1.Primary thyroid lymphoma:report of 7 cases and review of literature

Objective To investigate the clinical characteristics of primary thyroid lymphoma(PTL) and its prognostic factors.Methods Clinical and pathological data of 7 cases diagnosed as primary thyroid lymphoma were retrospectively analyzed.Results All of the 7 patients were diagnosed on thyroidectomy and presented with thyroid nodules,of whom 6 cases were middle-aged to elderly women and some had locally oppressive symptoms.Four cases were diagnosed as diffuse large B non-Hodgkin's lymphoma (DLBCL) and 3 were mucosa-associated lymphoid tissue B-cell lymphoma (MALT).Pathological subtype of mucosa-associated lymphoid tissue B-cell lymphomas and younger patients had better prognosis.Conclusion The possibility of PTL must be kept in mind in the differential diagnosis of thyroid nodules in middle-aged to elderly women.Age and pathological subtype are important prognostic factors.
2.Reform and Exploration of Practical Teaching System Based on Talents Training for Biomedical Specialty in Higher Vocational Education

OBJECTIVE:To provide reference for improving practical teaching system of biomedical specialty talents training. METHODS:Based on the present situation of practical teaching system in higher vocational education,combined with the develop-ment of biomedicine industry,referring to foreign experience about vocational education mode,using the elements of the practice teaching system as the breakthrough points,hardware and software of practical teaching were highlighted and strengthened;the practical teaching system was established fromability oriented,practical teaching as the leadingpractical ability of vocational stu-dents and vocational ability talent training mode. RESULTS:The practical teaching system of biomedical specialty talents training was established on the basis ofthree-capacity,one body two wingspractical teaching target system,three standards,hierarchi-cal and modularpractical teaching content system,system and institutions,hardware and software managementstandard man-agement system,progressive training base platform,dual-level teaching staff,practical teaching supporting system of talented teacher group. CONCLUSIONS:The practical teaching system strengthen comprehensive practice ability training, can realize win-win between college and government,and of society. It can provide reference for improving practical teaching system of bio-medical specialty talents training.
3.Effect of Yiqiyangyin Decoction on pulmonary fibrosis rat and expression of Smad2/Smad7 protein

Objective:To observe the effect of Yiqiyangyin Decoction( YQYY) on bleomycin-induced rats pulmonary fibrosis and the expression of Smad2 and Smad7.Methods:Sixty male SD rats were randomly divided into normal group,model group,YQYY group,hormone group,YQYY plus hormone group.The rats in the normal group injected saline via the trachea,the rest rats injected ble-omycin(5 mg/kg).From the second day of injection,YQYY group were treated with YQYY decoction(1 ml/100 g);hormone group were given 35%Prednisone acetate solution(1 ml/100 g);YQYY plus hormone group were treated with the mixture of YQYY decoction and hormone (1 ml/100 g);normal group and model group were given equivalent saline.After twenty-eight days,all the rats were killed,the rat lung histopathology was examined with HE staining and detect Smad2/Smad7 protein with immunohistochemical staining.Results:Compared with normal group, the model group rats alveolitis and pulmonary fibrosis scores were significantly increased;compared with model group,the hormone group,YQYY group,YQYY plus hormone group rats alveolitis and lung fibrosis scores were reduced,and the lung injury of YQYY group reduced obviously.The expression of Smad2 protein of model group in alveolar epithelial cells and the fibroblasts was significantly higher than normal group;compared with model group,the positive expression of Smad2 in hormone group,YQYY group,YQYY plus hormone group were reduced,the Smad2 of YQYY group was the lowest.Compared with normal group,the smad7 protein of model group decreased significantly;compared with model group,Smad7 protein expression in hormone group, YQYY group, YQYY plus hormone group were increased, the Smad7 expression of YQYY group was highest.Conclusion:Yiqiyangyin decoction has a good intervention and therapeutic effect on bleomycin-induced rats pulmonary fibrosis,its mechanism may be related to inhibiting the inflammatory reaction,reduce the formation and deposition of collagen and the intervention of Smad signal pathway.
4.MRI diagnosis of cerebral sparganosis in children.

