2.Research progression of the relationship between integrin α2β1 and drug-induced gingival overgrowth.

Drug-induced gingival overgrowth (DIGO) is characterized by fibrous gingival hyperplasia and increased gingival volume. DIGO is histologically associated with proliferation of cells and deposition of extracellular matrices, particularly collagen. Integrin α2β1 is related to collagen phagocytosis and involved in the occurrence and progression of DIGO. This paper reviews the progress of research on the relationship between integrin α2β1 and DIGO.

3.Quantitative analysis the effect of Ciclosporin on gingival tissue modality of rats.

OBJECTIVEQuantitative analysis the regional specificity and time dependence of Ciclosporin (CsP) on gingival tissue modality of rats.
METHODSEighty SPF grade male Wistar rats of 7 weeks old were randomly divided into experimental group and control group, and each group was divided into 4 subgroups according to the duration of treatment (10, 20, 30 and 40 days). The experimental objects were given intragastric administration of fresh milk including CsP, and the control ones were given intragastric administration of fresh milk the same as the experimental objects. After giving perfusion 4% paraform trans-heart to internal fixation, the specimens were get and made bucco-lingual paraffin sections at lower first molar and made HE staining. The area of buccal and lingual gingival epithelial and connective tissue, the length of the longest rete pegs were measured with the image analysis system. Data were analyzed by two-way analysis of variance of factorial design.
RESULTSThe rats of the experimental group took place gingival overgrowth, and the rete pegs of gingival epithelium of attached gingiva approach muco-gingival junction were prolonged. The area of buccal and lingual gingival epithelium and connective tissue, the length of buccal longest rete pegs of the experimental group were significantly higher than that of the control group rats (bucca P < 0.01, lingua 0.01 < P < 0.05). The length of lingual longest rete pegs of the experimental group were no difference than that of the control group rats. The area of buccal and lingual gingival epithelial, connective tissue, the length of longest rete pegs among experimental groups were no difference.
CONCLUSIONCsP may have the regional specificity on the rats' gingival epithelium of buccal attached gingiva approach muco-gingival junction, but the effect of CsP on the rats' gingiva was no time dependence.

With the increasing number of the orthodontic patients, the relationship between periodontal and orthodontic becomes increasingly close. Orthodontic treatment can improve periodontal status, but the adverse clinical problems of periodontal tissue during orthodontic treatment are relatively common. In this paper, we discuss the problems of soft tissue, including causes, prevention, and treatment of gingivitis, gingival enlargement, gingival recession, and gingival invagination in orthodontic treatment.

5.A Case of Severe Gingival Overgrowth Caused by Long-Term Amlodipine Treatment.

Amlodipine is one of the most commonly used calcium-channel blockers for the management of hypertension in Korea. Gingival overgrowth is an infrequent complication in patients receiving amlodipine treatment. A 52-year-old man on an amlodipine regimen of 10 mg/day for 25 months sought medical attention because of gradually progressive gingival enlargement. Examination of the oral cavity revealed severe gingival overgrowth. We stopped the amlodipine treatment and recommended the maintenance of good oral hygiene and a gingivectomy. Histological findings of the gingivectomy were typical of drug-induced gingival overgrowth, including epithelial thickening with proliferation, acanthosis with elongated rete ridges, and focal parakeratosis. A marked reduction in gingival overgrowth was evident 1 month after the gingivectomy and cessation of amlodipine. This report describes the case of a 52-year-old man who developed severe and histologically confirmed amlodipine-induced gingival overgrowth.

6.Spontaneous teeth migration after periodontal treatment in the patients with drug-induced gingival enlargement.

Anticonvulsants, calcium channel blockers and immunosuppressants are representative drugs related with gingival enlargement. Clinical signs and symptoms caused by drug-induced gingival enlargment frequently appear within 1 to 3 months after medication. At initial stage, it is limited to attached gingiva but may extend coronally and interfere with esthetics, mastication and speech. Interproximal spaces are common beginning area and pathologic teeth migration could be occurred by the lesion. Withdrawal or substitution of medication would be the most effective treatment of drug-induced gingival enlargement. However, periodontal treatment and further supportive periodontal therapy should be provided where change in medication is impossible. The present study reports the cases which show the resolution of inflammation with spontaneous teeth migration without change in medication. In all cases discussed in this report could be efficiently managed with proper periodontal treatment and further supportive periodontal therapy.

7.The Study on the Relationship between blood Cyclosporin A level and Gingival Overgrowth in rats.

The purpose of this study was to evaluate clinically and histopathologically the effects to the periodontal tissue in rats after Cyclosporin A(CsA) administration and to determine whether there is a relationship between dosage of CsA or blood CsA level and the severity of gingival overgrowth in rats. Twenty 6-week-old Sprauge-Dawley rats were randomized into 4groups. The control group received olive oil only and the test groups received daily CsA in olive oil via gastric feeding for 6weeks at a 3,10, and 30 mg/Kg. Rats were weighed to evaluate the systemic effect of drug and stone models were made from alginate impressions of upper and lower anterior region at 2-week interval. On completion of oral CsA administration, blood were collected and blood CsA levels were quantitated by TDxFLx analyzer. Rats were sacrificed and their upper and lower jaws were removed together with the surrounding gingiva and soft tissue for light microscopic examination. The results were as follows: 1. The weight gain of CsA-treated rats was much less than of the control group and central incisors were gradually displaced and separated in the test groups. 2. The extensive fibrovascular proliferation and scattered inflammatoy infiltrates in an edematous stroma were observed in enlarged gingiva of CsA-treated rats. 3. The increase in buccolingual, mesiodistal dimension of the anterior teeth and vertical height of the interdental papilla showed dose-dependent manner in CsA-treated rats. 4. Significant positive correlation exists between blood CsA level and the severity of gingival overgrowth in anterior teeth. This result indicates that the severity of gingival enlargement in CsA treated rats is correlated with dosage of CsA administration and blood CsA level.

10.Phenytoin-Induced Gingival Overgrowth in Un-Cooperated Epilepsy Patients.

Phenytoin-induced gingival overgrowth is a well-known and frequently reported gingival lesion, which was first detected in 1939. However, there are conflicts in the literature about the agents which affect the severity of the lesion. Un-cooperative dental patients are one of the most unsuccessfully treated periodontal patient groups because of the difficulty in maintaining their oral hygiene. This case report consists of two cases with the same characteristics: phenytoin usage, comprehension and speech defects and poor oral hygiene, but each case differs in the duration of the phenytoin therapy. Both of the cases received scaling, root planning and a gingivectomy.
