3.Extra-anatomic bypass for aorto-iliac occlusive disease in the elderly

Objective To evaluate the extra-anatomic bypass surgery for aorto-iliac occlusive disease in the elderly. Methods From January 2005 to December 2008,33 elderly patients/39 limbs (age ranged from 70 to 87 years, mean 76.0 ± 3.0 years) with aortoiliac occlusive disease were retrospectively analyzed. According to Fontaine classification, there were 5 phase Ⅱ b cases (7 limbs), 22 phase Ⅲ cases (26 limbs), and 6 phase Ⅳ cases (6 limbs), all cases were with TASC C, D lesions. Results These 33 cases/39 limbs were tr eated with a variety of bypass, including axillobifemoral bypass (6 cases/12 limbs) ,axillofemoral bypass ( 20 cases/20 limbs ), femorofemoral bypass ( 7 cases/7 limbs ). Intermittent claudication improved in 5 cases, rest pain disappeared in 22 cases, and ulcers healed in 6 cases after operations. The mean ABI increased from 0.29 ± 0.11 to 0.66 ± 0.13 ( t = 2.69, P < 0.05 ). All limbs were salvaged and there was no perioperative mortality within 30 days after operations. Postoperative complication rate was 9.1%. Patients were followed up for a median of 12 ±5 months (range, 6 to 28 months), primary patency rate was 89.7% (35/39), secondary patency rate was 94.9% (37/39), limb salvage rate was 92.3% (36/39). After dischargement the mortality was 6.1% (2/33) with one dying of myocardial infarction and another one of malignant tumor (gastric cancer). Conclusions Extra-anatomic bypass for aorto-iliac occlusive disease in the elderly is feasible, safe and effective, and the cardiopulmonary dysfunctions are the major risk factors for perioperative complications.
5.Research of the visual electrophysiology record system based on DSP.

An application of digital signal processor (DSP) in the visual electrophysiology record system is presented in this paper. The system's design and implementation are described in detail. Results of its simulation and the clinical experiments are acceptable.

6.Revascularization of lower limb artery ischemia for elder patients

Objective To evaluate the result of revascularization for lower limb artery ischemia in elder patients. Method During Jan 2006 to Nov 2008, 262 elder patients (60 years old and up), underwent artery revascularization for lower limb arterial ischemia. Mortality, morbidity, primary patency, secondary patency and limb salvage were analyzed. Result There were a total of 323 ischemia limbs in those 262 elder patients undergoing revascalarization, among those 102 limbs underwent artery bypass, 98 limbs underwent endoluminal angioplasty with or without stenting, 67 limbs underwent embolectomy or endarterectomy, 56 limbs underwent open surgery combined with endoluminal treatment due to multiple segment lesions. Operation success rate was 94.7%. Two patients died within 30 days. Perioperative morbidity developed in 15 cases. 245 patients (93.5%) were followed-up from 1 month to 35 months. Mortality was 6.1% (15 cases), primary patency was 80.5%, secondary patency was 92.7% and limb salvage rate was 95.2%. Risk factors analysis showed that history of cardiac disease and elder ages were associated with higher mortality. Diabetes mellitus, critical ischemia and multiple segment lesions were associated with poor long term patency and limb salvage. Conclusion Lower limb ischemia is severe disease related to elder patients' death and amputation. Careful patient selection, detailed pre-operation work-up, meticulous operative technique play crucial roles for a successful arterial reconstruction.
7.Use of fluorometry for determination of skim milk powder adulteration in fresh milk.

A FAST (fluorescence of advanced Maillard products and Soluble Tryptophan) method for identification of reconstituted milk made from skim milk powder in the fresh milk was developed. Considering milk and skim milk powders variations from different seasons and countries, milk was collected from different dairy farms in different seasons and skim milk powders were collected from different countries to measure the Tryptophan (Trp), advanced Maillard products (AMP) fluorescence values. The results showed that there were differences (P<0.01) between raw and reconstituted milk. The plot of values in each mixed level of raw and reconstituted milk had a correlation coefficient >0.97. The FAST method is a simple, rapid, low-cost and sensitive method enabling the detection of 5% reconstituted milk in fresh milk. The measurement of the Trp, AMP fluorescence values and calculation of the FAST index is a suitable method for large-scale monitoring of fresh milk samples.

8.Effect of opening point method of ziwu liuzhu on myocardial ischemia in the patient of stroke.

