1.Evaluation of Doppler echocardiograms on dilated cardiomyopathy in children English Column

Objective To evaluate the appraisal of Doppler echocardiograms on dilated cardiomyopathy(DCM) in children. Methods 26 patients with DCM were tested by Doppler echocardiograms. The left ventricular diastolic function, systolic function, and pulmonary artery systolic pressure (PASP)were tested before and after treatment. Results After treatment , the systolic function still changed, but situation got well obviously. Diastolic function was mostly false normal type (52.6% ), 3 patients returned to normal. Each index parameter (except E/A) of diastolie function was much different from the normal value(P<0.001). Serious continuous pulmonary hypertension is a prognosticate index for the children with DCM .Conclusions Doppler echocardiograms can be used to assess myocardium rehabilitation for the children with DCM.

Non-communicable-diseases(NCDs)have become leading death causes and major health challenges for the Chinese people,incurring direct and indirect economic burden of diseases on families and the society,and even posing a serious threat to social-economic development sustainability of the country.Given their complex causes,NCDs mostly result from unhealthy lifestyle of the individuals,along with complex multiple social determinants.Evidences from home and abroad prove NCDs as preventable and controllable,which call for powerful government policy commitments and leadership.The government should make health policies part of social policies,build such intervention platform as Healthy City,rebuild the health service system,promote universal coverage of cost-effective interventions,and involve the entire society,reinforce general measures such as planning and performance monitoring.All of which can achieve desired outcomes for NCD control in the end.
3.Research of using eIectroretinogram for diagnosising earIy diabetic retinopathy

·Electroretinogram (ERG) is an objective examination in evaluating retinal function, which is also suitable to evaluate retinal function of multiple ophthalmic diseases. ln recent years, studies have found that ERG can find functional changes prior to morphological changes of fundus examination in early diabetics, which provides a new way for researches of pathological mechanism, early diagnosis and prognosis evaluation for diabetic retinopathy, and then also provides a new therapy for diabetic retinopathy. ln this paper, using ERG in the diagnosis of early diabetic retinopathy was reviewed.
5.Alteration of intraocular structure before and after vitrectomy

AIM:To explore the possible alteration of refraction and estimate the potential impact of visual quality, investigate the impact of intraocular structure between the simple operation and the combination of phacoemulsification and intraocular lens implantation, and observe the influence among various tamponade materials application.
METHODS: Ultrasound biomicroscopy ( UBM ) and intraocular len ( IOL ) Master were measured for simple vitrectomy patients ( the simple group, 37 eyes ) and vitrectomy combined with cataract surgery patients ( the combined group, 11 eyes ) . Several parameters were measured such as the anterior chamber depth ( ACD ) , angle opening distance ( AOD ) , trabecular iris angle (TIA), axial length (AL), et al . The simple group was further divided into three sub - groups by various tamponade materials in vitreous: silicone oil group, gas group and equilibrium liquid group. All patients were measured including ACD, AOD250, AOD500, TIA500, AL and corneal curvature (K1, K2) parameters preoperatively and 2wk postoperatively.
RESULTS: AOD250, AOD500, TIA500 and AL were decreased 0. 13±0. 21mm, 0. 16±0. 22mm, 8. 68o±9. 72o and 0. 18±0. 24mm ( all P<0. 05 ) respectively in the simple group postoperatively. ACD, AOD250, AOD500, TIA500 and AL in combined group increased 0. 95±0. 54mm, 0. 13±0. 12mm, 0. 22±0. 20mm, 11. 49o±11. 06o, and 0. 45±0.30mm ( all P < 0. 05 ) respectively after operation. Alteration of ACD, AOD250, AOD500, TIA500 and AL were different between simple group and combined group ( P<0. 05). Of data from various tamponade materials sub-groups, ACD, AOD250, AOD500 and TIA500 of the gas group reduced 0. 19±0. 17mm, 0. 14±0. 09mm, 0. 20±0. 12mm, and 12. 02o±6. 64o respectively after operation ( P<0. 05), the AOD250, AOD500, TIA500 and axial length of silicone oil group decreased 0. 19±0. 25mm, 0. 21±0. 26mm, 10. 44o±9. 67oand 0. 23±0. 19mm respectively after operation ( P < 0.05 ) , while equilibrium liquid group had no significant changed in all intraocular structure (P>0. 05). However, neither the corneal thickness nor curvature (K1, K2) were changed postoperatively (P>0. 05).
CONCLUSION:Alteration of anterior segment and axial length in combined group are different from that of the simple group, while the corneal thickness and corneal curvature ( K1, K2 ) are not obviously changed in all groups. Meanwhile, various tamponade materials can also cause different impact on intraocular structure and refraction. In brief, alterations of intraocular structure might cause discrepant refractive changes and potential influence of visual quality.