OBJECTIVETo study the value of MRI in the diagnosis of cerebral sparganosis in children.
METHODSThe MRI appearances of 17 children with cerebral sparganosis proven by pathology or serological test were retrospectively studied. The diagnostic accuracy rate of cerebral sparganosis was compared before and after knowing the imaging features of this disease.
RESULTSEnhanced MRI was performed in 12 cases, and all of them demonstrated abnormal enhancement. Peripheral ring-type, tortuous beaded shape or serpiginous tubular shape enhancement were found in 8 cases. After analysis of follow-up MRI for 7 cases, a change in location and shape of lesions was found in 2 cases. Typical MRI patterns of cerebral sparganosis included abnormal enhancement such as peripheral ring-type, tortuous beaded shape or serpiginous tubular shape enhancement, and changes in location and shape of lesions in the follow-up MRI. The diagnostic accuracy rate for the 17 cases of cerebral sparganosis at the first visit, after first imaging examination and after several follow-up MR examinations were 0%, 11.8% and 28.6%, respectively. The diagnostic accuracy rate of this disease increased to 64.7%, 70.6% and 85.7% respectively after the radiologists were trained in the imaging characteristics of the disease.
CONCLUSIONSMR findings in children with cerebral sparganosis are specific. Mastering correctly the typical imaging features of the disease can be very helpful in reducing the rate of clinical misdiagnosis.

5.Estimated Financing Amount Needed for Essential Medicines in China, 2014.

BACKGROUNDAt the present time, the government is considering to establish the independent financing system for essential medicines (EMs). However, it is still in the exploration phase. The objectives of this study were to calculate and estimate financing amount of EMs in China in 2014 and to provide data evidence for establishing financing mechanism of EMs.
METHODSTwo approaches were adopted in this study. First, we used a retrospective research to estimate the cost of EMs in China in 2014. We identified all the 520 drugs listed in the latest national EMs list (2012) and calculated the total sales amount of these drugs in 2014. The other approach included the steps that first selecting the 109 most common diseases in China, then identifying the EMs used to treat them, and finally estimating the total cost of these drugs.
RESULTSThe results of the two methods, which showed the estimated financing amounts of EMs in China in 2014, were 17,776.44 million USD and 19,094.09 million USD, respectively.
CONCLUSIONSComparing these two results, we concluded that the annual budget needed to provide for the EMs in China would be about 20 billion USD. Our study also indicated that the irrational drug use continued to plague the health system with intravenous fluids and antibiotics being the typical examples, as observed in other studies.

6.The analysis of low cost medicine's accessibility in China: Based on the empirical study from 24 provinces and cities

Objective:To analyze the satiation of low cost medicine at present and provide suggestions for im-proving the low cost medicine policy since it has been conducted. Methods: The article uses the statistics from 24 provinces and cities to analyse the accessibility of low cost medicine with the angles of general condition,procurability and affordability. Results:The accessibility of low cost medicine is high through its policy knowledge is low. For pro-curability, there are only part of low cost medicine lacking in several areas momently; for affordability, the price of low cost medicine is affordable even through its price has been raised after the performance of policy. The reasons for shortage includes low price,check price transaction,lack of raw and processed material and small quantity demand of some medicine,etc. Conclusions:We suggest that government should improve the knowledge of this policy in hospi-tals and encourage low cost medicine in clinical applications. It also should lead companies to set the appropriate price and refine the purchasing system which will avoid the too high or too low price and help to ensure the amount and standard of companies that be selected in purchasing online. At the same time, government is supposed to strengthen the supervision on supply chain of low cost medicine and establish the communication mechanism for sup-ply system in order to guarantee the regulated supply of low cost medicine.
7.Effect of lipoxin A4 on lipopolysaccharide-induced endothelial hyperpermeability in human umbilical vein endothelial cell

