objective To study the relationships between TNF-β gene type in idiopathic dilated cardiomyopathy (IDC). Methods Eighty-six IDC patients were chosen as IDC group and 95 cases unrelated healthy people as control group in the First Hospital of Jilin University in 2008. The levels of TNF-β protein were detected with Enzyme-linked immunosorbent assay(ELISA ). TNF-β polymorphism were analyzed by polymerase chain reaction-restriction fragment length polymorphisms(PSR-RFLP). Results The levels of TNF-β protein in IDC patients[(1.876±1.013) μ/L] were significantly higher than controls[(1.018±0.645)μg/L], the difference being statistically significant (t=2.674,P<0.01). The frequency of TNFβ*2 genotypes in IDC patients[63.4%(109/172] was significantly higher than controls[47.9%(91/190)], the difference being statistically significant(OR=1.88, X2=6.78, P<0.05). Conclusions The TNFβ*2 allele might related to idiopathic dilated cardiomyopathy (IDC), can also be considered as one of susceptible genes of IDC

ObjectiveTo develop an experimental model of orthostatic hypotension (OH) after spinal cord injury (SCI) in rabbits and investigate the effect of tilting table exercise and Shengmaiyin on it. MethodsRabbit model of OH after SCI were induced by complete transection of spinal cord at the T5 spinal segment and raised to 60° head up tilt (HUT) on a tilting table. Tilting table exercise and Shengmaiyin were applied to different groups for 28 d from 4 days after operation. Systolic blood pressure (SBP) and pulse rate (PR) were observed with non-invasive sphygmobolometer of rabbit 3, 10, 17, 24, 31 d after the operation. Liquid chromatography with electrochemical detection(HPLC-ECD) was used to detect the concentrations of plasma norepinephrine (NE) and epinephrine(E) with horizontal position and HUT 60° on the 31st day. ResultsSBP with HUT 60° in rabbits of combined treatment group was significantly higher than that in rabbits of SCI group on the 24th day after operation, and the drop in SBP from horizontal position to HUT60° was significantly lower than that in rabbits of SCI group. SBP with HUT 60° in rabbits of exercise group and combined treatment group was significantly higher than that in rabbits of SCI group on the 31st day after operation. The drop in SBP from horizontal position to HUT 60° was significantly lower in rabbits of Shengmaiyin group, exercise group and combined treatment group than that in rabbits of SCI group on the 31st day after operation. The concentration of E and NE of SCI group, Shengmaiyin group, exercise group and combined treatment group was lower than that of sham group, but there was no significant difference among these groups. ConclusionThe treatment with tilting table exercise and Shengmaiyin can improve OH after SCI partially in rabbits without involving norepinephrine and epinephrine significantly. The period of treatment can be shorten when both therapies used together.

OBJECTIVE:To explore the optimal steaming time of Carapax trionycis during cleansing period, and to optimize and improve production technology of Carapax trionycis recorded by current Chinese Pharmacopoeia. METHODS:The mechanical processing replaced the artificial processing method in Chinese Pharmacopoeia. The content of protein and the appearance of Cara-pax trionycis were investigated after steaming for 30,60,90,120,180,240 min during cleansing period. The extract, decoction, ash content, appearance and property of Carapax trionycis decoction piece processed with vinegar were also investigated after cleansed Carapax trionycis decoction piece was processed by sand scalding and vinegar quenching method. RESULTS:The differ-ent steaming time obtained different quality of cleansed Carapax trionycis decoction piece and Carapax trionycis decoction piece pro-cessed with vinegar. Compared with decoction piece steamed for other duration, when the steaming time was 90 min,the content of protein in cleansed Carapax trionycis decoction piece was higher(31.16%),and its appearance was up to the requirement. Cara-pax trionycis decoction piece processed with vinegar had higher contents of extract and decoction(9.13%,11.39%)and lower con-tent of ash(66.29%),and its appearance was up to the requirement. CONCLUSIONS:Different steaming time have certain effect on the quality of cleansed Carapax trionycis and Carapax trionycis processed with vinegar, the optimal steaming time of Carapax tri-onycis is about 90 min during cleansing. The mechanical processing method maybe replace the artificial processing on Carapax tri-onycis for improving its production efficiency.

