1.Pathogenic Bacteria Distribution and Drug Resistance of Hospital Infection in Intensive Care Unit

OBJECTIVE To analyze the status quo of pathogenic bacteria distribution and drug resistance of hospital infection in intensive care unit,to provide evidence for the guidance of clinical rational administration and to decrease the hospital infection. METHODS Pathogenic bacteria were isolated from patients who suffered from hospital infection in intensive care unit from 2001 to 2004.K-B slip diffusion method was taken to carry out the sensitive test.Rate of drug resistance of the pathogenic bacteria was analyzed. RESULTS As the bacteria distribution,the highest isolated rate was 60.77%,which was isolated from the sample of respiratory tract.The main pathogenic bacteria were the Gram-negative bacilli(47.69%).And the second pathogenic bacteria were the mycetes(37.69%).The Gram-positive cocci(14.62%) occupied the third.The monitoring of the drug resistance showed that rate of the drug fast was rather higher.The detection rate of the meticillin-resistant staphylococci was 81.82%.The detection rate of the ESBLs from the Escherichia coli and the Klebsiella pneumoniae was 75.00% and 57.14%,respectively. CONCLUSIONS Intensive care unit is one of the highest risk departments for the hospital infection.Generally,the pathogenic bacteria are the multidrug-resistant ones.Enhanced monitoring on pathogenic bacteria distribution and drug resistance analyses of hospital infection in intensive care unit could benefit for the guide of clinical rational administration,the depression of multidrug-resistant bacteria,and the decrease in hospital infection.
2.Current status of occupational hygiene habits and Helicobacter pylori infection of domestic service personnel in a Shanghai community

To explore the current status of occupational hygiene habits and Helicobacter pylori(Hp) infection of domestic service personnel in a Shanghai community and propose the community control measures.A total of 76 domestic service personnel and employers were selected into the investigation and control groups.And they answered questionnaire and received 14C-urea breath test.The career duration of 65% domestic service personnel were over 3 years.Home-based services accounted for 45%.The number of 49% part-time service accounted for two or more.And 84% of them dined together with service families.The difference of occupational hygiene habits and awareness of Hp infection had no statistical significance (P >0.05).The rate of Hp infection of domestic service personnel was 50% vs.38% for control group (x2 =2.162,P =1.141).Their consistent rate was 62% (Kappa =0.237).A high risk of Hp infection spread within or between the families is correlated with the professional behaviors of domestic service personnel.Eradication therapy for an existing patient should be started as early as possible and occupational practices standardized.
3.The factors influencing the transfer of porcine endogenous retroviruses across the membrane in bioartificial livers

ObjectiveTo identify the factors influencing the transfer of porcine endogenous retroviruses across the membrane of a new bioartificial liver (BAL),which is a necessary caution to consider for BALs carrying porcine hepatocytes.MethodsA novel porcine BAL composed of two circuits was constructed using a plasma filter with membrane pore size of 500 nm and a plasma component separator with membrane pore sizes 10 nm,20 nm,30 nm,and 35 nm.Co-cultured cells of porcine hepatocytes and mesenchymal stem cells or single porcine hepatocytes were incubated in the bioreactors.The BAL was continuously worked for 72 hours during which the supernatant was collected from the internal and external circuits every 12 hours.PERV RNA,reverse transcriptase (RT) activity,and in vitro infectivity from the supernatant were detected.ResultsIn the plasma filter group,the PERV RNAlevels were the same in both circuits,suggesting little to no effect of the plasma filter on the PERV RNA's crossing.With plasma component separators,PERV RNA was found in the external circuits at different times without positive RT activity and HEK293 cell infection,but its level was reduced significantly.The larger the membrane pore size was,the earlier and the more RNA was detected.ConclusionsThe membrane pore size,the treatment time and the viral level in the internal circuit are contributing factors influencing the transfer of PERV RNA across the membrane in a BAL.
4.Analysis of diabetes prevalence and its influencing factors among residents in Yongchuan district of Chongqing

