1.Study on Quality Standards for Shufeng Zhike Extract

Objective To establish the quality standards for Shufeng Zhike Extract;To improve the controll ability of the extract and ensure medicine efficacy. Methods To establish a HPLC method for simultaneous differentiation of Paeoniae Radix Alba, Saposhnikoviae Radix, Polygoni Cuspidati Rhizomaet Radix, Glycyrrhizae Radix et Rhizoma, Belamcandae Rhizoma and Bupleurum in Shufeng Zhike Extract. Phenomenex Kintex C18 column (4.6 mm×100 mm, 2.6 μm) was used. Acetonitrile-0.05%phosphoric acid was as the mobile phase in gradient elution (0–25 min, 10%acetonitrile; 25–26 min, 10%→14% acetonitrile; 26–35 min, 14% acetonitrile; 35–36 min, 14%→34% acetonitrile;36–55 min, 34%acetonitrile) at flow rate of 0.8 mL/min, and column temperature was 40 ℃. The detection wavelength was 250 nm. To establish a HPLC method for simultaneous determination of Paeoniflorin and Prim-O-glucosylcimifugin in Shufeng Zhike Extract, Phenomenex Luna C18 column (4.6 mm×150 mm, 5 μm) was used; acetonitrile-0.05%phosphoric acid (12:88) was as the mobile phase at flow rate of 1 mL/min; column temperature was 40 ℃; the detection wavelength was 250 nm. Results The chromatographic peak separation with HPLC method for simultaneous differentiate of Paeoniae Radix Alba, Saposhnikoviae Radix, Polygoni Cuspidati Rhizoma et Radix, Glycyrrhizae Radix et Rhizoma, Belamcandae Rhizoma and Bupleurumin in Shufeng Zhike Extract was clear and negative control had no interference. The linear range of Paeoniflorin was 0.035 93–2.514 8 μg (r=0.999 5), and the average recovery was 100.54%(n=6). The linear range of Prim-O-glucosylcimifugin was 0.006 7–0.67 μg (r=0.999 5), and the average recovery was 100.39%(n=6). Conclusion The established quality standards are simple, reliable, and accurate. It can rapidly identificate Paeoniae Radix Alba, Saposhnikoviae Radix, Polygoni Cuspidati Rhizomaet Radix, Glycyrrhizae Radix et Rhizoma, Belamcandae Rhizoma and Bupleurum, and suitable for rapid determination of Paeoniflorin and Prim-O-glucosylcimifugin, which can provide the basis for the quality inspection of Shufeng Zhike Extract.
2.Features of Computed Tomography Perfusion of Mediastinal Lymphadenopathies: a Pathology-based Retrospective Study.

OBJECTIVETo explore the features of various mediastinal lymphadenopathies using computed tomography perfusion (CTP).
METHODSCTP parameters (CTPs) of the selected mediastinal nodes from 59 patients with pathology-proven malignant lymph nodes and of those from 29 patients with clinically diagnosed or pathology-proven inflammatory lymphadenopathies were collected. Patients were divided into subgroups by etiology and phase of primary disease, including different pathological malignant nodes and diverse inflammatory nodes. CTPs were defined as blood flow (BF), blood volume (BV), mean transit time (MTT), permeability (PMB), and time to peak (TTP). Differences of CTPs were compared between malignant and benign nodes, and among subgroups, respectively.
RESULTSIn the mediastinum, no significant differences of CTPs were found between malignant and benign groups (all P>0.05), the same for subgroups of malignant nodes (all P>0.05). Acute lymphadenitis had higher BF and BV than chronic inflammatory, lymphoid tuberculosis, sarcoidosis and malignant nodes. The BF of malignant nodes was markedly slower than that of acute lymphadenitis (P=0.01), but faster than chronic inflammatory nodes (P=0.04) and sarcoidosis (P=0.03), with no significant difference compared with lymphoid tuberculosis. Pneumonia-complicated lymphoid tuberculosis showed the longest MTT while sarcoidosis displayed the shortest MTT, and inflammatory nodes, lymphoid tuberculosis without complicated pneumonia and malignant nodes had moderate MTT.
CONCLUSIONCTPs show promising potential in distinguishing various lymphadenopathies in the mediastinum, but more studies are needed to improve their specificity.

3.Study on Quality Standard for Lianqiaobaidu Pills

Objective To imporve the quality standard of Lianqiaobaidu Pills. Methods The TLC method was established for ephedrae herba, sophorae flavescentis radix, paeoniae radix rubra, and RP-HPLC methed was established for the determination of chlorogenic acid in Lianqiaobaidu Pills. Zorbax SB-C18 (4.6 mm×150 mm, 5 μm) column was used with methanol-acetonitrile (2∶1) - 0.04% phosphoric acid (10∶90) as the mobile phase at flow rate of 0.8 mL/min. The detection wavelength was 326 nm. Results TLC chromatographic spots were clear, and negative control had no interference. The linear range of chlorogenic acid was 0.042 2-0.464 2 μg (r=1), the average recovery (n=6) was 98.96%. Conclusion The method is simple, reliable, with strong exclusive feature, and suitable for quality control of Lianqiaobaidu Pills.
4.Baseline survey of core competency of mobile nurses and lateral comparison with domestic nurses in three-class A level hospitals