8.A Comparison on Efficacy Between Laparoscopic and Open Radical Resection of Rectal Cancer

Objective To investigate the efficacy and safety of laparoscopic surgery of rectal cancer for different stages of rectal cancer . Methods A retrospective analysis was made on clinical data of 96 cases of rectal cancer from January 2009 to December 2013.Among 42 cases of TNM staged 0-Ⅰ, there were 22 cases of laparoscopic surgery ( laparoscopic group A ) and 20 cases of open surgery ( laparotomy group A ) .Among 54 cases of TNM staged Ⅱ-Ⅲ, there were 20 cases of laparoscopic surgery (laparoscopic group B) and 34 cases of open surgery (laparotomy B group).At the end of follow-up ended in May 2015, the tumor markers, postoperative complications , local recurrence , distant metastasis rate and 1-year and 3-year survival rates were compared in different stages of patients between the laparoscopic surgery and open surgery . Results As compared with the laparotomy group A , the laparoscopic group A had shorter surgery time [(121.7 ±13.5) min vs.(142.4 ±23.5) min, t=-3.530, P=0.000] and less blood loss [(80.9 ±10.6) ml vs.(136.3 ±32.6), t=-7.559, P=0.000].As compared with the laparotomy group B , the laparoscopic group B had less blood loss [(110.3 ±15.3) ml vs.(186.5 ±22.6) ml, t=-12.202, P=0.000].As compared with the laparotomy group A, the laparoscopic group A had significantly reduced distal resection margin distance [(2.3 ±0.6) cm vs. (3.0 ±0.7) cm, t=-3.489, P=0.001].As compared with the laparotomy group B, the number of lymph node metastasis was significantly reduced in the laparoscopic group B (2.7 ±0.4 vs.3.1 ±0.4, t =-3.298, P=0.002).The conversion rate to laparotomy in the laparoscopic group B was 20%.There were no significant difference between the laparoscopic group A and laparotomy group A in local recurrence rate [9.5% (2/21) vs.5.0% (1/20), χ2 =0.000, P=1.000], distant metastasis rate [4.8%(1/21) vs.5.0%(1/20),χ2 =0.000, P=1.000], and survival rate (log-rankχ2 =0.102, P=0.750).There were no significant difference between the laparoscopic group B and laparotomy group B in local recurrence rate [6.2%(1/16) vs.6.2%(2/32),χ2 = 0.000, P=1.000], distant metastasis rate [6.2%(1/16) vs.3.1%(1/32), Fisher test, P=1.000], and survival rate (log-rankχ2 =0.158, P=0.691). Conclusions Laparoscopic radical surgery for TNM stage 0-Ⅰ of rectal cancer has a good effect and safety.The conversion rate to laparotomy in TNM staged Ⅱ-Ⅲrectal cancer patients is still high with relatively high surgical risks , which requires strict preoperative assessment .
10.Case Report of Brucellar Meningitis in Children and Literature Review

Objective To explore the clinical characteristics and therapy of brucellar meningitis in children.Method One case of brucellar meningitis was reported and relevant literatures were reviewed.A 10 years old boy was admitted to hospital with a history of half a month fever and cough and 3 days headache.His family raised sheep,but he denied contacting closely with sheep and eating raw goat's milk and mutton.Results The course of this reported child was subacute.Clinical manifestations included fever,headache and meningeal irritation sign positive.The cerebrospinal fluid(CSF)findings were similar to those of tuberculous meningitis,where WBC count was elevated with predominant lymphocytes,glucose and chloride decreased and protein increased.The serum bacterium burgeri antibody was positive,agglutination titer was 1160.After brucellar meningitis was diagnosed,three antibiotics including rifampin,minocycline and ceftriaxone sodium and other symptomatic treatment were used.The CSF findings were normal 2 weeks later.This child taked orally minocycline,rifampin and trimethoprim-sulfamethoxazole for 8 weeks post discharge,the recovery was good.The domestic and foreign documents showed that the incidence of the nervous system complications of brucellosis in children was lower than that in adult.The main clinical manifestations were acute meningitis /meningocephalitis.Brucellar meningitis could be misdiagnosed as tuberculous meningitis,because the course of disease,clinical manifestations and CSF findings were similar.Therapeutic alliance including 3 kinds of antibiotics,which could easily pass through blood-brain barrier,was used to treat brucellar meningitis.The total course of treatment shouldn't be shorter than 8 weeks.Conclusions Brucellar meningitis should be screened through epidemiological investigation,serum agglutination test and cerebrospinal fluid culture,in case the reason of meningitis of meningoencephalitis is unknown,especially its clinical manifestation resembles to tuberculous meningitis.
