Thyroid peroxidase antibody (TPOAb) play an important role in the diagnosis of autoimmune thyroid disease.Chemiluminescence immunoassay (CLIA) is a new method been strongly recommended in recent years.The definition of positive value of TPOAb is inconsistent.The definition of TPOAb positive value is important for exploring the development of autoimmune thyroid disease.TPOAb is significantly increased in Hashimoto's disease and the titer is associated with the degree of infiltration and destruction of thyroid follicular.It is essential to monitor TPOAb in postpartum thyroiditis and early pregnancy.The positive of TPOAb is closely related to the adverse pregnancy outcomes caused by various risk factors.Graves disease combined with Hashimoto's must to be considered.

Objective To explore the causes,clinical feature,diagnoses and prognosis of children with acquired immunodeficiency syndrome (AIDS) and improve our awareness of this disease.Methods The clinical and laboratory data of 12 children with AIDS were analyszed retrospectively and the clinical feature was summed up.Results Among 12 children with AIDS,9 cases were boys and 3 cases were girls,aged from 45 days to 9 years old.The parents of 11 cases were positive in serological human immunodeficiency virus (HIV) antibody or died of AIDS.The main manifestations were developmental delay and various degree cacotrophia(12 case,100%),the others included abnormity fever,discontinuity cough and hepatosplenomegaly(10 cases,83.3%),recurrent diarrhea and lymphadenopathy (8 cases,66.7%),skin lesion(6 cases,50%),cerebral hypoplasia (4 cases,33.6%),oral candidiasis(3 cases,26.7%),and tympanitis(2 cases,16.7%).Laboratory tests showed:all of 12 cases were positive in serological HIV antibody by test for 2 times.The percent of CD4+ and the ratio of CD4+ to CD8+ were decreasing.One case was dead and the others had bad effect with allopathy.Conclusions Children with AIDS have short clinical latency,diversified clinical manifestation,low cell-mediated immunity function and bad effect with allopathy.To improve our awareness of the di-sease should be helpful for diagnosing and treating it early and prolonging the patients' life.

Objective To master of the characteristics of development and variation in plague natural foci of Marmota sibirica, and to analyze the plague monitoring results from 2004 to 2008. Methods From May to September every year, we monitored plague in Manzhouli, Chenbaerhuqi, Xinbaerhuzuoqi, Xinbaerhuyouqi,Ewenkeqi and Yakeshi city. The monitored area was 20 000 - 40 000 hm2 in every county. The density of Marmota,ground squirrel and moonlighting rats was surveyed by path method, one-day bow-clip method and a clamp was placed every five meters, respectively. The classification and identification of ectoparasite fleas were done by using low power lens. The fleas in the ground squirrel hole were collected by flannel rubber stick, then classified and identified. According to National Standard of Plague Diagnostic Criteria(GB 15991-1995), the collected rats and fleas were detected by isolation and cultured Yersinia pestis, the serums of collected rats were tested by indirect hemagglutination test. Results In five years, the density of Marmota and ground squirrel was 0.010/hm2 and 0.602/hm2, respectively. The capture rate of moonlighting rats was 2.69％ (258/9600). The flea infection rate of Marmota was 17.54％(10/57) and the fleas index was 2.54. The fleas infection rate of ground squirrel was 28.40％(213/750) and the fleas index was 1.01. Flea infection rat of ground squirrel hole was 5.60％(46/822) and the fleas index was 0.17. The total number of various hosts of Yersinia pestis detected was 1351, the groups number of the variety of cultured fleas was 127, and the pathogen test results were negative. The number of serum tested was 1064, positive number was 43, and the detection rate was 4.04％(43/1064). The highest positive titer was 1 : 1280. Other than 2006, the remaining four years were found positive for blood clotting material; positive serum was found in a total of three regions, they were Manzhouli, Xinbaerhuzuoqi, and Xinbaerhuyouqi; and 30, 12 and 1 copies of hemagglutination-positive sera were detected. Conclusions The epidemic of plague natural foci of Marmota sibirica is in a active state, and gradually expands the scope. We must continue to strengthen the inspection of the bacteria, bearing in mind the replacement of the region's main host, make every effort to prevent and control of human plague.

OBJECTIVETo explore the therapeutic effect of diagnosis and treatment program of integrative medicine (IM) on level 2 hypertension in the young and middle-aged patients and their ambulatory blood pressure.

