1.Value of ultrasound-measured changes in diaphragmatic excursion in predicting successful weaning from mechanical ventilation during spontaneous breathing trial

Objective To evaluate the value of ultrasound-measured changes in diaphragmatic excursion (DE) in predicting successful weaning from mechanical ventilation during the spontaneous breathing trial (SBT).Methods Sixty-three patients of both sexes,who were mechanically ventilated for more than 24 h and screened for ascertained readiness to wean,aged 45-64 yr,were enrolled in the study.Bilateral DE was measured using ultrasound at 0,10 and 30 min of SBT.The patients who successfully completed 30 min of SBT were extubated.The patients were divided into either success group or failure group according to whether or not weaning was successful.Receiver operating characteristic (ROC) curves were used to evaluate the value of bilateral △DE30-10 in predicting successful weaning.Results There were 48 cases in success group and 15 cases in failure group.Bilateral △DE30-10 was significantly higher in failure group than in success group (P<0.05).The area under the ROC curves of the right △DE30-10 in predicting successful weaning was 0.958,<0.175 cm was used to predict successful weaning,and the sensitivity and specificity were 93.3% and 87.5%,respectively.The area under the ROC curves of the left △DE30-10 in predicting successful weaning was 0.903,<0.275 cm was used to predict successful weaning,and the sensitivity and specificity were 80.0% and 89.6%,respectively.Conclusion The fight △DE30-10 measured using ultrasound can serve as an assistant index in predicting successful weaning from mechanical ventilation during SBT.

B→O blood conversion has very important meanings in the blood transfusion during the period of peace and war.In the leader of Professor Zhang Yangpei,from Academy of Military Medical Sciences,B→O blood conversion has been achieved by using gene engineering.They obtained the great breakthrough in the domain of blood type conversion.In order to realize standardization and automatization,it is necessary to develop a device applying to B→O blood conversion.This device automatically accomplishes the process of B→O blood conversion,shortens the time of B→O blood conversion to quarter than the time by manual operation,and ensures the standardization and obturation.In the condition of one-off pipeline,this device meets the clinical demands for blood,and lays a foundation for clinical expansion of B→O blood conversion.This paper presented the control system of B→O blood conversion device,including design of software and hardware.This device provides a safe and efficient tool for B→O blood conversion.

OBJECTIVE:
To evaluate the osteogenetic character and repairing maxillary sinus superior wall fractures capability of calcium phosphate cement (CPC) before and after combined with recombinant human bone morphogenetie protein-7(rhBMP-7).
METHOD:
A 10 mmX5 mm bone defect in the maxillary sinus superior wall was induced by surgery in all 24 New Zealand white rabbits. These 24 rabbits were randomly divided into two groups. The defects were repaired with CPC group (n = 12) and CPC/rhBMP-7 group (n = 12). The osteogenesis of bone defect was monitored by gro'ss observation, histological examination, observation under scanning electron microscope and measurement of ALP activity at 6 and 12 weeks after the implantation.
RESULT:
In group CPC,new bone was found to form slowly and little by little. In group CPC/rhBMP-7, however, new bone was observed to form early and massively. The ALP activity in group CPC showed significant statistical difference with that of group CPC/rhBMP-7 (P < 0.05).
CONCLUSION
The CPC/rhBMP-7 composite has osteoconductibility and osteoinductibility, comparing the use of CPC/rhBMP-7 with CPC for the repair of orbital fracture, the former show obvious advantage repairing ability in maxillary sinus superior wall defect.

6.Effects of the first metatarsophalangeal joint bending on human walking gait stability when walking on slippery floor.

The first metatarsophalangeal joint bending plays an important role in the foot movement. However, the existing researches mainly focused on the movement scope of the joint and the clinical treatments of related foot diseases. In order to investigate the effects of the first metatarsophalangeal joint bending on human walking gait stability, the present researchers recruited 6 healthy young men to perform the first metatarsophalangeal joint constraint (FMJC) and barefoot (BF) walking tests. Data of the temporal and spatial parameters, the joint angles of lower limbs, the ground reaction forces (GRF) and utilized coefficients of friction (UCOF) were collected and analyzed. The results showed that, since hip and knee could produce compensation motions, the FMJC had no significant effects on waking gait, but the slip and fall probability increased significantly.