Epithelial ovarian cancer is the most lethal gynecologic malignancy, drug?resistance will eventually appear after re?peated intermittent chemotherapy. However, second?line chemotherapy only works on a few patients, new drugs with better efficacy and less side effects are needed. There are a number of promising results of molecular targeted therapy in recent clinical trials for patients with recurrent epithelial ovarian cancer, such as Bevacizumab, Olaparib, Pazopanib, etc. They have been demonstrated to improve progression?free survival of patients with platinum?resistant ovarian cancer, which might become a new strategy for such patients. In this review, we summarized the current status and progress of targeted therapy for recurrent epithelial ovarian cancer.

Objective Gynecologic minimally invasive surgery has become popular in the treatment of tumor therapy in re-cent years, but improper application can result in tumor metastasis. In this paper, we presented 6 uterine neoplasm cases of tumor me-tastasis after minimally invasive surgery and analyzed the causes and the preventive measures. Methods Retrospective analysis was made on the clinical data and pathology characteristics of the 6 uterine neoplasm cases of tumor metastasis after primary minimally inva-sive surgery in our department from January 1, 2013 to 2015 June 30, and related literature were reviewed. Results The ages of 6 patients were 39-52 years old.The primary operation methods included 2 cases of hysteroscopic myomectomy, 3 cases of laparoscopic myomectomy and 1 case of radiofrequency ablation. The pathological diagnosis after primary operations were 4 cases of uterine sarcoma ( low grade endometrial stromal sarcoma in 2 cases and leiomyosarcoma in 2 cases) who were found metastatic tumor at 3-16 months after primary surgery and finally died of the disease and 2 cases of uterine fibroids who were found metastatic tumor in abdominal cavity and puncture hole at 60 months and 108 months respectively after primary operation followed by a good prognosis after the second surgi-cal resection. Conclusion Owing to uterine neoplasm by hysteroscopy, laparoscopy often needs certain pressure and morcellation which may result in easy plantation of crushing tumor tis-sue or metastasis with circulation and puncture under pressure.Ra-diofrequency ablation lack of histopathologic diagnosis has heating effect which is inclined to speed up the spread and transfer of tumor cells once it is diagnosed as malignant. Therefore, clinicians should know the defects and risk of being lack of histopathologic diagno-sis, diagnostic curettage pathology and fast pathology to avoid tumor metastasis induced by minimally invasive surgery.

OBJECTIVETo establish sleeve gastrectomy(SG) rats model of obese type 2 diabetes mellitus(T2DM) for the research of hypoglycemic mechanism.
METHODSNine male Sprague-Dawley (6-week-old) rats were fed with high-sucrose and high-fat diet for 4 weeks, developing diet-induced obesity (DIO) rats model. The rats were then randomly divided into two groups. Six rats of them underwent sleeve gastrectomy(SG) as the sleeve gastrectomy group[SGG, body weight (471.8±17.9) g] and the other three rats underwent a laparotomy and stomach manipulation as the sham operative group[SOG, body weight (467.0±8.4) g]. The body weight, caloric intake and peripheral blood concentration of total ghrelin of rats were recorded after operation.
RESULTSThe weight of all the rats declined progressively after operation. The weight of the rats in SOG began to rise on the 5th postoperative day(POD) and regain their preoperative levels on the average 22nd POD. However, the weight of the rats in SGG began to rise slowly from the 9th POD, but was still lower than that of SOG[(487.4±10.1) g] and preoperative levels[(471.8±17.9) g] on the 28th POD[(420.1±18.6) g](P=0.001). Average caloric intake of rats in SGG was significantly lower than that of SOG after operation, but there was no significant difference between the two groups(P=0.121). The ghrelin level of SGG showed a continuous decreasing trend after intervention, decreased by 17.4% compared with the preoperative level (1595.1±14.4 ng/L) on the 28th POD[(1316.8±14.8) ng/L]. The ghrelin level of SOG did not change obviously before and after operation and both groups differ statistically(P=0.000).
CONCLUSIONSA SG rat model is successfully established. This model can be used for the further study of mechanism analysis of T2DM resolution after surgery.