Objective To understand diabetes prevalence and its factors is to provide the suggestion for diabetes prevention and control in Yongchuan district .Methods Residents aged above 18 years old from 3 towns and 1 street were sampled by stratified clustered random sampling method .Residents were investigated with the questionnaire and physical examination and taken blood sample to test fasting glucose ,2‐h glucose .The data were analyzed to calculate diabetes prevalence by SPSS17 .0 .The factors of dia‐betes was compared by chi‐square and the independent influencing factors of diabetes were identified by step backward logistic re‐gression .Results A total of 1 202 were investigated .The rate of cognition about diabetes was below 30% .21 .46% of residents smoke every day .31 .78% of them do exercise .The prevalence of overweight ,obesity and diabetes was 24 .63% ,6 .07% and 10 .48% .The result of multi‐variant logistic regression indicated that the independent influencing factors of diabetes prevalence in‐cluded age(OR=1 .41 ,95% CI=1 .24-1 .62) ,the rate of cognition about the dose of salt intake per person per day recommended by Chinese guideline of diet(OR= 2 .12 ,95% CI= 1 .13 -3 .96) ,smoking at present(OR= 1 .44 ,95% CI= 1 .10 -1 .89) ,body weight(OR=1 .06 ,95% CI= 1 .00 -1 .12) ,blood pressure(OR=0 .76 ,95% CI=0 .59 -0 .98) .Conclusion Diabetes prevalence was high in Yongchuan district .The risk factors such as unhealthy diet ,smoking ,physical inactivity were common .Government should take the major role for diabetes prevention and control .
5.The relationship among plasma visfatin,endothelin, and insulin resistance in type 2 diabetic patients before and after treatment

The relationship between visfatin.endothelin, and insulin resistance in the pathogenesis of type 2 diabetes mellitus was explored. Endothelin, visfatin, and other markers were compared. Plasma visfatin was reduced after therapy. Endothelin was positively correlated with fasting insulin, HOMA-IR, HOMA-p, and visfatin. Visfatin was positively correlated with endothelin, HbA1C , fasting insulin, and HOMA-IR. HOMA-IR was an independent related factor in influencing visfatin. The visfatin is related to endothelin and insulin resistance, which may contribute to the pathogenesis of insulin resistance and type 2 diabetes mellitus.
6.LC-MS/MS assay of methylphenidate: stability and pharmacokinetics in human.

The study aims to solve the instability problem of methylphenidate (MPH) in plasma, and establish a LC-MS/MS method for simultaneous determining of MPH in human plasma. The stabilities of MPH in different media were studied, and the degradation characteristics of MPH in these media were also investigated by HPLC and LC-MS/MS. To a 200 microL aliquot of freshly collected plasma sample, 10 microL 2% formic acid was added immediately to prevent the hydrolysis of MPH in human plasma samples. Chromatographic separation was performed on a Sapphire C18 column using the mobile phase of methanol - 5 mmol.L-1 ammonium acetate buffer solution containing 0.1% formic acid (46 : 54). MPH was quantified by tandem mass spectrometry operating in positive electrospray ionization mode with multiple reaction monitoring. The detection used the transitions of protonated molecules at m/z 234.2-->84.1 for MPH and m/z 260.3-->183.1 for propranolol (IS), separately. The intra- and inter-assay precisions were all below 5.0%. The accuracies were all in standard ranges. The linear calibration curve was obtained in the concentration range of 0.035-40 ng.mL-1. The methods fulfilled the demand. The method was used to determine the concentration of MPH in human plasma after a single dose of 36 mg MPH tablet to 6 healthy Chinese volunteers. The method is suitable for the precisely determination of MPH and for pharmacokinetic study of MPH in human plasma.

7.Analysis on hypertension prevalence and its influencing factors among residents in Yongchuan district of Chongqing municipality

Objective To understand the hypertension prevalence and its influencing factors in Yongchuan district of Chongqing to provide the suggestion for the prevention and control of hypertension .Methods The permanent residents aged above 18 years old were sampled from 3 towns and 1 street by adopting the stratified clustered random sampling method and performed the questionnaire survey ,physical examination and detection of fasting glucose and postprandial 2‐h glucose .The data were analyzed to calculate the hypertension prevalence by the SPSS16 .0 software .The influencing factors of hypertension were analyzed by chi‐square and the multivariate Logistic regression analysis .Results A total of 1 202 persons were investigated .The survey found that the cognition level on hypertension in the residents was below 30 .00% .21 .46% of residents smoked every day .31 .78% of them did exercise .The proportions of overweight and obesity were 24 .63% and 6 .07% respectively ,the hypertension prevalence rate was 23 .88% .The multivariate Logistic regression analysis results indicated that the independent influencing factors of hypertension prevalence included age (OR=1 .95 ,95% CI:1 .74-2 .17) ,cognition level on hypertension(OR=2 .11 ,95% CI:1 .18-3 .79) ,oil in‐take per person per day(OR=0 .80 ,95% CI:0 .64-0 .99) ,body weight(OR=1 .97 ,95% CI:1 .58-2 .46) and blood glucose(OR=0 .63 ,95% CI:0 .50 -0 .78) .Conclusion The hypertension prevalence among the residents in Yungchuan district is high .The knowledge about the prevention and treatment of hypertension is deficient .The high risk factors of smoking ,drinking ,irrational diet and insufficient body movement are common .The comprehensive prevention and treatment work of hypertension should be practi‐cally strengthened .
8.Minimally invasive surgery for degenerative lumbar spine stenosis

