1.Clinicopathological features of renal cell carcinoma : a primary study of 705 cases

Objective To investigate the clinicopathological features of renal cell carcinoma (RCC).Methods From December 1956 to August 2012,the clinicopathological features of RCC were studied in 705 cases and related literatures were reviewed.Results The diameter of RCC ranged from 0.6 to 18.0 cm,which the average size was 4.6 cm.The proportions of the clear cell,papillary,multilocular clear cell,chromophobe and unclassified histologic subtype were 88.9% (627/705),4.1% (29/705),3.3% (23/705),1.3% (9/705) and 2.4% (17/705),respectively.According to the Fuhrman grading system,the proportions of grade 1,2,3,4 were 19.0% (116/612),58.3% (357/612),18.1% (111/612)and 4.6% (28/612),respectively.The rates of invasion into the renal pelvis,perirenal fat and vascular were 10.9% (66/603),10.6% (64/603) and 4.8% (29/603),respectively.Of 705 cases,464 (76.6%)cases were in T1,65 (10.7%) cases in T2,73 (12.0%) cases in T3,and 4 (0.7%) cases in T4.As to the lymph node and distant metastasis,the rate was 2.8% (17/606) and 3.5% (21/606).The percentages of stage Ⅰ,Ⅱ,Ⅲ and Ⅳ RCC were 74.3% (450/606),9.9% (60/606),11.7% (71/606) and 4.1%(25/606),respectively.The 3-,5-,10-and 15-year disease-specific survival rate for RCC was 92.8%,86.9%,76.8% and 55.5%,respectively.To those patients with clear cell RCC,the disease-specific survival at the same time point was 92.8%,88.1%,77.4% and 55.4%,respectively.Multivariate analysis showed that the stage was the only independent prognostic factor for RCC.Conclusions Tumor stage of RCC is the independent prognostic factor for disease-specific survival.The evaluation of renal sinus invasion and lymph node should be noted in the diagnosis of RCC.
2.Progress of event-related functional magnetic resonance imaging in field of rewarding stimuli in major depression

Reduced responsiveness to reward stimuli is a central feature of major depressive disorder (MDD). Study on the changes of neural substrates and behavior contributes to understand the mechanism of MDD. Functional magnetic resonance imaging (fMRI) has the unique advantage of investigating reward processing, which is of importance in the brain imaging study. The progresses of event-related fMRI (ER-fMRI) in field of rewarding stimuli in major depressive disorder were reviewed in this article.
3.Nonsurgical spinal decompression and common cervical traction compared

Objective To compare the effects of nonsurgical spinal decompression system (SDS) cervical traction and common cervical traction using surface electromyography (sEMG).Methods Twenty-two volunteers' sEMG signals were recorded before,during and after two modes of cervical traction:the conventional traction in sitting and SDS traction in supine.The interval between the two modes of traction was no shorter than 24 hours.The mean and peak amplitudes of their sEMG signals were observed before,during and after traction.The sEMG signals of the posterior cervical muscle while sitting or lying were compared at different stages of the traction.Results There was no statistically significant difference in sEMG signals between the subjects' right and left posterior cervical muscles.The sEMG signals decreased significantly during both traction trials.With common cervical traction the mean and peak amplitudes of the left posterior cervical muscle pre-and post-traction were higher than during traction,and the same as that of the right posterior cervical muscle.In the SDS trials the sEMG signals during traction and post-traction were significantly lower than those pre-traction.After traction the sEMG signals while sitting (common cervical traction) were significantly higher than those post-traction while lying (SDS cervical traction),and the sEMG signals post-traction while either lying or sitting were stronger than during traction while lying.The post-traction signals in sitting were stronger than those in lying.Conclusions Both nonsurgical SDS and common cervical traction can relax the posterior cervical muscles.The effect of nonsurgical SDS was better than that of common cervical traction.
4.Trends in Gleason scores of Chinese prostate carcinoma from 1995 to 2014