Objective To explore whether lipoxin A4 (LXA4)could prevent lipopolysaccharide (LPS)-induced human umbilical vein endothelial cells (HUVEC) monolayer hyperpermeability and its possible mechanism. Methods Human umbilical cords were obtained from women with normal pregnancy immediately after delivery from Tongji Hospital Affiliated of Tongji Medical College. Primary HUVEC were isolated from umbilical veins and subcultured, then, HUVEC were divided into four groups:control group;LPS group (10 mg/L of LPS); LPS + LXA4 group(10 mg/L of LPS and 100 nmol/L of LXA4); LPS +LXA4 + BOC-2 group [10 μmol/L of BOC-2, an effective antagonist of formyl peptide receptor like 1 (FPRL-1)]. All expriments were performed after cells were treated for 24 hours. Endothelial permeability was measured by fluorescein isothiocyan-ate labelled bovine serum albumin (FITC-BSA) clearance across the monolayer; tumor necrosis factor α(TNF-o) mRNA and secretion were detected by reverse transcriplase (RT) -PCR and ELISA assay respectively, and nuclear factor κB(NF-κB) protein change was determined by western blot. Results (1) LPS induced a significant increase in the permeability [Pa value of LPS group was (183.1 ±1.7)%], while co-administrating with LXA4 obviously attenuated this LPS-induced hyperpermeability, Pa value of LPS + LXA4 group was (103.1 ±2.2)%, LPS + LXA4 + BOC-2 group was (162.2 ± 2.8)%, control group was 100%, the permeability of HUVEC monolayer was significantly increased by LPS which was (83.1 ± 1.7)% of control (P <0.01), however, it was notably inhibited by LXA4 (P<0.05); the blockade of FPRL-1 could attenuate the effect of LXA4, that is, there was no difference between the LPS + LXA4 + BOC-2 group and the LPS group. (2) After treatment with different concentration of LPS(0,0.1, 1,10 mg/L), the mRNA expressions of TNF-α were increased (1.11 ±0.11,1.27 ± 0.03, 1.60 ± 0.06, 1.82 ± 0. 04, respectively), compared with the control group, at the concentration of 1,10 mg/L LPS, the difference was statistically significant (P<0. 05). (3) The increased levels of NF-κB and inflammatory mediator TNF-α in the LPS group were both inhibited by LXA4. Levels of NF-κB protein and TNF-o mRNA secretion in LPS treated group (0.53 ±0.06 and 0.81 ±0.09 ,respectively)were both inhibited by LXA4 (0.19 ± 0.05 and 0.41 ± 0.07, respectively, and both had significant difference, P<0.05). (4) Levels of TNF-α in HUVEC culture medium of LPS group [(31.94 ±0.01)ng/L] was significantly higher than the control group [(18.17 ± 0.03) ng/L, P<0.05], LPS + LXA4 group [(15.72 ± 0.07) ng/L] was significantly lower than the LPS group (P<0.05). Conclusion Our findings demonstrated that LXA4 could prevent the endothelial cell hyperpermeability induced by LPS in HUVEC under which the possible mechanism was through inhibiting the expression of NF-κB and its related cytokines through receptor-dependent.

A large amount of evidence has showed that sexually transmitted infection is an important synergistic factor of human immunodeficiency virus (HIV) infection.Therefore, this paper reviews the current situation of sexually-transmitted diseases (STD) and HIV infection, introduces HIV prevention and intervention measures and problems for STD patients at home and abroad, and proposes that behavior-psychology-society integrated intervention model should be constructed based on the characteristics of STD patients.
9.The biological function of auto-induced expression of the hepatitis C virus soluble core protein.

OBJECTIVETo investigate the biological role of auto-induced expression of hepatitis C virus (HCV) core protein (protein C) using a recombinant protein in an in vitro cell-based system.
METHODSThe PCR-amplified full-length HCV protein C gene (573 bp) was inserted into the pET28a prokaryotic expression vector. The recombinant plasmid was transformed into BL21(DE3)pLysS E. coli to achieve high-concentration expression of the recombinant C protein by auto-induction. The recombinant protein C was purified by Ni-NTA affinity chromatography, and tested in a protein binding assay for its ability to bind the HCV NS3 protein.
RESULTSThe transformed E. coli produced a large amount of recombinant protein C, as detected in the sonicated supernatant of the bacteria culture. The antigenic reactivity of the recombinant protein C was confirmed by western blotting. However, the recombinant protein C could not be purified by Ni-NTA affinity chromatography, but co-precipitated with the HCV NS3 protein.
CONCLUSIONSoluble recombinant protein C was successfully expressed by auto-induction, and shown to interact with the HCV NS3 protein, which provides a novel insight into the putative biological activity of this factor in HCV-related molecular processes. Future studies of this recombinant HCV protein C's crystal structure and antigenicity may provide further clues to its biological function(s) and potential for clinical applications.

10.Development of a SPA-ELISA method for detecting anti-coronavirus IgG antibodies in serum samples from fulvous fruit bats.

OBJECTIVETo develop an simple and sensitive method for detecting anti-coronavirus IgG antibodies in bat sera based on enzyme-linked immunosorbent assay (ELISA).
METHODSA commercial ELISA kit for detecting SARS-CoV antibody was modified for detecting coronavirus antibodies in bat serum samples. The second antibody in the kit was replaced with horseradish peroxidase-conjugated protein-A (HRP-SPA) based on the characteristics of binding between Staphylococcus aureus protein A (SPA) and mammal IgG Fc fragment. The sera of 55 fulvous fruit bats (Rousettus dasymallus) were tested using the SPA-ELISA.
RESULTSThe test results of the positive and negative controls in the kit and the serum samples from convalescent ;patient were consistent with expectation. Coronavirus antibody was detected in 2 out of the 55 bat serum samples. Serum neutralization test confirmed the validity of the SPA-ELISA method.
CONCLUSIONThis SPA-ELISA method is applicable for detecting coronavirus antibody in bat sera.
