1.Surgical treatment for hyperparathyroidism in 368 patients

Objective To analyze the surgical treatment and pathological manifestations in patients with hyperparathyroidism. Methods The clinical data of 368 patients undergoing parathyroidectomy with hyperparathyroidism in Peking Union Medical College Hospital between Oct 1974 and Jan 2009 were retrospectively analyzed. Results 368 cases underwent parathyroidectomy. There were 249 women (67.7%) and 119 men (32. 3% ). Parathyroid lesions were located in normal positions in 318 cases (86. 4% ), and in ectopic positions in 50 cases (13.6%). The pathology of 264 cases (71.7%) was adenoma, 91cases (24.7%) was hyperplasia, 11 cases (3.0%) was carcinoma, and two cases was parathyroid cyst. Postoperatively 257 cases (68.0%) presented with hypocalcaemia, and 159 cases (66. 0% ) recovered within 7 days. Conclusions The main pathological diagnosis was adenoma. It is common for a ectopic parathyroid gland in which a parathyroid lesion is located.
2.Inferior vena cava partial interruption and filter placement in the prevention of pulmonary embolism: experience of 44 cases

ObjectiveTo evaluate partial inferior vena cava (IVC) interruption and filter placement in the prevention of pulmonary embolism caused by lower extremity deep vein thrombosis (LEDVT).MethodsAmong 44 cases of LEDVT, 10 cases underwent partial IVC interruption, 34 cases received IVC filter placement.Results7 out of 10 cases undergoing IVC occlusion experienced post operative relief of non-fatal pulmonary embolism symptoms with no perioperative mortality or severe complications.On follow-up of 8 months, 3 cases experienced non-lethal pulmonary embolism. 24 out of 34 filter placement cases had postoperative relief of pulmonary embolism symptoms with no perioperative mortality. 2 cases suffered from inhospital pulmonary embolism with one death. 20 cases were followed up for an average of 10 months without recurrence of pulmonary embolism. Conclusion Both partial IVC interruption and filter placement are safe and effective surgical procedures for the prevention of recurrence of pulmonary embolism. The result of filter placement is more favourable.

4.The protection of spinal cord disfunction caused by acute ischemia

Objective To evaluate ischemia preconditioning (IP) and intermittent ischemia (II) in ameliorating the incidence of paraplegia. MethodCross clamping of the infrarenal aorta in New-Zealand rabbit was used to establish a spinal cord ischemia model. 25 rabbits were randomly divided into 3 groups. The continuous ischemia(CS) group (n=10) with the aorta clamped for 40 minutes. While in the IP group (n=6) the aorta was clamped for 3 sessions each lasting for 5 min, with 5 min interval between the sessions before 40 minutes clamping, in the Ⅱ group (n=9) the 40 min clamping was interposed by a 15 min interval. 5.Studies on the differential expression of signal transduction related gene in human abdominal aortic aneurysms

ObjectiveTo study the differential expression of signal transduction related gene between the tunica media in human abdominal aortic aneurysms (AAA) and normal aorta, and inspect their role in the etiology of abdominal aortic aneurysms.MethodsGene chip technique was applied to study the differential expression of signal transduction related gene in human AAA, then screening some significant differential expressed genes for further research by RT-PCR, Western Blot, and immunohistochemistry.ResultsForty-five differentially expressed genes related to cellular signal transduction were detected accounting for 1.09% of the chip′s total 4?096 genes,among them 28 genes were up-regulated and 17 down-regulated in aortic aneurysm. ASK1, ERK1 gene were found up-requlated in aortic aneurysm.ConclusionsDifferential expression of signal transduction gene could play a key role in the occurrence of AAA.

This paper reports the methods of inducing animal model of abdominal aortic aneurysm(AAA) and the research on etiology.The formation of AAA is the result of cooperation of multifactors such as genetic and biochemistric facfors.Different animal models of AAA can provide a possibility in researching the etiology and pathogenesis of AAA etc, and useful in judging the therapy and prognosis .
8.Uterine involution after cesarean section promoted with acupuncture: a randomized controlled trial.

OBJECTIVETo explore the effect and feasibility of acupuncture in uterine involution after cesarean section.
METHODSSeventy cases of cesarean section in primipara were randomized into an observation group (33 cases) and a control group (37 cases). In the control group, the conventional treatment was adopted after cesarean section. In the observation group, on the basic treatment as the control group, acupuncture was applied at Sanyinjiao (SP 6), Hegu (LI 4), Qihai (CV 6) and Guanyuan (CV 4). In 2 h after operation, acupuncture started, once a day, continuously for five times. The daily height of uterine fundus, daily uterine fundus decreasing degree, postpartum blood loss, lochia duration and ultrasonic B test in 42 days of postpartum were compared between the two groups.
RESULTSThe therapeutic methods of the two groups all promoted uterine contraction. The postpartum height of uterine fundus in the observation group was lower than that in the control group (all P<0.05) and the uterine fundus decreasing degree was higher than that in the control group (all P<0.05). The blood loss in 2 h, 6 to 12 h and 24 to 48 h of postpartum was less than that in the control group (all P<0.05). The lochia duration in the observation group was shorter than that in the control group (P<0.05).
CONCLUSIONAcupuncture promotes uterine contraction, reduces postpartum blood loss and lochia duration and benefits uterine involution after cesarean section.

9.Progress of management and standardization strategy research in diabetics with insulin pump therapy outside hospital

The role of insulin pump in the treatment of diabetes is becoming more and more significant. Based on the systematic literature review, collation and analysis, this article introduces the management of insulin pump outside the hospital from the development trend of insulin pump, application and common problems. In order to enhance the therapeutic satisfaction of patients and improve their quality of life, our aims are to put forward the strategy of standardized management and provide a reference for establishing and improving the standardized management system of insulin pump therapy outside hospital.
10.Surgical management of ectopic hyperparathyroidism: report of 66 cases

Objective To discuss the features and surgical management of ectopic hyperparathyroidism.Methods Clinical data of 66 cases of ectopic hyperparathyroidism were retrospectively analyzed.Results There were 66 cases of ectopic hyperparathyroidism accounting for 11.5% (66/575) of all ectopic hyperparathyroid cases admitted during the period from 1982-2010.Prevalence of mediastinal ectopic lesions was 71.2% (47/66),among those 65.2% (42/66) was in anterosuperior mediastinum,and 28.8% (19/66) in the non-typical loci of the neck.Radionuclide imaging of parathyroid glands was the most sensitive (87.0%) method among all common positioning examinations.Average number of operation episode was 1.47,and all lesions were finally resected.After surgery 49 cases presented with transient hypocalcemia,and were cured by calcium administration.52 cases were followed up,with recurrent hyperparathyroidism in 1 case.Conclusions Diagnosis and treatment of ectopic hyperparathyroidism are dependent on the understanding of common locations of ectopic parathyroid glands.Preoperative correct location and surgical expertise are helpful for successful resection.