Objective:To assess the changing trends in Gleason score (GS)of Chinese prostate carci-noma (PCa)from January 1995 to December 2014.Methods:In the study,875 patients admitted to hospital from January 1995 to December 2004 (1995 -2004)and from January 2005 to December 2014 (2005 -2014)were divided into two groups.The mean levels and proportions of GS,primary and se-condary grades were studied.The patients were divided into four groups according to age: <60,60 -69,70 -79 and ≥80 years.Types of specimen included needle biopsy (NB),transurethral resection of the prostate (TURP)and radical prostatectomy (RP).Histological types were made up by acinar carci-noma and other types (including atrophic,pseudohyperplastic,foam,signet ring cell and ductal carcino-ma,and so on).The total prostate-specific antigen (tPSA)involved groups of <20.0 μg/L and ≥20.0 μg/L.We observed the mean levels and proportions of GS in age,types of specimen,histological types and total prostate-specific antigen in different periods,and used SPSS 17.0 software for statistical analysis.Results:Compared with 1995 -2004,the mean levels of GS,primary and secondary grades decreased 0.32 (P =0.003),0.19 (P =0.001)and 0.12 (P =0.016)in 2005 -2014,respectively. The proportions of ≤6 in GS increased 10.9% (P =0.003),and ≥8 decreased 14.0% (P <0.001). The difference of GS 7 was not statistically significant.In the primary grade,the ratio of grades≤3 in-creased 12.8% (P =0.001 ),and grade 4 decreased 7.4% (P =0.037),grade 5 decreased 5.5%(P =0.007).The ratio of secondary grades≤3 increased 7.6% (P =0.037).The difference of grades 4 and 5 was not statistically significant.Conclusion:GS in Chinese patients with PCa showed a down-ward trend,which is one of the notable features in the past 20 years in China.The types of specimen and age are important factors in GS,while the histological types and tPSA have less impact on the GS.
5.Attentional Bias for Negative Emotional Facial Expressions in Major Depressive Disorder

Objective:To investigate the attentional bias for negative emotional facial expressions in major depressive disorder(MDD).Methods:Twenty MDD participants were selected from a larger pool of patients (n=35),diagnosed as depression with the Chinese Classification and Diagnostic Criteria of Mental Disorders Version 3 (CCMD-3),according to the Hamilton Depression Scale (HAMD) and Beck Depression Inventory (BDI).And 20 non-depression control participants(NC)matched with MDD group on age,gender and education level.All participants completed an exogenous cueing task which consisted of two kinds of cue types(valid and invalid trial)and two kinds of face types(neutral faces and negative faces).Results:Patients with MDD showed more larger cue validity effect for negative faces compared with neutral faces(21.73 ms vs.3.91 ms,P<0.01).They showed a stronger attentional engagement for negative faces in comparison with non-depressed participants(17.25 ms vs.1.64 ms,P<0.001).The NC group directed attention away from negative faces,more rapidly disengaging their attention compared with MDD,but the differences showed no significant(-1.50 ms vs.0.57 ms,P>0.05).Conclusion:These results support the assumption that MDD is associated with attentional bias for negative information,and deficits protective bias for it.
7.Impact of International Association of Urological Pathology Gleason scoring system on prostatic carcinoma grading: a preliminary analysis of 185 cases.

OBJECTIVETo explore the impact of the 2005 International Association of Urological Pathology (ISUP) Gleason score (GS) system on prostatic carcinoma grading.
METHODSUsing the 1977 revision and ISUP version of GS system, 112 needle biopsies, 18 transurethral resections of the prostate and 55 radical prostatectomies were scored. The proportion of grading discrepancy was observed and compared between the two versions of GS.
RESULTSGleason scores of 3+3, 3+4 and 4+3 accounted for 47.0% (87/185), 11.4% (21/185) and 17.3% (32/185) in the modified system, and accounted for 25.9% (48/185), 21.6% (40/185) and 27.6 % (51/185) in ISUP system, respectively. The percentages of primary grade by modified vs. ISUP system were 62.7% (116/185) vs.50.8% (94/185) for grade 3, and 31.4% (58/185) vs. 41.6% (77/185) for grade 4. The percentages of secondary grade by modified vs. ISUP system were 65.9% (122/185) vs. 54.6% (101/185) for grade 3, and 21.1% (39/185) vs. 31.4% (58/185) for grade 4.
CONCLUSIONSISUP system is different from the modified system. Compared with the modified system, the proportion scored by ISUP system tends to decline for GS 3+3 but increase for GS 3+4 and 4+3.

