Objective To investigate the relationship between red cell distribution (RDW) and serum inflammatory factor in di‐lated cardiomyopathy (DCM ) patients .Methods 47 dilated cardiomyopathy patients and 40 healthy volunteers were chosen .Ac‐cording to NYHA classification ,12 patients in NYHA class Ⅱ ,15 patients in NYHA class Ⅲ and 20 patients in NYHA class Ⅳ . RDW ,High sensitivity C‐reaction protein (hs‐CPR) ,tumor necrosis factor‐α (TNF‐α) and interleukin‐1β (IL‐1β) were detected in all the DCM patients and healthy volunteers .The correlation between RDW and serum inflammatory factors were analyzed . Results Compared to healthy volunteers ,RDW ,hs‐CRP ,TNF‐α and IL‐1β were elevated significantly in DCM patients (all P<0 .05) .Comparisons among the DCM patients groups ,RDW ,hs‐CRP ,TNF‐α,IL‐1β in NYHA class Ⅲ and NYHA class Ⅳ were higher than those in NYHA class Ⅱ(all P<0 .05) .There were positive correlation between RDW and hs‐CRP ,RDW and TNF‐α, RDW and IL‐1β(r=0 .422 ,0 .390 ,0 .325 ,P<0 .05) .Conclusion RDW is one of the indicators evaluating cardiac functions ,and there is a significant correlation between RDW and serum inflammatory factors in DCM patients .

Interleukin-24 (IL-24) is a novel cancer suppressor cytokine. It can suppress cancer cell growth, induce cancer cell apoptosis, inhibit angiogenesis, stimulate the PBMC secretion of cytokines, and enhance the antitumor activity of radiotherapy, chemotherapy and targeting gene-virotherapy. It is reasonable to suggest that IL-24 may be a powerful and effective protocol for combinatorial cancer treatment in the clinic.

The mature Wistar rats were fed with high lipid diet for 8 weeks together with 3 levels (30, 60, or 120mg/day) of dl-?-tocopheryl acetate supplementation. At the end of the experiment, serum vitamin E and lipids were measured by fluorescence analysis and enzymologic method, liver crude fat was measured by Soxhlet method, liver glycogen content and fat accumulation in parenchymal cells were estimated by histo-chemical procedure and liver lipid peroxidation was evaluated by fluorescence analysis respectively. The results showed that the high lipid diet induced hypercholesterolemia and fatty liver associated with a decrease in serum high density lipoprotein cholesterol and an increase in liver peroxidation. Overall vitamin E caused no significant decrease in serum cholesterol and triglyceride levels and no significant increase in high density lipoprotein cholesterol level, except serum vitamin E level had a positive correlation with serum cholesterol level, but could inhibit the neutral fat accumulation in liver cells and liver peroxidation in rats.The levels of serum cholesterol, or the serum vitamin E and VE/CHO were higher in females than those in males significantly when dl-a-tocopheryl acetate was given to the rats fed with high lipid diet.

Objective To observe the effect of metoclopramide combined with early enteral nutritional on patients with mechanical ventilation after craniocerebral operation.Methods A prospective randomized control study was conducted.56 patients with mechanical ventilation after craniocerebral operation were randomly divided into metoclopramide combined with early enteral nutritional group (observation group) and parenteral nutrition group (control group),28 in each group.The patients in the observation group received early enteral nutrition (24-48 h after operation),and intramuscular injection of metoclopramide (10mg Q8H) for 7 days.The patients in the control group received parenteral nutrition.The nutritional parameters,mechanical ventilation (MV) time,success rate of the ventilator weaning and the occurrence rate of pneumonia associated with ventilation (VAP) in one week,the incidence of adverse reaction were compared between the two groups.Results After 1 week therapy,the nutritioual parameters of the two groups showed no statistical significance(P ＞ 0.05);the observation group compared with the control group,3 、7days of blood glucose(P1 、P2) decreased significantly[(8.9 ± 1.6) mmol/L vs (11.4 ± 3.2) mmol/L,(8.2 ± 1.3) mmol/L vs (10.6 ± 2.1) mmol/L] (P1 =0.002,P2 =0.011),Mechanical ventilation (MV) time (P3) and the occurrence rate of VAP(P4) in one week also declined[(6.5 ±2.3)d vs (8.9 ±3.1)d,21.4％ vs 46.4％] (P3 =0.022,P4 =0.048),but within a week of success rate of the ventilator weaning improved (67.9％ vs 39.3％,P5 =0.032).Return rate (P6),gastrointestinal bleeding (P7) was significantly lower than those of the control group (17.9％ vs 42.9％,14.3％ vs 42.9％,P6 =0.042,P7 =0.038),but abdominal distention,diarrhea were no significant contrast (P ＞ 0.05).Conclusion Metoclopramide combined with early enteral nutritional may obviously improve the nutrition parameters,shorten the MV time,decrease the occurrence rate of VAP,not increase in side effects.

Objective To discuss the technical skills and the contraindication of transjugular intrahepatic portosystemic shunt stent (TIPSS) in portal vein cancercous thrombosis (PVCT) complicated with portal hypertension. Methods There were 16 cases of PVCT with portal hypertension, and average age of 53.6 yr. There were 9 cases with complete occlusion of portal vein trunk and 7 cases with incomplete thrombosis. There were 5 cases with cavernous transformation of the portal vein(CTPV). 1 case of simple upper gastro intestinal tract (GIT) massive bleeding, 4 with refractory ascites and 11 with upper GIT massive bleeding and refractory ascites. Results The procedure of TIPS was successful in 11 cases, the successful rate reached about 68.8%. The mean portal vein pressure was reduced from 4.9kPa to 2.4kPa with average 2.5kPa reduction. Ascites decreased, bleeding stopped and the clinical symptoms disappeared. The average survival period was 136 days. The procedure failed in 5 cases. Conclusions TIPSS is an effective method to control the bleeding and ascites caused by PVCT. The PV cavernous transformation was the contraindication of TIPSS.

Objective To understand morphological characteristics of sinoatrial node cells(SNCs) by taking purified isolation and culture of neonatal rat's SNCs in vitro and to understand the expression characteristics of surface antigens hyperpolarization activated cyclic nucleotide gated cation channel 4(HCN4) and connexin 45(Cx45) on SNCs.Methods The SNCs were isolated from sinus node tissues removed from neonatal rats by trypsin and purified cultured with differential attachment and 5'-bromodeoxyuridine(BrdU) treatment.Results Three different types of cells were observed among the purified cultured SNCs: spindle,triangular and irregular cells.The spindle cells took up the greatest proportion,had the fastest beats,and had less developed organelle and scarce muscular fibrils.Immunofluorescence staining was positive against HCN4 and Cx45 on the spindle cells.Small amounts of HCN4 was observed on triangular cells.Conclusion ① Isolation by trypsin combined with differential attachment and BrdU treatment is a reliable approach to culturing sinoatrial node cells.② The spindle cells with the fastest beats and positive expressions of HCN4 and Cx45 may be the sinus node pacemaker cells.
