3. The clinical value of using xMAP technology for detection of high-risk human papillomavirus

Objective: To explore the clinical values of using flexible multi-analyte profiling (xMAP) technology for detection of high-risk human papillomavirus (HR-HPV). Methods: Six hundred and thirty eight cervical exfoliated cell samples were collected from women who lived in the region with high-incidence of cervical cancer in Shanxi province. The samples were tested with xMAP technology and Hybrid Capture 2 ® Assay (hc2). Pathological diagnosis was used as a hallmark to evaluate the detection results. HR-HPV infection rate and its genotyping distribution were analyzed and the coincidence of two methods above was assessed. Results: The sensitivity, specificity and accuracy of xMAP technology were 77.78%, 93.59% and 92.42%, respectively. The negative and positive predictive values were 98.13% and 49.30%, respectively. The prevalence rate of HR-HPV was 11.7%. HPV16 and 52 were the dominant types in non-neoplastic disease and cervical intraepithelial neoplasia (CIN) grade 1; HPV16 and 58 were the most common types in CIN2 samples; HPV16 was the most frequent type in CIN3 and cervical cancer. The lesion progress was positively related to the infection rate of HPV16, 52 or 58 (P<0.01). The Kappa value for agreement between xMAP technology and hc2 was 0.60. Conclusions: Compared with the hc2 test, the xMAP technology was highly specific, and the sensitivity is 77.78%. The agreement of these two methods is satisfactory. More improvements should be made to elevate the sensitivity of xMAP method and to increase the numbers of genotypes that can be detected.

Epstein-Barr virus (EBV) infection is associated with some autoimmune diseases. We report here
a 32 months old girl with fever, isthmitis, lymphadenectasis, and excessive atypical lymphocytes,
who developed seizures, personality and behavior changes, with positive anti-N-methyl-D-aspartate
receptor (anti-NMDAR) antibody and EBV-early antigen (EA)-IgG antibody in cerebrospinal fluid
(CSF), which suggested the possibility that the acute EBV infection have triggered the anti-NMDAR
encephalitis, which has not been reported previously.

7.Technique improvement in establishing mice orthotopic tracheal transplantation surgery model

Objective To explore a simple and effective way of establishing a mice orthotopic tracheal transplantation model in studying bronchiolitis obliterans to reduce operation time. Methods Tracheas from C57BL/6 mice (n=30) were im?planted into Balb/c mice (n=30). Grafts and hosts were matched according to body weight.Orotracheal intubation and endo-stentwas used for trachea anastomose and animal model was establish. Operating time was also noted. Grafts were harvest?ed on Days 15, 30 and 60 after transplantation, using HE staining to observe pathological changes of allograft. Trachea block?ing rate was also calculated. Results Until survival of trachea transplantation, the operative time of donor′s and the recipi?ent′tracheas resection were (7.0±1.0) min and (7.0±1.0) min respectively.Orotracheal intubation the endo-stentestablish?ment cost(1 ± 0.5)min. Trachea anastomose took(20.0 ± 4.0)min and the operation time last(43.0 ± 10.0)min. Conclusion The improvement of orthotopic tracheas transplantation usingOrotracheal intubation the endo-stenfor trachea anastomose is esay-operated and did required complicate skills and instruments with high successful rate. So it is an ideal and stable model in studying bronchiolitis obliterans.
8.Application of LBL, PBL and PLTL Teaching Methods in Clinical Pharmacist Training

Objective:To improve the teaching quality of in-service clinical pharmacist training. Methods:According to the traits of in-service pharmacists and teaching methods, combined with problem-based learning ( PBL), peer-led team learning(PLTL) and lecture-based learning ( LBL) , a standard teaching mode for in-service clinical pharmacists was explored and established. Results:The teaching mode could not only improve the study enthusiasm of students, but also let them master the study methods, and team co-operation consciousness was strengthened. Conclusion:An integrated teaching mode of LBL, PBL and PLTL has a good teaching effect on clinical pharmacist training.
9.Comparative Study on the Effect of Implanted Stent or Selected Operation in Patients with Acute Occlusion of Right Coronary Artery

Objective To study the effect of implanted stent or selected operation in patients with acute occlusion of right coronary artery during the emergency coronary arteriongraphy. Methods Forty-three patients with acute occlusion of right coronary artery who underwent the emergency coronary arteriongraphy were divided into two groups: the implanted stent group ( n = 23) during the emergency coronary arteriongraphy and the selected operation group (n =20) during the emergency coronary arteriongraphy . Then we observed the general data, the coronary artery pathological changes, preoperative and postoperative thrombus, the blood flow, remained stenosis and the prognosis. Results The implanted stent group was similar to the control group in general condition, but implanted stent group was significantly different from the control group in the near, intermediate and distal coronary artery pathological changes ( <0.05) . The control group was significantly different from the implanted stent group in the thrombus and slowly blood flow ( <0.05) . After three months, we reviewed coronary arteriongraphy and selected operation, The implanted stent group was significantly different from the control in the remained stenosis and slowly blood flow (<0.05) . But the implanted stent group was similar to the control groug in the thrombus and new proceeding cardiovascular events. Conclusion When thrombus appears in acute occlusion of right coronary artery, we can implant stent during the emergency coronary arteriongraphy if the thrombus is little, or we can select operation after PTCA on the contrary.
10.Anti-infective Pharmaceutical Care of Clinical Pharmacists in Hospital

Objective:To investigate the breakthrough point and working method of anti-infective clinical pharmacists for involving the clinical therapy. Methods:Through the specific cases, the effect of clinical pharmacists in ward round and consultation was intro-duced and analyzed. Results:Through communication and learning, clinical pharmacists could play an important role in such areas as indications, multi-resistant bacterium and adverse reactions, especially in adverse reactions and individualized dosage regimen. Con-clusion:To a certain extent, clinical pharmacists are helpful to rational application of antibiotics in clinics through ward round, case discussion and consultation.