7.The effects of rat transplantated tumor capillary permeability under microbubble enhanced diagnostic ultrasound exposure

Objective To investigate the effects of transplantated tumor capillary permeability under microbubble enhanced diagnostic ultrasound exposure.Methods Fourty eight rats were randomly divided into four groups after Walker 256 carcinomas were subcutaneousely implanted,named the control group (A),microbubbles group (B),ultrasound group (C),microbubbles and ultrasound group (D).Using Evans blue(EB) as the indicator for tumor capillary permeability,the microbubbles were injected through the vein with diagnostic ultrasound frequency as 1.75 MHz,mechanical index as 1.4.The spillover of EB was estimated under confocal laser microscope,meanwhile the contents of EB in the tumor tissues were calculated according to the standard curve and spectrophotometry.Results EB spillover was observed around the capillaries in the group D,while EB spillover was only observable on capillary in group A,B and C.The EB content was also significantly higher than those in group A,B and C.Conclusions Exposure to both diagnostic ultrasound and microbubbles results in increased capillary permeability of rat tumor tissues,which might become a potential therapeutic method on clinical chemotherapy.
8.Clinical study of capecitabine or S-1 as maintenance treatment to patients with advanced gastric cancer

Objective To evaluate the efficacy and adverse effect caused by capecitabine compared with S-1 as maintenance treatment of patients with advanced gastric cancer (AGC). Methods A total of 123 AGC patients who did not suffer disease progression after first-line chemotherapy were randomized into three groups. The capecitabine group(Cap)received maintenance chemotherapy with capecitabine(1000 mg/m2 twice daily for 14 days,21 days/cycle),and the S-1 group(S1)received S-1(40,50,or 60 mg according to the body surface area and orally administered twice a day for 14 days ,21 days/cycle). The observation group was given the support-ive treatment. Patients kept this chemotherapy regimens until disease progressed or with intolerant toxicity. Re-sults The disease control rate was 70.7%in the Cap group and 80.5%in the S1 group(P=0.304). The median time of progression was 8.3 months in the Cap group and 8.5 months in the S1 group(P = 0.448). Maintenance chemotherapy groups showed better responses in the treatment group than the observation group ,which demonstrat-ed a median progression of 6.7 months(P<0.001). The median overall survival time was 15.3 months in the Cap group and 15.7 months in the S1 group(P = 0.637). Maintenance chemotherapy groups showed better responses than the observation group ,which demonstrated a median survival of 12.8 months (P < 0.05). The main side effects included hyperpigmentation,bone marrow suppression,nausea and vomiting and hand-foot syndrome. No death occurred in relation to the therapy. Conclusion The effectiveness of capecitabine and S-1 as maintenance chemotherapy in AGC patients after the first-line chemotherapy are similar,and both can prolong the time of overall survival. And the adverse reactions can be tolerated.
9.Comparative study on intestinal metaplasia in Barrett's esophagus, cardia and gastric antrum

Objective To compare the differences in mucin-histochemistry and etiology of Barrett's esophagus (BE) intestinal metaplasia (IM) , cardiac intestinal metaplasia (CIM) and gastric antrum intestinal metaplasia (GA-IM). Methods Alcian blue /periodic acid-schiff (AB/PAS) and high iron diamine / alcian blue (HID/AB) histochemical methods were used to classify IM in BE, cardia and gastric antrum, and IM were classified into three subtypes: complete small intestinal type (type Ⅰ ) , incomplete small intestinal type (type Ⅱ) and incomplete colonic type (type Ⅲ). Compared the prevalence of different subtypes of IM in above-mentioned sites, and investigated their relationships among the symptoms of gastroesophageal reflux disease (GERD) and Helicobacter pylori (Hp) infection. 10.Ischemia-reperfusion injury induces autophagy inhibition in aged rats

Objective To explore the mechanism of hepatic autophagy inhibition induced by ischemia-reperfusion injury in the aging liver.Methods The healthy male Lewis rats aged 3 months (3M) and 24 months (24M) were selected,and then were randomly divided into 3M IRI group,3M sham operation group,24M IRI group,24M sham operation group.In the experimental group,noninvasive vascular clamp was used to clamp the left and middle hepatic lobes (about 70o% hepatic ischemia).The liver was subjected to ischemia at 37 0.5℃ for 30min and reperfusion for 6h.The hepatic duodenal ligament was dissected only by sham operation.The serum levels of serum alanine aminotransferase (ALT) and aspartate aminotransferase (AST) were measured at 6 h after operation.Liver tissues of each group were examined by liver pathology and the number of autophagosome of LC3B in liver tissue of each group was observed under confocal microscopy.The changes of autophagyrelated protein were analyzed by Western blotting.Results The levels of serum ALT and AST in 24M IRI group were significantly higher than those in 3M IRI group,the difference was statistically significant (P<0.05).Pathological analysis showed that 3M IRI group showed spotty necrosis,the 24M IRI group showed massive necrosis and the infiltration of the inflammatory cells;Confocal microscopy showed that the number of autophagosome in the liver tissue of the 24M sham group was slightly lower than that of the 3M sham operation group and the number of autophagosome in the 24M IRI group was significantly lower than that in the 3M IRI group (P<0.05).The levels of autophagyrelated proteins (Beclin1 and ATG4B protein) in 24M IRI group were significantly down-regulated compare to 3M IRI group (P<0.05).Conclusion The ischemia-reperfusion injury of liver in aged rats inhibits autophagy,and its mechanism may be related to the decrease of autophagy-related protein level in hepatic ischemia-reperfusion injury.