ObjectiveTo explore the ideal minimally invasive surgical method for degenerative lumbar spine stenosis.MethodsFrom March 2008 to August 2010,73 cases with lumbar spinal stenosis underwent minimal invasive surgery were retrospectively analyzed.The patients were divided into different groups by clinical features,imaging manifestations,and concurrent diseases.The minimal invasive surgical methods were chosen according to the classification.Operation time,intra-operative bleeding,and complications were recorded.The lumbar function was evaluated by Oswestry disability index (ODI),and the clinical results were assessed by JOA pre- and postoperatively.ResultsMinimal invasive surgery was completed in all cases,which included 25 cases with bilateral decompression and 48 cases with unilateral approach for bilateral decompression.Twenty-three cases used interbody fusion and percutaneous pedicle screw fixation after decompression.Intraoperative blood loss and operation time were related to surgical methods.One case was complicated with dural tear.Cage was inserted into upper vertebral body during interbody fusion in 1case with osteoporosis,removed the cage and implanted bone fusion.Incisions of 3 cases were poor healing after surgery.The average follow up time was 13 months(range,10-35).JOA score and ODI before surgery were 9.2±5.1 and 62.3%±18.5% respectively; while 6 months after surgery,JOA score and ODI were 23.5±7.2 and 18.4%±6.4% respectively.JOA score and ODI showed statistically significant improvements after operation(P<0.01).Twenty-two cases got solid fusion at the final follow-up.ConclusionTreatment of lumbar spinal stenosis by minimal invasive surgery has satisfactory surgical outcomes,but the proper minimal invasive strategy should be chosen according to specific patients,surgeons and hospitals.
9. The two-stage urethroplasty with distal urethral preset for proximal pediatric hypospadias

Objective:
To investigate retrospectively a modified procedure of two-stage urethroplasty with distal urethral preset for proximal pediatric hypospadias and evaluate the feasibility and advantages of the method.
Methods:
From January 2013 to January 2018, two-stage urethroplasty with distal urethral preset was performed in 36 boys from 19 months to 15 years old (median age 36 months) with proximal hypospadias. The penile straightening and distal urethroplasty were performed at the first stage, meanwhile, the original urethral opening was preserved. After the distal reconstructed urethra performed in 6 months, the neourethra was connected with proximal meatus to re-establish the continuous urethra. The wound infection, urinary leakage, urethral stricture, appearance of penis and urination were observed after the second stage operation.
Results:
One case had skin infection around the wound after first stage operation and healed after wound management. Another one case had the distal urethra dehiscence but the wound has no signs of infection. The child had moderate anemia, and the hemoglobin is 80 g/L.It was therefore considered that anemia was the cause. The hemoglobin level was normal during the second stage operation, and repaired by Snodgrass method in the second stage. Five cases had the balanue dehiscence.Thirty case had being succeeded after the first stage, included a case of wound infection 5 days after operation. Three cases developed urethral fistula after the second stage repair, which were mended in the third stage. At a mean follow-up of 20 months postoperatively (14 to 48 months), all patients had satisfied appearance of penis, unobstructed urination and continuous urine line, without urinary fistula, urethral stricture and urethrocele at the second stage repair.
Conclusions
The distal urethral preset and staged procedure is a feasible operation that corrects proximal hypospadias with few complications and with good cosmetic and functional outcomes.
10.Relationship among optimism, entrepreneurial failure fear and entrepreneurial intention in college students

Objective:To explore the main characteristics of college students' entrepreneurial failure fear, and the mediating effect on the relationship between optimism and entrepreneurial intention among college students. Methods: Totally 427 college students were selected. Their optimism trait, entrepreneurial failure fear and entrepreneurial intention were accessed with the Optimism-pessimism Scale (OS), Failure Fear Scale (FFS) and Individual Entrepreneurial Intention Scale IEIS). Results: The average FFS score in college students was (2.8 ± 0.6). The scores of decrease of social value were higher in males than in females (P <0.01). The scores of decrease of self-esteem were higher in students from rural than those from cities (P < 0.05). The total FFS scores were lower in freshmen than in sophomores, juniors and seniors (P <0.01). OS scores positively predicted IEIS scores (β =0.19, P <0.001), and FFS scores partially mediated the relationship between the scores of OS and IEIS (effect size = 0.11, 95% CI:0.04-0.19). Conclusion: This study shows significant gender, birthplace and grade differences in college students' entrepreneurial failure fear. Entrepreneurial failure fear maybe partially mediate the relationship between optimism pessimism and entrepreneurial intention.
