Though SP programs have been used in Korean medical education for half a decade, few of those SP programs have demonstrated psychometric characteristics. In order to be a more useful educational tool, and in to more accurately evaluate Korean Medical students, the standardized patient program needs to be more thoroughly researched and developed. In the near future, if Korean medical educators lend their support and standardize the program in Korea, SPs will be widely adopted across the curriculum because of their potential advantages: from medical interviewing and physical diagnosis courses to clinical clerkships and residency training. This study demonstrates why Korean SP programs need early, systematic collaboration and cooperation among Korean medical colleges through a regional consortium in order to form an agenda for the implementation of SP programs, and realize the implications of this agenda throughout medical education.

Serological survey, using methylene blue dye test for toxoplasmosis was carried out upon 395 normal population, 21 cases of uveal lesions, and 95 cases of acute febrile and chronic systemic diseases. Toxoplasma gondii preferred for this test was "Anyang straio" from Veterinary Researh Anynag, Korea in March 1968. The results were as follows: 1. In normal population 9.62 per cent of the sera examined showed positive titers of 1:16 or above in adult and 1:8 or above in children. 2. The positive in serologic survey was prevalent 3rd to 5th decades of both sex. 3. 4 out of 21 cases of uveal lesions were positive of which titers showed 1:64 or above in adults and 1:32 in a girl. 4. In 95 cases of acute febrile and chronic systemic diseases 4.2 per cent of the sera examined showed positive titers of 1:16 or above.

A case of orbital injury with a plastic chopstick accompanied by meningeal irritation signs was presented. A 34-year-old house-wife was admitted to our clinic complaining of vision loss, proptosis and dull ocular pain in right eye on March 11, 1976. 17 days before admission she was stabbed on the right orbital region with a plastic chopstick. The clinical finical findings Were recorded normal in left eye. The right eye showed zero in vision, proptosis, fixed eyeball with total ophthalmoplegia and exposure Keratitis. On the 2nd hospital day a broken plastic chopstick, 9.5 cm in length, was carefully removed from injured site by exploratory eperation. In that afternoon meningeal irritation signs, vomiting and neck stiffness was abruptly developed. The laboratory findings of the cerebrospinalfluid revealed inflammatory signs. After having been administered massive antibiotics (crystalline penicillin, bayrena, and chloramphenicol) systemi-cally for 17 days, she was recovered from meningeal irritation signs containing remission of inflammatory findings of cerebro spinal fluid examination. On the 21st hospital day, 39 days after injury, the optic disc was seen totally pale. On the 25th hospital day she was discharged, at which right eye showed clear in cornea, totally pale disc, and total ophthalmoplegia with ptosis. When she returned about 5 months after discharge, the right eye revealed much regressed ptosis, superficial corneal opacity with loss of corneal sense, ocular motility limited in superolateral direction, fixed and dilated pupil and still pale disc. Authors attempt to evalurate this case throlgh the related literatues.

The clinical resemblance of carcinoma of the Meibomian gland to the relatively frequent chaiazion should be kept in mind by anyone performing eye lid surgery. A 46-year-oId Korean house wife was first visited here in May 2nd '69 with complaints of localized swelling, ulceration and dull pain on the left upper lid. She had been taken the operation for chaiazion on the same side of the left upper eye lid a year ago. Examination revealed a reddened hard, tender lesion elevating the skin on the medial one third of the left upper lid with ulceration at its center. The tarsal conjunctiva was noted yellowish patches with ulceraion extended to its margin. Local resection with full thickness containing normal tissue around he lesion of upper lid was performed and graft was made in lower lid and rotatory transplantation was given, and pathologic examination revealed adeno-carcinoma of Meibimian gland. There was no evidence of recurrence and metastasis to remote site a year after surgery.