METHODSA randomized, placebo parallel and controlled, multi-center clinical trial was performed. Totally 199 young and middle-aged level 2 hypertension patients were randomly assigned to the treatment group (99 cases) and the control group (100 cases). All received combined hypotensive treatment program by taking Nifedipine Sustained Release Tablet and Hydrochlorothiazide as basic drugs. Patients in the treatment group additionally took Western medicine (WM) combined Jiangyabao serial drugs (0.31 g per tablet, 2 tablets each time, twice daily), while those in the control group additionally took WM combined simulative agents of Jiangyabao serial drugs (0.31 g per tablet, 2 tablets each time, twice daily). The treatment course was 8 weeks for all, and 24-week follow-ups performed. 24 h ambulatory blood pressure and casual blood pressure, and their efficacies were compared between the two groups, and safety assessed as well.

RESULTSCompared with before treatment in the same group, daytime and night casual blood pressure, as well as 24 h ambulatory blood pressure were all obviously improved in the two groups (P < 0.01). Average diastolic and systolic blood pressures at night decreased more in the treatment group than in the control group with statistical difference (P < 0.05). There was no statistical difference in total efficacies of daytime casual blood pressure or ambulatory blood pressure (P > 0.05).

CONCLUSIONJiangyabao serial drugs combined WM in treating young and middle-aged level 2 hypertension patients showed obvious effect in improving night blood pressure, especially for night diastolic blood pressure.

A total of 267 patients with pressure ulcer were randomly divided into treatment and control groups.All of them underwent traditional debridement.The wounds in treatment group were spreaded with traditional Chinese medicine.According to the results of bacteria culture and drug sensitivity,antimicrobial agents were selected in control group.The observation indices of two groups included pain grade,swelling grade,ulcer area and skin temperature difference.Then the variations of observation indices before and after treatment were examined.In treatment and control groups,the different values of pain grade before and after treatment were 2.1 ± 0.9 and 1.2 ± 0.7,swelling grade 1.3 ± 0.6 and 0.7 ± 0.5,ulcer area 4.7 ± 3.1 and 2.4 ± 2.5 and skin temperature difference 0.12 ± 0.05 and 0.22 ± 0.10 respectively.Treatment group was superior to control group and there was statistical significance (P ＜ 0.05).The effective rates were 95.6％ and 65.4％ and the number of cured patients 86 and 33 cases respectively.And there was statistical significance (P ＜ 0.05).

Objective To study the effect of magnesium citrate combined with compound polyethylene glycol electrolyte powder in bowel preparation before colonoscopy.Methods One hundred and six colonoscopy patients with lower digestive tract symptom were divided into study group (54 cases) and control group (52 cases) by random digits table method,the bowel preparation in study group was magnesium citrate combined with compound polyethylene glycol electrolyte powder,in control group was compound polyethylene glycol electrolyte powder.The lesions detection rate,bowel preparation quality,time of first defecation,number of defecation,time of colonoscopy,results of the electrolyte level and liver and kidney function after medication and adverse reaction were compared between two groups.Results There were no statistical differences in lesions detection rate,time of colonoscopy,results of the electrolyte level and liver and kidney function after medication between two groups (P ＞ 0.05).The total effective rate of bowel preparation quality in study group was significantly higher than that in control group [96.3％(52/54) vs.82.7％ (43/52)],the time of first defecation was significantly shorter than that in control group [(32.5 ± 26.1) rmin vs.(47.2 ± 22.4) min],the number of defecation was significantly much than that in control group [(8.2 ± 2.9) times vs.(6.2 ± 2.6) times],there were statistical differences (P ＜ 0.05).The incidence of nausea in study group was significantly lower than that in control group [22.2％ (12/54) vs.48.1％ (25/52)],there was statistical difference (P ＜ 0.05).There were no statistical differences in the incidences of vomiting,abdominal distention and abdominal pain between two groups (P ＞ 0.05).Conclusion Magnesium citrate combined with compound polyethylene glycol electrolyte powder used in preoperative bowel preparation for colonoscopy has better clinical application value.

