Objective:To investigate clinic values of partial prostate sparing surgery in total cystectomy. Methods: 29 cases, age from 42~70 years old,(mean 61).Standard for being chosen:1.Distance was below 0.5cm between the carcinoma and bladder-neck.2. Biopsy negective at bladder-neck and urethral mucosa at random.3. PSA is below 4 ng/L.4.the surface of prostate was smooth and no scleroma by rectal examination;B-ultrasound had no unusnal echo in prostate before operation. All of the cases were bladder transitional cell cancer by Pathological examination. Invasive cancinoma 16 cases, superficial recurrence cancinoma 13 cases. Results: During the follow-up of 6 to 60 months(mean,38months). At 6 months follow-up,max flow rate of all the cases were beyond 20 ml/ s,residual volume above 100ml in 4 cases(4/29). All of the cases were fully continent during the day, and 27(93%) at night.16 cases had potency before operation, only 3 cases lost potency after operation;those 13 cases can have sex but no feel of ejaculation. Conclusion:Radical cystectomy with partial prostate sparing for bladder cancer is a relatively safe and reliable surgical procedure. It can improve the continence and potency without compromising the principle of tumor control.

Objective: To evaluate whether a prophylactic,tension-free vaginal tape (TVT)procedure,performed during prolapse repair, may prevent the development of postoperative SUI in these women.Methods:17 cases with genitourinary prolapse,in addition to genitourinary prolapse repair,underwent TVT.Results:The mean duration of follow-up was 13 months.None of the patients developed postoperative symptomatic SUI.Conclusion:To prevent the development of postoperative SUI,TVT as a prophylactic procedure during prolapse repair is effective

Objective:To assess the effects after the tension-free vaginal tape (TVT) procedure in women with urinary incontience.Methods:37 patients with stress incontinence underwent the TVT procedure.Results:The patients have been followed up for 3～16 months.All were cured by the TVT procedure.The main postoperative complication was retention (in 2 patients).Conlusion:Our results demonstrate that the TVT procedure is a safe and effective method for the treatment of stress incontinence in the female patient.

Objective: To discuss the practical value of cough test during the tension-free vaginal tape(TVT) procedure.Methods: 27 operations were performed according to the Ulmsten's method strictly,only that the stress of tape was adjusted in the light of cough test.The tape without stress was put down the urethral tract in other 18 operations,not adjusted by cough test.Results: 2 cases of urinary retention and 2 cases of difficulty of urination were found in the group of 27 cases,while difficulty of urination did not appear in other 18 cases.Conclusion: Adjusting stress of tape in accordance with cough test during the TVT can increase the opportunity of urinary retention or difficulty of urination after operation.

Objective To investigate the effect of preemptive cytomegalovirus infection(CMV) on acute rejection (AR) and assess the value of preemptive prophylactic antivirus therapy. Methods Retrospective study of 116 transplant recipients from September 2001 to February 2005 was made. The CMV infection was diagnosed by CMV-PP65 antigen tests. According to the result of CMV-PP65 antigen, 116 transplant recipients were divided into preemptive CMV infected group and non-infected group. The preemptive CMV infected recipients were subdivided into valacyclovir-treated group and non-valacyclovir-treated group. The impact of preemptive cytomegalovirus infection on acute rejection was analyzed. At the same time, CMV-PP65 antigen in 35 healthy subjects was detected. BACKGROUND: At present, there are considerable investigations and reports concerning the vascular remodeling cum pathology, molecular biology, immunohistochemistry, drug effect to the disease of the heart and great vessels. But varicocele is seldom reported. OBJECTIVE: To observe the change of the qualitation and quantitation of actin and collagen in varicocele vessel wall. DESIGN, TIME AND SETTING: A synchronized non-randomized control observation was carried out in the Department of Urinary Surgery, the First Affiliated Hospital of Chongqing University of Medical Sciences from March 2006 to June 2007. PARTICIPANTS: Thirty varicocele patients treated in Department of Urinary Surgery, the First Affiliated Hospital of Chongqing University of Medical Sciences from March 2006 to June 2007, were recruited for experimental group in this study. Their age was (26.29?9.12) years, and the course of disease was (2.76?1.83) years. Meanwhile, 24 normal spermatic veins from the donator of kidney transplantation, aged (39.56?8.32) years, were served as control group. All the patients and the donators' relatives signed the informed consent to the treatment. METHODS: Changes of collagen fibers and actin in 24 normal spermatic veins and 30 varicocele veins were studied quantitatively and qualitatively by the methods of V-G staining, immunohistochemistry, and scanning electron microscopy. The image analysis was conducted. Objective To investigate the reason why the misdiagnostic rate of testicular torsion is high and improve its diagnosis and treatment.Methods The clinical data of 20 cases of testicular torsion were retrospectively analysed.Results All 20 cases had the symptom of sudden colic of scrotum and went to hospital in 2 h.Only 5 cases were diagnosed as testicular torsion in 6 h by color Dopple ultrasonography.Three cases were cured by surgical detorsion and the testicles of 17 cases were removed for necrosis.The other testicle of all 20 cases was fixed.Conculusion The early diagnosis of testicular torsion is the most important and color Dopple ultrasonography is a reliable easy way.The sufficient knowledge of this disease by the doctors is crucial in improving the diagnosis and treatment.

Objective To evaluate the modified POSSUM surgical scoring system in urologic surgery patients over 70. Methods 180 patients over 70 were retrospectively analyzed with the mo dified POSSUM surgical scoring system.A comparison between the observed and the predicted morbidity was made. Objective To investigate the correlation between Gab2 expression and platinum drugs chemotherapy resistance by detecting the expression of Gab2 in ovarian cancer.Methods Immunohistochemical and Western blotting techniques were used to detect the expression of Gab2 in 107 specimens of serous ovarian tumor tissue and 12 specimens of normal ovarian tissues resected during bladder cancer radical operation in the department of gynaecology of our hospital from January 2011 to June 2015.The correlation between Gab2 expression with clinical stage and effect of combined chemotherapy based on platinum drugs was analyzed.Results The Gab2 expression level in ovarian cancer tissue was significantly higher than that in normal ovarian tissue,moreover had a relation with the FIGO clinical stage,the expression level of Gab2 increased with the increase of clinical stage,the difference was statistically significant (P<0.05);61 cases were sensitive to chemotherapy,46 cases were resistant to chemotherapy,the Gab2 expression level in the patients with chemotherapy resistance was significantly higher than that in the patients with chemotherapy sensitivity,the difference was statistically significant (P<0.05).Conclusion Gab2 may become one of effective indexes for predicting platinum drugs chemotherapy sensitivity.
