2.Efficacy of laryngeal mask airway proseal and laryngeal mask airway classic in gynecological patients undergoing laparoscopic surgery

Sixty gynecological patients undergoing laparoscopic surgery during October 2010 to June 2011were randomly divided into 2 groups:laryngeal mask airway proseal ( PLMA group,n =30 ) and laryngeal mask airway classic (CLMA group,n =30).Laryngeal mask was inserted after induction.The heart rate (HR),mean arterial pressure (MAP),airway sealing pressure,the average time of placement,the pulmonary satisfaction and complications were recorded.There was no significant difference in heart rate and MAP between the two groups(P>0.05).The airway sealing pressure [ (26 ±6) cm H2O ( 1cm H2O =0.098 kPa) vs.(16±5) cm H2O],the average time of placement [(36±5) vs.(24±6) s)] and the excellent lung ventilation(90% vs.73% ) in the PLMA group were more significantly increased than those in the CLMA group(P <0.05).PLMA can assure good airway sealing and is more suitable for gynecological patients undergoing laparoscopic surgery.

4.Percutaneous Transthoracic Cutting Needle Biopsy of Pulmonary Lesions: Comparison of the Use of 18 and 20 Gauge Needles.

PURPOSE: We evaluated the usefulness of a CT guided percutaneous transthoracic cutting needle biopsy (PCNB) using a 20 gauge (G) needle for pulmonary lesions after a comparison with the use of an 18 G needle for diagnostic accuracy and complications. MATERIALS AND METHODS: From August 2005 to July 2007, 433 patients underwent a CT guided PCNB. A total of 191 patients were excluded from the study as these patients had benign lesions seen after PCNB, but did not receive a confirmation biopsy or undergo follow-up (> 1 year). We evaluated the diagnostic accuracy for the use of PCNB using the Chi-squared test and analyzed which factors (location and size of lesions, diameter of the needle, distance between the pleura and lesions, presence or not of emphysema) were related to occurrence of a pneumothorax after PCNB using a multi-variant regression test. RESULTS: The diagnostic accuracy for malignant lesions with the use of an 18 G and a 20 G needle were 95.4% and 97%, respectively. The sensitivity, specificity, positive predictive value (PPV), and negative predictive value (NPV) for the use of an 18 G needle were 95.7 %, 100%, 100%, and 91.6%. The sensitivity, specificity, PPV, and NPV were 97.8%, 100%, 100%, and 95.0% for the use of a 20 G needle. A pneumothorax occurred in 5.5% (24/433) of the cases and was closely related to the distance from the pleura to the lesions. CONCLUSION: CT guided PCNB with the use of a 20 gauge needle provided good diagnostic accuracy and the procedure is safe to perform.

5.Effect of no mediator on kainic acid induced behavioral seizures in rats.

AIMTo further explore the roles of endogenous nitric oxide (NO) or NO derivatives in complex partial seizures and generalized convulsions.
METHODSThe effect of pretreatment with L-nitroarginine (L-NNA), an inhibitor of nitric oxide synthase (NOS), or L arginine (L-Arg), a precursor of NO on kainic acid (KA)-induced seizure in rats and the changes in the concentration of NO2 -/NO- in the hippocampus were determined.
RESULTSThe rats appeared with wet dog shakes (WDS) at 15 min and then occurred generalized convulsions during 1 h to 3 h after administration of KA (10 mg/kg i.p.). However, the pretreatment of L-NNA (50 mg/kg) so dramatically promoted and enhanced KA-induced behavioral seizures that the latency of generalized convulsion was shorten dramatically, and the mortality was greatly high. In contrast, the pretreatment with L-Arg (40 mg/kg) markedly delayed or weakened KA-induced behavioral changes, such as increasing latency of WDS and generalized convulsion, shortening time o f seizure and none of animal died during observed time. The concentration of NO2- /NO3- in the hippocampus increased immediately at 30 min and remained to 7 d after the administration of KA. Compared with control group (pretreatment with NS), the concentration of NO2- / NO3- in the hippocampus apparently increased at 3 h and 3 d after the administration of KA in the rats with L-Arg pretreatment.
CONCLUSIONThe endogenous NO (NO or NO derivatives) mediators may play an important role against excitotoxin induced seizures in rats.

6.A basic study and clinical trial of 585 nm pulsed dye laser in the treatment of congestive scars.

OBJECTIVETo study the biological effects of 585 nm pulsed dye laser (FLPDL) in the treatment of congestive scar.
METHODSBy histological study, collagen VG staining and microvascular staining, we investigated the changes of collagen fibers and the density of microvessels in the congestive scars after FLPDL treatment.
RESULTSHistological and immunohistochemistry examinations showed that FLPDL achieved normal vascularity in the scar after over 3 times of treatment.
CONCLUSIONSPDL treatment can change fundamentally the physiology of wound healing if applied in the early phases.

8.Clinical observation on therapeutic effect of the pressing plantar reflex area with wooden needle for treatment of patients with insomnia.