OBJECTIVETo observe effect of opening point method of ziwu liuzhu on myocardial ischemia in the patient of stroke.
METHODSThirty cases of stroke with myocardial ischemia were treated with opening acupoint according to time of the combination of reinforcing mother point and reducing son point of the heart channel in nazi method of ziwu liuzhu, and 30 cases in the control group were treated with acupuncture at Guangming (GB 37). Changes of electrocardiogram before and after treatment were recorded.
RESULTSST-T segment raised significantly 30 min after treatment with ziwu liuzhu needling method as compared with the control group (P < 0.05), the treatment group being better than the control group.
CONCLUSIONExcessive and deficient changes of the qi and blood in the meridians and collaterals are correlated with time rhythm, and ziwu liuzhu needling method can increase clinical therapeutic effect.

9.Vector analysis and comparison of surgically induced astigmatism after coaxial phacoemulsification with 2.2-and 2.8-mm clear corneal incisions

Objective To evaluate the surgically induced astigmatism (SIA)caused by coaxial phacoemuisification and intraocular lens (IOL) implantation with different clear corneal incision.Methods Clinical data of 97 patients 97 eyes underwent phacoemulsification and IOL implantation with 2.2 mm (2.2 mm group,44 eyes) or 2.8mm (2.8 mm group,53 eyes) clear corneal incisions were retrospectively reviewed.The corneal curvature were measured before and four weeks after the surgery.Then SIA were calculated,followed by analysis of the differences in mean SIA and centroid SIA between the 2.2 mm and 2.8 mm groups.Results At 4 weeks after surgery,the centroid and mean SIA of the 2.8 mm group was (0.234 ± 0.423) D@105° and (0.552 ±0.349) D,respectively;the corresponding data was (0.174 ± 0.464) D@104° and (0.582 ±0.392) D in the right eyes,and (0.272 ±0.382)D@106° and (0.545 ±0.300) D in the left eyes,respectively.At 4 weeks after surgery,the centroid and mean SIA of the 2.2 mm group was (0.108 ±0.417)D@98°and (0.506 ±0.362) D,respectively;the corresponding data was (0.145 ±0.404)D@81° and (0.5182 ±0.332)D in the right eyes,and (0.127 ± 0.418) D@120° and (0.516 ± 0.418) D in the left eyes,respectively.There was no significant difference in centroid SIA and mean SIA,centroid SIA and mean SIA of the right and left eyes,as well as both eyes between the two groups (all P>0.05);Atotal of 47 patients (48.5%) had SIA greater than 0.5 D,and 28 patients (52.8%) were in the 2.8 mm groups,including 12 patients (22.6%) in the right eyes and 16 patients (20.5%) in the left eyes;as well as 19 patients (43.2%) in the 2.2 mm group,including 10 patients (22.7%) in the right eyes and 9 patients (20.5%) in the left eyes.There was no significant difference in the proportion of SIA greater than 0.5 D and the proportion of SIA between the left and right eyes greater than 0.5 D between the two groups (all P > 0.05).Conclusion The mean SIA and centroid SIA are not statistically different in the 2.2 mm group than in the 2.8 mm group,but SIA were more stable in the 2.2 mm group than in the 2.8 mm group.The axis of the SIA were closely related to the position of the incisions,which were the vertical direction of the incisions.
10.Activation of mitochondrial aldehyde dehydrogenase 2 and inhibition of mitochondrial permeability transition pore involved in cardioprotection of ethanol postconditioning.

OBJECTIVETo investigate whether activation of mitochondrial aldehyde dehydrogenase 2 (ALDH2) and inhibition of mitochondrial permeability transition pore (mitoPTP) were involved in the cardioprotection of ethanol postconditioning in isolated rat heart.
METHODSHearts isolated from male Sprague-Dawley rats were perfused on a langendorff apparatus and subjected to 30 min of regional ischemia (occlusion of left anterior descending artery) followed by 120 min of reperfusion. The ventricular hemodynamic parameters and lactate dehydrogenase (LDH) release during reperfusion were measured. Infarct size was measured by TTC staining method and the expression of ALDH2 at mRNA level of left anterior myocardium was detected by RT-PCR.
RESULTIn contrast to ischemia and reperfusion, ethanol postconditioning improved the recovery of left ventricular developed pressure, maximal rise/fall rate of left ventricular pressure during reperfusion, reduced LDH release and infarct size. The expression of ALDH2 mRNA level was increased. Administration of mitoPTP activator atractyloside attenuated the effect of ethanol postconditioning, LDH release and infarct size were increased, and the recovery of hemodynamic parameters was inhibited. The expression of ALDH2 mRNA was decreased.
CONCLUSIONEthanol postconditioning has cardioprotection effect, which may be associated with upregulating mitochondrial ALDH2 mRNA expression and inhibiting the opening of mitochondrial permeability transition pore.