A high performance liquid chromatography-electrospray ionization mass spectrometry- charged aerosol detection ( HPLC-MS-CAD) method was established for the simultaneous quantitative analysis of four Lignans in Magnoliae Flos extract. The components were separated on a YMC-Pack ODS-A column (250 mm× 4. 6 mm, 5 μm) by gradient elution with methanol and water as the mobile phase at aflow rate of 1. 0 mL/min. Then the elution solution was routed into MS equipment at a flow rate of 0. 3 mL/min and CAD detector at a flow rate of 0. 7 mL/min by a split ratio of 3:7 for the further detection. The column temperature was 25 ℃ and the detection wavelength was 278 nm. A method was developed for the quantitative analysis of muti-components by single maker ( QAMS) to determine pinoresinol dimethylether, magnoli, 1irioresinol B dimethylethe and epi-magnoli A . Magnoli was selected as internal standard and the relative correction factors ( RCF) of the four Lignans were calculated. The contents of the four Lignans in Magnoliae Flos extract were determined by both external standard method and QAMS. The QAMS method was evaluated by comparison of its assay result and that of external standard method. Under the selected chromatographic condition, the limits of detection of pinoresinol dimethylether, magnoli, lirioresinol B dimethylethe and epi-magnoli A were 0. 34, 0. 55, 0. 50 and 0. 58 mg/L, respectively, while the linear range were within 6. 8-270 mg/L, 11-546 mg/L, 2. 0-101 mg/L and 2. 3-116 mg/L. The recoveries ( n=9 ) were 98. 2%-99. 5%, and the correlation coefficient were 0 . 9995-0 . 9998 . No significant differences were found between the quantitative results of external standard method and QAMS method. The developed method is accurate, feasible, and can be used for quality evaluation of Magnoliae Flos .

BACKGROUND: Bone marrow mesenchymal stem cells (BMSCs) are conveniently cultured and separated in vitro because theirimmunogenicity is low. Therefore, BMSCs are suitable for cell transplantation. Research has shown that BMSCs are potential to repair neurological defect. OBJECTIVE: To determine whether in vitro cultured BMSCs can be transplanted to repair peripheral nerve injury or not, and to investigate its mechanisms. DESIGN, TIME AND SETTING: Randomized controlled animal study This study was performed in Department of Toxicology, Public Health College of Jilin University from March 2006 to March 2007.MATERIALS: Fifty healthy female Wistar rats aging 2 months and six 1-week-old female Wistar rats were used for extraction of BMSCs. Rabbit-anti-nerve growth factor (NGF) monoclonal antibody was provided by Santa Cruz Company. METHODS:BMSCs were separated and cultured with adherent method. In the 3rd generation, BMSCs were preiabeled with bromodeoxyuridine (BrdU) 48 hours before transplantation. Fifty healthy Wistar rats were selected to prepare sciatic nerve crush injury models with clamping method.Subsequently, rats were randomly divided into transplantation group and control group, with 25 rats in each group. Rats in the transplantation group underwent transplantation of BrdU-labeied BMSCs at nerve injured sites; while, the same volume DMEM was injected into rats in the control group. MAIN OUTCOME MEASURES: Injured nerve in the transplantation group suffered from anti-BrdU staining 1, 2, 4, and 6 weeks after surgery. Distal injured nerve in both groups suffered from NGF immunohistochemical staining 1, 2, 4, and 6 weeks after surgery. Image analysis system was adopted to analyze integrated absorbance of positive expression. Gait analysis was performed every week after surgery to measure sciatic nerve function index, and it was also adopted to measure regenerated nerve conduction velocity 6 weeks after surgery. Subsequently, amount and inner diameter of medullated nerve fibers were calculated after luxol fast blue staining, while wet weight of experimental-lateral gastrocnemius muscle and cross section area of muscle fiber were measured at the same time. RESULTS: Fifty rats were included in the final analysis. BrdU-labeled positive cells could be found at injured nerve in the transplantation group 1, 2, and 4 weeks after surgery. Integrated absorbance of NGF protein expression in the transplantation group was significantly higher than that in the control group 1 and 2 weeks after surgery (P < 0.01), but there were no significant differences between the two groups 4 and 6 weeks after surgery (P > 0.05). Sciatic nerve function index in the transplantation group superiorly recovered to that in the control group 3-6 weeks after surgery. Furthermore, 6 weeks after surgery, nerve conduction velocity, amount and diameter of medullated nerve fibers, wet weight and cross section area of gastrocnemius muscle in the transplantation group were significantly higher than those in the control group (P < 0.05-0.01). CONCLUSION: BMSCs can be transplantated into injuried nerve tissue, and promote the recovery of nerve function in the micro-enviroment, improve NGF expression in an early phase may be one of its mechanisms.