OBJECTIVETo investigate the therapeutic effect of wooden needle on the patients with insomnia.
METHODSOne hundred and twenty patients with insomnia were randomly divided into a wooden needle group and a western medicine group, 60 cases in each group. In the wooden needle group, the patients were treated with wooden needle to press the plantar reflex areas, such as cerebellar, throid and cerebral areas. In the western medicine group, Alprazolam was taken orally. Before and after treatment, Pittsburgh Sleep Quality Index (PSQI) was used to evaluate the therapeutic effect of both groups.
RESULTSThe total therapeutic effect was 100.0% in the wooden needle group, while it was 90.7% in the western medicine group, there was no significant difference between the two groups (P > 0.05). Compared with PSQI before and after treatment, there was difference in the both groups (All P < 0.01), but there was no difference between the two groups (P > 0.05).
CONCLUSIONThe therapeutic effect of wooden needle is similar to that of Alprazolam on the insomnia, indicats that wooden needle is a better therapy for treating insomnia.

9.Prevention of pericardial constriction by transcatheter intrapericardial fibrinolysis with urokinase.

OBJECTIVETo investigate whether intrapericardial urokinase irrigation along with pericardiocentesis could prevent pericardial constriction in patients with infectious exudative pericarditis.
METHODSA total of 94 patients diagnosed as infectious exudative pericarditis (34 patients with purulent pericarditis and 60 with tuberculous pericarditis, the disease courses of all patients were less than 1 month), 44 males and 50 females, aged from 9 to 66 years (mean 45.4 +/- 14.7 years), were consecutively recruited from 1993 to 2002. All individuals were randomly given either intrapericardial urokinase along with conventional treatment in study group, or conventional treatment alone (including pericardiocentesis and drainage) in control group. The dosage of urokinase ranged from 200000 to 600000 U (mean 320000 +/- 70000 U). The immediate effects were detected by pericardiography with sterilized air and diatrizoate meglumine as contrast media. The long-term investigation depended on the telephonic survey and echocardiographic examination. The duration of following-up ranged from 8 to 120 months (mean 56.8 +/- 29.0 months).
RESULTSPercutaneous intrapericardial urokinase irrigation promoted complete drainage of pericardial effusion, significantly reduced the thickness of pericardium (from 3.1 +/- 1.6 mm to 1.6 +/- 1.0 mm in study group, P < 0.001; from 3.4 +/- 1.6 mm to 3.2 +/- 1.8 mm in control group, P > 0.05, respectively), and alleviated the adhesion. Intrapericardial bleeding related to fibrinolysis was found in 6 of 47 patients with non-blood pericardial effusion and no systemic bleeding and severe puncture-related complication was observed. In follow-up, there was no cardiac death, and pericardial constriction events were observed in 9 (19.1%) of study group and 27 (57.4%) of control group. Cox analysis illustrated that urokinase could significantly reduce the occurrence of pericardial constriction (relative hazard coefficient = 0.185, P < 0.0001).
CONCLUSIONThe early employment of intrapericardial fibrinolysis with urokinase and pericardiocentesis appears to be safe and effective in preventing the development of pericardial constriction in patients with infectious exudative pericarditis.

10.Surgical treatment of secundum atrial septal defects in adults over 30 years old.

OBJECTIVETo summarize the experience of surgical treatment of secundum atrial septal defects in adults over 30 years old.
METHODSThere were 469 patients with secundum atrial septal defects in our study (male 144, female 325; ages 30-68, mean 38.6 years old). There were 105 cases with pulmonary hypertension and 458 cases with arrhythmia in the group. Surgical closure of defects were performed in all patients. Surgical closure of 358 cases were done by patches including 305 autologous pericardial patches. The low dose (6 x 10(-6)) nitric oxide inhalation was used in 25 postoperative patients with pulmonary hypertension. Right sided maze procedures were done in 5 cases with atrial fibrillation.
RESULTSSurgical mortality was 0.6% (3 cases), the others were healed. In the group, there were 180 cases with arrhythmia, 27 cases with left ventricular function amyoplasia, 28 cases with low cardiac output syndrome, 12 cases in secondary operation for bleeding and 1 case with air-embolism. The level of mean pulmonary artery pressure of 25 postoperative patients with pulmonary hypertension inhaled nitric oxide was down 28.5%. After right sided maze procedures were done in 5 cases with atrial fibrillation, atrial fibrillation disappeared. 352 cases were followed up from 3 months to 20 years (mean 5.6 years). Twenty-nine cases were in class I-II of cardiac function, and the others were better than class I of cardiac function.
CONCLUSIONSAtrial septal defects in adult should be operated as early as possible. When patch is needed, an autologous pericardial patch is the first selection. Inhaled nitric oxide is an effective method to postoperative pulmonary hypertension. The maze operation should be performed for atrial septal defect with atrial fibrillation while the surgical closure of defect was done. During and after operation, much attention should be paid to preventing and curing arrhythmia and protecting and supporting left heart function.
