Objective To study the effect of Yiqi Huoxue Tablets on blood coagulation and fibrinolysis system of rats with cerebral apoplexy induced by artificial cold wave. Methods SD rats were randomized into 4 groups. The apoplexy-prone renovascular hypertensive rat model was established by the bilateral renal arteries clamping method. From the 14th day after the modeling operation, the treatment group was given Yiqi Huoxue Tablets (1.89 g·kg-1·d-1), the positive control group was given aspirin (80 mg·kg-1·d-1), and the model group and the blank group were given the same volume of distilled water, once a day. After 112 days, the rats were put into the artificial cold wave box staying for 3 days to inducing apoplexy, and then thrombin-activatable fibrinolysis inhibitor (TAFI) and protein C ( PC) , thrombin-antithrombin complexes ( TAT) and plasmin-antiplasmin ( PAP) levels were measured. Results The plasma levels of TAFI, PC, TAT, and PAP in Yiqi Huoxue Tablets group were lower than the model group, the difference being statistically significant ( P<0.05) . Conclusion The preventive and therapeutic mechanism of Yiqi Huoxue Tablets for cerebral apoplexy is probably related with the regulation of the balance between blood coagulation and fibrinolytic system of apoplexy-prone renovascular hypertensive rats.

Objective To develop a rapid PCR fingerprinting for discriminating between Candida albicans and Candida dubliniensis isolates. Methods Genomic DNA purified from the two species was amplified by single primer PCR. Oligonucleotide of the minisatellite-specific core sequence of the wild-type phage M13 (5′-GAGGGTGGCGGTTCT-3′) was used as primer and the amplified products were analyzed by gel electrophoresis and microfluidic DNA chip assays. Results Of 17 candida isolates, the PCR fingerprinting generated five strain-specific bands for C. albicans and C. dubliniensis respectively, allowing identification to species level between them. The other bands were minor different in their species. By microfluidic DNA chip, the DNA fragments in size of amplified products for the C. dubliniensis were 960,1177,1297,1495,1797 bp and for the C. albicans 653,1323,1531,2021,2875 bp. Conclusions C. albicans and C. dubliniensis have distinguishable pattern by PCR fingerprinting using the single primer. The microfluidic DNA chip is proposed here as a simple, rapid and highly reproducible tool, especially for the epidemiological investigation.

Objective To evaluate the efficacy and adverse effect caused by capecitabine compared with S-1 as maintenance treatment of patients with advanced gastric cancer (AGC). Methods A total of 123 AGC patients who did not suffer disease progression after first-line chemotherapy were randomized into three groups. The capecitabine group(Cap)received maintenance chemotherapy with capecitabine(1000 mg/m2 twice daily for 14 days,21 days/cycle),and the S-1 group(S1)received S-1(40,50,or 60 mg according to the body surface area and orally administered twice a day for 14 days ,21 days/cycle). The observation group was given the support-ive treatment. Patients kept this chemotherapy regimens until disease progressed or with intolerant toxicity. Re-sults The disease control rate was 70.7%in the Cap group and 80.5%in the S1 group(P=0.304). The median time of progression was 8.3 months in the Cap group and 8.5 months in the S1 group(P = 0.448). Maintenance chemotherapy groups showed better responses in the treatment group than the observation group ,which demonstrat-ed a median progression of 6.7 months(P<0.001). The median overall survival time was 15.3 months in the Cap group and 15.7 months in the S1 group(P = 0.637). Maintenance chemotherapy groups showed better responses than the observation group ,which demonstrated a median survival of 12.8 months (P < 0.05). The main side effects included hyperpigmentation, bone marrow suppression, nausea and vomiting and hand-foot syndrome. No death occurred in relation to the therapy. Conclusion The effectiveness of capecitabine and S-1 as maintenance chemotherapy in AGC patients after the first-line chemotherapy are similar, and both can prolong the time of overall survival. And the adverse reactions can be tolerated.

Objective To analyze the safety and effect of non-invasive pressure support ventilation in 32 patients by using a helmet and to give the appropriate way of patients who need non-invasive ventilation ( NIV) support after congenital heart disease surgery. Methods Patients over one year old after congential heart disease surgery were admitted in our Department of Cardiovascular Thoracic Surgery from July 2015 to December 2015. Patients who get clinically improved within one hour were divided into the early improved group( Group-E) ,otherwise they were classified to non-early improved group( Group-NE) . The general infor-mation,diagnosis, indication of NIV, ICU and hospital stay, complications, and mortality were collected. Results Thirty-two patients were engaged in this study, including 18 patients(56. 25%) in Group-E and 14 patients(43. 75%) in Group-NE. Patients who got improved in the first hour might have a higher incidence of avoiding reintubation[83. 33%(15/18) vs. 42. 86%(6/14),P=0. 02]. The heart rate, respiratory rate, pH,PaO2/FiO2 and lactate were improved in Group-E compared with Group-NE after the first hour by using helmet. At the end of NIV,the oxygenation showed no difference but the PaCO2 was lower in Group-E. In Group-E,the values showed a trend of improvement, while the values in Group-NE showed not only no statis-tical significance in different time points but also seemed to have a tendency of hypercapnia and reduced com-fort behavior scale in the end of NIV. There were 6 cases in Group-E and 10 cases in Group-NE developed ventilation associated pneumonia with the incidence of 33. 33%(6/18) and 71. 43%(10/14),respectively, which was significant difference (χ2 =4. 571,P =0. 03). The total duration of mechanical ventilation of Group-E was shorter than that of Group-NE [ ( 136. 72 ± 151. 49 ) h vs. ( 252. 79 ± 155. 33 ) h, P <0. 05 ] . Conclusion NIV through a helmet in children could be well tolerated and avoid re-intubation. Patients who get improved earlier may have more clinical advantages, such as less time of mechanical ventilation and lower incidence of postoperative complications. Early improvement can be considered as a valuable indicator wheth-er the patient needs to use NIV continuously.