8.Application of double supervisor teaching in practical teaching of rehabilitation technology specialty

Objective:To explore the effect of double supervisor teaching in practical teaching of rehabilitation technology specialty.Methods:Totally 45 students from rehabilitation technology specialty in our university were randomized into experimental group and control group. Experimental group (22 students) took double supervisor teaching method, which was jointly conducted by a doctor and a physiotherapist, while control group (23 students) adopted single physiotherapist teaching method. The practical effects were evaluated by theory test, clinical operation test, and questionnaire survey. The data were analyzed by SPSS 22.0.Results:The scores of tests in experimental group were significantly better than those in control group ( P<0.05). In addition, according to the questionnaire survey, the scores of indicators related to the teaching effectiveness in experimental group were significantly better than those in control group ( P<0.05). Conclusion:The application of double supervisor teaching in practical teaching of rehabilitation technology specialty can improve students' academic performance, improve students' learning ability and problem-solving ability, which is worthy of further exploration and promotion in rehabilitation medicine education.
9.Finite element analysis of the influence of new anterior cervical spine memory compression fixator on adjacent segments

Objective:To compare the effect of the new anterior cervical spine memory compression fixation device (GYZ memory alloy plate) and traditional titanium plate on the range of motion (ROM) and stress of the adjacent segment after anterior cervical discectomy and fusion.Methods:An adult male volunteer was recruited for a fee. After excluding cervical malformations, fractures, infections and other diseases, C 3-C 7 thin-layer CT scans were performed. Import the scanned data into the finite element modeling software to establish the finite element model of the physiological group and verify itseffectiveness. After C 5,6 discectomy, the intervertebral fusion device was inserted, and the anterior fixation was assisted by a conventional titanium plate or a new type of fixator. Thus, the finite element model of the traditional titanium plate group and the new fixer group was established. The three models were imported into the finite element analysis software ANSYS 16.0, and a vertical downward axial load of 73.6 N was loaded to simulate the head weight and the torque of 1.0 N·m to simulate the cervical spine flexion, extension, left lateral bending, right lateral bending, left rotation and right rotation.Compare the changes of intervertebral disc ROM and stress in adjacent segments of physiological group, traditional titanium plate group and new type fixator group. Results:The intervertebral disc ROM under six conditions was basically similar to the results of previous studies, and the model was effective. In the adjacent segment C 4,5, the three groups of activities in the flexion, extension, left lateral bending, right lateral bending, left rotation and right rotation conditions were: physiological group 3.9°, 4.2°, 3.7°, 3.7°, 2.2° and 2.2°, traditional titanium plate group 4.6°, 4.7°, 4.3°, 4.4°, 3.3° and 3.1°, and new fixture group 4.4°, 4.3°, 4.0°, 4.2°, 2.8° and 2.7°. The maximum stresses of the intervertebral discs under three different working conditions were: physiological group 1.81, 1.60, 3.99, 2.06, 3.63 and 3.41 MPa, traditional titanium plate group 1.86, 1.67, 4.21, 2.16, 3.82 and 3.63 MPa, and new fixture group 1.84, 1.64, 4.17, 2.14, 3.78 and 3.58 MPa. In the adjacent segment C 6,7, the activities of the three groups in six working conditions were: physiological group 3.1°, 3.2°, 2.5°, 2.5°, 1.2° and 1.3°, traditional titanium plate group 4.2°, 3.7°, 3.4°, 3.0°, 2.1° and 2.2°, and new fixture group 3.5°, 3.3°, 2.5°, 2.7°, 1.8° and 1.9°.The maximum stress of the intervertebral disc under three different working conditions was: physiological group 0.45, 0.66, 1.12, 0.85, 0.84 and 0.82 MPa, traditional titanium plate group 0.62, 0.93, 1.55, 1.24, 1.44 and 1.27 MPa, and new fixture group 0.61, 0.92, 1.54, 1.22, 1.07 and 1.24 MPa. The ROM and disc pressure of adjacent segments in the conventional titanium plate group were higher than those of the new fixator group. Conclusion:Compared with the traditional titanium plate, the new type of anterior cervical memory compression fixator has less effect on the ROM and stress of adjacent segments, which may slow down the process of adjacent segments degeneration to a certain extent.