A 47-years old korean farmer was first visited our department in Nov. 19, 1969 with complaining of proptosis, lateral deviation of eye ball and visual disturbance of 5 years duration. The onset was insidious. Past and family history were not contributory. On examination, right eye was within normal limit. The left eye was markedly proptotic (aoout 10 mm of exophthalmos) and displaced out-and downward. The vision of the left eye was L.P.. The ocular motility was slightly limited to all direction. Pupil was normal size and poorly reacted to light. Funduscopic examination revealed pale and slightly excavated in disc. X-ray of chest, skull and optic foramen showed normal. On Nov. 22. 1969 author adopted the Kronlein's approach and exceed well capsulated oval mass situated within the muscle cone, which attached to optic nerve. The tumor mass showed soft, dark reddish color of surface measuring 2 X 2 X 15 cm and its cut surface disclosed sponge like appearance. Histologically the section from tumorous mass showed many large cavernous spaces separated by connective tissue. The newly formed cavernouus space was for the most part lined by a single layer of endothelium and in part contained with red cell. We confirmed adiagnosis of cavernous hemangioma of the orbit in pathologic study. It was suggested that early loss of vision was caused by optic atrophy due to direct pressure of optic nerve by the tumor mass. A review of literature of recent years related to cavernous hemangioma of orbit was added.

BACKGROUND: In this study, we investigated the early time course of expression of the major histocompatibility (MHC) antigens, intercellular adhesion molecule-1 (ICAM-1), vascular cell adhesion molecule-1 (VCAM-1), interleukin-6 and the histopathological changes in the coronary arteries of cardiac allografts exchanged between inbred mice strains that differ in one loci of class I major histocompatibility antigen (B10.BR to B10.A). MATERIAL AND METHOD: No immunosuppressive therapy was used. Both allografts and the hearts of the recipients were harvested at 7 (group 1, n=6), 15 (group 2, n=6), 21 (group 3, n=6), and 30 (group 4, n=6) days after transplantation. They were examined by immunohistochemistry, microscopy and morphometry. All allografts had contractions at the time of harvest. RESULT: A strong MHC class I antigen expression was present on the endothelial and medial cells of the coronary arteries in group 1 and remained unchanged in the rest of the groups. However, MHC class II reactivity was none or very little at any time. Mild to moderate ICAM-1 expression was observed on the endothelial cells, but not on the medial cells at any time by 30 days. VCAM-1 expression was strong both on the endothelial and medial cells at any time. Moderate degree expression of interleukin-6 was observed from 7 to 30 day specimens. Histopathologically, percentage of affected vessels (vessels with intimal thickening) was less than 10 % in 7 day group and increased up to 50 % at 30 days. Mean percent narrowing of the lumen of the affected vessels revealed less than 20 % at 7 days and 40 % at 30 days. The area occupied by tropomyosin positive cells in the intimal lesion, graded from 0 to 3, showed gradual increase but remained between grade 0 to 1 by 30 days. Medial integrity was also well preserved at any time. Moderate perivascular mononuclear cell infiltration was observed at 7 days and it was progressively increased upto 30 days. Recipients' heart revealed no positive immunopathologic findings. CONCLUSION: In this study, the early time course of progression of the transplantation vasculopathy was demonstrated in the murine heterotopic heart transplant model.

For parthenogenetic activation as a model system of nuclear transfer, microinjection and electroporation as activation treatments in bovine metaphase II oocytes were administered to each of three groups as follows: control group (treatments with Ca2+, Mg2+ -free PBS+100 micro M EGTA), IP3 group (control+25 micro M IP3) and IP3+ ryanodine group (control+25 micro M IP3+10 mM ryanodine). In experiments using microinjection, no significant differences were observed between any of the developmental stages of the electroporation experiment. For electroporation, cleavage rates were significantly higher in the IP3+ryanodine group than in the IP3 or control group (85.6% vs 73.7% or 67.6%, respectively). In the subsequent stages of embryonic development, such as morula and blastocyst formation, the IP3 and ryanodine group exhibited significantly higher rates of morula fomation than the IP3 or control groups (40.6% vs 24.2% or 16.7%, respectively). Similarly, the rate of blastocyst formation in the IP3+ryanodine group was significantly higher than the control group (16.3% vs 6.9%) but did not differ significantly from the IP3 group (16.3% vs 9.5%). In nuclear transfer, activation was performed at 30 hpm by microinjection and elecroporation with 25 micro M IP3+ 10 mM ryanodine followed by 6-DMAP treatment. No significant differences were observed at any stage of embryonic development and none of the embryos activated by electroporation reached either the morula or blastocyst stage. However, 3.8% and 1.9% of embryos activated by microinjection sucessfully developed to the morula and blastocyst stages, respectively. In conclusion, activation treatments using IP3 and ryanodine are able to support the development of bovine parthenogenetic and reconstructed embryos.
