1.Pedicle omentum flap harvested by laparoscopy in breast oncoplastic sugery
China Oncology 2017;27(8):608-612
Surgery is one of the most important treatments for breast cancer. A part of the early breast cancer patients demand further oncoplastic breast surgery to reconstruct or restore the breast cosmosis after conventional breast conserving surgery, by oncoplastic breast surgery, which is a modern conception and technique including volume dis-placement and volume replacement. Oncoplastic breast surgery using pedicled omental flap is a new approach among volume replacement techniques. It has made some progress in recently years. A systematic review was therefore con-ducted to analyze and illuminate the present status.

Objective: To study the application of CT and MRI fusion technique in the diagnosis and treatment of intraocular and orbital tumors. Methods: 2D-images of 13 patients with intraocular and orbital tumors were fused by special-point registration and Iterative Local Closest Point(ILCP) method; 3D-fusion images were reconstructed by Ray Tracing method. Results: A 3D-CT-MRI fusion images of intraocular and orbital tumors were reconstructed and displayed. The CT and MRI data of intraocular and orbital tumors were displayed on the same image as a comprehensive whole, which provided a stereogram of 3D-structure of the normal and abnormal orbital tissues. Anatomical structure of the orbit was clearly visualized by 3D-CT-MRI image. Conclusion: The multi-modality fusion technique can provide more accurate and comprehensive information for clinical diagnosis of intraocular and orbital tumors, which is helpful for doctors' planning of surgical operations, clinical education and doctor-patient communication.

Journal of Biomedical Engineering 2014;31(2):283-287
Generally, P-wave is the wave of low-frequency and low-amplitude, and it could be affected by baseline drift, electromyography (EMG) interference and other noises easily. Not every heart beat contains the P-wave, and it is also a major problem to determine the P-wave exist or not in a heart beat. In order to solve the limitation of suiting the diverse morphological P-wave using wavelet-amplitude-transform algorithm and the limitation of selecting the pseudo-P-wave sample using the wavelet transform and neural network, we presented new P-wave detecting method based on wave-amplitude threshold and using the multi-feature as the input of neural networks. Firstly, we removed the noise of ECG through the wavelet transform, then determined the position of the candidate P-wave by calculating modulus maxima of the wavelet transform, and then determine the P-wave exist or not by wave-amplitude threshold method initially. Finally we determined whether the P-wave existed or not by the neural networks. The method is validated based on the QT database which is supplied with manual labels made by physicians. We compared the detection effect of ECG P-waves, which was obtained with the method developed in the study, with the algorithm of wavelet threshold value and the method based on "wavelet-amplitude-slope", and verified the feasibility of the proposed algorithm. The detected ECG signal, which is recorded in the hospital ECG division, was consistent with the doctor's labels. Furthermore, after detecting the 13 sets of ECG which were 15 min long, the detection rate for the correct P-wave is 99.911%.

Chinese Journal of Urology 2012;33(7):489-491
Objective To explore the safety and efficacy of small margin in nephron sparing surgery for early localized renal cell carcinoma (RCC). Methods A total of 325 cases of RCC with normal contralateral kidney and staged as Tla were retrospectivly studied.According to the margin size,125 cases were with surgical margin ≤ 5 mm (group ≤ 5 mm),102 cases with margin 6-9 mm (group 6-9 mm) and 98 cases with margin > 10 mm (group > 10 mm).The margin size and status was pathologically evaluated and clinical results including local recurrence, distant metastasis and overall survival rate were followed up and comparatively analyzed. Results None of the patients had positive surgical margins. The mean and median margin sizes were 2.2 and 2.0 mm for group ≤ 5 mm,6.7 and 6.0 mm for group 6-9 mm and 11.8 and 12.0 mm for group > 10 mm.The difference was statistically significant (P=0.025).The mean and median follow-up time for all the patients were 79 and 83 months (range 15-132 months),with 69 and 73 months (range,15-130 months) for group ≤ 5 mm,83 and 86 (range,17-132 months) for group 6-9mm and 82 and 82 (range 60-103 months) for group > 10 mm.Three patients in group ≤ 5 mm,5 in group 6-9 mm and 2 in group > 10 mm died of no-cancer related disease during follow-up.One patient in group ≤ 5 mm (0.74%) experienced ectopic recurrence in the same kidney and one in group 6-9 mm was detected local recurrence in situ (0.98%).No distant metastasis was detected in all the patients. The overall 5-year survival rate for patients in groups ≤ 5mm,6-9 mm and > 10 mm were 99.2%,99.0% and 98.0%,respectively.(Kaplan-Meier survival analysis,Log Rank,x2 =1.511,P=0.470). Conclusions Small margin in nephron sparing surgery is safe and effective in treating RCC with stage T1a,which provides excellent renal function preservation, favorable long-term progression-free survival rate, and is not associated with an increased risk of local recurrence.
