Journal of Medical Research 2007;53(5):114-119
Background: Tophi gout refers to the condition, where consistently high level of uric acid in the body causes the deposition of uric acid or monosodium urate crystals on the joints. Objective: To describe clinical and subclinical features of tophi gout. Subjects and method: A study conducted in 51 patients with tophi gout (according to Bennett-Wood criteria - 1968) in Department of Rheumatology, Bach Mai hospital, from February 2005 to February 2006. This was a cross-sectional prospective study. Results: The average time of tophi appearance lasted 5.3 \xb1 5.7 years. The average number of tophi was 4.5 \xb1 3.9. The most common positions of tophi were tarso - metatarsal - phalangeal joints and elbow joints (43-47%). 100% had gouty arthritis (in which 66.6% > 4 joints), 50-55% involved tarso - metatarsal - phalangeal joints (43.1% had deforming joints). On X-ray, 62.7% cases has injuries, para-articular erosions and overhanging margins (62.5%), joints space narrowing (71.9%), proliferation osseous change (43.8%). Renal stones examined by ultrasound were 41%, renal failure 24.6%. Blood analysis showed hyperuricemia (88.2%), hypertriglyceridemia (54.9%), hyperglycaemia (13.6%). Conclusions: Common features tophi gout were injuries in tarso - metatarsal - phalangeal joints; gouty polyarthritis over 4 joints. X-ray images showed para-articular erosions, joints space narrowing, proliferation osseous changes.

3.Ultrasonographic Assessment in Crystal-induced Arthritis.
Korean Journal of Medicine 2015;89(6):632-643
The latest recommendations for the diagnosis and management of crystal-induced arthritis, such as gout and calcium pyrophosphate dihydrate (CPPD) deposition disease, recognize the diagnostic potential of musculoskeletal ultrasonography (MSUS). MSUS allows rapid, highly sensitive, non-invasive detection of microcrystal aggregates in multiple anatomic areas, and can be used as a safe, reliable guide for aspiration of articular and periarticular specimens suitable for microscopic analysis. MSUS can also be used to monitor disease after treatment. Ultrasonographic differentiation between gout and CPPD deposition disease is based on the characteristics of crystal aggregates and their preferential localization in different anatomical areas. This rapid assessment may profoundly affect the clinical process, avoiding expensive, time-consuming diagnostic procedures. This article reviews the current status of and recent advances in MSUS imaging in crystal-induced arthritis.

Tophi is one of the clinical manifestations of gout. On the other hand, it does not draw the patient's attention when it is asymptomatic, which leads to delayed management. The current case is a typical example of delayed diagnosis and management. The authors' preferred management of tophi was medical not surgical, even though the hitherto therapeutic issue has been conservative versus surgical. The authors chose conservative treatment in the osteolytic lesion resulting from huge tophi in the patella, and the report the results of 6 years follow-up.

Candida arthritis is a rare joint infection. Its predisposing factors include prosthetic joint, intraarticular injection of corticosteroid and immunosuppression. Clinical diagnosis may be difficult because of the absence of specific signs and symptoms. Hence the presence of Candida species in synovial fluid should never be interpreted as laboratory contamination in patients with a risk factor, and therapy should be initiated promptly. We experienced a case of Candida arithritis secondary to repeated intraarticular corticosteroid injection in chronic tophaceous gout. We report the case with review of relevant literature.
