2.Inflammatory pseudotumor of an orbito-maxillary mass masquerading as a malignancy

Objective: To present a case of a benign orbito-maxillary tumor behaving as an invasive, expansive malignancy. Design: Case report Setting: A tertiary care hospital in Metro Manila Patient: A 4 1/2-year-old boy with a seven month history of right orbito-maxillary mass, proptosis and epiphora. Result: A series of biopsies were done before a definite diagnosis was made due to inconsistencies in the histopathologic findings, clinical picture, and the radiologic presentation of the case. After diagnosis, appropriate intervention resulted in a dramatic decrease in the size of the mass. At present, the patient is disease-free and asymptomatic. Conclusion: Histopathologic diagnosis of Inflammatory Pseudotumor is difficult and differentiating it from malignant tumors is often a concern for otolaryngologists and pathologists. In spite of an initial malignant biopsy result, the combination of clinical signs and symptoms and radiologic findings of an infiltrative mass lesion, should not discount the possibility of a benign entity such as Inflammatory Pseudotumor for which treatment is conservative. Inflammatory pseudotumor of the bladder is an unusual benign lesion arising from the bladder submucosa. This benign lesion must be differentiated histologically from several malignant lesions of the bladder. Complete surgical excision, either by transurethral resection or partial cystectomy, appears to be curative. We report a case and describe the clinical presentation, and radiographic and histological findings.

BACKGROUND AND OBJECTIVES: Inflammatory pseudotumor is not malignant, but similar to malignancy lesions with respect to its clinical and radiological manifestations. It is well-known to opthalmologists and neurosurgeons, but unfamiliar to otolaryngologists. The objective of this study is to determine the diagnosis and treatment of inflammatory pseudotumor of the orbit. SUBJECTS AND METHOD: We reviewed 17 patients who visited Kangnam St. Mary's hospital and were diagnosed with inflammatory pseudotumor with clinical manifestations and radiological studies between September, 1994 and December, 2006. RESULTS: Seventeen patients were treated with high dose steroid, and of these, 12 showed clinical improvement, although 7 of these patients later showed recurrence. Of the five patients who initially showed no clinical improvement, one received radiation therapy but did not show improvement whereas two patients showed no improvement. One patient's symptoms improved after surgical decompression. CONCLUSION: Our study suggests that the initial treatment of inflammatory pseudotumor should be high dose steroid treatment without improvement and the second treatment should be radiation therapy or surgical treatment.