Objective To measure baseline and influencing factors of core competency of mobile nurses when they join the team and lateral comparison with the domestic nurses,and provide the scientific basis to develop targeted training programs.Methods Using Competency Inventory for Registered Nurse (CIRN) to conducted a questionnaire survey on 56 mobile nurses.Results The core competencies of mobile nurses in our hospital scored (162.82±25.47) points,at a medium level throughout the country.The dimensions of legal and ethical practice were with the highest mean scores,and dimensions of critical thinking and scientific research ability were with the lowest mean scores,professional interests,career development prospects,degree of clinical ability had positive correlation with core competence scores of mobile nurses.Conclusions Core competencies of mobile nurses were at the middle to upper level compared with domestic nurses of the same seniority,but the cultivation for clinical thinking and research ability should be strengthened,and positive encouragement and opportunities for career development of mobile nurses would be necessary.
5.Emergency Treatment Experiences of Acute & Chronic Renal Failure with Hyperpotassemia

Objective To analyze the clinical conditions and emergency treatment experiences of acute and chronic renal failure with hyperpotassemia. Methods Retrospective analyzed 542 clinical cases of supervening hyperpotassemia, which were treated by hemodialysis on 186 patients of renal failure with hyperpotassemia for 5 years. Results 540 cases clinical symptom was improved, serum potassium concentration was down to normal range and hyperpotassemia disappeared in electro cardiogram. 2 cases died after salvage. Rate of success was 99.6%. Conclusion Hemodialysis therapy is the most effective treatment to hyperpotassemia. Use bicarbonate with kalium pH indicator of 3.0~3.5mmol/L in dialysis is useful to lower hyperpotassemia, and safe also. In the early stage of hemodialysis, pure ultrafiltration dehydration treatment is not suitable. There is still danger of heartbeat stop deal to high kalium in the early stage of dialysis; emergency treatments should be standby all the time.
6.Study on Determination Method of Luteolin-7-O-glucoside in Compound Luobuma Granule

Objective To establish determination method of luteolin-7-O-glucoside in Compound Luobuma Granule. Methods Phenomenex luna C18 column (4.6 mm×150 mm, 5 μm) was used;aceto-0.5% acetic acid (14∶86) was set as the mobile phase at flow rate of 0.8 mL/min;the detection wavelength was 348 nm. Results The linear range of luteolin-7-O-glucoside was in the range of 0.031 9-0.796 3 μg (r=0.999 6), and the average recovery was 100.85% (n=6). Conclusion The method is specific, simple, and can improve quality standard and increase the controllability of Compound Luobuma Granule.
7.Diabetes mellitus combined with spinal neurilemoma:A case report

[Summary] Limb numbness is a common symptom of diabetic peripheral neuropathy(DPN),which is similar to lumbar vertebra disease and spinal cord tumor compression syndrome. Diabetes mellitus (DM) combined with lumbar spondylosis and spinal cord tumor compression is rarely occurred and is difficult to distinguish from DPN . Here we reported a case of DM combined with spinal neurilemoma that was misdiagnosed as DPN for half a year.

9.Radiological evaluation of interstitial lung disease in diffuse connective tissue diseases

Objective To investigate the prevalence,radiological features and the association between HRCT changes and different diseases or clinical features in interstitial lung disease(ILD)caused by diffuse connective tissue diseases(CTD).Methods Retrospective analysis was performed based on the medical records of 412 in-patients with the diagnosis of diffuse connective tissue diseases from June 2003 to June 2005 in our hospital.268 cases were SLE,83 cases were SS,40 cases were DM/PM,21 cases were SSe.All patients had chest X-ray and the suspected cases had HRCT exam.The distribution of ILD among different diffuse CTD.the appearance between X-ray and HRCT appearances were compared.The correlation among HRCT appearance,different diseases and clinical features was studied.Results①In the 412 cases of CTD,ILD de- tected by HRCT was 9.7%(40 case in total),3,9% by X-ray.Among the 40 cases,DM/PM had the highest rate of ILD(25%),23.8% in SS,9.6% in SS and 6.3% in SLE.②The consensus rate between HRCT and X-ray was 40%.The rate of mis-diagnosis of ILD was 60% by X-ray.③The changes of interstitial involvement de- tected by HRCT were:ground glass change(41.0%),consolidation(32.8%),reticular change(16.4%)and hon- eycomb change(9.8%).The concomitant presence of two of the above changes were very common.④More level respiratory symptoms such as cough,panting,chest distress and chest pain and high level of complement were observed in patients with ground glass and consolidation changes,while high CRP level was seen in patients with consolidation and honeycomb changes(P

Phospholipid fatty acids are the major constituents of the membranes of all living cells, and different groups of microorganism synthesize a variety of PLFA through various biochemical pathways. Several PLFAs can be used as "signatures" to analyze changes in microbial biomass and microbial communities structure. More and more PLFA method was used in soil microbial analysis. This article briefly reviewed the applications of PLFA methods in soil microbial analysis.
