1.Minimally invasive treatments of spinal metastases: vertebroplasty, radiofrequency ablation and radiation therapy

BACKGROUND:The development of minimaly invasive technology significantly reduces the occurrence of surgical complications due to spinal metastases. Currently, the minimaly invasive treatment basicaly has three broad categories: vertebral cement augmentation, radiofrequency ablation combined with vertebroplasty, intraoperative radiotherapy combined with vertebroplasty. OBJECTIVE:To summarize the research progress of three kinds of minimaly invasive treatments for spinal metastases. METHODS:PubMed and Wanfang databases were searched using the keywords of spinal metastases, vertebroplasty, radiofrequency ablation, radiotherapy in English and Chinese, respectively. RESULTS AND CONCLUSION:Vertebral cement augmentation with good analgesic effect has been used widely, and the efficiency is up to 80%-90%. But its effect to kil tumors is very limited that is unable to control tumor growth. Radiofrequency ablation and radiation can kil the tumor, but cannot rebuild the vertebral stability. Therefore, the combination of different technologies can improve the therapeutic effect on spinal tumors. In recent years, intraoperative radiation and implantation of radioactive particles or radioactive bone cement have been developed as new technologies. However, there is no conclusion that these new technologies have better outcomes than the vertebral cement augmentation because of less reports and lack of long-term folow-up. Especialy in the metastatic patients with damaged vertebral posterior wal and tumors invaded into vertebral canal, the risk of nerve function damage caused by bone cement leakage is stil very high, even after many attempts. Above al, there is no a perfect minimaly invasive treatment for spinal metastases.
2.A study on the value of the serial position effect combined with delayed recall of auditory verbal learning test in distinguishing elderly patients with different levels of cognitive impairment

Objective:To explore the diagnostic value of the serial position effect combined with delayed recall of auditory verbal learning test in diagnosing the elderly patients with different levels of cognitive impairment.Methods:A total of 310 subjects were enrolled and divided into three groups: the normal control group(NC group, n=128), the mild cognitive impairment group(MCI group, n=133)and the mild Alzheimer's disease group(mild AD group, n=149)matched for gender, age and education level.Comprehensive neuro-psychological tests were performed on all subjects, and the Auditory Verbal Learning Test-Huashan version(AVLT-H)was used to analyze the serial position effect.Results:There were significant differences in the primacy effect of the second and third time of immediate recall, short-term and long-term delayed recall among the three groups( P<0.05), but no difference was found in the recency effect between the MCI and mild AD groups( P>0.05). The long-term delayed primacy effect combined with the short-term delayed recall showed a better validity to distinguish those elderly subjects with different levels of cognitive impairment than other memorial indexes, and the correlative sensitivity and specificity were 83.21% and 71.43%(NC group vs.MCI group), 85.71% and 92.86%(NC group vs.mild AD group), 66.67% and 75.57%(MCI group vs.mild AD group), respectively. Conclusions:The long-term delayed primacy effect combined with the short-term delayed recall can assist in distinguishing elderly patients with different levels of cognitive impairment.
3.Effect of Compound Rhizoma Coptidis on artery calcification in rats treated with warfarin and vitamin D

AIM:To investigate the effects of Compound Rhizoma Coptidis ( Rhizoma eoptidis,Radix suctellariae,Cortex phellodendri Chinensis,Fructus gardeniae and Radix et Rhizoma glycyrrhizae) on vascular calcification in rats treated with warfarin and vitamin D_3.METHODS: Thirty-two male SD rats were assigned randomly into control group,calcified group,Compound Rhizoma Coptidis prevented group and treated group.The later three groups were treated with warfarin,and subcutaneously injected with vitamin K_1 and vitamin D_3 for one week to induce extensive calcification of the aorta.Compound Rhizoma Coptidis was given before the first warfarin dose in prevented group and the drug was given after the modeling in the treated group.The control group was treated with normal saline.The calcification in the aorta was analyzed and osteoprotegerin (OPG) mRNA and protein were determined using histomorphometry,RT-PCR and immunohistoehemistry after 4 weeks of drug intervention.RESULTS : The results of 32 rats was analyzed compared to the control group,the area of darkly stained regions by Von Kossa staining and the level of calcium content in aorta wall increased significantly[(608.32±42.29) μg/g vs (1 139.47±230.03) μg/g,P <0.05].The OPGmRNA and protein decreased in aortic sections.No artery calcification could be detected in Compound Rhizoma Coptidis prevented group and a little artery calcification could be detected in Compound Rhizoma Coptidis treated group.The level of calcium content in aorta wall significantly lower[(854.77±12.99) μg/g,(875.78±27.23 ) μg/g].The expresion of OPGmRNA and the protein significanfly increased in Compound Rhizoma Coptidis prevented and treated groups.CONCLUSION:Compound Rhizo-ma Coptidis may prevent or regress the vegcular calcifiation,that seems dependent on the upregression of aortic OPG levels.
4.Interrelationship among allergic rhinitis,rhinosinusitis and asthma in pathogenesis

Allergic rhinitis and rhinosinusitis often complicate with asthma,and their relationship has long been investigated.From the view of epidemiology,all of these three diseases have higher prevalence,complicate with each other,are risk factors and prognostic factors for each other.Besides,they share common in anatomy and pathophysiology.In this paper,the interrelationship among allergic rhinitis,rhinosinusitis and asthma in pathogenesis is discussed.
5.Split-hand sign in the patients with amyotrophic lateral sclerosis

