1.Surgical treatment for hyperparathyroidism in 368 patients

Objective To analyze the surgical treatment and pathological manifestations in patients with hyperparathyroidism. Methods The clinical data of 368 patients undergoing parathyroidectomy with hyperparathyroidism in Peking Union Medical College Hospital between Oct 1974 and Jan 2009 were retrospectively analyzed. Results 368 cases underwent parathyroidectomy. There were 249 women (67.7%) and 119 men (32. 3% ). Parathyroid lesions were located in normal positions in 318 cases (86. 4% ), and in ectopic positions in 50 cases (13.6%). The pathology of 264 cases (71.7%) was adenoma, 91cases (24.7%) was hyperplasia, 11 cases (3.0%) was carcinoma, and two cases was parathyroid cyst. Postoperatively 257 cases (68.0%) presented with hypocalcaemia, and 159 cases (66. 0% ) recovered within 7 days. Conclusions The main pathological diagnosis was adenoma. It is common for a ectopic parathyroid gland in which a parathyroid lesion is located.
2.Renal lesion associated with Schimke immuno-osseous dysplasia: a case report and review of the literature

Objective To report a Chinese boy suffering from nephrotic syndrome associated with Schimke immuno-osseous dysplasia (SIOD). Methods The clnical data and pathological changes of renal biopsy were analyzed and associated literatures were reviewed. The clinicopathological features and diagnosis of SIOD were discussed. Results The first symptom of the patient was recurrent infections. Growth retardation, spondyloepiphyseal dysplasia accompanied by nephrotic syndrome and defective cellular immunity were seen as clinical features in this patient. Renal pathology showed focal segmental glomerulosclerosis. Conclusion Combining the clinical manifestation with renal pathology, the case is diagnosed as Schimke immuno-osseous dysplasia.
3.Treatment of depression fractures of posterolateral tibial plateau through a modified anterolateral approach.

OBJECTIVETo study the therapeutic effects of posterolateral depression fractures of the tibial plateau through a modified anterolateral approach.
METHODSFrom February 2011 to January 2012,13 patients with posterolateral depression fractures of the tibial plateau were treated through a modified anterolateral approach. There were 8 males and 5 females, ranging in age from 28 to 59 years old (49.2 years old on average). Data from patients were collected retrospectively as follows: X-ray, time of fracture healing and the complications of fracture healing. The patients were evaluated both clinically and radiologically according to the Rasmussen score system.
RESULTSAll the patients were followed up, and the duration ranged from 6 to 18 months (mean 13.7 months). All the patients got bony union. The average radiographic bony union time was 15.1 weeks (ranged, 11 to 17 weeks). No case of secondary articular depression was found. No complications such as malunion or joint stiffness were found. But 1 patient had superficial infection and 1 patient had common peroneal nerve injury. According to the Rasmussen score system,the mean radiological score was 16.50 ± 0.67 (ranged, 13 to 18), and the mean functional score was 25.20 ± 2.21 (ranged, 13 to 30). The mean range of knee motion was (125.3 ± 9.3)° (ranged, 0° to 135°).
CONCLUSIONTreatment of depression fractures of posterolateral tibial plateau with a modified anterolateral approach is a safe method with effective exposure, due to its stable fixation and relatively good outcome with minimal soft-tissue complications. It is regarded as an ideal procedure for depression fractures of posterolateral tibial plateau.

4.The protective effect of aminophylline on myocardium during cardiopulmonary bypass

Objective To evaluate the protective effect of aminophylline on myocardium in the patients undergoing prothetic valve replacement operation of heart.Mothods Thirty patients undergoing prothetic valve replacement operation of heart were randomized to be treated either with aminophylline(n=15)or without aminophylline treatment(n=15). Aminophylline(5mg/kg)was injected intravenously at 15 minutes after induction of anesthesia.Cardiac troponin I(cTnI), cyclic adenosine monophosphate(cAMP),myeloperoxidase(MPO),ratio of aortic blood neutrophil count to coronary vein sinus blood neutrophil count,hemodynamics,time of aortic cress-clamping and other clinical data were recorded during the operation.Results There were no differences between the two groups in the major perioperative variables.Plasm cTnI concentration in both groups increased after off-clamping than that before CPB,however,it was lower in aminophylline group than that in control group.Concentration of cAMP in both groups after off-clamping was lower than that before CPB, however cAMP concentration in aminophylline group after off-clamping was higher than that in control group.Myocardial MPO activity and neutrophil count ratio after aortic off-clamping in aminophylline group was significantly lower than that in control group.Conclusion These results suggest that aminophylline is helpful to unprotection of myocardial and decreases the sequestration of neutrophil in myocardium.The mechanism of the protection may be related to the cAMP increased in myocardium.
5.Protective effect of puerarin on testis in diabetic rats

