Among the complication of filtering surgery for glaucoma, late bacterial endophthalmitis is relatively uncommon, but very disatrous and so requires a prompt and vigorous treatment. Recently, authors experienced 2 cases of late onset endophthalmitis, each developed 13 years and 15 years after filtering surgery. And we performed pars plana vitrectomy as well as medical treatment. These cases had a good response to our medical and surgical treatment with no loss of vision.

Background: Penetrating eye injury is common cause blind or many long-term complications later. One of the complications such as glaucoma in the eye. There were some research on glaucoma after penetrating eye injury, but no reports about the treatment of glaucoma. Objectives: To describe the clinical characteristics and assess the outcomes of surgery in the treatment of glaucoma after penetrating eye injury. Subjects and method: The author performed a prospective analysis of 32 eyes the underwent surgery causes by glaucoma after penetrating eye injury at Trauma department of National Institute of Ophthalmology. Follow up period: 6 months. Data was processed by using medical statistics methods on SPSS 10.5 software. Results: Of the 32 patients, 21 were men (65.6%) and 11 were women (34.4%). Localization of the injury: in the cornea 93.8%; lens damages 100%; vitreous organization 56.3%; retinal detachment 12.5%. Functional success (visual acuity >0.02): 18.8%, 4 eyes were enucleated. The normal intraocular pressure after surgery was 71.9%. Conclusion: The clinical characteristics of glaucoma after penetrating eye injury are complicated and multivariable. The rate of normal pressure after treatment was 71.9%.

30 eyes of 28 patients (18 female, 10 male, aged 45-75) of primary glaucoma (25 eyes with closed angle glaucoma and 5 eyes with opened angle glaucoma) at the National Institute of Ophtalmology were operated. No complications occurred. In postoperative period, there were 4 eyes with low interocular pressure and very shallow vestibule. In discharge, 46.6% eyes got a visual acuity of 4/10 to >6/10, and 13.3% <1/10, interocular pressure was regulated with X=18.20.8 mmHg. All operated eyes had scars. After 1 year, acuity visual had been re-examined on 28 eyes, no change of visual acuity, vision field and scars. Interoculary pressure was normal level of X=19.61.2 mmHg

PURPOSE: To report the surgical outcome of juvenile glaucoma who had filtering surgery with Mitomycin C (MMC). METHODS: We have studied 38 patients (42 eyes, respectively) who were diagnosed as primary open angle glaucoma (31 patients), secondary open angle glaucoma (6 patients) and primary angle closure glaucoma (1 patients). The patients have been followed up for 24 months. RESULTS: Of total 42 eyes, 32 eyes had trabeculectomy only and the other 10 eyes had combined trabeculectomy and trabeculotomy. Mean preoperative intraocular pressure (IOP) was 34.4+/-12.0 mmHg, which was dropped to 13.3+/-4.0 mmHg postoperatively. There were statistically significant drop of IOP after filtering operation (P<0.05). However, success rate between group of age 10-25 and group of age 26-40 was not significantly different (90.5%, 76.2%, respectively). CONCLUSIONS: In conclusion, the filtering operation significantly decreases IOP in juvenile glaucoma.

The purpose of this study was to determine whether preoperative topical antiglaucomatous therapy influences the outcome of subsequent trabeculectomy. We investigated 50 eyes of 36 patients with primary open anlge glaucoma(POAG)and 86 eyes of 77 patients with primary angle closure glaucoma (PACG), who had undergone trabeculectomy. All the patients had received topical antiglaucomatous therapy preoperatively and had been followed up for at least 6 months postoperatively. Both groups, POAG and PACG were divided into two subgroups according to the duration of preoperative antiglaucomatous therapy: (a)for less than 1 month and (b)for more than 6 months. The criterion for success was maintenance of postoperative intraocular pressure(IOP)below 21 mmHg without medication. The rates of success were compared at 1 week, 1 month, 3 months, and 6 months postoperatively. In POAG patients, the overall success rate was clinically significantly higher in subgroup (a)than in subgroup (b)(P<0.05). The success rate in subgroup (a)of POAG patients remained higher irrespective of class of antiglaucomatous medications(P<0.05). In PACG patients, the success rate was also higher in subgroup A, but it failed to reach clinical significance. Postoperative mean IOP was slightly lower in subgroup (a)of all the participants, but it was not clinically significant. Our study suggests that long-term use topical antiglaucoma medications can affect adversely the result of filtering surgery in POAG.

BACKGROUNDNeovascular glaucoma is a refractory disease, and difficult to manage. The aim of this study was to evaluate the clinical outcomes of Ahmed glaucoma valve implantation (AGVI) in neovascular glaucoma (NVG) and non-NVG patients.

METHODSThis prospective, non-randomized study included 55 eyes of 55 patients with refractory glaucoma; 27 had NVG (NVG group) and 28 had non-NVG (non-NVG group). All of the patients underwent AGVI. The NVG group was adjunctively injected with intravitreal ranibizumab/bevacizumab (IVR/IVB) before AGVI. Intraocular pressure (IOP) was the primary outcome measure in this study. Surgical success rate, number of antiglaucoma medications used, best corrected visual acuity (BCVA), and postoperative complications were analyzed between the groups.

RESULTSAll of the patients completed the study (follow-up of 12 months). Kaplan-Meier survival curve analysis indicated that the qualified success rates in the NVG and non-NVG groups at 12 months were 70.5% and 92.9%, respectively; this difference was significant (P = 0.036). The complete success rates in the NVG and non-NVG groups at 12 months were 66.7% and 89.3%, respectively (P = 0.049). Compared with preoperative examinations, the postoperative mean IOP and use of medications were significantly lower at all follow-up time points in both groups (all P < 0.05). There were significant differences in BCVA between the two groups at the 12-month follow-up (χ(2) = 9.86, P = 0.020). Cox proportional hazards regression showed NVG as a risk factor for surgical failure (RR = 15.08, P = 0.033). Postoperative complications were similar between the two groups.

CONCLUSIONSAGVI is a safe and effective procedure in refractory glaucoma, but the success rate of surgery was related to the type of refractory glaucoma. The complete and qualified success rates of NVG patient adjunctive anti-vascular endothelial growth factor treatment are still lower than those of non-NVG patients.

We performed a new technique described by Schocket et al. in 6 cases of advanred, uncontrolled glaucoma and 2 cases of neovascular glaucoma. A silicone tube was inserted into the anterior chamber and aqueous was shunted through the tube to a reservoir around the encircling band. After a mean follow up of 103 days, 8 eyes (100 %) had a sucoessful outcome with the intraocular pressure below 20 mm Hg. Thus we recommend this procedure rather than destructive surgery in the treatment of advanced, uncontrolled glaucoma and neovascular glaucoma.

Various studies have been performed to identify change in the constituents of aqueous humor which was suggested either as a factor for fibroblast proliferation causing glaucoma filtering bleb failure or as a factor for the pathogenesis of primary open angle glaucoma. This study was designed to investigate change of aqueous humor composition after long-term topical antiglaucoma medication. After 16-week instillation of pilocarpine 4% and Betagan(TM) in ten rabbits, aqueous humor was obtained from eight control and eight experimental eyes. Protein bands were identified by SDS-PAGE and gold staining techniques. The density and distribution of these protein bands was significantly different between the two groups. The result of this study may give help for future study of pathogenesis of primary open angle glaucoma and causes of high failure rate after glaucoma filtration surgery in patients on chronic topical antiglaucoma medication.