Objective To investigate the diagnostic value of the split-hand sign in amyotrophic lateral sclerosis ( ALS).Methods Ninety ALS patients, 41 patients with other neuromuscular disorders and 71 normal controls were recruited for conventional nerve conduction study.Compound muscle action potential ( CMAP) amplitude recorded from abductor pollicis brevis ( APB) , abductor digiti minimi ( ADM) and first dorsal interosseous (FDI), CMAP amplitude ratios, CMAP amplitude differences and split-hand index ( SI) were analyzed.Results The APB/ADM CMAP amplitude ratio was significantly lower in the ALS patients (0.44(0.44)) than that in the patients with other neuromuscular disorders (1.31(0.87);z=6.967, P<0.01) and the normal controls (0.99(0.42);z=7.687, P<0.01).The FDI/ADM CMAP amplitude ratio was significantly decreased in the ALS patients ( 0.79 ( 0.46 ) ) compared with that in the normal controls ( 1.23 ( 0.39 ); z =5.899, P <0.01 ).The FDI/ADM CMAP amplitude ratio was comparable between the ALS patients and the patients with other neuromuscular disorders ( 0.93 ( 0.62 );z=1.737,P=0.081).SI was significantly lower in the ALS patients (2.42 (3.14)) than that in the patients with other neuromuscular disorders (10.10(6.54);q=7.947, P<0.05) and the normal controls (17.93(8.32);q=10.827, P<0.05).SI <5.2 can help differentiate ALS from mimic disorders, with a sensitivity of 83.33% and specificity of 96.43%.Conclusions The split-hand sign appears to be a specific feature of ALS.SI robustly differentiates ALS from mimic disorders and potentially facilitates an earlier diagnosis of ALS.

Objective To assess the diagnostic value of coronary CTA (CCTA) combined myocardial CT perfusion imaging (CTP) in coronary artery disease (CAD) with Meta-analysis.Methods Chinese and English literatures evaluating CCTA combined CTP in diagnosis of CAD in PubMed,Wed of Science,Cochrane Library,China National Knowledge Infrastructure (CNKI) and Wanfang Periodical Database from inception to December 2016 were searched.The extraction was conducted according to the criteria for diagnostic recommended by Cochrane Method Group.The quality of included studies was assessed with quality assessment of diagnostic accuracy studies-2 instrument.The statistical analysis was performed by Meta-disc 1.4 software,the diagnosis efficacy was calculated and the symmetric receiver operating characteristic curves (SROC) was drawn to obtain the area under curve (AUC).Results Five studies involving a total of 611 vessels were included.The pooled sensitivity (SEN),specificity (SPE),diagnostic odds ratio (DOR),positive likelihood ratio (PLR),negative likelihood ratio (NLR) were 0.96 (95%CI [0.92,0.98]),0.91 (95%CI [0.88,0.94],206.14 (95%CI [100,424.94]),9.33 (95%CI [5.57,15.61]),0.06 (95%CI [0.03,0.13]),respectively.The AUC of SROC was 0.977.Conclusion CCTA combined CTP has significant value in diagnosis of CAD.
8.Malignant Peripheral Nerve Sheath Tumors in the Pelvis and Sacrococcyx Originated from Neurofibromatosis: A Case Report and Literatures Review

Objective To observe the characteristics and treatment of malignant peripheral nerve sheath tumor originated from neurofibromatosis and the outcome of the patients. Methods A case was reported and the related literatures were reviewed. Results and Conclusion Malignant peripheral nerve sheath tumor is a rare soft tissue malignancy, which is with highly invasion. There are some difficulties with the diagnosis and treatment, and the outcome is poor. Early diagnosis is very important for treatment.
9.Postoperative Spondylodiscitis after Lumbar Intervertebral Disc Herniation:9 Case Report

Objective To investigate the clinical characteristics of postoperative spondylodiscitis after lumbar intervertebral disc herniation.Methods 9 cases with spondylodiscitis were reviewed,about their clinical symptoms,laboratory tests,bacteria culture,CT and MRI finging,etc.Results and Conclusion The clinical symptoms occurred 8~24 d after operation.Laboratory test and MRI were helpful for the early diagnosis.The outcome of both surgical and non-surgical treatment was satisfactory.
10.Effects of activation of ALDH2 by ethanol on the expression of JNK in kidney of diabetic rats.

OBJECTIVETo observe the effect of activation of aldehyde dehydrogenase 2 (ALDH2) by ethanol on the expression of c-Jun N-terminal kinase (JNK) in the kidney of diabetic rats.
METHODSEightheen healthy male SD rats were randomly divided into 3 groups (n = 6): normal control group, diabetes group and ethanol + diabetes group. After 8 weeks, 24 h urine samples from rats were collected to detect urinary protein content. The kidney was isolated and the ratio of kidney weight/body weight (index of kidney weight) was detected. The levels of fasting blood glucose, glycosylated hemoglobin serum urea nitrogen and serum creatinine were measured. Morphological changes of renal tissue were observed by optical microscope. The protein expressions of ALDH2 and JNK in renal tissue were detected by Western blot.
RESULTSCompared with the normal control rats, the levels of fasting blood glucose, glycosylated hemoglobin, serum urea nitrogen, serum creatinine and the index of kidney weight were increased markedly in diabetic rats. The expression of ALDH2 protein was decreased, while p-JNK, JNK protein expressions and the ratio of p-JNK/JNK were increased. The morphological observation was shown that the amount of glomerular mesangial matrix were increased, basement membrane were thickened and capillary lumen were narrowed. However,in ethanol + diabetes group, renal function was improved and the damage of renal structure was attenuated. The expression of ALDH2 protein was increased, while p-JNK, JNK and the ratio of p-JNK/JNK were decreased.
CONCLUSIONEnhanced ALDH2 expression can protect kidney in diabetic rats, which may be relevant with inhibitting the activity of JNK pathway.