AIM:To investigate the protective effect of puerarin on testis in diabetic rats.METHODS:The male Sprague-Dawley rats were randomly divided into normal control group,diabetic group,high dose puerarin(160 mg/kg),middle dose puerarin(120 mg/kg),low dose puerarin(80 mg/kg)treatment groups and aminoguanidine(100 mg/kg)treatment group.Corresponding drugs were intraperitoneal injected once a day.The animals in normal control group and diabetic group were given equal propylene glycol.12 weeks later,the rats were sacrificed and their testes were collected.Testicular structure was observed under transmission electronic microscope(TEM).The contents of nitric oxide(NO),malondialdehyde(MDA)and the activity of nitric oxide synthase(NOS),inducible nitric oxide synthase(iNOS),superoxide dismutase(SOD),Ca2+-ATPase and Na+-K+-ATPase in testis homogenate were detected biochemically.The levels of endothelin-1(ET-1)and calcitonin gene-related peptide(CGRP)were also measured by radioimmunoassay.In addition,the mRNA expressions of eNOS,iNOS,CGRP,ET-1 and AR in testis tissues were determined by reverse transcription polymerase chain reaction(RT-PCR).RESULTS:The pathologic changes such as cytoplasmic inclusion of sertoli cell and obvious vacuoles between sertoli cells and basal membrane of the contorted seminiferous tubules were observed under TEM in diabetic group.The contents of NO and CGRP,the activities of SOD,Ca2+-ATPase,Na+-K+-ATPase as well as the mRNA expressions of eNOS and CGRP decreased significantly(P
6.Changes of reactive oxygen species level and mitochondria fission-fusion hi cortical neurons of rats with chronic fluorosis

Objective To investigate the changes of reactive oxygen species(ROS) level and mitochondria fission-fusion-balance in cortical neurons of rats with chronic fluorosis and reveal the correlation between these two factors. Methods One hundred and twenty rats were randomly divided into 3 groups(control group, low-dose fluorosis group, high-dose fluorosis group) and 40 rats were in each group according to body weight and the experiments were carried out for 3 months or 6 months. The rats were fed with different concentrations of fluoride (NaF) to establish fluorosis models. Controls were fed with tap water( < 0.5 mg/L), experimental animals in low- or high-dose group were fed with water containing NaF 10.0,50.0 mg/L, respectively. The level of ROS and the morphology in mitochondria fission-fusion balance in neurons of the cortex of rat brains prepared with cortical frozen sections were detected with ROS fluorescent probe and MitoTracker RED probe, respectively. Results Significant differences of the level of ROS and the numbers of abnormal mitochondria in morphology in the cortical neurons were found between 3 groups at the experiment period of 3 month and 6 month(F= 3.07,3.06,3.05,3.07, all P < 0.05). As compared with control group(10.43 ± 5.98,4.12 ± 3.86) at the experiment period of 3 month, the level of ROS and the numbers of abnormal mitochondria in morphology in the cortical neurons were obviously increased in high-dose fluorosis group(25.48 ± 6.09,20.47 ± 6.09, all P < 0.05), whereas no significant changes were found in low-dose fluorosis group(11.67 ± 3.49,6.68 ± 3.48, all P> 0.05). Furthermore, the increases in both ROS level and abnormal numbers of mitochondria were significant observed in the cortical neurons of low-dose fluorosis group (63.02 ± 8.15, 49.33 ± 8.61) and high-dose fluorosis group(65.60 ± 7.40,53.10 ± 6.95) as compared with the control group (25.26 ± 6.41,20.26 ± 6.41) at the experimental period of 6 month (all P < 0.05). The abnormal numbers of mitochondria correlated with ROS level(r = 0.93,0.81, all P < 0.05). Conclusions Taking excessive amount of fluoride results in high level of oxidative stress and impaired the balance of mitochondrial fission-fusion,which is dependent on the feeding times and doses of fluoride. The mechanism of the mitochondrial abnormalities might be associated with the high level of oxidative stress induced by chronic fluorosis.
7.Expression of mitochondrial fission protein locus Fis1 and ultrastructural changes in the renal cells of rats with chronic fluorosis

