2.Controversy and consensus of the therapy for severe acute pancreatitis

With the characteristics of complicated etiologic factors,multiple complications and high mortality,severe acute pancreatitis (SAP) has been treated by all kinds of clinical management.Management of SAP is a hot topic in recent decades.Researches on the pathophysiology of SAP have advanced dramatically during the past 20 years.As the number of randomized controlled studies of SAP has steadily increased,mainly in the United States and Europe,evidence based management was demanded for the management of SAP.The main improvement of the treatment results was achieved following internationally accepted recommendations; however,some aspects of the management are different and need to be discussed.There has been no consensus regarding the treatment of SAP.Failure to provide definitive treatment may potentially put a patient at risk of a fatal episode of SAP.The aims of this study,therefore,were to survey the currently adopted practice of the management of SAP,to evaluate the impact of various subspecialty interests on the practice adopted,and to suggest pathways towards a better implementation of the guidelines and other evidence-based management approaches.
4. Relationship between serum adiponectin level and non-alcoholic fatty liver diseases

Objective: To observe the changes of serum adiponectin level in patients with non-alcoholic fatty liver disease (NAFLD),and to study the correlation between the serum level of adiponectin and NAFLD. Methods: A total of 110 patients with NAFLD who were hospitalized from Feb. 2006 to Mar. 2008 were included in this study. The patients were divided into non-alcoholic steatohepatitis (NASH) group (n=52,group 1) and simple fatty liver group (n=58,group 2). Fifty-four healthy volunteers were taken as normal control (group 3). The height, weight, and body mass index (BMI) were measured in all groups. The fasting blood samples were obtained to examine serum adiponectin, TC, TG, HDL-C and LDL-C. Stepwise regression method was used to analyze relationship between the serum level of adiponectin and NAFLD. Results: The serum adiponectin and HDL-C levels in group 1 and group 2 were significantly lower than those in group 3 (P<0.01); the levels of TC,TG,and LDL-C were significantly higher than those of group 3 (P < 0.01). The serum adiponectin in group 2 was significantly lower than that in group 1 (P<0.05). Lower serum adiponectin and HDL-C levels (P<0.05) and higher AST level (P<0.01) were found in group 2 than those in simple fatty liver patients. In non-alcoholic steatohepatitis patients,serum adiponectin level was negatively correlated with the levels of TC, TG (r = - 0.436, P < 0.05; r = - 0.567, P < 0.01, respectively) and positively correlated with the level of HDL-C (r=0.524,P<0.01). Conclusion: The serum adiponectin level is correlated with the severity of NAFLD patients, and examination of serum adiponectin may be helpful in understanding the status of NAFLD patients.
5. Relationship of local recurrence with expression of protein P53 and PCNA in laryngeal carcinoma and its surgical margins

Objective: To explore the relationship of the local recurrence with the expression of protein P53 and PCNA in the primary lesions and the surgical margins of laryngeal carcinoma. Methods: The primary lesions and the surgical margins of laryngeal carcinoma of 36 patients were made into serial sections. Immunochemical method (H-E staining) was used to detect the expression of P53 and PCNA protein. Results: The positive rates of protein P53 and PCNA were 55.6% (20/36) and 88.9% (32/36) in the primary lesions,and 25%(9/36) and 30.6% (11/36) in the surgical margins, respectively. The recurrent rates of the primary lesions positive for P53 and PCNA were 35% (7/20) and 25% (8/ 32), respectively, which were higher than those negative for them (12.5% [2/16] and 25% [1/4]). The recurrent rates of surgical margins positive for p53 and PCNA were 44.4% (4/9) and 72.7% (8/11), respectively, which were higher than those negative for them (18.5% [5/27] and 4% [1/25]). The expression of P53 and PCNA proteins in the primary lesions and surgical margins was abnormal; the recurrent rate of the laryngeal carcinoma was 100% (4/4) after surgery. Conclusion: P53 and PCNA protein can be used as biomarkers for local recurrence of laryngeal carcinoma after operation. Detection of P53 and PCNA in both the primary lesions and the surgical margins may be of greater significance in forecasting local recurrence.
6.Encephalopathy therapeutic tongue acupoint apparatus (ETTAA) for 42 cases of autism.

