operation,high reliability and man-machine interface,and could be used to realize remote realtime transmission and browsing of ECG signals between the hospital and medical communities.Conclusion The system may be a choice for ECG signals remote acquisition and transmission,and provides references for the development of telemedicine software.
5.Effect of phacoemulsification versus extracapsular cataract extraction in patients with intumescent senile cataract

Objective To compare the safety and effect of the phacoemulsification (PHACO) versus extracapsular cataract extraction (ECCE) in patients with intumescent senile cataract.Methods 200 eyes from patients with intumescent cataract were included and randomly divided into 2 groups:PE group (108 eyes,received PHACO) and ECCE group (92 eyes,received small incision ECCE) respectively.Superior quadrant sclera tunnel incisions were made with stabs of 2.8 mm diameters.Trypan-blue was used to show the anterior lens capsular membrane.In the PHACO group,a 4 mm diameters continuous curvilinear capsulorhexis (CCC) was made,which was enlarged to 6-7 mm after the intraocular lenses (IOL) implantation.For the ECCE group,a 8 mm-diameter CCC was made.Then the lens nucleus was either phacoemulsificated or delivered and IOL was implanted.The complications during and after surgery and the visual outcomes were recorded and statistically analyzed by SPSS 13.0 software.Results Age,sex and the hardness of the nucleus were comparable between the two groups.The best corrected vision acuity (BCVA) was 0.05 and worse in all patients before surgery.While 3 days after operation,in PE group,105 eyes (97.2%,105/108) gained postoperative vision 0.05 and better,and 82 of them were better than 0.3; in group ECCE,97.2% (89/92) of the operated eyes gained vision 0.05 and better,72 (78.3%,72/92) eyes were better than 0.3.No statistical differences were found between the two groups in postoperative vision recovery.While,there were more failure rates of the CCC,tear of the anterior and posterior capsular,loss of the vitreous and iris injury rate in the ECCE group than in the PE group (20.7% vs.8.3%,17.4% vs.12.0%,7.6% vs.0.9%,15.2% vs.0%,P=0.01,P=0.00,P=0.02,P< 0.001).Prolapse of iris and discoria were found in ECCE group.Conclusions With small CCC,phacoemulsification can lead to better surgical outcomes than small incision ECCE procedures,and the operative and post-operative complications are less in PE group than in ECCE group.
6.Analysis of morbidity, diagnosis and treatment in 26 pulmonary infection patients after renal transplantation

Objectives: To analyze the strategies of the diagnosis and treatment in patients with the pulmonary infection after renal transplantation. Methods: A retrospective analysis was made on 28 cases of pulmonary infections among 285 patients undergoing renal transplantation. Results: The morbidity of pulmonary infection was 9.82%(28/285). The majority pathogens of pulmonary infection in this study were pseudomonas, staphylococcus epidermidis, canadida and cytomegalovirus. Chest X ray was important in the diagnosis, but there was no specificity to determine the pathogens. The mortality in 28 pulmonary infection patients was 32.1%(9/28). Most of the patients suffered from pulmonary infection within 4 months after the operation, and the postoperative period from 2 months to 4 months was a high risk time window for death caused by pulmonary infections. Conclusions: The morbidity and mortality of pulmonary infections, especially severe pneumonia, in patients with renal transplantation, are higher than healthy people. Treatment following clinical epidemiology and strategy of microbiology play a very important role to reduce the mortality in those patients.
7.Perioperative cardiovascular abnormality in elder patients with silent coronary heart disease.

OBJECTIVETo explore the perioperative cardiovascular dysfunction and its relevance to age in patients with silent coronary heart disease (or silent myocardial ischemia), and explore the clinical treatment and recovery of perioperative arrhythmias.
METHODSOne hundred and eighty cases were selected from selective surgery patients with silent myocardial ischemia (SMI). Among the cases, 130 patients older than 51 years old were divided into 51 - 60 year-old group, 61- 70 year-old group and 71 - 80 year-old group. Control group was set up by other 50 patients younger than 51 years old. Electrocardiogram data of 24 h before the operation, 24 h after the operation and 48 h after the operation were continuously monitored by dynamic electrocardiogram (DCG). The electrocardiogram data of ST shifting, arrhythmia incidences of different type and at different time were analyzed by professional doctors. At the same time, the treatment and recovery of perioperative arrhythmia were recorded.
RESULTSAs the age increase, the magnitude and duration of ST shifting appeared upward trend compared to the control group (P < 0.05, P < 0.01). The incidence of ST elevation in 71 - 80 year-old group was higher than the control group (P < 0.05). The ST depression duration in 61 - 70 and 71 - 80 year-old group and ST elevation magnitude in 71 - 80 year-old group were higher than 51 - 60 year-old group (P < 0.05). Compared to the control group, the incidence of accelerated idioventricular rhythm (AIR) in 61 - 70 year-old group and the incidence of sinus bradycardia (SB), ventricular premature beat (VPB), ventricular tachycardia (VT) in 71 - 80 year-old group were higher (P < 0.05, P < 0.01). Compared to the 51 - 60 year-old group, the incidence of atrial fibrillation (AF) in 61 - 70 year-old group and the incidence of VP, VT, AF in 71 - 80 year-old group were higher (P < 0.05, P < 0.01). The arrhythmia incidences in 24 h after operation were higher than 48 h after operation and 24 h before operation (P < 0.01). As the age increase, the recovery incidence by removing inducement was decreased, but the recovery incidences by drug and electric-shock treatment were increased (P < 0.05).
CONCLUSIONOld SMI patients have high levels of perioperative myocardial ischemia and arrhythmia, and 24 h after operation is the period of high incidence.