Objective To observe the expression of mitochondrial fission protein locus Fis1 and ultrastructural changes in the renal cells of rats with chronic fluorosis,and to reveal the mechanism in mitochondrial damage of the renal cells.Methods Sixty SD rats were randomly divided into 3 groups according to sex and body mass(20 in each group):control group,lower fluoride group and higher fluoride group.All the rats were fed with different doses of sodium fluoride in drinking water(0,10 and 50 mg/L,respectively).Six-month later,the expression of Fisl in renal cells was determined by real-time fluorenscence quantitative PCR and immunohistochemistry technology,the mitochondrial morphology of renal cells was observed under transmission electron microscopy (TEM).Results As compared with the control group(28.70 ± 12.41),Fis1 mRNA levels(91.48 + 34.83 and 582.09 ± 184.69) in renal cells of the lower fluoride and the higher fluoride groups were increased(all P < 0.05).As compared with the control group(10.49 ± 7.66),Fisl protein levels(16.33 ± 10.26 and 21.50 ± 5.24) in renal cells of the lower fluoride and the higher fluoride groups showed a trend of increasing,the higher fluoride group was higher than that of the control group(P < 0.05).By TEM,mitochondrial crest in renal cells of the lower fluoride and the higher fluoride groups was vague or disappeared,mitochondrial division section appeared.Conclusions Fluoride is a kind of toxicant that can cause damage to mitochondrion of renal cells,induce the expression of Fis1 in transcriptional and protein level,and lead to the obstacles of mitochondrial fusion-fission and ultrastructural abnormality of mitochondrion,which may play an important role in mechanism of mitochondrial damage in the renal cells of rats with chronic fluorosis.
8.Correlation between percentages of PMN, MNC, FBC and wound age after skeletal muscle injury in rats.

OBJECTIVE:
To study the percentages of polymorphonuclear leukocytes (PMN), mononuclear cells (MNC) and fibroblastic cells (FBC) in different post-traumatic intervals after skeletal muscle mechanical injury in rats.
METHODS:
The rat model of skeletal muscle mechanical injury was established. The rats were divided into injured groups (6 h, 12 h, 1 d, 3 d, 7 d, 10 d and 14 d after injury) and control group. The percentages of PMN, MNC and FBC in different post-traumatic intervals after skeletal muscle mechanical injury were assessed with HE staining and image analysis.
RESULTS:
At post-injury 6-12h, the percentages of PMN and MNC infiltration appeared in injured sites and that of PMN reached peak. At 1 d, the percentage of MNC infiltration appeared and reached peak, while that of PMN decreased. At 3-7 d, the percentage of FBC gradually increased, while that of PMN and MNC decreased. At 10-14d, the percentage of FBC reached peak.
CONCLUSION
The percentages of PMN, MNC and FBC in injured zones showed time-dependent changes, which might be used as reference index for determination of age of skeletal muscle injury.

10.Clinical symptoms in 455 hyperparathyroidism patients.

OBJECTIVETo analyze the clinical symptoms of patients with hyperparathyroidism.
METHODSThe clinical data of 455 patients with hyperparathyroidism who were treated in Peking Union Medical College Hospital from October 1974 to January 2009 were retrospectively analyzed.
RESULTSOut of these 455 patients, there were 156 men (34.3%) and 299 women (65.7%), and the male/female ratio was 1:1.92. The mean onset age was (40.33±16.61) years (range: 5-86 years). Most patients (n=372, 81.8%) with skeletal manifestations, of whom 283 patients (62.2%) presented with ostealgia. The second most common symptom (n=252, 55.4%) was in urinary system, of whom 162 patients (35.6%) had nephrolithiasis. There were 200 patients (44.0%) with digestive system symptoms and 54 patients (11.9%) with neuropsychological symptoms. Twenty-one patients (4.6%) had asymptomatic hyperparathyroidism. There were 133 patients (29.2%) with decreased body height, and the mean decrease was(6.23±5.02)cm. Forty-nine patients (10.8%) presented with hypercalcemia crisis.
CONCLUSIONSWomen are more susceptible to hyperparathyroidism, which is featured by symptoms in skeletal system, urinary system, and digestive system. The asymptomatic hyperparathyroidism is relatively rare, which should be carefully considered during clinical practice.