OBJECTIVETo observe the efficacy of encephalopathy therapeutic tongue acupoint apparatus (ETTAA) in the treatment of autism.
METHODSEighty-four children of autism were randomly divided into a tongue acupuncture group (group A) and a conventional training group (group B), 42 cases in each group. The behavior training and sensory integration training were carried out in group B and the ETTAA was added in group A. The apparatus was switched on for 20 min every time and 3 times a day. Treatment of two months were carried out in both groups. The score of childhood autism rating scale (CARS) and clinical efficacy in both groups were observed before and after treatment.
RESULTSAfter treatment, the CARS in both groups were significantly reduced (42.39 +/- 6.86 vs 32.15 +/- 5.12, P < 0.001; 44.58 +/- 6.76 vs 39.72 +/- 7.11, P < 0.05), which was more significant in group A (P < 0.01). The totally effective rate in group A [90.5% (38/42)] was superior to that in group B [66.7% (28/42), P < 0.01].
CONCLUSIONThe clinical efficacy in tongue acupuncture group is apparently superior to that in conventional training group, ETTAA combined with conventional training have a better curative effect in the treatment of autism.

7.Establishment and Evaluation the Review Criteria for Sysmex KX21 Blood Cell analyzer

Objective To find a proper way for accurate results of blood counts,and provide reference for the user of 3 cluster blood cell analyzer.Methods Based on the results of blood cell analyzer KX21 and International Consensus Review Rules,set up the criteria for blood cell microscopic examination,1700 blood specimen were detected and the results were analyzed according to the criteria..Statistics analysis were made to evaluate the accordance between warnings of analyzer and manual examination,likewise the reliability of the criteria.Results According to the review rules and slide riview positive criteria[1],the true positive ratio,the false positive ratio,the true negative ratio and the false negative ratio were 11.88%(202/1 700),21.88%(372/1 700),64.29% (1 093/1 700) and 1.94% (33/1 700) respectively..No blast cell and immature cell was missed.Conclusion The smear review criteria suggested by the International Consensus Group could facilitate to establish the review criteria for 3 cluster blood cell analyzer..The review rules used on KX21 blood cell analyzer in this study are suitable for our laboratory.
8.Role of color Doppler ultrasonography in the differentiation of vein thrombosis and regional hematoma of hemodialysis arterio-venous fistula

A total of 62 cases with autogenous arteriovenous fistulas were examined bv color Doppler ultrasonography.Venous thrombotic manifestations (n =13) and large local hematoma (n =3) were detected.The diameter ranges were around (1.8 ± 0.7) cm for arteriovenous fistula and (2.0 ± 0.8) cm for local hematoma.And there was no statistical significance (t =1.63,P > 0.05).According to edge shape,presence or absence of flow signal and fistula searching,it could be used to identify venous thrombosis and local hematoma of arteriovenous fistula.Color Doppler ultrasound is helpful for the differential diagnosis and the mass properties of arteriovenous fistula so that it provides useful diagnostic information for the clinicians.
10.MRU features of renal tuberculosis in relation to pathologic changes

Objective To evaluate the diagnostic value of MR urography(MRU) in the diagnosis of renal tuberculosis. Methods MRU features of 18 cases of renal tuberculosis,nonvisualized or poorly visualized in IVU,were analyzed and compared with pathologic findings. Results MRU provided high-resolution images of the kidneys and upper urinary tract in all patients.MRU features of renal tuberculosis and corresponding pathologic changes were summarized. Conclusions MRU has a great value in diagnosing renal tuberculosis when the kidneys are nonvisualized or poorly visualized in IVU.It can provide diagnostic evidence and is helpful with choice of the therapeutic strategy.