8.Influence of eye healthcare information teaching for the visual development of children under “combination of medicine and education”

Abstract?AIM:To study the Influence and outcomes of eye healthcare information teaching for the visual development of children under“combination of medicine and education” in kindergartens.?METHODS:The children(5-6 years old) were randomly selected from 6 kindergartens in Nanjing, 3 in Yuhua District as the experimental group, and the other 3 in Jianye District as the control group.A one-year follow-up was conducted to evaluate the difference of visual development, including the rate of low vision, rate of referral caused by refractive abnormality, rate of astigmatism, the average of the equivalent spherical lens and the rate of lacking physiological hypermetropia, between the experimental group and the control group children.?RESULTS:One school year later,the rate of low vision, rate of referral caused by refractive abnormality and the rate of astigmatism, were significantly lower (P<0.05)in the experimental group under the mode of“combination of medicine and education”, which were also lower than those before experiment(P<0.05).The average of the equivalent spherical lens of experimental group increased and the rate of lacking physiological hypermetropia decreased significantly, compared with the control group ( P <0.05 ) and with those before experiment(P<0.05).?CONCLUSION:Using “combination of medicine and education” eye health care model,is good for children's visual development,so as to reduce the rate of low vision and delay the occurrence of myopia.
9.Preparation and Characteristics of Patterned TiO2 Thin Film onTC4 Made by Sol-gel Method

Objective To improve the long-term biocompatibility and mechanical stability of Titanium alloy implants. Method micro-nano patterned TiO2 thin films have been prepared on TC4 substrates via a sol-gel dip-coating method with Ti(OC4H9)4 (TEOT) as precursor and polyethylene glycol (PEG) as organic template. The influence of PEG concentration upon the morphology of the target films has been studied by using AFM. The wettability and tribological properties of the target films have been studied by means of optical contact angle measuring instrument and UMT-2. Results When increasing the PEG concentration to 3.5g/100mL, a parallel arranged groove structure can be obtained on the surface of the sample; compare with TiO2 sol-gel film without adding organic template, the wear resistance of the patterned film is better. Conclusions The feature of the patterned TiO2 films can be controlled by adding different amount of organic template; and it has been proved that a certain kind of patterned structure can improve the wear resistance of the film.
10.Effects of different anesthesia methods on perioperative hemodynamics and ECG in old CHD patients.

OBJECTIVETo explore an optimal anesthesia method with less impact on hemodynamics and electrocardiogram (ECG) of old patients with coronary artery disease (CHD) during abdominal operation.
METHODSThe 133 CHD patients waiting for abdominal operation were randomly divided into continuous epidural anesthesia (EA) group, general anesthesia group (GA) and combined spinal-epidural anesthesia (CSEA) group. Continuous monitoring was carried out during operation and mean arterial pressure (MAP), heart rate (HR), oxygen saturation (SaO2), abnormal ECG were measured respectively at different time for comparison and the differences of the above hemodynamic parameters and abnormal ECG features were compared among the 3 groups.
RESULTSAt the 15 min and 30 min point after anesthesia, SaO2 in GA group was significantly increased compared to that in the EA group (P < 0.05). At 15 min, 30 min and 60 min point after anesthesia, MAP in CSEA group was significantly increased compared to that in the EA group (P < 0.05). At 30 min point after anesthesia, HR in CSEA group was increased significantly compared to the EA group (P < 0.05). At 15 min and 30 min point after anesthesia, SaO2 in the CSEA group was increased significantly compare to the EA group (P < 0.05). Compared with preanesthesia (T0) in EA group, MAP, HR and SaO2 decreased significantly at 15, 30 and 60 min after anesthesia (P < 0.05). The fluctuation of the three parameters in GA and CSEA groups were relatively small (P > 0.05). As well as the comparison of abnormal ECG among the 3 groups was concerned, the incidence of ST-T changes in GA and CSEA groups were significantly lower than that in EA group at the time of 15 min, 30 min and 60 min after anesthesia and at the time of surgery termination (P < 0.05, P < 0.01). The incidence of arrhythmia in GA and CSEA groups were significantly lower than that in EA group at the time of 15 min, 30 min and 60 min after anesthesia (P < 0.05, P < 0.01). Compared with T0 in the same group, the incidences of ST-T changes and arrhythmia in GA or CSEA group at the time of 15, 30 and 60 min after anesthesia and at the time of surgery termination were significantly lower than that before anesthesia (P < 0.05, P < 0.01).
CONCLUSIONGA and CSEA have less impact on hemodynamics and have smaller incidence of abnormal ECG of old CHD patients with abdominal operation.
